Cemented versus uncemented fixation of second-generation Trabecular Metal glenoid components: minimum 5-year outcomes.

BACKGROUND: Total shoulder arthroplasty (TSA) with second-generation Trabecular Metal™ implants (Zimmer, Warsaw, IN, USA) has shown good short-term outcomes. Differences in outcomes between cemented and uncemented fixation are unknown. This study compared the clinical, radiographic, and patient-rated outcomes of TSA with cemented vs. uncemented TM glenoids at minimum 5-year follow-up. METHODS: Patients who underwent anatomic TSA with second-generation TM glenoid components for primary osteoarthritis were identified for minimum 5-year follow-up. The patients were divided into 2 groups: cemente

[Testing of cementless stepped-stem prosthetic devices replacing the hip shoulder joint of the dog. Comparison of two animal models (author's transl)].

In 6 dogs the hip joint was experimentally replaced using a ceramic stepped-stem prosthetic device implanted in the thigh bone. In six other dogs V4A steel stepped-stem prosthetic devices inserted in the arm bone was used for replacing shoulder joint. The six devices replacing the shoulder joint healed up without complications and proved to be mechanically stable. From six devices replacing the hip joint only one was successful; the remaining five implants did not heal up due to fractures of the stem, loosening in the thigh bone and osteomyelitis. We suggest that these different results are ba

Long-term clinical and radiological outcomes of a stemless reverse shoulder implant that is fallen out of favor - stemless nano-reverse shoulder arthroplasty.

BACKGROUND: The purpose of this study was to assess the long-term results of the stemless onlay Comprehensive Nano reverse total shoulder arthroplasty (rTSA) system. METHODS: We evaluated 35 shoulders at an average follow-up of 106 ± 14.6 months (range, 80-135) (follow-up rate 66%). Patients were evaluated via the Constant-Murley scale, the DASH score (Disabilities of the Arm, Shoulder and Hand) and the subjective shoulder value (SSV). Furthermore, a visual analog scale (VAS) for pain intensity was used. The passive glenohumeral range of motion and active total range of motion were recorded.

Long-term results and implant survival of revision reverse shoulder arthroplasty after a mean follow-up of ten years.

AIMS: With the growing number of primary arthroplasties of the shoulder being undertaken, revision arthroplasty becomes an increasing challenge. Most revisions are of reverse shoulder arthroplasties (RSAs); however, there is a paucity of long-term results. METHODS: This monocentre retrospective analysis included 101 patients who underwent revision RSA between June 2006 and August 2019. Clinical outcomes were assessed using the Constant-Murley Score (CMS), range of motion (ROM), Simple Shoulder Test (SST), Subjective Shoulder Value (SSV), and satisfaction questionnaires. Implant survival was d

The effect of glenosphere size on functional outcome for reverse shoulder arthroplasty.

PURPOSE: Reverse shoulder arthroplasty (RSA) is an effective surgery for a variety of patients with difficult shoulder pathology. Since postsurgical outcomes are often variable, there has been great effort made to optimize the design and use of these implants. Previous studies demonstrated an association between increased glenosphere size and improved range of motion. The purpose of this study is to assess the relationship between glenosphere size, range of motion, and functional outcome scores. METHODS: This is a retrospective cohort study of 140 patients (148 shoulders) undergoing reverse s

Bilateral alloplastic prostheses for temporomandibular joint reconstruction in a patient with ankylosing spondylitis.

Ankylosing spondylitis (AS) or Bechterew disease is a chronic, usually progressive, systemic inflammatory joint disease, which predominantly affects the spine and sacroiliac joints. In these joints, early inflammatory changes are followed by lumbosacral pain and progressive restriction of spinal movement associated with radiologically visible intervertebral ossification. Peripheral joint involvement occurs in 10 to 30% of patients and shows a predilection for the shoulders, knees, ankles, feet, and wrists. Temporomandibular joint (TMJ) involvement has been described, and its reported frequency

[Surface replacement shoulder: arthroplasty indications and limits].

Surface replacement arthroplasty of the shoulder offers the ability to restore anatomy and biomechanics of the glenohumeral joint, if sufficient humeral bone stock of at least 60% of the humeral head is preserved and no severe deformity is present. The implantation following the individual anatomy helps to avoid complications such as prosthetic malpositioning and periprosthetic fractures. While the use of a glenoid component is technically demanding, revision to a stem prosthesis is relatively easy because of bone stock preservation. Indications for humeral head resurfacing are osteoarthritis

[Clavicula pro humero--a new surgical method for malignant tumors of the proximal humerus].

In the majority of cases with malignant tumors in the proximal part of the humerus a limb saving tumor resection is possible. Reconstruction of the defect is necessary to maintain the length of the arm and to create a fulcrum for elbow flexion and extension. Several methods of reconstruction have been described in the literature including the fixation of distal humerus to the second rib or to the clavicle by means of Küntscher-nails, the implantation of a proximal humerus prosthesis without or with accompanying bone transplantation, a bridging of the defect using an allograft or an arthrodesis

Reliability of synovial fluid alpha-defensin and leukocyte esterase in diagnosing periprosthetic joint infection (PJI): a systematic review and meta-analysis.

BACKGROUND: Synovial fluid proteins had been applied as diagnostic biomarkers for periprosthetic joint infection (PJI) in recent research papers. Thus, this meta-analysis aimed to estimate the diagnostic efficiency of synovial fluid α-defensin and leukocyte esterase (LE) for PJI. METHODS: We conducted our systematic review by searching the keywords in online databases such as PubMed, Embase, Cochrane, Elsevier, Springer, and Web of Science from the time of database inception to October 2018. Inclusion criteria were as follows: patients who have undergone knee, hip, or shoulder joint replaceme

Assessment of virulence traits in Cutibacterium acnes from shoulder periprosthetic joint infections.

BACKGROUND: Cutibacterium acnes is the most common pathogen in shoulder periprosthetic joint infection (PJI) but remains difficult to distinguish from contamination due to its low virulence and presence as a skin commensal. Diagnostic ambiguity and variable antibiotic resistance further complicate management. METHODS: We analyzed 87 C acnes isolates from patients undergoing evaluation for shoulder PJI across 2 institutions. Isolates were obtained from intraoperative cultures and stored for subsequent analysis. Hemolysis, aerotolerance, and growth rate were assessed in vitro. Antimicrobial sus

A gender and size specific evaluation of Grammont-type inlay versus lateralizing onlay stem designs in achieving lateralization and distalization in reverse shoulder arthroplasty.

INTRODUCTION: In reverse shoulder arthroplasty (RSA) new designs enable greater amounts of lateralization to prevent instability and scapular notching and increase range of motion, however, excessive lateralization leads to stress upon the acromion that can result in scapular spine fatigue fractures. Aim of this study was to gender- and size-specifically assess the influence of glenosphere size and different humeral designs on lateralization, distalization, and bony impingement-free range of motion (ROM) in patients undergoing RSA. METHODS: Computed tomography scans from 30 osteoarthritic pat

Hemolytic strains of Propionibacterium acnes do not demonstrate greater pathogenicity in periprosthetic shoulder infections.

BACKGROUND: Hemolysis has been suggested as a feature conferring increased pathogenicity to certain Propionibacterium acnes strains in the setting of shoulder infection. The purpose of this study was to compare the virulence of hemolytic and nonhemolytic P acnes strains in patients undergoing revision shoulder arthroplasty. METHODS: Thirty-nine patients with at least 1 positive culture growth for P acnes at the time of revision surgery were identified with P acnes isolates available for hemolysis testing. Patients were grouped into those with P acnes isolates positive (n = 20) and negative (n

Evaluation of thirty eight cemented pegged glenoid components with variable backside curvature: two-year minimum follow-up.

BACKGROUND: The PERFORM™ pegged glenoid system has been used for shoulder arthroplasty since 2012. This system offers multiple backside curvatures per size to better match variable patient anatomy. As a result, less reaming is required and subchondral bone is preserved-a critical factor in preventing glenoid migration and loosening, thus enhancing implant longevity. PURPOSE: The purpose of this study was to analyze all radiographic modifications around this new glenoid implant. METHOD: Thirty-eight shoulders which received the PERFORM™ pegged glenoid component between June 2012 and January 2

[Periprosthetic humeral fractures: Strategies and techniques for osteosynthesis].

The prevalence of periprosthetic humeral fractures (PHF) is currently low and accounts for 0.6-2.4%. Due to an increase in the rate of primary implantations a quantitative increase of PHF is to be expected in the near future. The majority of PHF occur intraoperatively during implantation with an increased risk for cementless stems and when performing total arthroplasty. Additional risk factors are in particular female gender and the severity of comorbidities. In contrast, postoperative PHF mostly due to low-energy falls, have a prevalence between 0.6% and 0.9% and are significantly less common
