Sleeping sickness in Uganda: revisiting current and historical distributions.

BACKGROUND: Sleeping sickness is a parasitic, vector-borne disease, carried by the tsetse fly and prevalent in sub-Saharan Africa. The disease continues to pose a public health burden in Uganda, which experienced a widespread outbreak in 1900-1920, and a more recent outbreak in 1976-1989. The disease continues to spread to uninfected districts. OBJECTIVES: This paper compares the spatial distributions of sleeping in Uganda for the 1900-1920 outbreak period with current disease foci, and discusses information gaps and implications arising for future research, prevention and control. METHODS:

A heuristic mathematical model for the dynamics of sensory conflict and motion sickness.

The etiology of motion sickness is now usually explained in terms of a qualitatively formulated "sensory conflict" hypothesis. By consideration of the information processing task faced by the central nervous system in estimating body spatial orientation and in controlling active body movement using an "internal model" referenced control strategy, a mathematical model for sensory conflict generation is developed. The model incorporates and extends models proposed by von Holst, Held, and Reason, and is congruent with multisensory models for spatial orientation developed by Young and coworkers. T

The impact of pain and impaired role performance on distress in persons with cancer.

The observed upward trend in the incidence of many cancers is expected to continue for the next 20 years despite monumental basic research efforts. There are interpersonal and intrapsychic complications associated with cancer and its treatment leading to considerable distress which, in extreme cases, becomes clinical depression and anxiety. Pain and impaired role performance have been cited as being important sources of distress in people with cancer. This paper presents data from a community sample of 1,309 people living with cancer in Prince Edward Island, Manitoba and Quebec and examines ri

Frequency, Not Amplitude, of Latency Affects Subjective Sickness in a Head-Mounted Display.

BACKGROUND: Interactions between frequency and amplitude of latency in head-mounted displays (HMDs) are thought to affect simulator sickness. Many studies have linked system latency to subjective sickness, but recent research has found that at least with the case of inertia-based head tracking technology, latency is not a constant; rather it varies systematically over time due to sensor errors and clock asynchronization. The purpose of this experiment was to further explore the relationship between frequency and amplitude of latency as they relate to subjective sickness experienced in an HMD.

Developmental changes of alpha 1-adrenergic chronotropic action on human sinus node.

The important role of alpha 1-adrenoceptors in reperfusion ventricular arrhythmia is mentioned in the introduction. However, the present report is concerned with the sinus node. It is speculated that the decreased positive chronotropic action of alpha 1-adrenergic stimulation due to aging may contribute to the development of sick sinus syndrome.

Research strategies for motion and space sickness.

Research is urged for several motion sickness topics now receiving relatively little attention. The role of the area postrema, if any, is not completely resolved. The hypothesis that the cerebral spinal fluid might be a necessary neurochemical conduit should be confirmed or put to rest. It is surprising that the cerebellum, implicated more than 40 years ago, has not been reevaluated for its role in motion sickness in view of the explosion of new data on this structure. The sensory rearrangement theory, known by several names, needs direct experimental verification. One such experiment should d

Management of Plantar KeratodermasLessons from Pachyonychia Congenita.

Plantar keratodermas can arise due to a variety of genetically inherited mutations. The need to distinguish between different plantar keratoderma disorders is becoming increasingly apparent because there is evidence that they do not respond identically to treatment. Diagnosis can be aided by observation of other clinical manifestations, such as palmar keratoderma, more widespread hyperkeratosis of the epidermis, hair and nail dystrophies, or erythroderma. However, there are frequent cases of plantar keratoderma that occur in isolation. This review focuses on the rare autosomal dominant keratin

Somatization in cancer.

Somatic symptoms can occur in disease-free cancer patients. The causes of such symptoms in cancer can be many and varied. These could be due to anxiety, depression, somatization or a manifestation of illness behaviour. Somatic symptoms can also arise out of treatments for the cancer like radiation treatment or chemotherapy. Cancer related somatic symptoms have cognitive, psychological, and physiological causes, each of which is amenable to treatment. The occurrence of somatoform disorders in cancer patients is likely to complicate the treatment and outcome of the cancer. Common somatic symptom

Exploring concerns of children with cancer.

BACKGROUND: Aggressive treatment protocols in pediatric oncology have major effects on the lives of children with cancer. The effects of lifestyle changes such as hospitalization and home schooling on quality of life have not been investigated. This study explores lifestyle effects of cancer therapy on the quality of life of children with cancer. The goals of this study were to identify important quality-of-life issues from the perspectives of children with cancer and to identify how they think their experience with cancer treatment could be improved. MATERIALS AND METHODS: Pediatric oncology

Indoor air quality and health in offices and other non-industrial working environments.

BACKGROUND: Over the last 30 years, transformation of indoor environments--in particular in office blocks--has been associated with complaints from workers of discomfort, malaise and even diseases termed Building Related Illnesses (BRI) which are classified as specific (e.g. Legionnaire disease, asthma, hypersensitivity pneumonia) or non-specific (e.g. the Sick Building Syndrome). METHODS: A review was made of data from international public health organisations, epidemiological, clinical and experimental studies and congress proceedings from 1990 to 2006 on the topic of indoor air quality and

[Do psychological factors change in patients with spasmodic dysphonia after injections with botulinum-toxin?].

OBJECTIVE: A pre-/posttherapeutic comparative study was carried out to investigate possible associations of voice symptoms, selected psychological factors and their changing. SAMPLE: A total of 11 patients with spasmodic dysphonia (adductor-type) subjected to botulinum-toxin-therapy. Patients had received treatment without any complications for at least 2 years. During that period of time, they received a minimum of 3 and a maximum of 9 injections (mean: 6). METHODS: Psychosocial data were measured before and after treatment using standardized psychological methods. The time span between mea

[Torticollis spasmodicus, blepharospasm and hemifacial spasm. Subjective evaluation of therapy by patients].

Injections with botulinum toxin type A (BTX) are considered the first-line treatment for spasmodic torticollis (ST), blepharospasm (BL) and hemifacial spasm (HFS). Because BTX brings only temporary and partial relief, patients frequently try other additional therapies to minimize their symptoms. The subjective rating of all therapies ever tried by patients with ST, BL and HFS was evaluated by using a simple questionnaire. Two hundred questionnaires were considered (112 TS, 54 BL, 34 HFS). BTX was rated subjectively the best therapy in all three diagnostic groups (median: 2 = good effect). Desp

A group therapy approach to facilitate integration of risk information for women at risk for breast cancer.

OBJECTIVE: To describe and illustrate elements of a group counselling approach designed to enhance the communication of risk information on breast cancer (BC) to women with a family history of this disease. Breast cancer is a leading cause of female cancer death. The most important risk factor for BC is a positive family history in at least 1 first-degree relative, and approximately one-third of women with BC have a family history of the disease. Recent evidence suggests that there is a significant psychological impact associated with having a family history of BC, and this may influence the p

Somatic complaints associated with depressive symptoms in HIV-positive patients.

BACKGROUND: Although several studies have indicated the association of mood states including depression with somatic complaints in HIV patients, there has been no report on Japanese HIV patients. The purpose was to examine the relationship between somatic complaints and mood states in HIV patients without AIDS. METHODS: Subjects were 50 HIV patients without AIDS. As controls, we used 47 healthy persons without HIV, matched for age and gender of the subjects. After the Structured Clinical Interview for the DSM-III-R, several inventories including the Profile of Mood States (POMS) were administ

[Quality of life after implantation of a cardioverter-defibrillator (ICD)--results of an empirical study of 132 ICD patients].

All 181 patients with malignant arrhythmias who have been provided with an implantable cardioverter-defibrillator (ICD) at the University Clinics of Muenster during the last 3 years, were investigated by a clinical questionnaire on quality of life (QOL). 132 patients answered and could be included in the study (response rate 73%). The mean age of the patients was 55 +/- 11.8 years, 73% males, 60% had already retired. In the general view 51% of the patients reported fair to excellent quality of life, while 24% gave a negative description of their QOL. 26% of the patients suffered from side-effe

The pediatric psychologist's role in differential diagnosis: vocal-cord dysfunction presenting as asthma.

Presented the case of an 11-year-old boy with vocal-cord dysfunction (VCD) as an example of a rare clinical phenomenon that may result in clinical and systemic challenges for the pediatric psychologist. VCD presents as highly similar to asthma, yet is best treated with speech therapy and psychosocial intervention. The physical symptomatology of VCD and its conceptualization as a psychosomatic disorder are described. Difficulties inherent in integrating psychological factors into medical case formulation are discussed, and possible pitfalls and strategies are delineated.

Illness behavior influences the outcome of laparoscopic antireflux surgery.

BACKGROUND: While gastroesophageal reflux is amenable to medical and surgical treatment, the severity of pathophysiologic changes of the disease correlates poorly with the symptomatology. Similarly, the overall successful outcome of surgical therapy can be lessened by the poor outcome in a small number of patients despite the technical success of the operation. We conducted a study to determine the influence of illness behavior on the outcome and efficacy of laparoscopic Nissen fundoplication. STUDY DESIGN: From a larger group of patients undergoing a laparoscopic Nissen fundoplication, 77 pa

Psychological distress among adult patients being evaluated for bone marrow transplantation.

A sample of 437 patients being evaluated for bone marrow transplantation (BMT) completed interviews and questionnaires to assess their psychosocial adjustment. Nearly a third of the patients (31%) showed some degree of depression on the Center for Epidemiologic Studies Depression Scale. Scores on the Profile of Mood States Scale also indicated that these BMT candidates were experiencing a high level of psychological distress. This distress was found to be predicted by low scores on the Self-Rated Karnofsky Performance Scale and on scales measuring mastery and dispositional optimism. The value
