INTRODUCTION: Skull base osteomyelitis typically presents in an immunocompromised patient with severe otalgia and otorrhoea. Pseudomonas aeruginosa is the commonest pathogenic micro-organism, and reports of resistance to fluoroquinolones are now emerging, complicating management. We reviewed our experience of this condition, and of the local pathogenic organisms.
METHODS: A retrospective review from 2004 to 2011 was performed. Patients were identified by their admission diagnostic code, and computerised records examined.
RESULTS: Twenty patients were identified. A facial palsy was present in…

AIMS: The goal of this study was to report skull vault injuries characteristics among motorcyclists after crash.
MATERIALS AND METHODS: It was descriptive study performed at the Departmental Hospital of Borgou, Benin from January 2008 to April 2009. It concerned motorcyclists hospitalized after traffic accidents. They undergoing skull X-ray and or Brain CT-scan.
RESULTS: On 106 patients answering the criteria of inclusions, 40 (37,7%) presented a lesion of the skull vault. Sex ratio M:F was 4,71. The mean age was 24,55 years. Skull X-Ray was performed in 25 cases, CT- scan in 9 and both in 6…

OBJECTIVE: The primary purpose of this study was to evaluate the efficacy of gamma knife radiosurgery (GKRS) when used as a treatment modality for cavernous sinus or posterior fossa skull-base meningiomas (SBMs), with particular attention given to whether or not intentional partial resection followed by GKRS constitutes an appropriate combination treatment method for larger SBMs.
PATIENTS AND METHODS: Of the 101 SBM patients in this series, 38 were classified as having cavernous sinus meningiomas (CSMs), and 63 presented with posterior fossa meningiomas (PFMs). The patients with no history of…

During retrosigmoid and far-lateral skull base surgical approaches, the head may be positioned at the extreme limits of rotation and flexion. In rare instances, patients may develop acute sialadenitis after surgery as a result of this positioning technique. Over a 4-year period, five patients developed postoperative sialadenitis after undergoing either a retrosigmoid craniotomy in the supine position (n = 4) or a far-lateral craniotomy in the park-bench position. Based on all the retrosigmoid and far-lateral approaches performed by the senior author (RFS), the incidence of sialadenitis was 0.8…

We report on a 74 year old patient who was admitted to the Emergency Department of Cologne University after an attempt to commit suicide. Clinical examination showed no neurological deficits but two bleeding bullet holes in his head bilateral in the temporal areas initially suggesting a complete passage of a bullet through the head. The patient was able to answer questions but showed also a slight bleeding from his mouth while talking. Emergency CT showed a bullet between the internal and external lamina of the temporal bone on both sides and one bullet in the hard palate. With general anesthe

This report documents a 47-year-old male who developed acute deficits of the IX, X, XI, XII cranial nerves and Horner's symptoms, consistent with Villaret's syndrome. Neuroimaging studies demonstrated an osteolytic lesion in the skull base involving the clivus and jugular foramen. The patient recovered after the antibiotic treatment for proteus mirabilis infection. We suggest that Villaret's syndrome can be a rare presentation of skull base osteomyelitis.

OBJECTIVE: To evaluate the efficacy of a therapeutic regimen in the treatment of patients with culture-negative skull base osteomyelitis.
STUDY DESIGN: Retrospective case review.
SETTING: Tertiary referral hospital.
PATIENTS: Eight patients with diabetes mellitus presented with otalgia and were found to have positive technetium and gallium scans of the temporal bone. These patients, however, all had negative cultures of their external auditory canals. All patients had been treated with ototopic drops and two patients had undergone a 2-week course of oral quinolones.
INTERVENTIONS: All pati…

Facial nerve rerouting techniques were developed to facilitate re-section of extensive tumors occupying the skull base. Facial nerve rerouting has its own limitations and risks, requiring microsurgical expertise, additional surgical time, and often some degree of facial nerve paresis. This article presents different degrees of anterior and posterior facial nerve rerouting, techniques of facial nerve rerouting, and a comprehensive review of outcomes. It then reviews anatomic and functional preservation of the facial nerve in acoustic neuroma resection, technical aspects of facial nerve dissecti…

The asymmetric skull shape, or plagiocephaly has become of particular concern since the advent of the "Back to Sleep Campaign," as increasing numbers of children present with this deformity within the first year of life. In this article we review the diagnosis and treatment options for the management of various forms of plagiocephaly. Of particular concern are the various forms of craniosynostosis which may be mistaken for deformational plagiocephaly.

BACKGROUND: The standard surgical treatment of meningiomas is total resection of the tumour. The complete removal of skull base meningiomas can be difficult because of the proximity of cranial nerves. Stereotactic radiosurgery (SRS) is an effective therapy, either for adjuvant treatment in case of subtotal or partial tumour resection, or as solitary treatment in asymptomatic meningiomas.
METHOD: Between September 1992 and October 1995. SRS using the Leksell Gamma Knife was performed on 46 patients (f:m 35:15), ranging in age from 35 to 81 years, with skull base meningiomas at the Neurosurgica…

Anterior craniofacial resection has become a popular operation for nasoethmoid tumours involving the skull base. Many papers have been published since the first by Ketcham et al. in 1963. However, there is still controversy about the method for reconstruction of an anterior skull base defect after resection. The simple reconstruction of Ketcham has been followed by more sophisticated procedures using galeal-pericranial flaps, free flaps with microvascular anastomosis and bony or alloplastic augmentation. The main purposes of the reconstructions are to prevent brain herniation, to avoid intracr…

Lesions in the parasellar and paracavernous regions can be removed by various skull base approaches involving basal osteotomies. A major complication of intradural skull base approaches is CSF leak and associated meningitis. We have managed 5 patients with skull base lesions with a pure extradural approach using wide basal osteotomies. The operative techniques are described.

Skull base tumours represent a special challenge to surgeons due to the complex anatomy of the area. While small tumours are easy to remove, large lesions can pose complex situations. The most difficult aspects are not only the approach and removal, but specially the repair of the defects created by the resection of the tumour. We present here our experience with the surgical removal of tumours on the anterior skull base. To achieve a good approach, we resort to a bifrontal craniotomy including the cilliar arches. To obtain a skull base bone flap that can be used for repair at the end of the p…

Vasospasm after resection of skull base tumors is a rare complication that often produces serious ischemic sequelae. In four of the authors' recent cases, vasospasm complicated the patient's postoperative course. A review of the literature produced a number of cases that can help determine possible causes of vasospasm after tumor resection, ways to prevent it, and methods to evaluate it when it occurs. The cause appears to be multifactorial and the surgical approach may contribute to the pathogenesis of vasospasm. Physicians must have a high degree of suspicion to detect vasospasm at an early …
