Role of melatonin in the induction and maintenance of sleep.

Pharmacokinetic studies of melatonin in young and elderly human volunteers, and the measurement of hypnotic effects in chicks under alternate light-dark or permanent light conditions, show that melatonin is a bioprecursor of hypnotic acetyl metabolites produced by the enzymatic acetylation of both melatonin and 2-oxomelatonin under the control of serotonin N-acetyltransferases (NATs), which are present in the pineal gland. The acetyl metabolite of melatonin, which we call carbo2, is an N-acetyl-β-carboline. The electroencephalographs (EEG) architecture of the sleep produced by this compound is

Homeostatic sleep regulation in patients with idiopathic hypersomnia.

OBJECTIVE: To determine whether the spectral activity during sleep of patients with idiopathic hypersomnia (IH) differs from that of healthy subjects. METHODS: Spectral analysis of the electroencephalogram (EEG) was performed in 10 patients with IH and in 10 age-matched control subjects. We compared the time course of absolute power for slow wave activity (SWA: 0.75-4.5 Hz), and for theta, alpha, sigma and beta bands for the first 4 non-rapid-eye movement (NREM) episodes. RESULTS: Compared to controls, IH patients had less SWA across the night although the exponential decay was preserved. Th

Depression, sleep, and antidepressants.

Sleep disturbances are an integral feature of depressive disorders. Like the disorders themselves, the sleep disturbances associated with depression are heterogeneous, ranging from hypersomnia to marked difficulties maintaining sleep. These difficulties are to some extent age dependent and reflect abnormalities of central nervous system arousal. Moreover, the sleep disturbances associated with depression have both reversible, or state-dependent, and more persistent trait-like characteristics. Polysomnographic recordings can be used to document sleep maintenance difficulties, and they often als

Anesthesia with abdominal surgery leads to intense REM sleep during the first postoperative week.

Characteristics of nocturnal sleep were investigated in six patients after anesthesia and cholecystectomy and in another six after anesthesia and gastroplasty. All night polysomnographic recordings were obtained while each patient slept in a private surgical ward room through two nights before and five or six nights after operation. Anesthesia included thiopental, N2O, isoflurane, and fentanyl. Postoperative analgesia was provided with parenteral morphine. Other aspects of care were routine. Nocturnal sleep was markedly disturbed after both surgical procedures. Throughout the operative night a

Neural memory of the genioglossus muscle during sleep is stage-dependent in healthy subjects and obstructive sleep apnoea patients.

KEY POINTS: In most patients with obstructive sleep apnoea (OSA), there is a spontaneous resolution of the breathing disorders during slow wave sleep (SWS) for yet unknown reasons related to non-anatomical factors. Some recently identified forms of neural memory specific of upper airway muscles may play a role in this phenomenon. In the present study, we show for the first time that a form of memory of the genioglossus (tongue) muscle is greatly enhanced during SWS compared to non-rapid eye movement stage 2 sleep. The present study represents a step forward in understanding the mechanisms resp

The influence of two behavioral regimens on the distribution of sleep and wakefulness in narcoleptic patients.

Thirty-two hours (night-day-night) of polygraphic recordings were performed on 14 patients with a diagnosis of narcolepsy-cataplexy. Half of the patients stayed in bed during the day, whereas the other half were seated at a table. Patients were free to nap whenever they wanted to. Patients under continuous bedrest slept 2-3 times more during the day than patients who were sitting at the table. Rapid-eye-movement (REM) sleep and slow-wave sleep (SWS, stages 3 and 4) were nearly absent during daytime sleep in the table group, but not in the bed group. The differential behavioral regimes during t

EEG effects of a single low dose of ethanol on afternoon sleep in the nonalcohol-dependent adult.

Three polygraphic recordings (PGR) of afternoon sleep (ANS) related to the duration of one sleep cycle, i.e., 90 min, were performed in 14 healthy adult volunteers (7 men and 7 women): two reference PGR, on two consecutive days (before ingestion of alcohol). Only the second being retained: reference PGR = P1; another recording, on day 3, 50 min after the start of single slow oral ingestion of the equivalent of 0.25 g 95% ethyl alcohol (ETOH) per kg body weight. Alcohol was ingested as 40 degrees whiskey, and the volume administered ranged from 34.5 to 66 ml (ETOH polygraphic recording = P2). A

Immune control of sleep pressure via interferon-γ in mice.

Sleep pressure reflects the brain's homeostatic need to sleep, but the mechanisms underlying its regulation remain poorly understood. In mice, a subset of cortical inhibitory neuronal nitric oxide synthase (nNOS) positive interneurons tunes the electroencephalographic slow wave activity in the delta band (<4 Hz), marker of sleep pressure. Here, we demonstrate that in mice the natural killer (NK) cells and innate lymphoid cells (ILC)1 depletion inhibits nNOS+ interneurons and EEG delta activity reducing the time spent in the non-rapid eye movement (NREM) sleep. The optogenetic re-activation of

Lack of efficacy of music to improve sleep: a polysomnographic and quantitative EEG analysis.

An increasing number of studies have been examining non-pharmacological methods to improve the quality of sleep, including the use of music and other types of auditory stimulation. While many of these studies have found significant results, they suffer from a combination of subjective self-report measures as the primary outcome, a lack of proper controls, often combine music with some type of relaxation therapy, or do not randomise subjects to control and treatment conditions. It is therefore difficult to assess the efficacy of music to induce or improve sleep. The present study therefore exam

Naltrindole retards tolerance development to morphine-induced effects on EEG and EEG power spectra.

In the present study, EEG and EEG power spectra were used to assess the effects of naltrindole, a selective delta opioid antagonist, on the development of tolerance to morphine. Adult female Sprague-Dawley rats were implanted with cortical EEG electrodes and permanent indwelling i.c.v. and i.v. cannulas. Twice daily for 7 days, rats were pretreated with either i.c.v. naltrindole (20 nmol) or i.c.v. water, 20 min before i.v. morphine (10 mg/kg) injections. The treatments produced EEG slow-wave bursts and associated behavioral stupor. The amount and duration of these effects decreased less rapid

Effects of some H1-antagonists on the sleep-wake cycle in sleep-disturbed rats.

The present study was undertaken to investigate the effects of some H(1)-antagonists on the sleep-wake cycle in sleep-disturbed rats in comparison with those of nitrazepam. Electrodes were chronically implanted into the frontal cortex and the dorsal neck muscle of rats for the electroencephalogram (EEG) and electromyogram (EMG), respectively. EEG and EMG were recorded with an electroencephalograph. SleepSign ver. 2.0 was used for EEG and EMG analysis. The total times of waking, non-rapid eye movement (non-REM), and rapid eye movement (REM) sleep were measured from 10:00 to 16:00. Nitrazepam sh

Epileptic Nocturnal Wanderings in a Young Female: A Rare Case Report and Review of the Literature.

Sleep-related hyperkinetic seizures are a frequent symptom of frontal lobe epilepsy that occurs at night. Although temporal lobe-originating sleep-related seizures have been documented, they often lack hyperkinetic activity. Furthermore, episodic nocturnal wandering is rarely observed and is believed to represent an unusual form of nocturnal epilepsy that responds to anti-seizure medications. In this report, we present a case of a 14-year-old right-handed girl with recurrent sleepwalking and wandering. Interictal electroencephalography revealed an epileptogenic focus in the left temporal lobe

Cortical layer 6b mediates state-dependent changes in brain activity and effects of orexin on waking and sleep.

One of the most distinctive features of the mammalian cerebral cortex is its laminar structure. Of all cortical layers, layer 6b (L6b) is by far the least studied, despite exhibiting direct sensitivity to orexin and having widespread connectivity, suggesting an important role in regulating cortical oscillations and brain state. We performed chronic electroencephalogram (EEG) recordings in mice in which a subset of L6b neurons was conditionally 'silenced', during undisturbed conditions, after sleep deprivation (SD), and after intracerebroventricular (ICV) administration of orexin. While the tot

Rapid Eye Movement (REM)-Predominant Central Sleep Apnea With Alpha-Delta Sleep Pattern: A Case Report.

Central sleep apnea (CSA) occurring predominantly during rapid eye movement (REM) sleep represents an unusual clinical phenomenon, as CSA typically occurs during non-rapid eye movement (NREM) sleep. We present the case of a 32-year-old male with severe obstructive sleep apnea (OSA), with an apnea-hypopnea index (AHI) of 65.5/hr, who exhibited central apneas mainly during REM sleep (central apnea index: 35.9/hr in REM vs. 5.2/hr in NREM), along with concurrent alpha-delta sleep pattern during slow-wave sleep. The patient was taking tramadol 100 mg and pregabalin 300 mg daily for chronic back pa

Opioids, sleep architecture and sleep-disordered breathing.

Opioid use whether acute or chronic, illicit or therapeutic is prevalent in Western societies. Opioid receptors are located in the same nuclei that are active in sleep regulation and opioid peptides are suggested to be involved in the induction and maintenance of the sleep state. mu-Opioids are the most commonly used opioids and are recognized respiratory depressants that cause abnormal awake ventilatory responses to hypercapnia and hypoxia. Abnormal sleep architecture has been reported during the process of opioids induction, maintenance and withdrawal. During induction and maintenance of opi

Automatic Oxford-Medilog 9200 sleep staging scoring: comparison with visual analysis.

We report the results of a comparison study between automatic sleep staging using the ambulatory Oxford-Medilog 9200 system and visual analysis in a group of 30 subjects. Sleep analysis was performed during the second nighttime period of each 48-h polygraphic monitoring. The automatic analysis determined a significant underestimation of total sleep time and stage 2 non-rapid eye movement (NREM) sleep and an overestimation of stage 1. No significant differences were found between the two analyses regarding slow-wave sleep and REM sleep. The differences between automatic and visual analysis of s

Subdiaphragmatic vagotomy does not block sleep deprivation-induced sleep in rats.

Cytokines, such as interleukin-1beta (IL-1beta), are involved in physiological sleep regulation and in the sleep responses to sleep deprivation. Sleep deprivation increases systemic cytokine levels and recent evidence suggests that cytokine-to-brain communication occurs via the vagus nerve. Furthermore, the vagus nerve plays a role in sleep responses elicited by feeding and vagal activity affects electroencephalographic (EEG) activity. Thus, this study examined sleep-wake activity and brain temperature (Tbr) responses to sleep deprivation in subdiaphragmatically vagotomized and sham-operated r

The diagnostic value of the sleep EEG with and without sleep deprivation in patients with atypical absences.

Hitherto it has not been known whether or not the sleep EEG after sleep deprivation is more effective than the simple or drug-induced sleep EEG. To investigate this, we recorded for 32 patients both sleep EEGs without sleep deprivation and, on the following day, sleep EEGs after 24 h of sleep deprivation. All the patients had atypical absences which were almost exclusively combined with generalized seizures or some other seizure types. All patients were receiving antiepileptic therapy. Sleep without sleep deprivation was induced by oral administration of 2 mg/kg body weight Protactyl (promazin

Comparison of auditory brainstem responses and pneumograms in intensive care nursery infants.

Central slowing of auditory brainstem responses (ABRs) and excessive breathing irregularity on pneumogram recordings indicate dysfunction in central auditory pathways and brainstem respiratory control mechanisms, respectively. These centers are anatomically proximate within the brainstem so that ABR slowing and respiratory instability might be expected to occur concomitantly, reflecting overall dysfunction of this part of the central nervous system. To examine the relationship between these two assessments, testing results were compared for 15 infants in the intensive care nursery who had ABRs

Simultaneous stomach-brain electrophysiology reveals dynamic coupling in human sleep.

Sleep involves continuous communication between the brain and body, yet the dynamics of peripheral signals during human sleep remain poorly understood. Here we tested whether gastric electrophysiology exhibits infraslow structure and coordinated fluctuations with cortical rhythms indicative of sleep physiology. Simultaneous high-density electroencephalography (EEG) and electrogastrography (EGG) were recorded across sixty participants and three nights. Gastric power was consistently higher during NREM than REM sleep and declined across successive cycles, consistent with stage-dependent autonomi
