A case of disseminated intravascular coagulation in a 13-year-old-boy caused by a bite from a twig snake (Thelatornis kirtlandii capensis) is described. The coagulant enzyme was found to be a single-chain mole-activate prothrombin and factor X as well as weakly accelerating fibrinolysis. Its action on synthetic chromogenic substrates is described. Boomslang antivenom was unable to block twig snake venom, and the implications of this as regards treatment are discussed.

The image of a queen bitten by a snake is controversial and the facts, such as the swiftness of her death and her servants, and scientific experiments are in favour of a deadly poisoning. The author reminds that in the ancient texts the snake had sacred virtues and it was a symbolic image to embellish the suicide of the one who was sentenced to death by the Romans. Octaves set up the myth of a fatal bite which became an iconographic image for the cinema.

Passive immunotherapy was discovered in 1894. It is the only etiological treatment of envenomations by snakes or scorpions. Immunotherapy is based on administration of antibodies produced by an animal hyperimmunised against venom. Improvement of whole antivenomous sera was obtained, first by separating the antibodies from other components of blood plasma, then by using enzyme digestion of immunoglobulins G and, finally, by purifying the final product. Efficacy and also tolerance were significantly increased. Antivenom administration should be performed through the intravascular route to ensure

Snakebites and the scorpion stings constitute a neglected public health problem, first because their incidence is poorly known and, second, because low income populations are involved. Accidents of envenoming are still regarded as inevitable, leading to limitations of their management. A better knowledge of the incidence and severity of the envenoming should result in a better control. Antivenoms are well tolerated, due to high purification of immunoglobulin, and became the most effective treatment. But they remain inaccessible to the populations of underdeveloped countries because of its high

Every year large numbers of venomous snake bites occur around the world, especially in tropical areas. The World Health Organization classifies venomous snake bites as one of their highest priority neglected tropical diseases, one of the reasons being the short supply of antivenom compared to the number of snake envenomations. The standard of care for snake envenomation is administration of antivenom. Many antivenoms are polyvalent, which are produced using venoms from multiple species of snakes. The polyvalent antivenoms can treat envenomation from snake venoms used in the production, but als…

Snakebite envenomation affects thousands of people annually in Nepal. Published hospital-based studies of snakebite treatment in Nepal are s … oxicity developed up to 25.8hr after suspected elapid snakebites. This was not observed for viperid snake bites. No enrolled patients received any of the currently recommended first aid for snake bite. The prevalence of nocturnal elapid snake bites, the practice of inappropriate first aid measures and highly variable administration of antivenom were identifie …

Snake bites represent a serious public health risk in many regions of the globe, especially in tropical areas. Clinical signs and postmortem changes are well described in human and other mammalian species. However, detailed case reports about venomous snake attacks in avian species are limited. This report describes presumptive fatal envenomations in three psittacines caused by pit vipers in a Brazilian zoo. In one case, a Brazilian lancehead (Bothrops moojeni) was captured in the aviary. In all three cases the dermis around the suspected snake bite area exhibited hemorrhages and edema. Histol…

Data abstracted from 34 death certificates indicate that the three venomous animal groups most often responsible for human deaths in California from 1960 through 1976 were Hymenoptera (bees, wasps, ants and the like) (56 percent), snakes (35 percent) and spiders (6 percent). An average incidence of 2.0 deaths per year occurred during these 17 years, or an average death rate of 0.01 per 100,000 population per year. Nearly three times more males than females died of venomous animal bites and stings. Half of the deaths from venomous snake bites occurred in children younger than 5 years of age. Su…

Antivenom is the most effective method currently available for the treatment of poisonous snake bite. Allergic reactions to antivenom have been reported in the past. Here we shared a case of allergic reactions to antivenom in an old male patient who was bitten twice by the same snake (probably same one) at the same biting site within a month whereas the patient did not show any allergic disorder in the first bitten. Envenomations twice in a short period time by the same kind of snake are very rare. Physician should be alert to the occurrence of allergic reactions in treating this type of patie

Malayan cobra (Naja naja sputatrix) is the venomous snake of the Elapidae family which involves at least three species of Asian spitting cobras, according to the new taxonomy. This snake occurs naturally in southeastern Asia and in Poland it is kept only in the private breedings. Its venom mainly contains neurotoxins which have paralyzing activities to the nervous system and cardiotoxins which act cytolytically. The present study shows a case of the forty-one-year-old man professionally engaging in venomous reptiles who was bitten in his left ring finger by the Malayan cobra. No general sympto

The development and application of immunoassay techniques in relation to snake venom research is reviewed. Enzyme linked immunosorbent assay (ELISA) is compared with radioimmunoassay, immunodiffusion, immunofluorescence, haemagglutination and immunoelectrophoresis. It is concluded that ELISA is the most versatile immunoassay technique so far applied to the field of venom research, its main advantages over other methods including relatively high levels of sensitivity and specificity, reproducibility, simplicity and ease of sample collection. It can also be readily modified into kit form and is

This case report describes a young male German Shepherd dog that presented with bleeding from the lip 24 hours after being bitten by a boomslang (Dispholidus typus). Bites from this snake are infrequently encountered in dogs due to the shy habits of the snake. The boomslang venom is a potent procoagulant causing a consumption coagulopathy with resultant profuse haemorrhage. The only effective treatment is the administration of specific antiserum. Antiserum administration in this patient resulted in cessation of bleeding within one hour after administration. The article also discusses treatment

A 49-year-old female patient presented at the hospital with a history of herpetic blisters, frequent episodes of vomiting and loose stools, bilateral upper and lower limb weakness, and diminishing sensorium. She was diagnosed with hyponatraemia and respiratory failure and later became unconscious with absent brainstem reflexes. The patient was initially treated for herpetic encephalitis, a chronic obstructive pulmonary disease with acute exacerbation, hyponatraemia and neuroparalytic snake bite. Further evaluation, however, identified the uncommon Guillain Barre syndrome presentation with over

Despite widespread use of antivenoms, many questions remain about their effectiveness in the clinical setting. The almost universal acceptance of their value is based mainly on in vitro studies, animal studies and human observational studies. Numerous examples exist where they demonstrate clear benefit, such as consumption coagulopathy in viper envenoming, prevention of neurotoxicity in Australasian elapid bites, systemic effects in scorpion and funnel-web spider envenoming. There are also concerns about the quality and efficacy of some antivenoms. However, it is important not to confuse the e

The epidemiology features and clinical symptoms associated with 121 cases of snakebite requiring admission to 24 hospitals in Jordan, during 2018-2020, are discussed. Ninety-six of them (79%) brought with them to the hospital the snake that bit them or a photograph of it. Echis coloratus was responsible for 68 of the bites and 6 fatalities. Sex ratio was 3.2 males: 1 female, with an overall average age of 27 ± 14.36 years. The highest incidence of bites was reported in September. Bites were most common on hands and legs. The period of hospitalization ranged from 1 to 36 days. Irbid and Karak g

BACKGROUND AND OBJECTIVES: Although exposure therapy is an efficacious treatment for anxiety disorders, fear often returns after treatment. From an inhibitory learning perspective, long-term improvement depends not only on learning that feared stimuli are safe, but also that it is safe to experience the emotional response triggered by these stimuli. Accordingly, the current study examined the effects of occasional threat reinforcement during repeated exposure to multiple cues on the return of fear in snake phobia by incorporating reminders of the feared outcome.
METHODS: Snake fearful communi

Struan Sutherland (1936-2002) was the doyen of medical research in the field of envenomation and the ultimate authority on the medical management of envenomated victims in Australia for almost 3 decades. In 1981 as Head of Immunology Research of Commonwealth Serum Laboratories (CSL), he produced an antivenom against the Sydney Funnel-web Spider (Atrax robustus)-an accomplishment that had defied numerous previous attempts. Struan also invented the pressure-immobilisation technique of first-aid for snake bite. This ingenious, simple but safe and effective technique revolutionised first-aid manag

There is a broad spectrum of renal involvement following snake envenomation. At the clinical level the renal manifestation may be absent or minimal. Mild proteinuria with abnormal urinary sediment may be observed. Significant proteinuria is uncommon. Hematuria and hemoglobinuria are seen in envenomation by vipers or crotalids, while myoglobinuria follows envenomation of sea snakes or elapids. Acute renal failure can occur in these snake bites. All renal structures can be involved. Mesangial proliferative glomerulonephritis is common. Tubular necrosis is the important pathological counterpart o…

OBJECTIVE: Villagers are commonly poisoned by kraits and cobras in India, and resulting deaths are common. An inadequate understanding of appropriate snakebite treatment often delays proper treatment of those who are bitten. A lack of simple airway management equipment such as resuscitation bags and laryngoscopes compounds the difficulty in treating many patients and increases mortality in neurotoxic (elapid) venom poisoning. This article discusses the clinical signs and symptoms of krait and cobra envenoming and medical intervention in a rural setting.
METHODS: We enrolled 30 subjects of pre
