Recovery of Dysarthria in Anti-GluK2 Antibody-Associated Autoimmune Cerebellar Ataxia: A Longitudinal Acoustic and Perceptual Case Report.

Introduction Temporal coordination of speech segments is thought to involve cerebellar contributions to the coordination and timing of articulatory movements. Although cerebellar dysfunction leads to dysarthria, speech symptoms in inflammatory cerebellar disorders may improve following immunotherapy. However, longitudinal changes in speech characteristics during recovery remain insufficiently described. Previous studies have reported abnormalities in syllable timing, speech rate, and articulatory coordination in cerebellar dysarthria, but longitudinal changes in these acoustic features during

Bone Conduction Implants: Comparative of Audiometric Results and Quality-of-Life Bonebridge® versus Osia®.

INTRODUCTION: Bone conduction implants have been indicated for patients with conductive hearing loss, mixed hearing loss, and even profound unilateral sensorineural hearing loss. With the introduction of Bonebridge®, new transcutaneous implant options emerged. The latest is Osia®, a direct-drive variant or active systems, where the implant directly generates and applies vibration to the bone. MATERIALS AND METHODS: Retrospective study of two cohorts of patients treated with active bone conduction implants at a single center, one with the Bonebridge® device and the other with Osia®. OUTCOMES:

Hyperacusis scores and listening characteristics: Extended high-frequency audiograms from a young population without tinnitus and with low noise exposure.

Hyperacusis describes the condition in which mid-level sounds are perceived as uncomfortably loud. This study assessed 36 listeners (mean age = 25.75 years) with low noise-exposure history (Noise Exposure Structured Interview, NESI) and no tinnitus, to investigate the association between assessed hyperacusis scores (Hyperacusis Questionnaire, HQ), measured uncomfortable loudness levels (ULLs), and speech intelligibility in noise (Matrix Speech test). Thresholds were ≤ 20 dB HL, for the standard audiometric signal frequencies (0.25-8 kHz). ULLs were measured for signal frequencies of 0.5-, 2-,

[Evaluative audiometry after cochlear implant provision. German Version].

BACKGROUND: One of the main treatment goals in cochlear implant (CI) patients is to improve speech perception. One of the target parameters is speech intelligibility in quiet. However, treatment results show a high variability, which has not been sufficiently explained so far. The aim of this noninterventional retrospective study was to elucidate this variability using a selected population of patients in whom etiology was not expected to have a negative impact on postoperative speech intelligibility. MATERIALS AND METHODS: Audiometric findings of the CI follow-up of 28 adult patients after 6

Single and multiple microphone noise reduction strategies in cochlear implants.

To restore hearing sensation, cochlear implants deliver electrical pulses to the auditory nerve by relying on sophisticated signal processing algorithms that convert acoustic inputs to electrical stimuli. Although individuals fitted with cochlear implants perform well in quiet, in the presence of background noise, the speech intelligibility of cochlear implant listeners is more susceptible to background noise than that of normal hearing listeners. Traditionally, to increase performance in noise, single-microphone noise reduction strategies have been used. More recently, a number of approaches

Radial forearm free flap for reconstruction of a large defect after radical ablation of carcinoma of the tongue and floor of the mouth: some new modifications.

BACKGROUND: A modified radial forearm free flap was designed to rehabilitate function and to reduce the complications at both donor and recipient sites. METHODS: Between 2003 and 2007, 15 patients with infiltrating squamous cell carcinoma (T(3)-T(4)) of the tongue and/or floor of the mouth underwent hemiglossectomy and resection of the floor of the mouth with microvascular reconstruction using a modified radial forearm flap. The mean size of the forearm flap was 7.5 x 14 cm, and the de-epithelialized area was 7 x 6 cm, requiring no skin graft from the abdomen. Speech intelligibility tests wer

Long-term treatment of severe dysarthria: a case study.

This case study describes the long-term treatment and changing symptoms in a single subject with dysarthria secondary to basilar artery thrombosis. Initially, the subject was anarthric. Treatment efforts thereafter were directed toward modifying speech respiration, velopharyngeal function, articulatory precision, speech intensity, and speech intelligibility. A variety of treatment and measurement techniques are illustrated. The behavioral change resulting from each of the treatments was small. However, when combined, these small gains in conjunction with some neurological recovery resulted in

Radial forearm versus anterolateral thigh flap reconstruction after hemiglossectomy: functional assessment of swallowing and speech.

The authors retrospectively compared the results of postoperative speech and swallowing in patients who had undergone hemiglossectomy for carcinoma of the anterior tongue. Immediate reconstruction in 16 patients was with a free radial forearm flap and in another 16 with an anterolateral thigh flap. Clinical speech pathology evaluation included the Fletcher time-to-time maximum syllable repetition rate, multiple rhyme test, and overall quality and intelligibility of the patients' speech. Evaluation of swallowing included deglutition duration, bolus volume, and ingestion rate. The functional res

Seeing a Talker's Mouth Reduces the Effort of Perceiving Speech and Repairing Perceptual Mistakes for Listeners With Cochlear Implants.

OBJECTIVES: Seeing a talker's mouth improves speech intelligibility, particularly for listeners who use cochlear implants (CIs). However, the impacts of visual cues on listening effort for listeners with CIs remain poorly understood, as previous studies have focused on listeners with typical hearing (TH) and featured stimuli that do not invoke effortful cognitive speech perception challenges. This study directly compared the effort of perceiving audiovisual speech between listeners who use CIs and those with TH. Visual cues were hypothesized to yield more relief from listening effort in a cogn

Speech, Language, and Communication Skills of Adolescents With Craniofacial Microsomia.

Purpose Youth with craniofacial microsomia (CFM) have anomalies and comorbidities that increase their risk for speech, language, and communication deficits. We examined these outcomes in youth with and without CFM and explored differences as a function of CFM phenotype and hearing status. Method Participants included youth ages 11-17 years with CFM (n = 107) and demographically similar controls (n = 306). We assessed speech intelligibility, articulation, receptive and expressive language, and parent and teacher report measures of communication. Hearing status was also screened at the study vis

Velopharyngeal insufficiency managed by autologous fat grafting in patients with aberrant courses of internal carotid arteries.

INTRODUCTION: Velopharyngeal insufficiency (VPI) is usually managed, besides speech therapy, by performing a velopharyngoplasty. An alternative approach is autologous fat grafting (AFG) of the posterior pharyngeal wall. About 5% of the population has internal carotid arteries (ICA) with an aberrant course. This anatomic variation can be responsible for surgical difficulties while when performing a velopharyngoplasty, and therefore lead surgeons to only consider a speech reeducation of VPI. However, AFG is does not bear such surgical morbidity. OBJECTIVE: The aim of this study is to retrospect

Functional outcomes after surgical reconstruction of the base of tongue using the radial forearm free flap in patients with oropharyngeal carcinoma.

BACKGROUND: Historically, the prevention of functional morbidity after multimodal treatment for squamous cell carcinoma in patients with large base of tongue lesions has been challenging. The purpose of the present research is to report prospectively collected speech and swallowing outcomes for patients with disease that encompassed half or more of the base of tongue. METHODS: Speech and swallowing data were gathered at 4 evaluation times that spanned the first year of treatment for 32 patients treated with surgery and adjuvant radiation therapy. RESULTS: Speech intelligibility remained abov

Improved whole-brain reconfiguration efficiency reveals mechanisms of speech rehabilitation in cleft lip and palate patients: an fMRI study.

INTRODUCTION: Cleft lip and/or palate (CLP) patients still have severe speech disorder requiring speech rehabilitation after surgical repair. The clarity of language rehabilitation is evaluated clinically by the Language Rehabilitation Scale. However, the pattern and underlying mechanisms of functional changes in the brain are not yet clear. Recent studies suggest that the brain's reconfiguration efficiency appears to be a key feature of its network dynamics and general cognitive abilities. In this study, we compared the association between rehabilitation effects and reconfiguration efficiency
