[Acute rhinosinusitis and sinusitis].

During the common cold in the adult, involvement of the sinusal mucosa, confirmed by CT scan, is virtually constant and justifies the term "rhinosinusitis" that is now applied. The presence of bacterial superinfection can only be demonstrated by positive laboratory results on a sample, which is never performed in usual practice; thus the practitioner must rely on presumptive factors. Among them, an essential element is the unilateral nature of painful symptoms and purulent rhinorrhoea. When symptoms are bilateral, antibiotics should not be prescribed except when the condition has not been reso

CPAP-induced sphenoid sinus pressures after endoscopic sinus surgery.

Positive pressure transmitted from continuous positive airway pressure (CPAP) to the sinuses and skull base in the early post-operative period has not been studied in live subjects and controversy exists in when to restart this post-operatively. This study found that approximately 32.76% and 13.52% of the delivered CPAP pressures reached the post-surgical sphenoid sinus and the mid-nasal cavity, respectively, suggesting that surgical factors such as tissue edema, nasal packing, blood, and nasal secretions may provide a protective effect.

[Retrobulbar optic neuropathy secondary to an undifferentiated carcinoma of the sphenoid sinus].

Undifferentiated carcinoma of the paranasal sinuses is a rare malignant tumor, characterized by rapid growth, local/regional invasion, metastatic potential and poor prognosis despite aggressive treatment. Clinically, this tumor may manifest as episodes of epistaxis, headache or ophthalmic signs, particularly oculomotor nerve palsies, optic atrophy or even proptosis in the case of orbital extension. We report the case of a patient admitted with a left retrobulbar optic neuropathy, which led to a diagnosis of undifferentiated carcinoma of the sphenoid sinus.

Transalar sphenoidal meningoencephalocele associated with buccal angiomatosis. One case.

Basal encephaloceles are uncommon congenital malformations occurring with an estimated frequency of one case in 35,000 births. They are frequently occult since they protrude into the orbit, nasal cavity, nasopharynx or paranasal sinuses. Basal encephaloceles are divided into five anatomical types. We report a case of transsphenoidal encephalocele which differed from the classical type in that it protruded through a congenital defect in the greater wing of the sphenoid bone. This case had an unusual clinical presentation, being responsible for trigeminal neuralgia and associated with an angioma

Intrasphenoidal meningoencephalocele: a case report.

A case of intrasphenoidal meningoencephalocele is presented, representing the eighth case report in English language literature. The major presenting symptom was retro-orbital headache. The patient also had cerebrospinal fluid rhinorrhea and a history of recurrent meningitis. Diagnosis was suspected on the basis of plain radiographs of the sinuses and was confirmed by computed tomography scan. The meningoencephalocele was reduced via a sublabial, transseptal sphenoidotomy approach with improvement of the patient's symptoms. Management and treatment of isolated sphenoid sinus lesions are discus

Radiology of olfactory neuroblastoma.

Olfactory neuroblastoma is often misdiagnosed pathologically. The authors reviewed eight such cases in which the pathological findings met strict criteria. These neoplasms are locally invasive in the nose and paranasal sinuses, with frequent intraorbital and intracranial extension as well as distant metastases. A rare primary presentation in the sphenoid sinus-clivus region is described. Computed tomography is the procedure of choice in defining intraorbital extension and is complementary to precision laminagraphy for defining intracranial extension. Angiography frequently shows hypervasculari

CT of Anatomic Variants of the Paranasal Sinuses and Nasal Cavity: Poor Correlation With Radiologically Significant Rhinosinusitis but Importance in Surgical Planning.

OBJECTIVE: The purpose of this study was to determine the incidence of sinonasal anatomic variants and to assess their relation to sinonasal mucosal disease. MATERIALS AND METHODS: A retrospective evaluation of 192 sinus CT examinations of patients with a clinical history of rhinosinusitis was conducted. The CT scans were evaluated for the presence of several anatomic variants of the sinonasal cavities, and the prevalence of each variant was calculated. Prevalences of all sinonasal anatomic variants were compared between patients who had minimal to no apparent imaging evidence of rhinosinusit

Sinonasal manifestations of cystic fibrosis: a correlation between genotype and phenotype?

BACKGROUND: Patients with Cystic Fibrosis are prone to develop sinonasal disease. Studies in genotype-phenotype correlations for sinonasal disease are scarce and inconclusive. METHODS: In this observational study several aspects of sinonasal disease were investigated in 104 adult patients with CF. In each patient a disease specific quality of life questionnaire (RSOM-31), nasal endoscopy and a CT scan of the paranasal sinuses were performed. Patients were divided into two groups, class I-III mutations and class IV-V mutations, based on their CFTR mutations. RESULTS: The prevalence of rhinosi

Prevalence of incidental paranasal sinuses opacification in pediatric patients: a CT study.

A prospective evaluation of the paranasal sinuses was performed on a consecutive series of 137 pediatric patients referred for cranial CT. Approximately one-half of the patients less than 13 years of age had some degree of maxillary or ethmoid sinus opacification. The prevalence and severity of opacification was approximately the same for the maxillary and ethmoid sinuses. Sphenoid sinus abnormality was less common (16% of patients) and was usually minimal or mild. No incidental frontal sinus abnormalities were observed. This study confirms previous reports, based on plain film radiography, of

Mucin impaction tumor of the paranasal sinuses: a new clinical entity?

A variety of uncommon benign lesions occur in the paranasal sinuses and have been reported to masquerade as carcinomas. Nearly all of such cases have been limited to the frontal, sphenoid, and ethmoid sinuses with an isolated report of maxillary sinus involvement. The classic roentgenologic picture is that of bony destruction. Heretofore described destructive lesions of the maxillary sinus include the mucocele, mucous retention cyst, pseudocyst, pyocele and cholesteatoma. This report deals with a previously undescribed entity, the mucin impaction tumor located in a septate maxillary sinus. Thi

Surgical strategy for cerebrospinal fluid rhinorrhea repair.

OBJECTIVE: Cerebrospinal fluid (CSF) rhinorrhea is leakage of CSF from the nasal cavity caused by cranial base or meningeal defects. Surgical treatment of CSF rhinorrhea is still problematic. We evaluated the clinical outcomes of 132 consecutive cases of CSF rhinorrhea treated via transcranial or transnasal endoscopic approaches according to the patient's condition. The indications for the approaches are discussed. METHODS: Of 132 patients with CSF rhinorrhea, a transnasal endoscopic approach was used in 98 to repair cranial base defects in the ethmoid and sphenoid sinuses. A transcranial int

Repeated loss of frontal sinuses in arctoid carnivorans.

Many mammal skulls contain air spaces inside the bones surrounding the nasal chamber including the frontal, maxilla, ethmoid, and sphenoid, all of which are called paranasal sinuses. Within the Carnivora, frontal sinuses are usually present, but vary widely in size and shape. The causes of this variation are unclear, although there are some functional associations, such as a correlation between expanded frontal sinuses and a durophagous diet in some species (e.g., hyenas) or between absent sinuses and semiaquatic lifestyle (e.g., pinnipeds). To better understand disparity in frontal sinus morp
