Duodenal perforation in course of endoscopic retrograde cholangiopancreatography-endoscopic sphincterotomy. Therapeutic considerations.

AIM OF THE STUDY: To define the therapeutic program for the treatment of perforative complication of the duodenum in course of endoscopic retrograde cholangio-pancreatography (ERCP) with endoscopic sphincterotomy (ES). MATERIAL OF THE STUDY: In the period from 1997 to 2003, 5 duodenal perforations occurred during 101 ERCP/ES (4.95%), executed in two digestive endoscopic centres. Three patients were operated in emergency (duodenostomy, external biliary drainage, gastric-enteric-anastomosis). The other two were treated conservatively with nasal-duodenal drainage in aspiration. RESULTS: The post…

Influence of juxtapapillary diverticulum on hepatic clearance in patients after endoscopic sphincterotomy.

BACKGROUND AND AIM: The role of juxtapapillary diverticulum ( JPD) in biliary stone formation is controversial. This study was designed to understand the relationship between the size of JPD, hepatic clearance and recurrent bile duct stones in patients after endoscopic sphincterotomy. METHODS: Five hundred and twenty patients with choledocholithiasis who had received endoscopic sphincterotomy were enrolled. They were divided into three groups: group A (n = 268) without diverticula; group B (n = 156) with small diverticula; and group C (n = 96) with large diverticula. All patients were regular…

Sphincterotomy and the treatment of detrusor-sphincter dyssynergia: current status, future prospects.

STUDY DESIGN: Literature review of current treatment options for detrusor-sphincter dyssynergia (DSD) in spinal cord injury. OBJECTIVES: To review the outcomes and complications associated with external sphincterotomy and to summarise the results and complications of alternative treatment options for detrusor-sphincter dyssynergia in spinal cord injury. In addition, we propose a potential alternative future drug treatment for external sphincter dyssynergia based upon recent research on the neuropharmacology of the external urethral sphincter. SETTING: The National Spinal Injuries Centre, Sto…

Early refeeding after endoscopic biliary or pancreatic sphincterotomy: a randomized prospective study.

BACKGROUND AND STUDY AIMS: Patients who have undergone endoscopic sphincterotomy (ES) are usually left to fast for arbitrary reasons until they are examined on the following day. The aim of this study was to check whether this systematic fasting after ES is actually justified. PATIENTS AND METHODS: A blinded randomized prospective study, involving 146 patients, was carried out from January 1999 to September 2001. All patients undergoing biliary and/or pancreatic endoscopic sphincterotomy during this period were randomly allocated to one of two groups: group 1 patients were re-fed 4 hours afte…

Open lateral sphincterotomy is still the best treatment for chronic anal fissure.

Chronic anal fissure is the main cause of anal pain and is very common. Many treatment modalities have been tried-none is ideal. Recently, topical nitric oxide releasing agents and botulin toxin injections have been used, trying to replace surgery. No long-term experience is available. Personal experience with 2,108 open, ambulatory, lateral sphincterotomy with follow-up of 4-20 years is presented.

Reappraisal of endoscopic sphincterotomy for post-traumatic neurogenic bladder: a prospective study.

PURPOSE: We assessed the results of endoscopic sphincterotomy in patients with detrusor-sphincter dyssynergia secondary to spinal cord injury. MATERIALS AND METHODS: A prospective study was done of 92 patients with a mean followup of 20.6 months. RESULTS: The morbidity rate was 10.9%. Results demonstrated objective (assessed by the decrease in voiding pressure) and subjective (expressed by the patient) improvement in quality of micturition in 83.7% and 73% of the cases, respectively. Subjective autonomic dysreflexia resolved in 93.2% of the cases. CONCLUSIONS: Sphincterotomy is a useful and…

Transduodenal sphincterotomy for stenosing papillitis and massive choledocholithiasis after Billroth II gastrectomy.

Management of stenosing papillitis with massive cholecystodocholithiasis in patients previously operated upon with a Billroth II (B-II) type gastrectomy, is a challenging clinical problem. Endoscopic papillotomy in these patients has been described, but the success rate is low especially in the presence of a long afferent loop of the gastrojejunostomy and/or extensive adhesions. Long-standing common bile duct (CBD) stones lead to varying degrees of papillary stenosis. The latter could predispose to new stone formation because of the damaged CBD mucosa and varying degrees of stasis. In this pap

Sphincter of Oddi response to caerulein after endoscopic sphincterotomy for papillary stenosis.

Using a percutaneous transhepatic cholangioscopy (PTCS) catheter, sphincter of Oddi motility was measured in a patient with papillary stenosis secondary to bile duct stones. Prior to sphincterotomy, intramuscular injection of 20 micrograms caerulein did not inhibit pathological contraction waves of the sphincter of Oddi or relieve abdominal pain. Endoscopic sphincterotomy of the lower segment of the sphincter of Oddi resulted in recovery of the normal response to caerulein, i.e. relaxation of the sphincter of Oddi. This observation indicates that the pathological contraction and lack of relaxa…

Endoscopic sphincterotomy for bile duct stones: an institutional review of 272 patients.

The results of the first 283 endoscopic sphincterotomies (ES) attempted in Oxford for bile duct stones are described. Endoscopic sphincterotomy was achieved in 272 patients (96%) and complete duct clearance was achieved in 88% of these patients; an overall success rate of 85%. Complications occurred in 26 patients (10%), five of whom required an emergency laparotomy. Eight patients died within 30 days of ES but only two deaths were attributed directly to the procedure. Although the sphincterotomies were performed by several endoscopists in varying stages of training, the results are similar to…

Endoscopic sphincterotomy: long-term results in 408 patients with complete follow-up.

Endoscopic sphincterotomy was performed in 469 patients for the treatment of biliary calculi, with procedure-related morbidity of 6.3% and mortality of 0.4%. Long-term follow-up to 10.5 years (mean 3.7 years) was completed in all of 408 patients at least six months postsphincterotomy. Recurrent stones developed in 21 patients (5.8%) after a mean of 2.4 years (range 4 months to 7 years); in 6 after 3 years. Eight patients reformed stones more than twice at a mean interval of 1.8 years (range, 5 months to 3.5 years). In the 122 patients with gallbladders in situ, acute cholecystitis occurred in …

Endoscopic balloon tamponade for conservative management of severe hemorrhage following endoscopic sphincterotomy.

We report on two patients in whom balloon tamponade was applied for management of severe hemorrhage following endoscopic sphincterotomy (EST). In both patients after EST severe hemorrhage from the papillary orifice was treated by immediate introduction of a special balloon catheter (balloon catheter, model Mainz, Vygon, Aachen). In both patients after 10 minutes no further bleeding was visualized. We conclude that this simple procedure should be taken into account for the conservative treatment of severe hemorrhage following EST.

Outcome of lateral internal sphincterotomy for anal fissure in a retrospective cohort of patients.

OBJECTIVE: Chronic anal fissures (CAF) is a common cause of severe anorectal pain in adults causing poor quality of life. In case of failure of conservative treatments, lateral internal sphincterotomy (LIS) is an effective cure. We aimed to analyse the outcomes of LIS in a consecutive patients' series operated in a high volume Surgical Coloproctology Unit. METHODS: A retrospective cohort study was conducted on 422 patients with CAF undergoing surgical treatment between January 2015 and December 2019 at our Surgical Coloproctology Unit. Patients over the age of 18, not responding or recurring …

Antiplatelet Monotherapy Is Associated with an Increased Risk of Bleeding After Endoscopic Sphincterotomy.

BACKGROUND: Clinical guidelines recommend continuing antiplatelet monotherapy with aspirin and, in certain situations, other antiplatelet agents in patients undergoing endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy. AIMS: Given the scant evidence supporting this recommendation, our primary objective was to determine if the risk of post-sphincterotomy bleeding was increased in patients on antiplatelet monotherapy. METHODS: We performed a systematic search of Cochrane Library, Ovid Embase, Ovid Medline, Pubmed, Scopus, and Web of Science Core Collection databases. In…

The Role of Lateral Internal Sphincterotomy in Haemorrhoidectomy: A Study in a Tertiary Care Center.

Background Hemorrhoids are a common condition that presents with bleeding per rectum, pain at rest and defecation, mucosal discharge, and prolapse. Surgical hemorrhoidectomy is the treatment method of choice for Grade 3 and Grade 4 hemorrhoids. Hemorrhoidectomy is associated with postoperative pain and no single surgical technique has been proved to significantly reduce the pain. We analyzed in our study the effect of lateral internal sphincterotomy with hemorrhoidectomy on postoperative pain, anorectal function, and retention of urine after the Milligan and Morgan technique. Methods This rand…

Predictors of outcomes after biliary and pancreatic sphincterotomy for sphincter of oddi dysfunction.

BACKGROUND: There are few data on combined pancreatic and biliary sphincterotomy for sphincter of Oddi dysfunction (SOD), especially regarding clinical features that might predict outcomes. We sought to examine the relative importance of various clinical features and the presence or absence of objective biliary abnormalities in determining responses to endoscopic therapy. METHODS: A cohort of consecutive patients with suspected SOD was treated with biliary sphincterotomy, with additional pancreatic sphincterotomy at initial or subsequent endoscopic retrograde cholangiopancreatography if there…

Endoscopic sphincterotomy in the treatment of postoperative biliary fistulas of hepatic hydatid disease.

BACKGROUND: Ten patients with postoperative external biliary fistula treated by endoscopic sphincterotomy are reported. METHODS: Nine of these patients were operated for hepatic hydatid disease and one for a liver abscess. Mean daily output of bile through the fistulae which were present for 5-39 days was approximately 500 cc. RESULTS: Treatment was successful in nine patients with closure of the fistulae in 2-15 days (mean, 7 days). No response was obtained in one patient who was reoperated, and an intrahepatic biliary duct was found to be completely eroded by the cyst wall. CONCLUSIONS: E…
