Two cases of plant thorn synovitis. Difficulties in diagnosis and treatment.

Two patients with inflammatory monoarthritis of the knee due to joint penetration by plant thorns are reported. Histopathology demonstrated granulomatous synovitis in both and visualized fragments of black thorn. Synovectomy was partly effective. Awareness of this cause of chronic monoarthritis will avoid delay in diagnosis and treatment.

[Detritus synovitis in chronic polyarthritis: a clinical and operation histologic evaluation].

In rheumatoid arthritis (RA) joint inflammation is due to two processes: 1) the underlying inflammatory process (UIP) characterized by a lymphoplasmacellular infiltration of the synovial tissue, as well as pannus formation, and 2) the detritogenic synovitis (DS), a synovial response to articular wear products from cartilage and bone (detritus) that induces a preferentially fibrinous inflammation. In order to estimate the role of DS in the clinical presentation of such joints, 40 patients with RA undergoing knee-joint surgery on 48 occasions were evaluated for clinical parameters, radiological …

Polyarticular pigmented villonodular synovitis in children: evidence for a genetic contribution.

Pigmented villonodular synovitis rarely occurs in multiple joints, and only once has been noted in siblings. We identified this disorder in 2 generations and found evidence for its presence in a third. It occurred in 2 sibling children at multiple sites. All affected individuals also had a rare autosomal dominant disorder, the multiple lentigines syndrome. Among the developmental abnormalities in this kindred were fibrous dysplasia, hypertelorism, pectus excavatum, high arched palate, and primary lymphedema. The etiology of pigmented villonodular synovitis is uncertain, and suggested causes in…

Secondary malignant synovitis: report of three cases and review of the literature.

Metastatic malignant deposits in joints are rare and only 16 cases have been described. The details of three further cases are presented, a girl with Hodgkin's lymphoma in a sternoclavicular joint, an elderly lady with carcinoma of the ascending colon and knee metastases and a middle-aged man, also with knee deposits, from an adenocarcinoma of lung. The clinical features together with details of synovial fluid cytology of all 19 cases are analysed.

Ultrasonography in hip joint effusion. Report of a child with transient synovitis.

In a 7-year-old boy with clinical symptoms of transient synovitis of the hip, an intracapsular effusion was diagnosed with computed tomography and ultrasonography. An intracapsular pressure of 50 mmHg was recorded prior to aspiration of 5.5 ml of clear fluid. Repeated ultrasonography immediately after aspiration verified a reduction in effusion, and serial examinations on the following days did not reveal any recurrence of the effusion.

[Thermographic monitoring of the evolution and therapy of chronic synovitis and cysts in hemophiliacs].

The authors have studied the evolution of chronic synovitis and of cysts in hemophilic patients, through measurement of the articular circumference or of the cyst. To obtain this, they have used a folding rule and liquid cristal strips, that have the characteristic, that they change colours from braunish to orange, from greenish to bluish according to the thermic gradient revealed. The authors have studied 21 hemophilic patients, affected from chronic synovitis for a total of 23 articulations and 5 patients with cyst. All patients have undergone several controls reporting dates revealed at 2,6…

Notopterol attenuates synovitis via α7nAChR-dependent metabolic reprogramming of macrophage polarisation.

Synovitis, a characteristic feature of inflammatory arthritis (IA), is often driven by an aberrant macrophage-mediated inflammatory response that promotes irreversible joint damage. The α7 nicotinic acetylcholine receptor (α7nAChR) is a key regulator of the cholinergic anti-inflammatory pathway. Despite its therapeutic potential, how α7nAChR coordinates macrophage metabolic reprogramming to resolve synovitis remains underexplored. This study aims to elucidate the therapeutic effects of Notopterol, a bioactive constituent derived from the rhizome of Notopterygium incisum Ting ex H.T. Chang, a t…

Primary Remitting Seronegative Symmetrical Synovitis With Pitting Oedema: A Case Report.

Remitting seronegative symmetrical synovitis with pitting oedema (RS3PE) syndrome is a rare disease that typically presents in older men and is characterised by bilateral and symmetrical peripheral arthritis, pitting oedema in the dorsal hands and feet, and seronegativity for rheumatoid factor. A notable feature of this syndrome is its excellent response to systemic corticosteroid therapy. This article presents a case of a 75-year-old White male patient recently diagnosed with bilateral carpal tunnel syndrome. He reported complaints of distal and symmetrical inflammatory polyarthralgia for thr…

Pulsed Electromagnetic Field Inhibits Synovitis via Enhancing the Efferocytosis of Macrophages.

Synovitis plays an important role in the pathogenesis of arthritis, which is closely related to the joint swell and pain of patients. The purpose of this study was to investigate the anti-inflammatory effects of pulsed electromagnetic fields (PEMF) on synovitis and its underlying mechanisms. Destabilization of the medial meniscus (DMM) model and air pouch inflammation model were established to induce synovitis in C57BL/6 mice. The mice were then treated by PEMF (pulse waveform, 1.5 mT, 75 Hz, 10% duty cycle). The synovitis scores as well as the levels of IL-1β and TNF-α suggested that PEMF red…

Remitting seronegative symmetrical synovitis with pitting edema syndrome in maintenance hemodialysis.

INTRODUCTION: The remitting seronegative symmetrical synovitis with pitting edema syndrome primarily occurs in elderly individuals to represent symptoms of edema, pain, and joint swelling. It could be misdiagnosed in elderly maintenance hemodialysis patients, as hemodialysis patients often present with pain and joint swelling induced by hypervolemia, inflammation, amyloidosis, and/or chronic kidney disease. Here, we describe a maintenance hemodialysis patient with remitting seronegative symmetrical synovitis with pitting edema syndrome. CASE PRESENTATION: A 71-year-old man on maintenance hemo…

Spondylitis-psoriasis-enthesitis-enterocolitis-dactylitis-uveitis-peripheral synovitis (SPEED-UP) treatment.

Axial spondyloarthritis (axSpA), psoriatic arthritis (PsA), psoriasis, inflammatory bowel disease (IBD), and noninfectious uveitis form a distinct group among the immune mediated inflammatory diseases. Thus, many patients suffer from more than one of these disease manifestations. Here, we will use the term spondylitis-psoriasis-enthesitis-enterocolitis-dactylitis-uveitis-peripheral synovitis (SPEED-UP) spectrum disease. The aim is to review the new targeted pharmacological treatment options for all these diseases. All biological or targeted synthetic drugs with U.S. Food and Drug Administratio…

Can optical spectral transmission assess ultrasound-detected synovitis in hand osteoarthritis?

OBJECTIVE: To determine whether optical spectral transmission (OST) can be used to assess synovitis in hand and wrist joints of patients with hand osteoarthritis (OA). DESIGN: Hand and wrist joints of 47 primary hand OA patients with at least one clinically inflamed hand or wrist joint were assessed for synovitis by OST and ultrasound (US). Associations between standardized OST and US synovitis were studied in linear mixed effects models, across all joint types together and individually for wrist, proximal interphalangeal (PIP), and distal interphalangeal (DIP) joints, and were adjusted for O…

Pigmented villonodular synovitis: Four pediatric cases and brief review of literature.

Pigmented villonodular synovitis (PVNS) is a rare benign disorder of a joint which affects its synovium, tendon sheaths and bursas. While most cases of PVNS are adult patients aged between 20-50 years, few afflicted children have also been reported. Clinical signs of PVNS are insidious and non-specific. Pain, swelling and stiffness are the major symptoms. Magnetic Resonance Imaging (MRI) is the best radiological method for diagnosis of PVNS, as the initial X-ray is normal in early phase of disease in most cases. Therefore, diagnosis is often delayed or confused with mechanical disorders, haemo…

Pigmented Villonodular Synovitis - Various Manifestations, Inconsistent Terminology and Treatment. Cases Study.

Pigmented villonodular synovitis (PVNS) as well as tenosynovial giant cell tumor (TGCT) are both diagnosed and described in two forms, namely a localized and diffuse type. They form a diverse group of proliferative lesions in articular synovium and tendon sheaths. The inconsistent terminology impedes communication between patients, orthopedic surgeons, radiologists and pathologists, leading to misunderstandings and delaying treatment. In this paper, we present three cases of PVNS/TGCT (involving the fourth toe extensor tendon sheath, hip joint after resurfacing and flexor hallucis longus tendo…

MicroRNA-29a Counteracts Synovitis in Knee Osteoarthritis Pathogenesis by Targeting VEGF.

Synovitis contributes to the development of osteoarthritis (OA) of the knee. MicroRNAs regulate joint microenvironment homeostasis and deterioration. This study was undertaken to characterize the actions of microRNA-29a (miR-29a) to synovial remodeling in OA joints. Synovial specimens isolated from patients with end-stage OA knees showed abundant fibrotic matrix and vessel histopathology concomitant with weak miR-29a expression. In vitro, miR-29a knockdown caused synovial fibroblasts to exhibit high expressions of collagen III, TGF-β1, MMP9, MMP13, and ADAMTS5, whereas miR-29a overexpression d…
