Imaging of the Pes Cavus Deformity.

Direct-type cavus foot deformities are most commonly encountered and are primarily sagittal plane deformities. Direct deformities should be delineated from rarer triplane pes cavovarus deformities. The lateral weight-bearing radiograph is the cornerstone of imaging evaluation of direct pes cavus foot deformity. The apex of Meary talo-first metatarsal angle on the lateral radiograph represents the pinnacle of the cavus deformity and assists in subclassification of the deformity. With routine application, ancillary radiographic imaging techniques, such as the modified Saltzman view or the modifi

Clubfoot.

Clubfoot or talipes equinovarus deformity is one of the most common anomalies affecting the lower extremities. This review provides an update on the outcomes of various treatment options used to correct clubfoot. The ultimate goal in the treatment of clubfoot is to obtain a fully functional and pain-free foot and maintain a long-term correction. The Ponseti method is now considered the gold standard of treatment for primary clubfoot. Relapse is common after primary treatment with the Ponseti method, and other interventions are discussed that are used to provide for long-term successful outcome…

Pedal manifestations of musculoskeletal disease in children.

Musculoskeletal disease presenting in a child's foot is usually an isolated mechanical problem rather than part of a systemic disease; however, the pediatric flatfoot may be a more serious problem, caused by tarsal coalitions, congenital vertical talus, or part of a syndrome. The most common congenital pediatric foot deformities, including flatfoot, talipes equinovarus, digital anomalies, and osteochondritis, have characteristic and easily identifiable features that should be familiar to the practitioner. In this article, these deformities are described as they occur in isolation and as the re…

Review of Elmslie's triple arthrodesis for post-polio pes calcaneovalgus deformity.

Twelve patients with residual talipes calcaneovalgus deformity due to poliomyelitis were reviewed. A two-stage Elmslie's arthrodesis was performed on nine patients with rigid deformity. Tendon transfer (peroneus brevis in one, peroneus brevis and longus in two) to the tendo Achillis insertion is used to correct the calcaneus deformity, after reduction of the calcaneus in the subtalar joint, using a Steinmann pin passed through the calcaneus. Good results were obtained in patients treated by arthrodesis. However, the deformity recurred in patients treated only with tendon transfer.

[Surgical treatment of foot deformities].

The authors analyse their many-year experience gained in the treatment of patients with foot deformities of various origins, carried out at the traumatology clinic of the Kuibyshev D. I. Ulyanov Medical Institute. New surgical techniques for the treatment of congenital talipes, valgus deformity of the first thumb combined with transverse platypodia, paralytic loose, paralytic pes planus and calcaneus are described. The causes of valgus deformities in the patients with infantile cerebral paralysis after achillotomy are discussed on the basis of EMG data. The described methods are superior to th…

Low back pain in persons with congenital club foot.

Ninety-three persons with congenital talipes equinovarus were investigated in order to analyse the frequency of LBP and to relate these findings to the deformity of the foot. Eight-eight per cent of the feet had some residual deformity. One third of the individuals had a difference in the length of the lower extremities and one fifth did walk with a limp. The frequency of LBP and LBP resulting in sick compensation was not increased in this group in comparison with the whole population in the same age group. There was no over-representation of LBP in the groups with residual deformity, with lim…

Calcaneo-valgus deformity.

A discussion of the essential deformity in calcaneo-valgus feet develops a theme originally put forward in 1961 on the relapsed club foot (Evans 1961). Whereas in the normal foot the medial and lateral columns are about equal in length, in talipes equino-varus the lateral column is longer and in calcaneo-valgus shorter than the medial column. The suggestion is that in the treatment of both deformities the length of the columns be made equal. A method is described of treating calcaneo-valgus deformity by inserting cortical bone grafts taken from the tibia to elongate the anterior end of the cal…

Principles of Triple Arthrodesis and Limited Arthrodesis in the Cavus Foot.

Cavus foot is a complex podiatric deformity that requires precise and in-depth work-up through an objective, physical, and radiographic examination. The goal of surgical treatment is to eliminate pain while establishing a plantigrade foot structure. Triple arthrodesis has proven to be an effective surgical procedure for treatment of moderate to severe rearfoot deformity with or without the presence of rearfoot arthritic changes. The foot and ankle surgeon must always be aware that no two cavus deformity cases are alike, therefore one may require additional surgical procedures including soft ti

A case of Kabuki (Niikawa-Kuroki) syndrome associated with manifestations resembling C-trigonocephaly syndrome.

We report a 2-year-old boy with overlapping manifestations of the Kabuki (Niikawa-Kuroki) and Opitz trigonocephaly syndromes. The proband was initially diagnosed with Opitz trigonocephaly syndrome on the basis of metopic craniosynostosis, upslanting palpebral fissures, high arched palate, redundant skin folds at the back of the neck, ventricular septal defect, broad flat thumbs, bilateral single palmar creases, left talipes equinovarus, eventration of right hemidiaphragm, and bilateral inguinal hernia. However, as he grew older, his facial profile changed to that typical of Kabuki syndrome.

Gordon syndrome: literature review and a report of two cases.

The aim of this article is to publish a literature review and report on two new cases of Gordon syndrome (GS), a rare syndrome documented to have an autosomal dominant inheritance pattern or to occur sporadically; it is characterized by camptodactyly, cleft palate, and talipes equinovarus. We report two exceptional cases of GS where both patients were also diagnosed with congenital myopathy, and one developed malignant hyperthermia. These are the first two cases reported where patients were diagnosed with both GS and congenital myopathy or where GS is associated with malignant hyperthermia.

Trismus-pseudocamptodctyly syndrome: report of one case.

Trismus-pseudocamptodactyly syndrome is a rare neuromuscular disorder that manifests as a combination of hand, foot, and mouth anomalies. Limited range of motion for hands, feet, and mouth are typical presentations. A male term newborn baby was noted to have trismus, ulnar deviation of both wrist joints, limited stretch of digits during hand dorsiflexion but with full digital extension during volarflexion, and talipes equinovarus of the right foot. These anomalies were not seen in his parents. Hecht syndrome, or trismus-pseudocamptodactyly syndrome, was diagnosed with a negative result of elec…

Congenital musculoskeletal malformations in neonates.

A total of 13321 consecutive live births were studied for the incidence of congenital musculoskeletal malformations (CMSM) in neonates in relation to mother's age, parity, social class, sex and birth weight of the child. In all, 64 CMSM were observed in 54 neonates (4.054 per thousand live births). Congenital talipes equinovarus was the commonest CMSM while polydactyly and arthrogryposis were the next common CMSM in order of frequency. CMSM were more commonly seen in lower socio-economic class and in low birth weight babies.

An Approach to the Management of Severe Clubfoot Deformities on Global Humanitarian Programs: The Role of Talectomy.

The approach to treatment of severe untreated or recurrent congenital talipes equinovarus deformities is very different in the world where patients are mobile, have access to repeated return visits for follow-up treatment, and where more sophisticated options for gradual correction with external fixation are available. For treatment, talectomy may be the only option to treat certain neglected clubfoot deformities during humanitarian programs and it may still have to be used as a salvage procedure used in modern foot surgery. Our extensive experience with these deformities has been on global hu…

Correlation between interdental occlusal plane and plantar arches. An EMG study.

The Authors carried out an experimental study on a homogeneous group of young people to provide evidence of functional correlation among masticatory muscles and, indirectly, between changes to the interdental occlusal plane and modifications of the plantar arches due to talipes valgus and flat foot. In the two analysed conditions, the masticatory muscles undergo different functional alterations. This is due to the fact that the mechanoreceptors in the tendons of the muscles governing the plantar arch configuration are stimulated in different ways during the activation of long osteoarthromuscul…

Foot and ankle deformities in arthrogryposis multiplex congenita.

When 51 patients with arthrogryposis multiplex congenita followed an average of 12 years were reviewed, talipes equinovarus was the most common foot and ankle deformity. The best results were obtained by talectomy. The majority of tendo Achillis lengthenings, posteromedial releases, and triple arthrodeses were unsuccessful and were associated with a high complication rate. Congenital convex pes valgus was the second most common deformity. Surgical treatment by open reduction generally resulted in unsuccessful outcomes. Ambulation was achieved in 46 patients. Residual foot deformity was the pri…
