The study was performed on cerebral tissue resected during temporal lobectomy in 16 patients whose long-standing cryptogenic epilepsy did not submit to anticonvulsive drugs. Cases presenting definite etiological factors such as CNS trauma, infection or neoplasm were excluded. Neuropathological investigations disclosed microangiomas and focal vascular malformations in the meninges and tissue in 7 patients. Neuronal heterotopias in the white matter and of the white matter in the cortex were observed in 3 cases. Main cortical changes were: neuronal loss, chronic neuronal degeneration, perineurona

We examined the clinical significance of bisynchronous spike-wave complexes which appear in EEGs of some patients with focal spikes in one or both temporal lobes. Few features distinguished patients with bisynchronous spike-waves from those with only focal temporal spikes: a higher incidence of patients with more than two grand mal seizures a year (43 vs. 7%) and a younger age at last recording (22.5 vs. 28.3 years) were the only statistically reliable factors. Proportion of patients with complex partial seizures (CPS) (100%), age of CPS onset, incidence of febrile convulsions, intellectual le

Experiential phenomena occurring in spontaneous seizures or evoked by brain stimulation were reported by 18 of 29 patients with medically intractable temporal lobe epilepsy who were investigated with chronic, stereotaxically implanted intracerebral electrodes. The phenomena mainly consisted of perceptual (visual or auditory) hallucinations or illusions, memory flashbacks, illusions of familiarity, forced thinking, or emotions. Experiential phenomena did not occur unless a seizure discharge or electrical stimulation involved limbic structures. For such phenomena to occur, seizure discharge or e

Fourteen patients with temporal lobe epilepsy (TLE) and 14 patients with primary generalized epilepsy (GE) were compared on a self-report questionnaire (Personal Inventory) which assesses 18 personality and behavioral traits hypothesized to characterize persons with TLE. Four traits (sense of personal destiny, dependence, paranoia, philosophical interest) were significantly elevated in the TLE group, thus lending some support to the notion of changes in behavior and thought which occur in TLE but not necessarily in other forms of epilepsy.

We studied a patient with hippocampal epilepsy who had frequent nocturnal and diurnal seizures. Depth electrode recording showed that focal seizure discharges in the right hippocampus were of shorter duration in REM and non-REM sleep. However, awakening, especially from REM sleep or shortly after a REM period, facilitated the occurrence of a generalized seizure. There was no ultradian fluctuation in frequency or duration of seizure during diurnal recording. Night terrors (but not nightmares) disappeared after right temporal lobectomy, suggesting that pavor nocturnus was an ictal manifestation

INTRODUCTION: Temporal lobe epilepsy is a form of focal epilepsy that originates in the temporal lobes, often presenting with a variety of symptoms including altered consciousness, automatisms, and focal seizures with or without impaired awareness. Given such a diversity of manifesting symptoms, recognizing temporal lobe epilepsy in the emergency department (ED) can be challenging. Early identification is crucial for appropriate management, including timely initiation of antiepileptic therapy and differentiation from other neurological emergencies.

The anterior temporal lobe (ATL) is suggested as a semantic hub that may support reading inconsistent words via semantic access. Surface alexia is characterized by difficulty in reading these inconsistent words, and often co-occurs with ATL atrophy and semantic impairments. However, the role of ATL in word reading is unclear, as evidenced by cases of alexia without semantic impairments and semantic impairments without surface alexia. To test its role in reading, we stimulated the ATL using transcranial Direct Current Stimulation in neurotypical participants performing a Word Naming task includ

OBJECTIVE: Memory impairment is common in people with temporal lobe epilepsy (TLE). Recent studies in healthy subjects showed a positive correlation between sleep spindles coupled to slow waves (SWs) and memory performance. We aimed to determine differences in spindle-SW coupling in TLE patients compared to healthy controls using combined high-density electroencephalography and polysomnography.
METHODS: The study population consisted of 20 patients (12 female, 36.5 ± 9.9 years old) with unilateral drug-resistant TLE (10 left temporal) and 20 age- and sex-matched controls (12 female, 31.2 ± 6.

BackgroundAlzheimer's disease (AD) is a complex neurodegenerative disorder with intricate pathophysiological mechanisms. Transcriptome analysis has been used to investigate the pathogenesis of AD from the perspectives of mRNA expression, alternative splicing, and alternative polyadenylation. However, these 3 transcriptomic regulatory layers have not been comprehensively explored, limiting our understanding of the transcriptomic landscapes of AD pathogenesis.ObjectiveWe aimed to describe the transcriptomic landscapes of AD pathogenesis, detect the contributions of different regulatory layers to

BACKGROUND: There is growing interest in using Hadamard Encoding and Reconstruction for MEGA-Edited Spectroscopy (HERMES) within the mesial temporal lobe (MTL). For cross-sectional group comparisons and longitudinal repeated measures designs, an understanding of the internal and test-retest validity of γ-aminobutyric acid (GABA+) and glutathione (GSH) is critical. We therefore evaluated the reproducibility of the consensus recommended semi-localization by adiabatic selective refocusing (sLASER) localization for edited-MRS acquisitions in a challenging region, the MTL.

Noncoding RNAs are known to be importantly involved in a variety physiological and pathophysiolgical processes. Their role in the pathogenesis of hypertrophic scars remains unclear, however. After preliminary screening of the microRNA (miRNA) gene expression profiles, we explored the role of miR-495 in the development of hypertrophic scar by comparing expression of miR-495 and focal adhesion kinase (FAK) between hypertrophic scar and normal skin tissue. We also used 3-(4,5-dimethyl-2-thiazolyl)-2,5-diphenyl-2-H-tetrazolium bromide and annexin V-fluorescein isothiocyanate/propidium iodide assay

The human medial temporal lobe (MTL) includes a number of cortical regions surrounding the hippocampus implicated in long-term memory and high-level perception, including temporopolar cortex (TPC), entorhinal cortex (ERC), perirhinal cortex (PRC), and parahippocampal cortex (PHC). A critical prerequisite to studying the function of these areas is to define their anatomical boundaries. While several atlases or segmentations methods for automated boundary definition exist, they have limitations: 1) they are derived from small sample sizes; 2) most do not include TPC; and 3) they are volumetric r

Quantifiers like 'all' or 'some' play a key role in how we construct meaning from language, but their underlying neurobiological mechanisms are still unclear. One difficulty in determining the neural correlates of quantification is that the interpretation of these elements depends on multiple elements of a sentence, unlike other determiners, such as 'the.' This makes it difficult to isolate neural signals that are specific to the logical properties of quantifiers but orthogonal to the intricacies of predictive processing. In this magnetoencephalography study, we overcame this challenge by empl

BACKGROUND: Temporal lobe epilepsy (TLE) predisposes individuals to cognitive difficulties and psychosocial consequences. Evaluating its impact on quality of life (QOL) is essential for patient care.
OBJECTIVE: To identify factors influencing QOL in low-income patients with TLE.
METHODS: An institution-based cross-sectional study was conducted on 40 patients with TLE during neurological consultations at a day clinic in Agadir, Morocco. The Quality of Life in Epilepsy Inventory-31 (QOLIE-31) was used to measure QOL. Multivariate linear regression analysis was performed to assess the associati

Temporal lobe epilepsy (TLE) is one of the most common drug-refractory epilepsies. However, the diagnosis and treatment of TLE may be improved by better understanding its complex network. In this article, the authors summarize their experience with TLE and discuss their process for using stereo-electroencephalography (SEEG) as part of presurgical evaluation in the past 10 years. The authors demonstrate the value of SEEG in different types of TLE and discuss how their findings have impacted treatment options. Ultimately, the authors' experience will help other centers in addressing TLE cases.

Recent work has focussed on how patterns of functional change within the temporal lobe relate to whole-brain dimensions of intrinsic connectivity variation (Margulies et al., 2016). We examined two such 'connectivity gradients' reflecting the separation of (i) unimodal versus heteromodal and (ii) visual versus auditory-motor cortex, examining visually presented verbal associative and feature judgments, plus picture-based context and emotion generation. Functional responses along the first dimension sometimes showed graded change between modality-tuned and heteromodal cortex (in the verbal matc

Combining information from multiple senses is essential to object recognition, core to the ability to learn concepts, make new inferences, and generalize across distinct entities. Yet how the mind combines sensory input into coherent crossmodal representations - the crossmodal binding problem - remains poorly understood. Here, we applied multi-echo fMRI across a 4-day paradigm, in which participants learned three-dimensional crossmodal representations created from well-characterized unimodal visual shape and sound features. Our novel paradigm decoupled the learned crossmodal object representat
