A 37-year-old man, amateur rugby player sustained a hyperextension injury to his lower thoracic spine during a scrum collapse. The patient developed extreme hyperpathia in the T10-12 dermatome, and parasthesia from T12 to S1 in the left lower limb. Medical Research Council grade 5 power was regained rapidly within minutes of the accident, and the hyperpathia resolved within a week. MRI showed contusion of the spinal cord at T10 level but no associated osseoligamentous injury. Six months later, parasthesia and subjective weakness remained in the left lower limb. To our knowledge, this is the fi

BACKGROUND: Recent advancements in the diagnosis and management of blunt thoracic aortic injury have improved outcomes after operative repair. With consideration of these advancements, we analyzed our level 1 trauma experience with blunt traumatic aortic injury.
METHODS: Between January 1997 and March 2009, 255 patients with blunt traumatic aortic injury were entered into the Utah Trauma Registry-Trauma Center Registry. Of these, 145 (58%) patients underwent thoracic aortic repair, with 12 (5%) undergoing aortic exploration without repair. Median age was 32 years (range, 13 to 87), with 43 (3

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) is accepted treatment for blunt aortic injury (BAI). We hypothesized that immediate TEVAR would reduce deaths from aortic rupture in patients with BAI.
METHODS: Review of 81 patients with BAI who arrived alive at a level I trauma center over a 10-year period.
RESULTS: Twenty-three patients (28%) died within 4 hours of admission, including 12 who died of aortic rupture. Fifty-eight patients (72%) survived beyond 4 hours, and 8 (14%) ultimately died of associated injuries. Forty patients (69%) underwent aortic repair (30 open repair, 10 T

The case of an upper oesophageal perforation as a concomitant injury of an isolated fracture of the upper thoracic spine without neurological compromise has not been described so far. A Case report and review of the literature is presented here. Concomitant oesophageal perforations carry a high risk of being missed initially. CT alone can visualize the subtle indirect signs like peri-oesophageal air. The literature revealed that only peri-oesophageal air might be a valid indicator of oesophageal injury. There are no systematic data on thoracic spine fractures with concomitant oesophageal perfo

Perioperative airway management in trauma victims presenting with penetrating thoracic spine injury poses a major challenge to the anesthesiologist. To avoid further neurological impairment it is essential to ensure maximal cervical and thoracic spine stability at the time of airway manipulation (e.g., direct laryngoscopy and endotracheal intubation). Airway management in the prone position additionally increases the incidence of cervical/thoracic spine injury, difficult ventilation, and difficult airway instrumentation. Although awake fiberoptic intubation of the trachea is considered the gol

BACKGROUND: Thoraco-abdominal-aneurysm surgery predicts high mortality. Propofol and sevoflurane are commonly used anaesthetics for this procedure. Halogenated anaesthetics induce organ protection similar to ischaemic preconditioning. We investigated which anaesthetic regimen would lead to a better protection against ischaemia-reperfusion injury induced by temporary thoracic-aortic occlusion.
METHODS: Following initial fentanyl-midazolam anaesthesia for surgical preparation, 18 pigs were randomly assigned to two groups: group one received propofol (n=9) and group two sevoflurane (n=9) before,

BACKGROUND: Traumatic injury of the thoracic aorta is a life-threatening complication in patients who sustain deceleration or crush injuries. The magnitude of force necessary to cause blunt thoracic aortic injury results in a high proportion of concomitant injuries, posing a significant challenge for prioritizing management. Open surgical mortality is increased in the presence of coexisting head, lung, and abdominal injuries. Spinal cord ischemia may occur following aortic cross-clamping and operative hypotension. Endovascular stent-graft placement offers a safe, effective, and timely treatmen

Traumatic rupture of the thoracic aorta is a common cause of death after vehicle collisions. Associated injuries are common, and patients with lung injury, cardiac contusion, abdominal bleeding, and head injury comprise a group at high risk for conventional surgical or medical therapy. In this particular population, existing commercially available stent-grafts may provide a life-saving repair option. The Ancure and AneuRx stent-grafts, designed for abdominal aortic aneurysm application, were successfully placed in three patients. Accommodation for the short length of the delivery device was ac

Ultrasound (US)-guided cannulation of the internal jugular vein (IJV) has become the preferred approach for venous access as a result of its higher success rate and lower incidence of complications. This report describes a case of thoracic duct injury during US-guided left IJV catheterization. The normal and variant anatomy of the thoracic duct in the neck is illustrated.

Acute burn injury is usually associated with pain in the injured and nearby areas. However, we have recently reported that a thoracic scald induces hindpaw hyperalgesia during the healing stage in rats. The present study investigated the cause of the remotely occurring hyperalgesia. Behavioral testing using the von Frey test revealed that rats developed hyperalgesia in the neck and flank as well as the hindpaw 2-3 weeks after injury. The concentration of nerve growth factor (NGF) in the skin of the chest increased markedly during the healing stage. Moreover, rats injected daily with anti-NGF s

BACKGROUND: The study was concerned with the secondary functional compromise persons with long-term paraplegia contend with, that has been shown to be associated with wheelchair use. Of particular interest was the relationship between the level of thoracic spinal cord injury (SCI) and factors predisposing to rotator cuff disorders (RCD), one of the most common upper extremity musculoskeletal complications linked with long-term SCI. Disorders of these peri-articular structures have previously been associated with impaired trunk postural control and abnormal muscle strength ratios in both able-b

OBJECTIVE: The objective was to determine the course of the long thoracic nerve relative to the scapula as an aid to the prevention of proximal long thoracic nerve injuries.
METHODS: Eighteen fresh cadavers (7 male, 11 female) were studied. Each was sequentially placed in the transaxillary and posterolateral thoracotomy positions, and the distance of the long thoracic nerve from the scapular tip and anterior scapular border was measured. The measurements were made bilaterally; the mean, standard deviation, and 99% confidence interval were calculated for each position by gender.
RESULTS: Dist

To characterize pleural healing, we quantitated leukocyte accumulation in the pleural cavity and histological changes after two types of thoracic surgery. Rabbits underwent intercostal thoracotomy followed by abrasion of the parietal pleura and ligation and resection of the right middle lobe of the lung (group A), or only abrasion of the parietal pleura (group B). After surgery, the influx of leukocytes into the pleural exudate was characterized by an increase in the number of polymorphonuclear neutrophils (PMNs) followed by monocytes/macrophages. In group A, the total number of leukocytes rea

Penetrating thoracic injuries involving retained foreign bodies (RFBs) present a significant clinical challenge. While removal is generally indicated to prevent complications, invasive surgical approaches carry inherent risks. We present the case of a 60-year-old male who sustained an accidental nail gun injury to his left chest. Imaging revealed a 10 cm nail lodged in the lung, 3 mm from a branch of the pulmonary artery. Despite the proximity to a major vessel, the patient's haemodynamic stability permitted a conservative approach. Under local anaesthesia and mild sedation, a prophylactic che…

INTRODUCTION: Traumatic thoracic injuries constitute a major global health burden, associated with high mortality, severe complications, and complex care needs. The acute and unpredictable nature of trauma care often limits early patient involvement despite the importance of person-centred care for safety and recovery. Although nurses provide crucial technical and holistic support, organisational pressures may hinder psychosocial care.
AIM: This study aimed to explore how patients with traumatic thoracic injuries experience nursing care and participation in a trauma care unit.
METHODS: Sixte…

Aortobronchial fistula is a rare but life-threatening cause of hemoptysis. We report the case of a 68-year-old man who developed an aortobronchial fistula 38 years after undergoing open repair of a blunt thoracic aortic injury from a plane crash. Initial thoracic endovascular aortic repair was performed covering the area of the previous open repair (zones III/IV) where fistulas typically form; however, persistent symptoms required extension with subsequent resolution of hemoptysis. Six years later, a suspected endograft infection following bronchitis was managed with antibiotics and chronic su

PURPOSE: To explore differences based on level of pediatric spinal cord injury (SCI), we compared cervical and thoracic SCI in pediatric trauma patients (PTPs), hypothesizing higher mortality and length of stay (LOS) for cervical SCI.
METHODS: The 2017-2021 Trauma Quality Improvement Program was queried for all PTPs ≤ 17 years-old with cervical or thoracic SCI. Bivariate analyses compared the two groups. The primary outcome was mortality and secondary outcomes included hospital LOS and injury severity scores (ISS). Logistic regression models were used to determine independent risk factors for
