Extracorporeal Shock-Wave Therapy or Low-Level Laser Therapy: Which is More Effective in Bone Healing in Bisphosphonate Treatment?

OBJECTIVES: Bisphosphonate-related osteonecrosis of the jaw is potential side effect of long-term bisphosphonate therapy. Different treatment modalities have been used in this and investigate the effects of low-level laser therapy (LLLT) and extracorporeal shockwave therapy (ESWT) on socket healing after tooth extraction in rats given long-term bisphosphonates. METHODS: Forty male Wistar-Albino rats were used in this experimental study. About 0.1 mg/kg zoledronic acid was administered intraperitoneally to all animals 3 times per week for 8 weeks. Four groups were made which were control, LLLT

Postextraction infections, prevention, and treatment.

Postoperative infection is one of the most common complications of tooth extraction. It may manifest as localized infection or develop to systemic infection. Clinically, oral surgeons can prevent postoperative infections by urging patients to strengthen oral hygiene, applying antibiotics in a rational and compliant manner, and choosing appropriate surgical methods for tooth extraction. For the treatment of infection, the oral surgeon should formulate a response strategy on the basis of different diagnoses. For local infections such as dry socket, delayed alveolar osteitis, gap infection, and m

Immediate Implant Placement Using the Socket Shield Technique: Clinical, Radiographic, and Volumetric Results Using 3D Digital Techniques-A Case Series.

Extraction-site alveolar remodeling is a major concern due to negative volumetric hard and soft tissue changes that inevitably limit rehabilitation options and diminish esthetic outcomes. Surgical techniques employed to minimize alveolar dimensional changes are not always predictable. Utilizing a socket shield with an immediate surgical implant procedure helps maintain a thin portion of the root in the vestibular area and thus minimizes bone resorption, especially at the coronal cortical aspect. This case series assesses the dimensional changes in peri-alveolar structures via superimposition o

Case report: autotransplantation for a missing permanent maxillary incisor.

Patients with nonrestorable or missing anterior teeth are typically seen by their general or pediatric dentist who directs the course of consultation, referral, and treatment. In the mixed dentition stage, loss of permanent maxillary incisors is usually treated by various forms of removable/fixed prosthetic appliances. Because premolars are developing during this time period, transplantation of an available premolar to an incisor position is a viable alternative, that may provide a better biological substitute for a missing incisor than other choices. The purpose of this case report was to des

Digital-guided Autotransplantation of Teeth with Autogenous Bone Augmentation: A Case Report.

Long-term dental defects are often accompanied by severe alveolar bone resorption, which compromises the maintenance of alveolar bone height and thickness, thereby increasing the complexity of restoration. This case report presents a 24-year-old female patient with a 6-month dental defect and alveolar bone resorption. The treatment consisted of autotransplanting the left mandibular third molar to the defect site, with the use of a three-dimensional printed donor tooth replica and the surgical guide for precise alveolar socket preparation. Simultaneously, autogenous bone harvested intraoperativ

Open collagen membrane technique in socket preservation.

Both hard and soft tissue undergo change after tooth extraction. In particular, the bone tissue surrounding teeth with fenestration or dehiscence defects undergoes dramatic change following tooth extraction, which can compromise further rehabilitation of the area. Adequate alveolar bone volume and keratinized mucosa are critical to the success of implant therapy. Therefore, the anatomic dimension of the alveolar ridge must be adequate to achieve an esthetically acceptable outcome of implant therapy. Previous studies have proposed many clinical techniques for preserving the extraction socket. T

Techniques for Extraction Socket Regeneration for Alveolar Ridge Preservation.

BACKGROUND: Alveolar bone undergoes volumetric changes after extraction due to physiologic bone remodeling. The amount of alveolar bone available during prosthodontic treatment can affect the esthetic outcome of the treatment and make implant placement challenging. Socket preservation techniques are advocated postextraction to maintain the bone's vertical and horizontal alveolar bone dimensions and prevent its atrophy. AIM: This review is oriented toward a clinician, describing the different materials and techniques in practice today for socket preservation. REVIEW RESULTS: A variety of meth

A Clinical Classification System for the Treatment of Postextraction Sites.

A postextraction socket is always open to different treatment possibilities. A straightforward clinical classification may help evaluate which surgical approach is best suited for the case being treated. Four different classes are defined on the basis of the local anatomy of the site, available bone volume, and soft tissue level. For every clinical situation, either immediate placement, early placement, alveolar ridge preservation, or staged approach can be selected as a treatment modality according to the classifications listed.

What is the Optimum for Alveolar Ridge Preservation?

In the dental implant era, there has been growing interest in exploring the most effective methods to minimize morphologic alteration in the postextraction site. Despite modern methods of ridge preservation, resorption of the ridge is inevitable. The optimal approach to minimizing the rate and amount of these changes is still a subject of controversy. This article provides a contemporary review of the different approaches to preserve alveolar ridge dimensions. A suggested classification for single extraction sockets is presented along with multiple treatment options for each class.

The effect of biologic factors and adjunctive therapies on orthodontically induced inflammatory root resorption: a systematic review and meta-analysis.

OBJECTIVE: Biologic factors and adjunctive therapies are known to affect the degree of orthodontically induced inflammatory root resorption (OIIRR). However, a systematic overall assessment of their impact on OIIRR has not previously been reported. We, therefore, aimed to systematically assess effects on OIIRR of biologic factors and adjunctive therapies in human and animal subjects. METHODS: A comprehensive search strategy was performed for all major electronic databases. Two reviewers independently selected studies, undertook data extraction and assessed the risk of bias for all included st

Endoscopic removal of a maxillary third molar displaced into the maxillary sinus via the socket.

Removal of a maxillary third molar is a common dental procedure, and oral surgeons rarely experience accidental displacement of a maxillary impacted third molar into the maxillary sinus as a complication of this procedure. In the case in which such displacement occurs, the molar is removed via a transantral (Caldwell-Luc) approach through bone removal or window osteotomy of the anterior wall of the maxillary sinus. However, this approach is highly invasive because 2 surgical fields are required at the socket and the anterior wall of the maxillary sinus. Here, we report as a minimally invasive
