Curricular Interventions in Medical Schools: Maximizing Community Engagement Through Communities of Practice.

This article explains the importance of a communities of practice (CoP) model for continually aligning medical education and clinical transformation with contemporary health issues. It describes the evolution and advantages of using CoP as a model for transforming medical education and clinical practice and applies the CoP methodology to addressing the changing needs of socially vulnerable populations (LGBTQ [lesbian, gay, bisexual, transgender, and queer/questioning], persons experiencing homelessness, and migrant farm workers). In conclusion, this article describes CoP-led activities, achiev

Meningiomas in three male-to-female transgender subjects using oestrogens/progestogens and review of the literature.

Sex hormones have been proposed as a possible risk factor for the development and growth of meningiomas. Hormonal therapy plays a fundamental role in the treatment of male-to-female transgenders and needs to be continued after sex reassignment surgery. Usually, this treatment leads to no adverse events; however, its impact on hormone-related tumours such as meningiomas has not yet been investigated thoroughly. We searched our cohort of 2810 male-to-female transgender persons, who have been treated between 1975 and 2010, for patients with meningiomas. Additionally, we conducted a literature sea…

What's in a Guideline? Developing Collaborative and Sound Research Designs that Substantiate Best Practice Recommendations for Transgender Health Care.

Transgender medicine presents a particular challenge for the development of evidence-based guidelines, due to limitations in the available body of evidence as well as the exclusion of gender identity data from most public health surveillance activities. The guidelines that have been published are often based on expert opinion, small studies, and data gathered outside the US. The existence of guidelines, however, helps legitimate the need for gender-affirming medical and surgical interventions. Research conducted on transgender populations should be grounded in gender-affirming methodologies an

Intersectional Stigma and Implementation of HIV Prevention and Treatment Services for Adolescents Living with and at Risk for HIV: Opportunities for Improvement in the HIV Continuum in Sub-Saharan Africa.

Adolescents in sub-Saharan Africa, specifically adolescent girls and young women, young men who have sex with men, transgender persons, persons who use substances, and adolescents experiencing homelessness experience intersectional stigma, have a high incidence of HIV and are less likely to be engaged in HIV prevention and care. We conducted a thematic analysis informed by the Health and Discrimination Framework using a multiple case study design with five case studies in 3 sub-Saharan African countries. Our analysis found commonalities in adolescents' intersectional stigma experiences across …

Suicidality and self-harm among sexual minorities in Japan.

In this study, I used ethnographic methods to examine suicidality and nonsuicidal self-harm among gay, lesbian, bisexual, and transgender persons in Japan. Participants (N = 84) indicated that suicidality and self-harm are serious problems among sexual minorities and tend to be driven by (a) a homophobic/transphobic environment and the negative consequences of sexual minorities either disclosing their true selves or remaining hidden and silent within such an environment; (b) various antecedents to poor mental health; and (c) factors not directly related to being a sexual minority, particularly…

The health and wellbeing of incarcerated trans, gender diverse, and non-binary people: An international scoping review.

Globally there is a growing visibility of trans, gender diverse, and non-binary people in contact with prison systems, and growing questions raised by stakeholders about the health and wellbeing of these populations under state care. This scoping review aims to map the global evidence on the key health and wellbeing issues of incarcerated trans, gender diverse, and non-binary adults and young people to enhance researchers, community and policy makers' capacity to use international evidence to develop locally relevant policies and care models. The review follows Arksey and O'Malley's (2005) fiv

Masculinizing gender-affirming surgery for trans men and non-binary individuals: what you should know.

Gender dysphoria, the discordance between one's gender identity and anatomy, affects nearly 25 million people worldwide, and the prevalence of transgender and non-binary identities is increasing because of greater acceptance and awareness. Because of the improved accessibility to gender-affirming surgery (GAS), many providers will care for patients during and after gender transition. For trans men (female-to-male), GAS represents a combination of procedures rather than a single surgery. The particular combination of masculinizing procedures is chosen on the basis of informed patient-provider d

Experienced and Anticipated Intersectional Violence and Psychological Distress Symptom Severity Among Black Transgender Women in the United States of America.

BACKGROUND: Black transgender women experience disproportionately high rates of violent victimization rooted in intersecting systems of oppression, including cisgenderism and anti-Black racism. Although victimization is linked to psychological distress, the mental health impacts of intersectional violence, which targets overlapping marginalized identities, remain understudied. OBJECTIVES: To examine the associations between anticipated and experienced intersectional victimization and psychological distress among Black transgender women. METHODS: Online survey data from 151 Black transgender

Using Tenofovir Diphosphate Levels to Evaluate Factors Associated With PrEP Non-Adherence and Performance of Indirect Adherence Measures in Latin America: A Prospective, Single-Arm, Open-Label, Multicentre Implementation Study (ImPrEP).

INTRODUCTION: Tenofovir-based oral pre-exposure prophylaxis (PrEP) is pivotal for HIV prevention in Latin America. Monitoring PrEP adherence is crucial for maximizing its impact and identifying individuals who may benefit from additional support. We evaluated the performance of indirect adherence measures and factors associated with PrEP non-adherence in Brazil, Mexico and Peru. METHODS: ImPrEP was a prospective, multicentre, PrEP implementation study which enrolled 9509 persons from 6 February 2018 to 30 June 2021. For this analysis, we included men who have sex with men (MSM) aged 18-24 yea

Standards of Care for the Health of Transgender and Gender Diverse People, Version 8.

Background: Transgender healthcare is a rapidly evolving interdisciplinary field. In the last decade, there has been an unprecedented increase in the number and visibility of transgender and gender diverse (TGD) people seeking support and gender-affirming medical treatment in parallel with a significant rise in the scientific literature in this area. The World Professional Association for Transgender Health (WPATH) is an international, multidisciplinary, professional association whose mission is to promote evidence-based care, education, research, public policy, and respect in transgender heal

Talking About the Anti-Trans Sociopolitical Climate with Transgender and Gender Diverse Children and Their Caregivers.

BACKGROUND: Transgender and gender diverse (TGD) children and adolescents in the United States are currently experiencing an oppressive sociopolitical climate that is likely impacting their mental wellbeing. TGD children, defined as 5- to 10-year-olds, have the capacity to be aware of the anti-trans sociopolitical climate and, within the context of developmental readiness, TGD children and their caregivers may benefit from having conversations about the sociopolitical climate with a mental health provider. However, there are no clinical guidelines for how mental health providers may approach c

Gender minority stress, testosterone, and blood pressure responses to stress in transgender and nonbinary adults.

Trans and nonbinary adults assigned female at birth (TNBF) are at elevated cardiovascular risk, an effect proposed to occur secondary to testosterone use. However, gender minority (GM) stress may also contribute to cardiovascular risk. Therefore, we examined the relative association of GM stress and testosterone with blood pressure (BP) and its regulation in TNBF adults. In 24 TNBF adults (12 not using and 12 using testosterone), GM stress was derived from a standardized composite score of seven psychosocial scales, and total testosterone was measured via radioimmunoassay. BP was assessed at r

Chronic liver disease and hepatology care in transgender and gender diverse populations.

Transgender and gender diverse (trans) people experience unique minority stressors related to their gender identity, along with additional psychosocial and systemic stressors that contribute to health disparities. Many risk factors for chronic liver diseases, such as viral hepatitis, alcohol use disorder, and metabolic dysfunction, disproportionately affect trans people. There have also been historical concerns about the hepatic effects of gender-affirming hormone therapy (GAHT) that are predominantly based on data extrapolated from cisgender populations or hormone formulations that are no lon

[Ethical issues of gender reassignment surgery].

Gender reassignment raises complex ethical issues. It questions gender identity and the right to self-determination. Some highlight the right to access the medical care necessary to live according to one's perceived gender. Others worry about the potential rush into major medical decisions, especially among minors. The need for a medical diagnosis of gender dysphoria can be seen as pathologizing. The requirement for surgery for marital status change, while not mandatory in many places, remains controversial. The balance between informed consent and access to treatment, as well as stigma and di

The Population-Level Impact of Doxycycline Postexposure Prophylaxis on Syphilis in King County, WA: An Interrupted Time Series Analysis.

BACKGROUND: An initial evaluation found that doxycycline postexposure prophylaxis (doxy-PEP) has a population-level effect on syphilis among cisgender men and transgender women who have sex with men. Whether doxy-PEP's impact is generalizable across the United States and will affect syphilis incidence among cisgender women and pregnant people-populations who seldom receive doxy-PEP-and congenital syphilis (CS) is unknown. METHODS: We used public health surveillance data from January 2017 to June 2025 to conduct an interrupted time series analysis of monthly syphilis diagnoses (all stages) ove

Brain Health Outcomes in Sexual and Gender Minority Groups: Results From the All of Us Research Program.

BACKGROUND AND OBJECTIVES: Sexual and gender minority (SGM) groups have been historically underrepresented in neurologic research, and their brain health disparities are unknown. We aim to evaluate whether SGM persons are at higher risk of adverse brain health outcomes compared with cisgender straight (non-SGM) individuals. METHODS: We conducted a cross-sectional study in the All of Us Research Program, a US population-based study, including all participants with information on gender identity and sexual orientation. We used baseline questionnaires to identify sexual minority (lesbian, gay, b

Breast Cancer in Transgender Patients Following Medical and Surgical Intervention.

BACKGROUND: Current breast cancer (BCa) screening guidance does not adequately address transgender populations despite differences in hormone exposure and post-surgical anatomy. MATERIALS AND METHODS: Using the TriNetX network (2003-2023), transgender adults were identified using ICD-10 codes and grouped by sex assigned at birth and intervention: no intervention (NI), hormone therapy (HT ≥ 1 year), or HT with surgery (SX). BCa was defined as ≥ 2 diagnoses of malignant breast neoplasm (ICD-10 C50.x). RESULTS: Among 33,775 transgender women, 12 (0.036%) developed BCa. HT alone was not associat
