Causes and types of voluntarily reported unintentional medication discrepancies in care transitions: a cross-sectional study.

INTRODUCTION: Medication incident reporting systems enable healthcare professionals to report incidents and their analysis may help prevent reoccurrences. While previous research has primarily focused on medication errors in general, incidents involving unintentional medication discrepancies remain unexplored. AIM: Therefore, this study aimed to identify the causes of voluntarily reported medication incidents describing unintentional medication discrepancies occurring in care transitions. The secondary objective was to characterise the reported medication incidents regarding type of incident,

Pharmacist-led Transitions of Care Intervention to Address Infectious Disease Tests Pending at Discharge (TPAD).

BACKGROUND: Infectious disease test results frequently finalize after hospital discharge, and many are clinically actionable. Standardized workflows for reviewing these results remain limited. Although pharmacist-led culture follow-up is well established in emergency departments, evidence evaluating inpatient transitions-of-care models is sparse. This study assessed the impact of a pharmacist-led service on outpatient antimicrobial appropriateness and clinical outcomes. METHODS: We conducted a single-center, retrospective pre-post study at a community hospital comparing patients discharged wi

Hospital-to-Home Neurological Transition Care: A Scoping Review Across Selected Chronic Neurological Disorders.

BACKGROUND: Returning home after neurological hospitalization, rehabilitation, or specialist care transfers responsibility to patients, caregivers, and community services. We mapped mechanisms and gaps across dementia/Alzheimer's disease and related dementias (ADRD), Parkinson's disease (PD), multiple sclerosis (MS), and amyotrophic lateral sclerosis (ALS). METHODS: Following JBI guidance and PRISMA-ScR, eligibility was derived using population-concept-context. We included empirical reports involving adults with a target condition, a post-discharge, return-home, rehabilitation, telehealth, ca

Impact of Transitioning Elective Orthopedic Surgeries to the Ambulatory Setting on Acute-Care Hospital Operations and Quality.

Introduction Total joint arthroplasty (TJA) and spine surgeries are increasingly being performed in the ambulatory surgery center (ASC) setting. As high-volume elective surgeries shift out of the acute-care setting, the acuity and complexity of patients requiring hospital care have increased. The current study evaluates how the transition of TJA and spine procedures from the hospital to the ASC affected patient acuity and outcomes in a historically specialized hospital joint and spine unit (JSU). Methods A retrospective review of 12,067 patients receiving care at the JSU during calendar years

The "controlled flow" migration strategy and regimes of humanitarian care for people "in transit" through Darién, Panama.

Since 2021, over one million people have traveled on foot across the Darién border region separating Panama and Colombia en route to the U.S.-Mexico border. Extreme physical conditions of the route, crossing rivers and mountains through the Darién Forest, present lethal vulnerabilities to violence and impacts on migrant health. As migration across Darién has increased and diversified, complex medical emergencies, including migrant death, sexual violence, and injury, present critical areas for social service response. Since 2016, the Panamanian State has implemented the "controlled flow" migrat

Optimising Paediatric Transition to Intensive Care for Adults (OPTICAL): study protocol for a mixed method study.

INTRODUCTION: An increasing number of teenagers and young adults (TYA) with chronic conditions and complex needs are transitioning from paediatric to adult services, including admission to intensive care units (ICUs). As these services are often ill-equipped to care for TYA, there is a risk of compromised care. Despite recent guidelines from the UK Paediatric Critical Care and Intensive Care Societies highlighting the importance and urgency of improving ICU transition, current recommendations are not evidence-based and established pathways for ICU transition remain limited. METHODS AND ANALYS

Challenges of Care Transition From Hospital to Home for Older Colorectal Surgery Patients: Surgeons' Perspectives.

OBJECTIVE: Postdischarge transitions from the hospital to home in older (≥65 years) colorectal surgery patients have a high risk of medication errors, complications, and worsening of existing conditions. Up to 14% are readmitted within 30 days, costing ~$180 million annually. The anticipated 50% increase in colorectal cancer surgeries in older adults by 2040 necessitates an improvement in care transitions and outcomes. METHODS: We conducted semi-structured qualitative interviews with 10 surgeons from 8 US health systems to inform the design of a multicomponent care transition model. We select

Diagnostic value of cardiac magnetic resonance imaging during transition care in adolescents with Turner syndrome.

BACKGROUND: Turner Syndrome (TS) is a chromosomal disorder frequently associated with congenital cardiovascular abnormalities, particularly bicuspid aortic valve (BAV), coarctation of the aorta (CoA), and aortic dilatation. These conditions substantially increase the risk of aortic dissection. Although echocardiography (ECHO) is commonly used for cardiac monitoring, its limitations in evaluating aortic morphology require cardiac magnetic resonance imaging (CMR), as recommended by recent guidelines. CMR offers a comprehensive alternative, especially during the transition from pediatric to adult

The Journey to Adulthood: A Systematic Review of Interventions in Type 1 Diabetes Paediatric to Adult Transition Care.

Young people with type 1 diabetes mellitus (T1DM) transition from paediatric to adult services when they reach late adolescence. This can be a risky period for young people, and it has been associated with a deterioration in glycaemic control and disengagement from diabetes services. This review aimed to identify current interventions addressing the following questions: What adolescents with T1DM healthcare transition interventions have been evaluated? What are the underlying theories and components of these interventions? What outcomes have been considered in these evaluations? Databases, tri

Implementation of a transition of care pathway for low-risk patients presenting to the emergency department with venous thromboembolism.

Inpatient management of low-risk patients with venous thromboembolism (VTE) places a large resource burden on the healthcare system. Adult patients diagnosed with deep vein thrombosis (DVT) or pulmonary embolism (PE) in the emergency department (ED) have historically been hospitalized and treated with therapeutic anticoagulation. However, over the last two decades, outpatient treatment of patients with acute DVT and low risk PE has become increasingly accepted as an effective and safe option for patients given the low risk of short-term clinical deterioration. The purpose of this project was t

Feasibility and Acceptability of a Codesigned Health Care Transition Intervention for Young People With Spinal Cord Injuries.

BACKGROUND: Due in part to medical complications, adults with a pediatric onset spinal cord injury (SCI) are at higher risk of experiencing dissatisfaction with life and lower perceived physical health when compared to their peers with no disability. To support the prevention of medical complications, young people with SCI must successfully transition to adult health care. Health care transition (HCT) interventions can support young people with chronic conditions in their move to adult health care. OBJECTIVES: To evaluate the feasibility and acceptability of a web-based HCT intervention codes

Transition of Mental Health Care for Transgender Youth Aged Between 16 and 24 Years Old.

BACKGROUND: The transitional age (16-24 years old) is a clinically vulnerable period particularly important in mental health care. While several studies focus on this problem, few consider the specificity of the transgender population, a minority at risk of mental health issues. The purpose of this paper is to establish a review of the current literature addressing the transitional age and the transgender population in order to identify the issues met by this specific population in the transition of mental health care. METHODS: A systematic search of the English literature was conducted in Pu

Scenario-Based Evaluation of Team Health Information Technology to Support Pediatric Trauma Care Transitions.

BACKGROUND: Clinicians need health information technology (IT) that better supports their work. Currently, most health IT is designed to support individuals; however, more and more often, clinicians work in cross-functional teams. Trauma is one of the leading preventable causes of children's death. Trauma care by its very nature is team based but due to the emergent nature of trauma, critical clinical information is often missed in the transition of these patients from one service or unit to another. Teamwork transition technology can help support these transitions and minimize information los
