Endovascular embolization of high-grade cerebral dural arteriovenous fistulas - assessment of long-term recurrences.

INTRODUCTION: Endovascular therapy has shown to be safe and effective for the treatment of cerebral dural arteriovenous fistulas; however, recurrence after complete occlusion is not uncommon, and the timing of recurrence remains unknown. METHODS: A retrospective single-center cohort study was conducted from January 2005 to December 2020. Patients with high-grade (≥Borden II-Cognard IIB) dural arteriovenous fistulas treated with endovascular therapy were included in this study. Clinical and angiographic characteristics were collected for hospitalization and at follow-up. RESULTS: A total of 5

Dynamic Magnetic Resonance Angiography Provides Collateral Circulation and Hemodynamic Information in Acute Ischemic Stroke.

BACKGROUND AND PURPOSE: Contrary to usual static vascular imaging techniques, contrast-enhanced dynamic magnetic resonance angiography (dMRA) enables dynamic study of cerebral vessels. We evaluated dMRA ability to assess arterial occlusion, cerebral hemodynamics, and collateral circulation in acute ischemic stroke. METHODS: Twenty-five acute ischemic stroke patients with proximal anterior circulation occlusion underwent dMRA on a 3T scanner within 12 hours of symptoms onset. Diffusion weighted imaging, Tmax6 s lesion volumes and hypoperfusion intensity ratio as volume of Tmax>6 s/volume of Tm

An Unusual Case of Cerebral Venous Sinus Thrombosis With Negative D-dimer Mimicking Giant Cell Arteritis.

Cerebral venous sinus thrombosis (CVST) is a rare, potentially life-threatening condition that presents with variable symptoms such as headache, visual disturbance, and seizures, often mimicking an arterial stroke or intracranial lesion. Diagnosis can be challenging, and delays may result in severe outcomes. Although an elevated D-dimer may suggest CVST, a negative result does not exclude it. We report a rare case of extensive CVST initially treated as giant cell arteritis (GCA) due to unilateral temporal headache, elevated CRP, and a negative D-dimer. A 62-year-old man presented with acute le

Hemodynamic assessments of unilateral pulsatile tinnitus with jugular bulb wall dehiscence using 4D flow magnetic resonance imaging.

BACKGROUND: Pulsatile tinnitus (PT) is a type of tinnitus characterized by a rhythmic sound that is synchronous with the heartbeat. One of the possible causes of PT is the jugular bulb wall dehiscence (JBWD). However, the hemodynamics of this condition are not well understood. To elucidate this issue, the present study aimed to compare the blood flow of PT patients with JBWD, PT patients with sigmoid sinus wall dehiscence (SSWD), and volunteers. METHODS: A retrospective case-control study was conducted, which enrolled patients with unilateral PT who had undergone both computed tomography angi

Does measurement of the jugular foramen diameter on MRI help to differentiate transverse sinus thrombosis from unilateral transverse sinus hypoplasia?

BACKGROUND AND PURPOSE: The transverse sinus (TS) is a frequent location of cerebral venous thrombosis. However, unilateral TS hypoplasia is a frequent variation and radiological imaging pitfall in the diagnosis because it may mimic unilateral TS thrombosis. The purpose of this study is to find a cutoff value for bilateral jugular foramen (JF) diameter ratios on magnetic resonance imaging (MRI) for differentiating TS thrombosis from TS hypoplasia. MATERIALS AND METHODS: We retrospectively reviewed magnetic resonance venography results for 174 patients with reduced unilateral TS caliber result

[Yellow nail syndrome: two pediatric case reports].

BACKGROUND: Yellow nail syndrome (YNS) is a combination of specific nail dystrophy (yellowish or green nails, transverse ridging, increased curvature with "a hump" and distal onycholysis), lymphoedema and respiratory tract involvement (mainly bronchial hyper-responsiveness, recurrent pneumonia, bronchiectasis and pleural effusion), with or without sinusitis. This rare condition is most often seen in middle age. We report two pediatric cases. PATIENTS: A 9-year-old boy had 20 yellow nails and lymphedema of the lower and upper left limbs. He was born at 32 weeks of gestation. At birth, he had f

Image-guided surgical planning using anatomical landmarks in the retrosigmoid approach.

OBJECTIVE: The suboccipital lateral or retrosigmoid approach is the main neurosurgical approach to the cerebellopontine angle (CPA). It is mainly used in the treatment of CPA tumors and vascular decompression of cranial nerves. A prospective study using navigation registered with anatomical landmarks in order to identify the transverse and sigmoid sinuses junction (TSSJ) was carried out in a series of 30 retrosigmoid craniotomies. The goal of this study was to determine the accuracy of this navigation technique and to establish the relationship between the location of the asterion and the TSSJ

Imaging of head and neck tumors--methods: CT, spiral-CT, multislice-spiral-CT.

Spiral-CT is standard for imaging neck tumors. In correspondence with other groups we routinely use spiral-CT with thin slices (3 mm), a pitch of 1.3-1.5 and an overlapping reconstruction increment (2-3 mm). In patients with dental fillings a short additional spiral parallel to the corpus of the mandible reduces artifacts behind the dental arches and improves the diagnostic value of CT. For the assessment of the base of the skull, the orbital floor, the palate and paranasal sinuses an additional examination in the coronal plane is helpful. Secondary coronal reconstructions of axial scans are h

Development of acquired arteriovenous fistulas in rats due to venous hypertension.

Dural sinus thrombosis has been hypothesized as a possible cause of dural arteriovenous fistulas (AVF's). The pathogenesis and evolution from thrombosis to actual development of an AVF are still unknown. To study dural fistula formation, a surgically induced venous hypertension model in rats was created by producing an arteriovenous shunt between the carotid artery and the external jugular vein. The external jugular vein beyond the anastomosis was ligated 2 to 3 months after surgery and angiography was performed to identify any new acquired AVF's. Forty-six male Sprague-Dawley rats, each weigh

Migration of missile fragments into the heart via the cerebral venous sinuses: A report of 3 cases and a comprehensive review of the literature.

Vascular injury from projectiles can lead to the migration of missile fragments, resulting in arterial or venous missile embolism. In the context of penetrating craniocerebral trauma, this complication is exceedingly rare. This can only occur when a missile breaches a dural venous sinus or the jugular vein, leading to subsequent embolization into the right cardiac chambers or the pulmonary arteries. We present 2 cases of right ventricular missile embolism and 1 case of pulmonary artery missile embolism via dural sinuses and provide a comprehensive review of 7 additional cases from the literatu

Venous sinus stenting: where does it fit in the treatment paradigm for cerebrospinal fluid leaks?

OBJECTIVE: Spontaneous CSF leaks are traditionally managed with surgical repair. However, a growing body of evidence suggests that some of these leaks may result from idiopathic intracranial hypertension (IIH) and/or elevated intracranial pressure (ICP) due to underlying venous sinus stenosis-an association that remains underrecognized. Venous sinus stenting (VSS) may address CSF leaks by restoring normal venous outflow and reducing ICP. However, few series have evaluated VSS as a primary or adjunctive treatment for CSF leaks. Herein, the authors present a case series highlighting the role of

Brain parenchyma deformation and strain in patients with spontaneous intracranial hypotension.

BACKGROUND: Spontaneous intracranial hypotension (SIH) is mainly associated with orthostatic headache but can also cause cognitive impairment, with symptoms mimicking behavioral-type frontotemporal dementia in severe cases. SIH is caused by a spinal cerebrospinal fluid (CSF) leak and can lead to brain sagging. The specific brain regions affected by this motion, their deformation, and the magnitude of the associated mechanical strain have not been quantitatively characterized. We investigated whether brain MRI in patients with SIH shows quantifiable strain, expansion, and compression of brain s

Developmental venous anomaly associated with dural arteriovenous fistula: Etiopathogenesis and hemorrhagic risk.

INTRODUCTION: Developmental venous anomalies (DVAs) have traditionally been defined as non-pathological congenital lesions. Compared to isolated DVAs, the association of DVAs with arteriovenous shunts seems to have a more adverse clinical connotation. In this review, we describe the association between DVA and dAVF and discuss the hemorrhagic risk. We also advance a hypothesis about the potential de novo formation of a DVA and challenge the dogma about their "developmental" or "congenital" nature. METHODS: A systematic review of the literature on the association of DVA and dAVF was performed

A case series of closed head trauma with pituitary stalk disruption resulting in hypopituitarism.

INTRODUCTION: Traumatic brain injury (TBI) is one of the main causes of morbidity and mortality in young trauma patients with resultant multi-organ effects. Hypopituitarism following TBI can be debilitating and life threatening. TBI which causes hypopituitarism may be characterized by a single head injury, such as from a motor vehicle accident, or by chronic repetitive head trauma, as seen in combative supports including boxing, kick-boxing, and football. In the majority of cases, a diagnosis of hypopituitarism can be entirely missed resulting in severe neuro-endocrine dysfunction. We present
