Subscapularis tendon injuries in adolescents: a report of 2 cases.

BACKGROUND: Although shoulder injuries in adolescents are relatively common, injuries to the rotator cuff are relatively rare and not well characterized in the literature. We review 2 cases of adolescent subscapularis tendon avulsions treated surgically. METHODS: The 2 patients were high-level athletes, 1 is a baseball pitcher and the other is a boxer. Both were injured during participation in their respective sports. In each case, the diagnosis was made by magnetic resonance imaging. Patients underwent an open repair by a deltopectoral approach, using suture anchors for fixation. RESULTS: A

Re-evaluating the neck injury index (NII) using experimental PMHS tests.

OBJECTIVE: The neck injury index, NII, developed in ISO 13232 (2005) as a testing and evaluation procedure for assessing the risk of injury to the AO/C1/C2 region of the cervical spine in motorcycle riders is reevaluated using an existing postmortem human subjects (PMHS) data set and resulting in a reformulated NII criterion applicable to PMHS tests. METHODS: A recent series of 36 PMHS head/neck component tests was used to examine the risk of neck injury in frontal impacts and to assess the predictive capability of NII for impacts of various orientations. Using force and moment load cell PMHS

Differences between neurosurgeons and orthopedic surgeons in classifying cervical dislocation injuries and making assessment and treatment decisions: a multicenter reliability study.

Variability exists in the management of cervical spinal injuries. The goal of this study was to assess the effect of training specialty (orthopedic surgery vs neurosurgery) on management of cervical dislocations. Twenty-nine spine surgeons reviewed 10 cases of cervical dislocation injuries. For each of the 10 cases, the surgeons evaluated 3 clinical scenarios, which included a neurologically intact patient, a patient with an incomplete spinal cord injury (SCI), and a patient with complete SCI. Surgeons determined whether a unilateral or bilateral facet dislocation was present and whether pretr

Influence of a previous neck sprain on recovery after whiplash injury.

The impact of a previous neck sprain on recovery from whiplash injury is unknown as published studies have produced conflicting results. This article reviews the literature on the prognosis of a second whiplash injury, distinguishing between previous injuries with and without complete recovery. The best available evidence suggests that a previous injury with incomplete recovery represents an adverse prognostic indicator. However, where there has been complete recovery, the prior injury does not influence the prognosis.

What predicts injury from physical punishment? A test of the typologies of violence hypothesis.

OBJECTIVE: This study examined the power of child, perpetrator, and socio-economic characteristics to predict injury in cases of reported child physical abuse. The study was designed to assess the validity of the assumption that physically injurious incidents of child physical abuse are qualitatively different from those that do not result in injury, that their generative factors are distinctive, and that the quality of caregiving in these two types of incidents is different. METHOD: A weighted, nationally representative sample of 8,164 substantiated punishment abuse cases in Canada was used.

Validation of the ocular trauma score for intraocular foreign bodies in deadly weapon-related open-globe injuries.

BACKGROUND AND OBJECTIVE: To evaluate the prognostic value of the Ocular Trauma Score (OTS) in cases of deadly weapon-related open-globe injuries with intraocular foreign bodies. PATIENTS AND METHODS: A retrospective, interventional case series included 20 eyes of 20 patients who had deadly weapon-related open-globe injuries with intraocular foreign bodies. The OTS was calculated for each patient by adding the determined numbers of OTS variables at presentation (initial visual acuity, rupture, endophthalmitis, perforating injury, retinal detachment, and afferent pupillary defect). Patients we

Acute stroke in the burn patient.

A 3-year-old boy was brought to our hospital with 62% TBSA burn injury by boiled water. He received modern burn therapy, including early débridment and skin grafts. The patient discharged 70 days after being burned. Unfortunately, he sustained an acute right-sided hemiparesis 7 days after discharge. Magnetic resonance imaging revealed left middle cerebral artery territory infarction, and a magnetic resonance angiogram showed nonvisualization of left distal intra-cranial carotid artery and proximal middle cerebral artery. Thickened walls of these arteries indicated acute dissection. The patient

Reconstruction of crashes involving injured child occupants: the risk of serious injuries associated with sub-optimal restraint use may be reduced by better controlling occupant kinematics.

OBJECTIVE: To determine whether injuries to sub-optimally restrained child occupants in real-world crashes were likely to be preventable by alternative restraint usage practices and to assess the usefulness of crash reconstruction for exploring injury mechanisms in child occupants. METHODS: Real-world crashes in which child occupants sustained significant injuries were reconstructed on a laboratory crash sled using the Hybrid III family of child dummies. Alternative restraint scenarios and cases in which children were not seriously injured were also simulated to compare dummy kinematics and d

Rapid assessment of exposure to chlorine released from a train derailment and resulting health impact.

OBJECTIVES: After a train derailment released approximately 60 tons of chlorine from a ruptured tanker car, a multiagency team performed a rapid assessment of the health impact to determine morbidity caused by the chlorine and evaluate the effect of this mass-casualty event on health-care facilities. METHODS: A case was defined as death or illness related to chlorine exposure. Investigators gathered information on exposure, treatment received, and outcome through patient questionnaires and medical record review. An exposure severity rating was assigned to each patient based on description of

Expansive mortar-induced ocular injury.

We describe here a case of bilateral chemical injury (with an expansive mortar which is being used in recent times to cut the rocks). On examination limbal ischemia was more in the left eye (9 clock hours) than the right eye (2 clock hours). The case was managed by bilateral removal of foreign bodies, along with conjunctival resection and amniotic membrane transplantation in the left eye. At six-month follow-up, patient had best corrected visual acuity of 20/30 and 20/60 in the right and left eyes respectively. Since this being an occupational hazard, proper eye protection gear should be used

Thoracic bite trauma in dogs: a comparison of clinical and radiological parameters with surgical results.

OBJECTIVES: Canine bite wounds may cause severe underlying tissue trauma even with no clinically evident puncture wounds. In order to assess the ability of pre-operative diagnostic parameters to predict the extent of internal damage inflicted by a thoracic bite wound, the clinical, radiological and surgical data of 45 dogs that sustained thoracic bite trauma were recorded. METHODS: Clinical, radiographic and surgical parameters from 45 dogs of various breeds with thoracic bite trauma, were analysed (P<0.05). All dogs were treated according to a previously described protocol and had explorator

[Computed tomography in diagnosing and determining treatment policy in patients with posttraumatic pathology of the eye and orbit].

The paper analyzes the diagnostic value of computed tomography (CT) in posttraumatic lesion of the eye socket, ball, and appendage and identifies the basic groups of patients in whom CT is an essential study. In 2004, the Department of Traumatology, Reconstructive Surgery, and Eye Prosthesis, Helmholtz Research Institute of Eye Diseases, performed CT in 323 patients (18.2% of the inpatients and 3.4% of the outpatients). Analyzing all CT images identified the basic groups of patients in whom this study was essential. The CT scans characterizing examples from each group of patients are given. Th

Racial differences in severe perineal lacerations after vaginal delivery.

OBJECTIVE: The purpose of this study was to determine the relationship between maternal race and rates of third- and fourth-degree laceration after vaginal delivery. STUDY DESIGN: An electronic audit of the medical procedures database at Thomas Jefferson University Hospital from 1983 through 2000 was completed. Univariate and multivariable models were computed with the use of logistic regression models. RESULTS: From the database, 34,048 vaginal deliveries were identified, with 3487 deliveries resulting in third- or fourth-degree laceration (10.2%). Overall severe laceration rates by race wi

Pregnancy-associated assault hospitalizations.

OBJECTIVE: To determine the rate of pregnancy-associated hospitalized assaults in a multistate population and whether they have any increased assault risk versus nonpregnant counterparts. METHODS: An International Classification of Diseases, 9th Revision (ICD-9-CM) pregnancy and injury criterion was applied to 1997 statewide hospital discharge data from 19 states (52% of the female population age 15-49 years). Cases were classified as assault-related with and without pregnancy-associated diagnoses. Pregnancy-associated and all women were compared using demographic specific rate ratios and sev

[Evaluation of the management of blunt renal trauma and indication for surgery].

OBJECTIVES: Appropriate management of renal trauma is still controversial. Many of the patients have minor injuries and conservative treatment can achieve excellent outcomes without any complications. For major injuries of deep lacerations or ruptures, we have been performing early surgical treatment to salvage the kidney in the selected cases after the precise evaluation of the injury. To obtain the optimal management options, we evaluated the clinical results of our procedures. PATIENTS AND METHODS: We conducted a retrospective study, which included 106 cases of blunt renal trauma with evid

The 2002 Moyer Award. Metabolic effects of vitamin D on serum calcium, magnesium, and phosphorus in pediatric burn patients.

Severe burn injury results in profound metabolic derangements. Recently, we have shown that vitamin D metabolism is disturbed after burn injury. Vitamin D is essential for calcium and phosphorus homeostasis and skeletal bone integrity. The role of vitamin D on magnesium homeostasis is not well understood. The purpose of this study was to assess the effects of vitamin D deficiency on serum electrolytes. Forty-one pediatric burn patients with a mean (+/- SEM) total body surface area burn of 53.1 +/- 2.9% and full-thickness injury of 44.2 +/- 4.1% were studied from July 1996 to December 2000. The

[Therapeutic approach in vascular injuries of the lower extremity: Amputation or limb salvage].

The management of lower extremity trauma with vasculary involvement should be directed toward to the salvage of the extremity or to the primary amputation according to the additional pathologies, parameters of the patient and the extremity. We investigated the efficiency of Mangled Extremity Severity Score (MESS) system which is proposed as an grading system to evaluate the change to extremity salvage or the risk for onset of systemic complications. 81 patients with lower extremity trauma were analyzed according to MESS criteria. 79 of the patients were men and mean age was 23 +/- 4. Local mitochondrial function following traumatic brain injury in rats.

The effect of lateral fluid percussion injury on mitochondrial function in the rat brain was investigated by quantitative imaging of changes in the regional activity of succinate dehydrogenase (SDH), a mitochondrial enzyme of the tricarboxylic acid cycle for adenosine triphosphate production. Regional SDH was measured in the frontal, parietal, temporal, and occipital cortices, CA1 and CA2-3 of the hippocampus, thalamus, corpus callosum, caudate/putamen, and cerebellum 1 hour and 72 hours after low, medium, and high pressure injury. No regional difference between the hemispheres in the activity
