Tetanus: an uncommon cause of dysphagia.

A 53-year-old woman was examined at our medical center because of progressive dysphagia of 14 days' duration and a severe inability to open her mouth and swallow saliva. A barium esophagogram showed no obstruction, but pooling of barium in the hypopharynx suggested a neuromuscular disorder. The clinical diagnosis of tetanus was confirmed by electromyography. With appropriate therapy, the patient recovered during a period of 6 weeks. This case illustrates both an uncommon cause of dysphagia and an uncommon initial manifestation of tetanus.

Modified Technique for Difficult Secondary Tracheoesophageal Puncture.

Tracheoesophageal puncture (TEP) with voice prosthesis (VP) placement is commonly used to restore voice in laryngectomy patients. The conventional procedure utilizes a rigid esophagoscope to open and visualize the pharyngeal inlet. However, this approach is challenging in patients with postradiation changes, reduced neck extension, or trismus. Here, we demonstrate a modified technique involving flexible endoscopy and endotracheal tube placement to reestablish the TEP tract in a patient with a challenging anatomic profile. This step-by-step video demonstrates retrieval of a dislodged prosthesis…

Myositis Ossificans Traumatica of the Head and Neck in a Child.

Myositis ossificans traumatica (MOT) is a focal reactive ossification of muscle that typically occurs secondary to a single acute high-impact trauma or multiple chronic low-grade traumas. The disease is often limited to a single muscle and can occur throughout the body. Ossification of the muscles in the head and neck region is extremely rare but can occur after falls, motor vehicle accidents, blunt force trauma, or surgical procedures such as extractions and local infiltration. The purpose of this paper is to present the case of a seven-year-old girl with trismus and calcification within her …

Bilateral peritonsillar abscess: a case study and literature review.

While peritonsillar abscesses are the most common deep neck infections, bilateral forms are rare. A peritonsillar abscess occurs when pus accumulates in the peritonsillar space, located between the tonsils and superior constrictor muscle, causing medial displacement of the uvula, trismus, odynophagia or even upper airway obstruction. High clinical suspicion is needed to diagnose bilateral peritonsillar abscess due to frequent history, computerized tomography scan of the neck with IV contrast facilitates accurate diagnose and a full assessment of the patient. Incision and drainage are needed to…

[Intensity-modulated radiotherapy for head and neck cancer. Dose constraint for salivary gland and mandible].

Intensity-modulated radiation therapy (IMRT) is the gold standard for head and neck irradiation. It allows better protection to the organs at risk such as salivary glands and mandible, and can reduce the frequency of xerostomia, trismus and osteoradionecrosis. At the time of treatment planning, the mean dose to a single parotid gland should be kept below 26Gy, the mean dose to a single submandibular gland below 39Gy, the mean dose to the mandible below 60 to 65Gy and the D2% to a single temporomandibular joint below 65Gy. These dose constraints could be further improved with data extracted fro…

Maintenance of increased mouth opening in oral submucous fibrosis patient treated with nasolabial flap technique.

Oral submucous fibrosis (OSMF) is an insidious chronic disease affecting any part of the oral cavity and sometimes the pharynx with epithelial atrophy leading to stiffness of the oral mucosa, causing trismus and inability to eat. However, a more serious complication of this disease is the risk of the development of oral carcinoma. A case of OSMF reported with initial interincisal mouth opening; 8 mm which was treated surgically with nasolabial flap technique followed by active mouth opening exercises for 6 months with Hister's jaw exerciser. The patient could maintain mouth opening of 32 mm at…

Myositis ossificans traumatica of the medial pterygoid muscle following a third molar extraction.

Myositis ossificans (MO) is a rare disease involving heterotopic ossification in the muscle or soft tissue. Myositis ossificans traumatica (MOT) disease presents as a calcification within the injured muscle, resulting from a single or repetitive injury. There are few reports of MOT in the masticatory muscles. The case of a patient with MOT in the medial pterygoid muscle caused by a complication related to the extraction of an erupted upper third molar is reported. The major symptom was severe trismus. Despite surgical treatment, the disease relapsed. MOT can lead to serious consequences for th…

Cephalic tetanus presenting as acute vertigo with bilateral vestibulopathy.

Cephalic tetanus is a rare form of tetanus, defined by paralysis of more than one cranial nerve. The seventh cranial nerve is the most frequently involved. We report a 58-year-old man with cephalic tetanus and bilateral vestibulopathy. The patient's initial symptoms were rotatory vertigo and hypertension. He then developed trismus and cranial nerve palsies of the fifth and seventh nerves. The caloric test and vestibular evoked mygenic potentials in response to air-conducted clicks revealed absent responses on both sides, although audiometry and auditory brainstem responses were normal in both …

Synovial chondromatosis of the left temporomandibular joint superficially resembling chondrosarcoma: a case report.

A 25 year-old woman was referred to the clinic complaining of pain in the left temporomandibular joint (TMJ) and trismus. According to the x-ray images, a solitary mass was observed in the anterior pouch of the lower joint cavity. The mass was removed by means of a synovectomy and a diskectomy. Upon light microscopic examination, the tissue removed showed high cellular activity. As a differential diagnosis, we had to consider the possibility of chondrosarcoma based on the histopathological features; however, since no invasive nor metastatic finding was recognized, we made a diagnosis of synovi…

[Removal through the coronal approach of the upper wisdom teeth. Apropos of a case of bilateral migration into the temporal fossa].

This report presents a case of double migration of third maxillary molar in temporal fossa and their removal by a coronal approach. A 15 years of old boy, was seen for major trismus. 2 months before, he underwent under general anaesthesy impacted third maxillary molar removal. The radiologic examination showed that superior third molar has been pushed and left into the temporal fossa in both sides. After computed tomography examination, teeth were removed by a coronal approach. This was the only approach in order to preserve facial nerve and to allow with good chances to remove the teeth. Anesthesia Mumps With Transient Airway Compromise Following Spinal Anesthesia for Emergency Cesarean Delivery.

The authors present a 44-year-old woman who developed bilateral parotid swelling following spinal anesthesia for an emergency cesarean section. Shortly after surgery, the patient developed acute facial swelling associated with pain, trismus, and transient respiratory distress. Ultrasonographic evaluation was consistent with acute parotitis. Conservative management, including intravenous hydration, corticosteroids, analgesics, and supportive care, resulted in complete clinical recovery within 3 days. Although perioperative parotid swelling is an uncommon complication after spinal anesthesia, th…

Chronic Sclerosing Osteomyelitis of the Mandible: A Case Report.

Chronic sclerosing osteomyelitis (CSO) is a rare, non-suppurative inflammatory condition of the bone characterized by progressive sclerosis and fibrosis, often requiring a multifaceted diagnostic approach. This case highlights an eight-year-old female with progressive mandibular swelling and trismus unresponsive to prior treatments. Imaging and histopathological evaluation revealed features consistent with CSO, excluding infectious or neoplastic causes. Intraoperative findings prompted a shift from planned segmental resection to conservative bone shaving, preserving mandibular integrity while …

Variants in tropomyosins TPM2 and TPM3 causing muscle hypertonia.

Patients with myopathies caused by pathogenic variants in tropomyosin genes TPM2 and TPM3 usually have muscle hypotonia and weakness, their muscle biopsies often showing fibre size disproportion and nemaline bodies. Here, we describe a series of patients with hypercontractile molecular phenotypes, high muscle tone, and mostly non-specific myopathic biopsy findings without nemaline bodies. Three of the patients had trismus, whilst in one patient, the distal joints of her fingers flexed on extension of the wrists. In one biopsy from a patient with a rare TPM3 pathogenic variant, cores and minico…

Comparative evaluation of acute stress and clinical efficacy between piezosurgery and conventional rotatory technique in transalveolar extraction of impacted mandibular third molars-A randomized controlled trial.

BACKGROUND: Surgical extraction of impacted mandibular third molars is commonly associated with postoperative pain, swelling, and trismus. Conventional rotatory osteotomy, though widely used, may cause thermal and mechanical trauma. Piezosurgery has emerged as a minimally invasive alternative that may improve clinical outcomes and reduce physiological stress, which can be objectively assessed using salivary biomarkers. PURPOSE: To compare acute stress response and clinical efficacy between piezosurgery and conventional rotatory technique in transalveolar extraction of impacted mandibular thir…

Is ADDM graft practical? A comparative study to evaluate ADDM graft in third molar surgery.

BACKGROUND: It has been outlined that LTM (Lower third molar) extracted from patients in which grinding, cleaning, sterilization & demineralization prove to be highly effective as graft material for filling the alveolar socket of the very same patient. These investigations aim to assess the efficiency of ADDM (Autogenous Demineralized Dentin Matrix) graft in third molar extraction sockets. PURPOSE: To check the effectiveness of ADDM as graft material in extraction socket by evaluating pain, swelling, trismus, PD (Probing Depth) and bone density. STUDY DESIGN, SETTING, SAMPLE: A prospective, …

Evaluation of Efficacy of Intraligamentary Injection Technique for Extraction of Mandibular Teeth-A Prospective Study.

INTRODUCTION: Fear of dental pain is one of the most common reasons for delaying dental treatment. Local Anaesthesia (LA) is the most commonly employed technique of achieving pain control in dentistry. Pterygomandibular Nerve Block (PNB), for achieving mandibular anaesthesia has been the traditional technique used and is associated with a few set of complications which include pain, nerve injury, trismus, and rarely facial nerve palsy, and sustained soft tissue anaesthesia. These complications have resulted in a rapid need for research on alternative local anaesthetic techniques. AIM: This st…

Reconstruction of the buccal mucosa following release for submucous fibrosis using two radial forearm flaps from a single donor site.

BACKGROUND: Oral submucous fibrosis is a collagen disorder affecting the submucosal layer and can severely limit mouth opening. The use of bilateral forearm flaps to fill buccal defects following trismus release has proven to be effective and reliable. However, it requires the sacrifice of radial arteries from both forearms. We have developed a technique that allows for the harvest of two independent flaps from a single forearm donor site. METHODS: Two separate flaps are designed on the same radial artery and concomitant vein pedicle. The distal flap is marked in the standard fashion and the …

Post-assault myositis ossificans of the masseter muscle: A case report.

Myositis ossificans (MO) is a reactive, localized, benign lesion that typically affects the extremities in younger people. It is a unique and uncommon condition that has been difficult to fully understand regarding its causes, development, and effective treatment. Commonly seen in muscles of arms, and thighs, it seldom includes the masticatory muscles. Severe trismus results if it affects the masticatory muscles. Magnetic resonance imaging and computed tomography scans are useful methods for identifying the calcified mass. Histopathology is essential for achieving an accurate diagnosis and pla…
