Correlation of vascular endothelial growth factor with the clinical regression of tubercular granuloma.

Tubercular granulomas are a common manifestation of intraocular tuberculosis. These are said to be hypoxic granulomas with increased expression of vascular endothelial growth factor (VEGF). Management of these granulomas includes a combination of antitubercular therapy (ATT) and oral corticosteroids. We report a case of tubercular granuloma with exudative retinal detachment which was treated with weekly intravitreal anti-VEGF and antibiotic injections along with ATT and corticosteroids. The VEGF levels measured paralleled with the clinical regression of the granuloma.

Cryptococcal osteomyelitis of the spine.

We have treated seven patients with cryptococcal spondylitis. Five presented with a neurological deficit and one was HIV-positive. Amphotericin-B and 5-flucytosine were used in five patients and ketoconazole was given orally in the remaining two. Three patients made a complete neurological recovery. Since these lesions mimic spinal tuberculosis, which is commonly seen in our environment, we draw attention to the importance of obtaining a tissue diagnosis.

The Oral and Dental Significance of Heroin and its Resurgence in New York State.

Heroin is a highly addictive, illegal drug, which is processed from the resin of poppy plants. It was first manufactured in 1898 as a treatment for tuberculosis and morphine addiction. Its use poses both direct and indirect consequences for oral health. An increased incidence of dental caries and periodontal disease is seen in heroin users. This is in addition to a number of systemic manifestations. Currently, New York State is experiencing an epidemic of fatal or near-fatal incidents involving heroin use. This is largely due to the convergence of two deadly influences: the transition by many

OROFACIAL GRANULOMATOSIS: CASE REPORT.

An 18-year-old girl was referred to the Department of Oral Medicine with upper lip swelling. She was in good general health and laboratory tests were within the normal range. Histopathologic diagnosis did not reveal the presence of granulomas. This is consistent with the finding that 30% of patients with orofacial granulomatosis do not have granulomas on their biopsies. The patient was treated with intralesional steroids once a week for three weeks. The lesion subsided, but not completely, and recurred partially after ketchup intake. This case report highlights the fact that in every patient w

Obstructive Jaundice Due to Periportal Tuberculous Lymphadenopathy in a Patient With Multidrug-Resistant (MDR) Tuberculosis: A Case Report.

Extrapulmonary tuberculosis is a common manifestation of tuberculosis, with lymph node involvement being the most frequent form. Among its presentations, periportal lymph node involvement leading to obstructive jaundice is extremely uncommon. Jaundice developing during anti-tubercular therapy (ATT) is frequently ascribed to drug-induced hepatotoxicity, which may contribute to delays in accurate diagnosis. We report a 17-year-old male with multidrug-resistant (MDR) lymph node tuberculosis who presented with progressive jaundice and right upper quadrant abdominal pain. Laboratory evaluation show

Inhalable locust bean gum microparticles co-associating isoniazid and rifabutin: Therapeutic assessment in a murine model of tuberculosis infection.

Tuberculosis is a leading cause of death worldwide. Although the development of new antimycobacterial drugs is an obvious and necessary strategy to address the disease, improving the therapeutic performance of drugs already approved constitutes a valuable alternative approach. As the lung is the most affected organ, where M. tuberculosis is able to survive and proliferate, the direct pulmonary delivery of antitubercular drugs comprises a highly promising therapeutic strategy. In this work, spray-dried locust bean gum (LBG) microparticles were used to deliver a combination of two first line ant

Short-course treatment for multidrug-resistant tuberculosis: the STREAM trials.

Multidrug-resistant (MDR) tuberculosis (TB) is a threat to global TB control, as suboptimal and poorly tolerated treatment options have resulted in largely unfavourable outcomes for these patients. The last of six cohort studies conducted in Bangladesh which assessed a new shorter regimen using currently available TB drugs showed promising results and offered the possibility of a more acceptable and more effective regimen than the one recommended by the World Health Organization (WHO). The aims of stage 1 of the STREAM (Evaluation of a Standardised Treatment Regimen of Anti-tuberculosis Drugs

Mechanical complication of endobronchial tuberculosis.

A 19-year-old Vietnamese lady was diagnosed with culture positive, left upper lobe pulmonary tuberculosis for which medical treatment was initiated. Four months into treatment, she developed a 'rubber-band-like' stretching sensation in her left chest with wheezing and shortness of breath. Decreased respiratory excursion over the left lung was present on physical-examination. Chest-Xray revealed left-upper-lobe collapse with leftward deviation of the trachea and mediastinum. CT thorax revealed a long segment of stenosis in the left mainstem bronchus. FEV1 was 1.26 L (45% predicted), FVC 1.53 L

A rare case of multiple brain abscesses caused by Nocardia abscessus co-infection with tuberculous meningitis in an immunocompetent patient.

BACKGROUND: Nocardial brain abscesses are extremely rare and predominantly affect immunocompromised patients, exhibiting a high overall mortality rate. Tuberculosis infections, although they can occur in immunocompetent individuals, are more prevalent in those with compromised immune systems. Tuberculous meningitis (TBM), the most severe manifestation of tuberculosis, is associated with a high fatality rate. Co-infection with both pathogens is unusual. To our knowledge, this is the first reported case of multiple brain abscesses caused by Nocardia abscessus (N. abscessus) in a young immunocomp

Pulmonary fungal ball due to Trichophyton successfully managed with oral itraconazole: a case report.

Pulmonary fungal balls are caused by long-term fungal infection of the lung. They are sometimes a complication of previous cavitary pulmonary tuberculosis. Pulmonary fungal balls caused by Trichophyton are extremely rare. A 65-year-old man who worked in a leather recycling factory was admitted because of a productive cough and shortness of breath. He had a history of tuberculosis with lung destruction. A chest radiograph showed an opacity surrounding an air lucency over the left lung field, and chest computed tomography showed a mass within a cavity, producing a ball-in-hole appearance, over t

Amphiphilic xanthones as a potent chemical entity of anti-mycobacterial agents with membrane-targeting properties.

Tuberculosis (TB) remains a deadly disease and infects one-third of the world's population. Given the low success rates encountered in clinical development, there is an urgent need to identify structurally novel antimicrobials for tuberculosis. The present report details the anti-mycobacterial activities, structure-activity relationships (SARs) and mechanism of action of amphiphilic xanthone derivatives. The xanthones exhibited potent MIC, rapid time-kill and no cross-resistance with the current anti-TB drugs. Evidence is presented that these compounds disrupted the inner membrane and led to A

Adverse Drug Reactions Associated With Longer All-Oral MDR/XDR-TB Regimens: Prospective Pharmacovigilance Evidence From a High-Burden Tertiary Care Setting in North India.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) and extensively drug-resistant tuberculosis (XDR-TB) impose a disproportionate burden on high-endemic countries, with India contributing the largest global share of rifampicin-resistant cases. The National TB Elimination Program (NTEP) introduced longer all-oral regimens to replace injectable-based therapy; however, prospective pharmacovigilance data characterizing the real-world adverse drug reaction (ADR) profile of these regimens from India remain scarce. OBJECTIVE: To prospectively evaluate the incidence, nature, system organ class dis
