[A case of pleurisy with effusion in the course of rheumatoid arthritis].

The case of pleurisy with effusion in the rheumatoid arthritis (RA), observed in the Chest Diseases and Tuberculosis Clinic in Szczecin is described. Diagnosis was based on the presence of rheumatoid factor and low glucose level in the pleural effusion, (the glucose level in serum was normal) and the aggravation symptoms of RA. Pleural biopsy did not confirmed the presence of rheumatoid nodules.

A Case of Tuberculous Pleurisy Highlighting the Need to Consider Primary Tuberculosis in Interferon-Gamma Release Assay-Negative Patients.

We report a case of tuberculous pleurisy in a 54-year-old man who initially presented with negative interferon-gamma release assay (IGRA) results and elevated pleural fluid adenosine deaminase (ADA) levels. Bacteriological confirmation was obtained only when the pleural fluid culture grew Mycobacterium tuberculosis one month later. The IGRA yielded positive results approximately two months after symptom onset. This case highlights the limitations of IGRAs and ADA as definitive diagnostic tools and underscores the need to consider primary tuberculosis in early IGRA negative cases. Clinicians sh

Sirolimus Therapy in Generalized Lymphatic Anomaly With Tuberculosis: A Case Report.

We report the case of a critically sick previously healthy 14-year-old girl who presented with subacute dry cough, respiratory distress, chest pain, hepatosplenomegaly, lytic bone lesions, splenic involvement, and bilateral chylothorax. Radiological evaluation established a diagnosis of Generalized Lymphatic Anomaly (GLA). The patient showed limited response to propranolol and pleurodesis but improved after initiation of sirolimus and vincristine. Notably, pleural fluid culture yielded Mycobacterium tuberculosis, leading to the addition of antitubercular therapy (ATT). The child subsequently d

Use of adenosine deaminase as a diagnostic tool for tuberculous pleurisy.

BACKGROUND: A statistical audit of adenosine deaminase (ADA) in pleural effusions was undertaken. METHODS: ADA analysis, cytological and microbiological examinations, and differential cell counts were performed on 462 pleural fluid samples. RESULTS: ADA activity in tuberculous effusions was higher than in any other diagnostic group. At a level of 50 U/l the sensitivity and specificity for the identification of tuberculosis was 90% and 89%, respectively. CONCLUSIONS: ADA activity remains a useful test in the evaluation of pleural effusions.

Laboratory and radiological discrimination between tuberculous and malignant pleural effusions with high adenosine deaminase levels.

BACKGROUND/AIMS: Pleural fluid adenosine deaminase (ADA) levels are useful in discriminating tuberculous pleural effusions (TPEs) from malignant pleural effusions (MPEs). However, some patients with MPE exhibit high-ADA levels, which may mimic TPEs. There is limited data regarding the differential diagnosis between high-ADA MPE and high-ADA TPE. This study aimed to identify the predictors for distinguishing high-ADA MPEs from high-ADA TPEs. METHODS: Patients with TPE and MPE with pleural fluid ADA levels ≥ 40 IU/L were included in this study. Clinical, laboratory, and radiological data were c

Role played by the programmed death-1-programmed death ligand pathway during innate immunity against Mycobacterium tuberculosis.

Tuberculous pleurisy allows the study of specific cells at the site of Mycobacterium tuberculosis infection. Among pleural lymphocytes, natural killer (NK) cells are a major source of interferon gamma (IFN-gamma), and their functions are regulated by activating and inhibitory receptors. Programmed death-1 (PD-1), programmed death ligand 1 (PD-L1), and programmed death ligand 2 (PD-L2) are recognized inhibitory receptors in adaptive immunity, but their role during innate immunity remains poorly understood. We investigated the PD-1:PD-L1/PD-L2 pathway on NK cell effector functions in peripheral

Serological analysis of pulmonary and extrapulmonary tuberculosis with enzyme-linked immunosorbent assays for anti-A60 immunoglobulins.

IgA, IgG, and IgM antibodies to mycobacterial antigen A60 were measured by ELISA in blood, pleural fluid, and cerebrospinal fluid from 560 patients with pulmonary and/or extrapulmonary tuberculosis who were being treated at hospitals in northern China and from 734 uninfected controls. Among 529 healthy persons (most of whom had been vaccinated with bacille Calmette-Guérin [BCG] and 287 of whom were tuberculin-positive), the rate of false-positive results was negligible; this observation ruled out interference of remote BCG vaccination with A60 assays at the chosen cutoff level. Rates of positi

[Ultrasonic diagnosis of pleurisy in patients with pulmonary tuberculosis].

Ultrasound proved helpful in diagnosis of pleurisy in tuberculous patients. It is able to localize exudate, to determine its volume, size and characteristic features as well as to show structure of the pleural layers, the existence of pleural adhesions. Echography differentiates changes in the adjacent pulmonary tissue and pleura though it is difficult in patients with rapidly progressive tuberculosis, especially in caseous pneumonia because affected pulmonary tissue is also hypoechogenic. Ultrasound is superior to X-ray in low volumes of the liquid in the pleural cavity or atypical position o

Granulomatous pleuritis caused by Francisella tularensis: possible confusion with tuberculous pleuritis.

Granulomatous pleuritis, diagnosed by pleural biopsy, is rarely caused by disease processes other than tuberculosis. We present a case of granulomatous pleuritis caused by Francisella tularensis that occurred in a 55-yr-old male sheep shearer with an exudative, lymphocytic, pleural effusion. Confirmation of the diagnosis was made by elevated serum agglutination titers against F. tularensis and detection of the organism in a pleural biopsy specimen by the glucose oxidase immunoenzyme technique. We conclude that F. tularensis, not previously described as a cause of granulomatous pleuritis, must

[Paragonimiasis: a rare little known disease].

BACKGROUND: Paragonimiasis, caused by a lung fluke, is an parasitic disease rarely encountered in France. CASE REPORT: A 52-year-old man developed dyspnea, cough, mild fever and chest pain. Pleural effusion suggested possible pulmonary embolism or tuberculosis. Cell counts in blood and pleural effusion fluid revealed major eosinophila in this patient who had recently returned from a trip to Japan. Paragonimiasis was confirmed by ELISA. Treatment with praziquantel led to complete clinical and radiographic recovery. DISCUSSION: The clinical and radiographic features of paragonimiasis are often

Hepatosplenic tuberculosis with hypersplenism: CT evaluation.

Hepatosplenic tuberculosis of macronodular form is extremely rare. We present one case of hepatosplenic tuberculosis with its appearance on computed tomography (CT). The CT findings of hepatosplenic tuberculosis were multiple, well-defined, round or ovoid, low-density masses with a range of +35 to +45 HU. Lymphadenopathy in the abdominal and mediastinal area and pleural effusion were also found. Although CT does not confirm a diagnosis of hepatosplenic tuberculosis, it is a valuable examination to support the diagnosis and define the extent of disease.

Microbiological diagnosis of tuberculosis: a comparison of old and new methods.

The aim of our study was to evaluate the diagnostic value of various methods widely used in microbiological diagnosis of tuberculosis: direct smear examination for acid-fast bacilli, cultural identification in Lowestein-Jensen (L-J) medium, the radiometric BACTEC 460 system, and Polymerase Chain Reaction (PCR). Three hundred and ninety-three clinical samples of sputum (375), gastric aspirate (3), pleural fluid (12) and urine (3) were taken from 125 patients hospitalized at our Institute for suspected pulmonary tuberculosis, between January 1995 and June 1997. On completion of diagnosis, 35 wer
