UNLABELLED: PURPOSE PATIENTS AND METHODS: Helicobacter pylori (HP) is present in more than 90% of duodenal ulcers (DUs). To investigate the pathophysiology in those patients with DU who are HP-negative compared with those who are HP-positive, we interviewed consecutive patients prior to endoscopy regarding factors often associated with ulcer disease. At esophagogastroduodenoscopy, antral biopsy specimens were obtained for urease test, culture, and Warthin Starry staining for HP in all patients with DU who did not have active bleeding.

763 patients from the "Honorio Delgado" Regional Hospital of Arequipa city (2300 mts ht) are studied, all suffering of gastroduodenal peptic ulcer disease. The authors describe the gastro-kinetic drugs that act on functional dyspepsia including metoclopramide, domperidone, clebopride cisapride. Moreover, in some forms of non-ulcer dyspepsia it is useful to give sulglicotide, a cytoprotective drug that has been shown to induce marked improvement of clinical symptoms and endoscopic findings.

Marjolin's ulcer is the malignant lesion which develops in a burn scar or chronic fistula. Due to the low prevalence of this lesion, there has been disagreement regarding its clinical features, methods of treatment, and prognosis. We evaluated 19 cases of patients who had been admitted to Severance Hospital from Jan. 1970 to Dec. 1985. Chronic gastroduodenal ulceration is the end product of an imbalance between acid levels, peptic hostile factors, and mucosal defenses. This condition differs significantly from stress ulceration, in which the primary factor is decreased mucosal blood flow, and from nonsteroidal anti-inflammatory drug-induced injury, in which there is local vascular injury and inhibition of prostaglandin synthesis. The identification of H pylori as a cause of chronic gastritis, duodenitis, and peptic ulcer is required for specific antibacterial therapy.

The prevalence of peptic ulcer disease was retrospectively analysed in 35 patients affected by primary hyperparathyroidism consecutively observed from 1977 through 1987. The results of 96 simultaneous operations for peptic ulcer are discussed. The indications for their performance are substantiated and the principles of their conduction in various diseases aggravating one another are determined. The low incidence of postoperative complications (4.2%) and lethality (1.04%) are evidence of the expediency of the suggested tactics.

To study the role of Campylobacter pyloridis (CP) in the etiology of peptic ulcer and chronic gastritis, biopsies were obtained from gastric and duodenal mucosa of 54 ulcer patients and 8 healthy controls. In normal histological appearance and moderate gastritis without signs of exacerbation CP were not registered contrary to atrophic antral gastritis demonstrating CP irrespective of ulcer location. A case of tuberculous skin ulcer is reported. The biopsy specimen did not reveal acid-fast bacteria but cultures grew Mycobacterium tuberculosis. A high index of suspicion is needed to diagnose mycobacterial ulcers correctly. The classification of the cutaneous tuberculoses is discussed.
