The catheterized bladder environment induces dysregulation of macrophage polarization exacerbating bacterial UTI.

Urinary catheterization causes bladder damage, predisposing hosts to catheter-associated urinary tract infections (CAUTIs). CAUTI pathogenesis is mediated by bladder damage-induced inflammation, resulting in accumulation and deposition of the blood-clotting protein fibrinogen (Fg) and its matrix form fibrin, which are exploited by uropathogens as biofilm platforms to establish infection. Catheter-induced inflammation also results in robust immune cell recruitment, including macrophages (Mϕs). A fundamental knowledge gap is understanding the mechanisms by which the catheterized-bladder environm…

Implementing a clinical practice guideline to manage postpartum urinary retention.

Postpartum urinary retention is a common condition in obstetric units. A Clinical Practice Guideline was implemented in a high-risk obstetrical unit to decrease variance of clinical practice, rate of postpartum urinary retention, and number of urinary catheterizations and increase awareness of this common condition. Guideline implementation met the 4 aims, including a decreased rate of urinary retention.

The diagnosis and management of neonatal urinary ascites.

Urinary ascites in a newborn infant is unusual and most commonly indicates a disruption to the integrity of the urinary tract. The following report describes a case of urinary ascites, probably due to bladder rupture caused by umbilical artery catheterization, associated with hyponatremia, hyperkalemia and elevated serum creatinine. This unusual biochemical profile is characteristic of urinary 'autodialysis' and was corrected by bladder drainage.

Morbidity of urodynamic investigation in healthy women.

The morbidity of diagnostic catheterization in healthy women has never been described. In order to further elucidate the natural history of postcystometry bacteriuria we studied a group of healthy women without lower urinary tract symptoms. Thirty female volunteers aged 39-72 years underwent urine sampling prior to and at day 3 after invasive urodynamic evaluation. One case of asymptomatic bacteriuria was detected among pretest samples. On post-test sampling 10 of 29 cultures were positive. With a cut-off level of >10(5) CFU/ml only 2 cases could be classified as significant bacteriuria. Only

Postpartum urinary retention.

BACKGROUND: Urinary retention is a common and frustrating complication in women during the immediate postpartum period. Physiologic changes in the bladder that occur during pregnancy predispose patients to develop symptomatic retention of urine during the first hours to days after delivery. METHODS: The incidence and characteristics of postpartum urinary retention were researched through a literature review and are illustrated by a case report. RESULTS AND CONCLUSIONS: Postpartum urinary retention has a reported incidence ranging from 1.7 to 17.9 percent. Factors associated with postpartum u

Development, Implementation, and Impact of an Electronic Medical Record Alert System for Implanted Artificial Urinary Sphincters.

INTRODUCTION: The presence of an artificial urinary sphincter can be overlooked resulting in inadvertent urethral catheterization and cuff erosion. A hard-stop best practice advisory was created in the electronic medical record to alert for the presence of an artificial urinary sphincter. We evaluated its utilization and impact on patient outcomes. METHODS: Our advisory fired for men with an artificial urinary sphincter implanted or in place between August 1, 2016, and September 30, 2023. We calculated exposure years (total time in years of implant duration during each time period), which wer

Feasibility and safety of same-day discharge and shortened bedrest after atrial fibrillation ablation.

BACKGROUND: While initial studies suggest that same-day discharge or shortened bedrest may be feasible for some patients following atrial fibrillation (AF) ablation, the risks and benefits of this approach remain unclear for patients undergoing hemostasis with figure-of-eight (FO8) suture technique. METHODS: We prospectively evaluated access site bleeding, length of hospitalization, urinary catheterization, and other clinical outcomes in patients undergoing AF ablation with 3 hours of bedrest between April and May 2021, and compared them to a control group that had undergone AF ablation with …

Postoperative Urinary Retention After General Surgical Procedures: Incidence, Risk Factors, and Short-Term Outcomes.

BACKGROUND: Postoperative urinary retention (POUR) is a frequent but sometimes missed complication after general surgical procedures. If not recognized early, it may cause bladder overdistension, patient discomfort, urinary tract infection, unexpected catheterization, delayed mobilization and longer hospital stay. This study was conducted to determine the incidence, associated risk factors, and short-term outcomes of POUR in patients undergoing general surgical procedures. METHODS: This prospective observational study included adult patients who underwent elective or emergency general surgica

Providing effective evidence-based catheter management.

Around a quarter of people admitted to hospital are catheterized at some point, however, urinary catheterization can have catastrophic effects on the bladder and a persons ability to regain continence in the future (Patel and Arya, 2001). Although most urinary catheters are used for a short-term period, bladder drainage complications, such as pain or bypassing, can occur (Niël-Weise and van den Broek, 2005). Sometimes, when these problems develop, or if the catheter is expelled, nurses react by changing the catheter, even though this is not necessarily the most appropriate solution. In fact, c…

Efficacy and Safety of Catheter Interventions for Postoperative Urinary Retention After Primary Hip and Knee Total Joint Arthroplasty.

INTRODUCTION: Postoperative urinary retention (POUR) is a common barrier to rapid-discharge hip and knee total joint arthroplasty (TJA). We evaluated the efficacy and safety of catheterization intervention methods for POUR before and after discharge. METHODS: A total of 1,659 primary TJAs were retrospectively reviewed. POUR resolutions before and after discharge were evaluated relative to catheterization type and other covariates. Complications before and within 90 days of discharge were quantified. A total of 113 POUR cases comprised the analysis sample of 76 hips and 37 knees in 51 women an

Adoption of Single-Use Clean Intermittent Catheterization Policies Does Not Appear to Affect Genitourinary Outcomes in a Large Spinal Cord Injury Cohort.

PURPOSE: In April 2008, Medicare amended its policy for clean intermittent catheterization, increasing coverage from 4 reused catheters per month to up to 200 single-use catheters. The primary reason for the policy change was an assumed decrease in risk of urinary tract infection with single-use catheters. Given its economic/environmental impact (∼50-fold increase in cost and plastic waste) and a paucity of supporting evidence, we retrospectively evaluate the policy's effect in a prospective spinal cord injury registry. MATERIALS AND METHODS: We accessed data for the years 1995 to 2020 from t

Significance of urinary cytology in the early detection of transitional cell cancer of the upper urinary tract.

Cytologic study of urine from 100 patients with transitional cell cancer of the upper urinary tract revealed that findings on the voided and catheterized specimens correlated well with grade and stage of the tumor. However, there was a significantly greater positive yield with urine specimens obtained by ureteral catheterization than by voiding. Consequently, because concomitant bladder tumors, diagnosed or not, can give questionable positive results ureteral catheterization specimens should be used for cytologic study.

A review of strategies to decrease the duration of indwelling urethral catheters and potentially reduce the incidence of catheter-associated urinary tract infections.

The use of indwelling urinary catheters in hospitalized patients presents an increased risk of the development of complications, including catheter-associated urinary tract infection (CAUTI). With regard to the risk of developing a CAUTI, the greatest factor is the length of time the catheter is in situ. The aim of this article is to review the evidence on the prevention of CAUTI, particularly ways to ensure timely removal of indwelling catheters. Published studies evaluating interventions to reduce the duration of catheterization and CAUTI in hospitalized patients were retrieved. The research

Catheter burden following urogynecologic surgery.

BACKGROUND: Data on the experience that women who undergo urogynecologic surgery have with postoperative catheterization are severely limited. As the importance of our patients' perioperative experience becomes more valued, assessment of the burden of postoperative catheterization, which has not yet been performed, is increasingly needed. OBJECTIVE: The aim of this study was to compare catheter burden in women who self-selected use of an indwelling Foley catheter vs clean intermittent self-catheterization for voiding dysfunction after reconstructive pelvic surgery.
