Risk factors and a predictive model for steinstrasse formation after shock wave lithotripsy in pediatric urolithiasis.

Steinstrasse is an important complication following extracorporeal shock wave lithotripsy; however, predictive models for risk factors in pediatric populations remain limited. This study aimed to develop and externally validate an explainable machine learning model for predicting steinstrasse formation after ESWL in children. This two-center retrospective study included 807 pediatric patients (< 18 years) who underwent ESWL for renal stones (2015–2025). The internal cohort comprised 685 patients (Adıyaman), while the external validation cohort consisted of 122 patients (Adana). Twenty-six preo

Performance of large language models in urological decision support: a guideline-based comparative evaluation in urolithiasis.

Large language models (LLMs) are increasingly investigated for their potential role in guideline-based clinical information support. However, their consistency with subspecialty guidelines, particularly in urolithiasis, remains underexplored. This study aimed to evaluate the performance of four large language models (LLMs); GPT-4, GPT-4-turbo, Claude, and Gemini in generating guideline-concordant responses to clinical questions related to urolithiasis. A total of 105 clinical questions were independently developed by the authors based on urolithiasis management principles. Each LLM generated r…

Sex-specific signatures of gut microbiota and systemic inflammation in patients with urolithiasis: a cross-sectional study.

BACKGROUND: Urolithiasis is a globally prevalent disease with a distinct male predominance; however, the pathophysiological heterogeneity within diagnosed cohorts remains underexplored. This study delineates the sex-specific signatures of gut microbiota and systemic inflammation in urolithiasis patients to inform sex-stratified management. METHODS: This cross-sectional study enrolled 60 urolithiasis patients (40 males, 20 females). Systemic inflammatory cytokines were quantified via peripheral blood assays, and gut microbiota was profiled using 16S rRNA sequencing. Data were integrated to eva…

Assessing associations of clinical factors and feeding practices with infantile urolithiasis: Insights from a nationwide study.

INTRODUCTION: The incidence of infantile urolithiasis (IU) has been rising, attributed to factors such as climate change, the widespread use of ultrasonography, dehydration, and alterations in nutritional practices. Studies investigating the influence of infant feeding practices on stone formation remain limited. OBJECTIVES: This study aims to identify the risk factors associated with stone formation in infants and to evaluate whether formula feeding influence the development of IU. STUDY DESIGN: Infants aged 1-12 months who were diagnosed with IU and followed up at pediatric nephrology clin…

Head-to-head comparison of the CHOKAI and UROLITHIASIS scores for predicting ureteral stones: A multicenter prospective validation.

OBJECTIVE: The CHOKAI score, which includes seven clinical factors, namely, age, sex, pain features, hematuria, and hydronephrosis, was designed to predict uncomplicated renal colic and rule out serious alternative conditions. The more recently introduced UROLITHIASIS score adds laboratory data, such as urine microscopy and serum creatinine. This study compared the diagnostic accuracy of the CHOKAI and UROLITHIASIS scores for identifying ureteral stones and determined the optimal UROLITHIASIS cutoff in a Japanese cohort. METHODS: We conducted a multicenter prospective observational study betw…

Case report: Urolithiasis, nephrolithiasis and a urinary bladder malformation in a seven-month-old alpaca cria.

Urolithiasis is a common condition in male small ruminants where predisposing factors have been identified. Occasionally, urolithiasis is diagnosed in South American camelids (SACs). However, nephrolithiasis is rarely diagnosed in ruminants. To our knowledge, this is the first report focusing on a combined appearance of nephrolithiasis and urolithiasis in an alpaca cria. A 7-month-old alpaca cria suffering from impaired urinary flow was presented for examination. On admission, the alpaca had a wet prepuce and showed a standing posture with a wide-based stance. Ultrasonographic examination of t…

Outcome and complications in goats treated by perineal urethrostomy for obstructive urolithiasis: 25 cases (2010-2017).

BACKGROUND: Obstructive urolithiasis commonly affects male goats. Perineal urethrostomy (PU) can be a permanent treatment option but is generally considered undesirable because of the risk of stricture of the urethral stoma. Limited information exists regarding long-term outcome and complications in goats undergoing PU for treatment of obstructive urolithiasis. OBJECTIVES: To determine short-term and long-term outcome and complications in goats undergoing PU for treatment of obstructive urolithiasis. ANIMALS: Twenty-five client-owned goats. METHODS: Multi-institutional retrospective case se…

Kelley-Seegmiller Syndrome: Urolithiasis, Renal Uric Acid Deposits, and Gout: What is the Role of the Urologist?

Kelley-Seegmiller syndrome (KSS) is a disorder that occurs when there is a partial deficiency of the enzyme hypoxanthine guanine phosphoribosyl transferase. It is involved in the metabolism of purines, clinically manifesting as hyperuricemia, hyperuricosuria, gout arthritis, and urolithiasis. The aim of this article is to present the case of a 33-year-old male with KSS, with left ureteral colic, and a 5-mm, 323-HU ureteral calculi, successfully managed with conservative management. It is critical to recognize that most urologists are not familiar with this inborn metabolic error and 75% of the…

Postrenal acute kidney injury in a patient with unilateral ureteral obstruction caused by urolithiasis: A case report.

RATIONALE: In patients with bilateral ureteral obstruction, the serum creatinine levels are often elevated, sometimes causing postrenal acute kidney injury (AKI). In contrast, those with unilateral ureteral obstruction present normal serum creatinine levels, as long as their contralateral kidneys are preserved intact. However, the unilateral obstruction of the ureter could affect the renal function, as it humorally influences the renal hemodynamics. PATIENT CONCERNS: A 66-year-old man with a past medical history of hypertension and diabetes mellitus came to our outpatient clinic because of ri

Risk of Urolithiasis in Anorexia Nervosa: A Population-Based Cohort Study Using the Health Improvement Network.

This population-based retrospective cohort study sought to determine if anorexia nervosa (AN) is associated with a higher risk of urolithiasis. Nine thousand three hundred two females with AN were compared to 92 959 randomly selected age-matched and practice-matched females. Cox regression was used to estimate the hazard ratio (HR) for urolithiasis and evaluate effect modification by age. Twenty-three participants with AN (0.25%) developed urolithiasis compared with 154 unexposed participants (0.17%) over a median of 4 years of observation. The risk of urolithiasis varied significantly with ag…

Design and challenges of a randomized clinical trial of medical expulsive therapy (tamsulosin) for urolithiasis in the emergency department.

Urolithiasis or urinary stone disease has been estimated to affect about 1 in 11 Americans. Patients with urinary stone disease commonly present to the emergency department for management of their acute pain. In addition to providing analgesia, administration of drug (medical expulsive therapy) is often prescribed to assist passage of the urinary stone. In this methodology paper, we describe the design of a prospective, multi-center, randomized, double-blind placebo controlled clinical trial of the alpha-adrenergic blocker, tamsulosin, to evaluate its effectiveness as medical expulsive therapy…

Urolithiasis and psoas abscess in a 2-year-old boy with type 1 glycogen storage disease.

We report on a pyogenic psoas abscess secondary to an impacted calcium oxalate ureteric stone in a 2-year-old boy with glycogen storage disease type 1 (GSD-1). The patient had a drainage of the abscess through a flank incision followed by percutaneous nephrostomy and open ureterolithotomy. Metabolic acidosis, hyperuricemia, hypocitraturia, and hypercalciuria appear to be significant in the pathogenesis of urolithiasis in patients with GSD-1. Regular ultrasonography of the abdomen along with optimal metabolic control may delay or prevent urolithiasis and its complications in GSD-1 patients.

Silicon-zinc interactions and potential roles for dietary zinc and copper in minimizing silica urolithiasis in rats.

Two 8-wk experiments were conducted with Sprague-Dawley weanling rats to determine whether interactions occurring between Zn and Si, or a nu … ously in the rat, may not be of a sufficient magnitude to be applicable to the prevention of silica urolithiasis. The data further demonstrate that Zn deficiency and, to a lesser extent, Cu deficiency contributed to silica urolith formation in rats fed diets having a high content of absorbable Si. However, 540 mg of Si/kg of diet may potentiate the metabolic a …

[Extracorporeal shockwave lithotripsy in the treatment of urolithiasis--experiences from a center with the Piezolith 2200 and HM3 lithotriptors].

The Piezolith 2200 allows not only a qualitatively identical treatment of urolithiasis like the HM-Dornier systems or the Siemens Lithostar, but the application of lithotriptable urinary calculi could be extended to cardiac risk patients, to patients with skeletal deformities and to those with unusual body height and weight. As the piezolithotripsy does not cause pain, treatment is possible without anaesthesia or analgesia. Combined with internal ureteral stenting by self-retaining double-J-ureteral catheter also calculi with larger stone masses can be treated advantageously by exclusive piezo…

Citrate in urine and serum and associated variables in subgroups of urolithiasis. Results from an outpatient stone clinic.

Outpatient renal stone formers belonging to the established urolithiasis subgroups and controls were examined with respect to urinary and serum citrate (Cit) and several associated variables. Only in the normocalciuric majority of calcium and in uric acid stone formers was Cit in 24-hour urine decreased, but was normal in 2-hour fasting morning, and in 3-hour postprandial urine following a Cit-free test meal. Serum Cit was elevated in normocalciuria, renal and resorptive hypercalciuria. This Cit constellation was associated with either normal (absorptive, renal hypercalciuria) or low (normocal…
