Hemipylorectomy with transverse pyloroplasty and vagotomy was made in 922 patients with perforating and bleeding ulcers of pyloric canal. 510 of them had perforation of ulcer, 412--ulcer bleeding. In all the cases ulcer was excised together with anterior semicircle of the pylorus, the defect was sutured transversely. In experiment on white rats in was established that after the truncal vagotomy performance in the stress gastric ulcer the lowering of the serotonin and histamine blood content was noted as well as the disorder of ratio of their concentration in gastric wall. Insufficiency of serotonin causes the disorder of gastric motor-evacuation function after the operation. Administration in postoperative period of serotonin adipinate had promoted the raising of serotonin concentration in blood and gastric tissues, histamine level lowering, enhancement of serotonin/histamine ratio.

The main purpose of this study was to investigate the effects of (i) truncal vagotomy + pyloroplasty, (ii) proximal cell vagotomy and (iii) anterior proximal cell vagotomy + posterior truncal vagotomy on the gastric secretion and gastric emptying in rats. We report the results of a prospective study of all patients undergoing highly selective vagotomy (HSV) for bleeding duodenal ulceration (BDU) at Concord Hospital between 1979 and 1989. Highly selective vagotomy was undertaken in 63 patients (58 male, 5 female) with a median age of 69 years (range: 16-89). Experience in the treatment of 70 patients with stenotic duodenal ulcer by surgery is generalized. In addition to SPV the patients underwent duodenoplasty as a draining operation. There were 61 (87.1%) males and 9 (12.9%) females. Their ages ranged from 18 to 70 years. The indications for highly selective vagotomy have expanded in recent years, with the technique being applied to selected cases of perforation and bleeding. Its use in obstruction is controversial, but two options are available for managing the stenotic pylorus or duodenum: dilatation or duodenoplasty. The influence of the proximal selective vagotomy (PSV) on the origin and the extent of experimental gastric ulcer were investigated in rats. The lesions of the gastric mucosa were caused in three groups: by stress through swimming-test, by application of phenylbutazone, and by ischemia (ligature of the left gastric and the right gastroepiploic vessels). The PSV practised a protective influence on the pharmacodynamic etiology, however, not on the stress ulcer.
