[Excretion-secretion antigens from adult Dirofilaria immitis in the diagnosis of human filariasis by solid phase immunoenzyme assay].

The solid phase enzymatic immuno-assay (ELISA) was normalized for detecting antibodies against. Filaria using excretion-secretion antigens (ESA) from Dirofilaria immitis adults in a group of asymptomatic and microfilaraemic patients infected by different species of filariae (loa loa, Wuchereria bancrofti, Onchocerca volvulus, Mansonella ozzardi and Mansonella perstans), and in another group of symptomatic and aminorofilaraemic patients, temporary residents in an area with endemic loiasis. The ESA-ELISA specificity permitted the distinction between filariasic and non-filariasic patients.

Microfilaria in achylous hematuria: Can it imitate urolithiasis?

Wuchereria bancrofti is the major species resulting in filarial cases in India. Filariasis may present with various clinical presentations: asymptomatic, microfilaremia, lymphedema, acute adenolymphangitis and chronic lymphatic disease. Microfilariae of W. bancrofti have been detected in various clinical samples, but incidental detection of microfilaria in achylous urine is a rare finding with unexplained pathology. We report a case of microfilaria in achylous urine of a 30-year-old female who presented with pain abdomen and ureteric calculus on ultrasonography.

Bancroftian filariasis in an isolated hunter-gatherer shifting horticulturist group in Papua New Guinea.

A survey for Wuchereria bancrofti microfilaraemia using membrane filtration was carried out among the Hagahai, a recently contacted Papua New Guinea group of hunter-gatherer shifting horticulturists. Adult men had a significantly higher microfilaraemia rate than women. Children aged > 15 years had significantly fewer infections than adults and the microfilaraemia densities were considerably lower. Two subjects exhibited matutinal microfilarial periodicity, peaking at approximately 06 h 00. The possible reasons for the epidemiological and microfilarial findings are discussed.

Breast filariasis or inflammatory breast carcinoma? Reaching a diagnosis.

Lymphatic filariasis is a neglected tropical disease caused by the parasite Wuchereria bancrofti. Involvement of the breast is relatively uncommon and may affect only 10% of individuals with lymphatic filariasis. We present a case of an elderly woman with breast pathology. The condition clinically appeared as a malignancy. Routine diagnostic tests including tissue histopathological studies failed to clinch a diagnosis. However, a serological test helped in identifying the pathology, which turned out to be a manifestation of breast filariasis, and aided in instituting appropriate therapy for th…

Disseminated microfilaremia associated with lung cyst and empyema: an autopsy report.

Clinical manifestations of extralymphatic disease caused by filariasis are varied and range from symptoms due to tropical pulmonary eosinophilia to hematuria, proteinuria, splenomegaly, and rarely arthritis. Disseminated microfilaremia in association with loculated lung cyst and empyema is of rare occurrence and to the best of our knowledge has not been documented in the literature so far. We report here a case of disseminated microfilaremia due to Wuchereria bancrofti infection accompanied by a lung cyst and empyema in a 21-year-old Indian man.

The PacELF programme: will mass drug administration be enough?

The Pacific Programme to Eliminate Lymphatic Filariasis is a regional, mass drug administration-based campaign in 22 countries and territories with the aim of eliminating filariasis transmission and alleviating the suffering caused by Wuchereria bancrofti. The challenges to filariasis elimination campaigns based on mass drug-administration alone are reviewed in this article. These challenges together with the previous successes of mosquito control campaigns in eliminating filariasis from regions in the Pacific argue for inclusion of entomology components in the control of filariasis and the mo…

Non-parasitic chyluria: a case report and review of the literature.

Chyluria is due to the passage of chyle into the urine giving it a typical milky appearance. Although rarely seen in our countries it is well-known in areas where infections with Wuchereria bancrofti are endemic. Chyluria can also have non-parasitic causes of which only a few cases have been described. We briefly discuss etiologies and pathogenesis of chyluria and present a case of non-parasitic chyluria successfully treated by surgery. A short and critical review of the different treatment modalities is made.

Reduced lymphocyte response to mitogens in patients with Bancroftian filariasis.

Peripheral blood lymphocytes from patients with acute and chronic Wuchereria bancrofti infections responded poorly to concanavalin A, phytohaemagglutinin and pokeweed mitogen when cultured in heat-inactivated pooled normal serum. The lymphocyte response to mitogens in carriers of microfilariae (mff) were normal. The suppression of transformation to mitogens was not reversible by the removal of plastic adherent cells. Incubation with mitogens and the adult filarial worm antigen (BmA) did not alter the mitogen response either in control subjects or in filarial patients. The possible mechanism of…

Polymerase chain reaction-based detection of lymphatic filariasis.

PCR-based diagnostic assays are promising tools for the monitoring and evaluation of the Global Programme for Elimination of Lymphatic Filariasis. Sensitive and specific assays have been described for the detection of Wuchereria bancrofti, Brugia malayi, or Brugia timori infection in blood, sputum, and vectors. These techniques can be most cost-effective when employed for pool screening, which is important in the later stages of control programs when infection rates of humans and vectors are low, and large numbers of samples must be examined.

Microfilariae in fine needle aspirates from epididymal lesions.

Microfilariae of Wuchereria bancrofti were observed in fine needle aspiration smears from three epididymal nodules, and degenerating microfilariae suggestive of Brugia malayi were found in the smears from a fourth case. The smears in all four cases showed a polymorphonuclear inflammatory cell component as well as epithelioid cell granulomata. While blood eosinophilia was present in all four cases, eosinophilia was present in the aspiration smears in only one case. Microfilariae could be demonstrated in the peripheral blood in only one case.

Antigenic activity in adult Dipetalonema viteae in the indirect immunofluorescent test against sera from filariasis patients--the immunofluorescent histological search for "pure" antigen.

Using IFAT, it has been shown that isolated egg-shells and uterine fluid of Dipetalonema viteae are the most potent antigens in heterologous systems using human sera from patients infected with Wuchereria bancrofti, Onchocerca volvulus and Loa loa, as well as in homologous systems using sera from animals infected with D. viteae. It is suggested that these antigens are unlikely to be highly species-specific, and that anatomical isolation of antigens is a necessary prerequisite to immunochemical analysis aimed at the preparation of a "pure" antigen.

Mapping of lymphatic filariasis in Nepal.

BACKGROUND: Human infection with Wuchereria bancrofti causes a disabling parasitic disease known as lymphatic filariasis, which is a major public health and socio-economic problem in many parts of the world. At the onset of the study, little was known of the distribution of filariasis and its current importance as a public health problem in Nepal. METHODS: Epidemiological mapping was undertaken to determine the prevalence of infection by Wuchereria bancrofti in 37 districts of Nepal between July to December 2001. The study population above 15 years of age was selected, and the immunochromatogr…

Seroepidemiology of bancroftian filariasis in the Seychelles Islands.

Wuchereria bancrofti is the only human filarioid present in the Seychelles archipelago. The last parasitological survey carried out in Mahé revealed a microfilaraemia rate of 3.6%. Serum samples from 417 native individuals living in Mahé were tested for the presence of filarial antibodies by ELISA method, using crude soluble extract of Brugia pahangi adult worm as antigen. The results seem to show a proportion of the population (17%) has been exposed to W. bancrofti (with OD values greater than 0.5) and 7% (with OD greater than 0.7) have specific filarial antibodies. The pattern of distributio…

Ocular filariasis: "Dancing sensation in the anterior chamber".

We report a case of ocular filariasis in a 28-year-old male who presented with a 2-month history of irritated acute red eye. Examination revealed a living and active large worm dancing in the anterior chamber of the left eye causing acute iridocyclitis, leading to drop in vision to 6/36. The worm whose size was approximately 33 mm in length was extracted under local anesthesia in toto using viscoelastics without damaging the ocular structures. Microscopic examination confirmed adult Wuchereria bancrofti.

Retroperitoneal filariasis diagnosed by fine needle aspiration: a case report.

BACKGROUND: Filarial worms lodge anywhere, from body fluids to subcutaneous tissue, as various case reports prove. CASE: A 21-year-old Nepali male presented with bilateral groin swellings. Imaging studies showed dilated retroperitoneal lymphatics forming a mass, suggestive of lymphangioma or lymphangioleiomyoma. Fine needle aspiration (FNA) of the retroperitoneal mass revealed microfilariae of Wuchereria bancrofti. CONCLUSION: This is the first reported case of retroperitoneal filarial lymphangiectasia diagnosed on FNA, thus stressing the importance of FNA as a simple yet effective diagnosti…

Indocyanine green - guided laparoscopic renal pedicle lymphatic disconnection: A novel, targeted treatment for chyluria.

Chyluria, or the passage of chyle into the urine from anomalous lymphatic connections, results in a characteristic milky urine. In severe cases, it can cause signifi cant morbidity from nutritional losses and immune suppression. Although predominantly associated with Wuchereria bancrofti infections, non-parasitic cases have also been described. Traditionally, surgical treatment has involved renal lymphatic disconnection using open or minimally invasive methods, occasionally aided by pre-operative imaging techniques like lymphangiography, or by identifi cation of structures with laparoscopic ma…

Tirandamycins from Streptomyces sp. 17944 inhibiting the parasite Brugia malayi asparagine tRNA synthetase.

Lymphatic filariasis is caused by the parasitic nematodes Brugia malayi and Wuchereria bancrofti, and asparaginyl-tRNA synthetase (AsnRS) is considered an excellent antifilarial target. The discovery of three new tirandamycins (TAMs), TAM E (1), F (2), and G (3), along with TAM A (4) and B (5), from Streptomyces sp. 17944 was reported. Remarkably, 5 selectively inhibits the B. malayi AsnRS and efficiently kills the adult B. malayi parasite, representing a new lead scaffold to discover and develop antifilarial drugs.

[New Guinea and Australia: their common pathology].

New Guinea and Australia have been joined together during several hundred millions of years. Inhabited by men 40,000 years ago, these populations (papous and aborigenes) have been separated of the others during several millenaries. The first invaders brought with them plasmodiums, Necator americanus, Wuchereria bancrofti and Dengue virus. Brugia malayi, because lack of primates, took non root in New Guinea; the virus of Murray Valley encephalitis is autochthonous; the high frequency of Burkitt lymphoma in New Guinea is, perhaps, a racial feature.
