Consensus guidelines for initial management of neutropenic fever.

This update of the Australasian consensus guidelines for the initial empiric management of neutropenic fever occurs in the context of major changes in cancer treatment paradigms, as well as advances in the management of sepsis and new models of care for infections since the last version of these guidelines in 2011. Acknowledging the important role of antimicrobials in the disruption of the gut microbiome and emerging antimicrobial resistance, as well as the changing epidemiology of antimicrobial resistance more broadly, these guidelines address current evidence for optimal empiric neutropenic …

Progress in African Swine Fever Vector Vaccine Development.

African swine fever (ASF) is a highly lethal, infectious, hemorrhagic fever disease, characterized by an acute mortality rate approaching 100%. It is highly contagious, and results in significant losses to the global hog industry as it spreads. Despite incremental progress in research on the African swine fever virus (ASFV), a safe and effective commercial vaccine has yet to be developed. Vector vaccines, a promising type of vaccine, offer unique advantages, and are a primary focus in ASFV vaccine research. This paper focuses on the characteristics of viral, bacterial, and yeast vector vaccine…

Atypical Patients With Severe Fever With Thrombocytopenia Syndrome.

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne disease with a high fatality rate. The clinical diagnosis criteria mainly rely on white blood cell (WBC) and platelet (PLT), which, however, are of limited usage in identifying atypical SFTS. A multicenter study was performed in two hospitals from 2011 to 2023. SFTS patients were categorized as atypical or typical based on the clinical diagnosis criteria. Clinical progress and outcomes were compared between the two groups. A total of 2876 laboratory-confirmed SFTS patients were included in this study, 90.54% (2604/287…

Epidemiological Characteristics of Severe Fever with Thrombocytopenia Syndrome.

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging infectious disease primarily reported in Asia. This review aims to summarize studies on the epidemiological characteristics of SFTS. Literature from PubMed and Scopus was searched up to February 14, 2024. A total of 76 articles were eligible. Infections were reported in China, Japan, South Korea, and several other countries in Asia. The incidence of SFTS has been rising and reported from new areas across Asia. The incidence rate was highest in China, ranging from fewer than 0.1 to 4.2 cases per 100,000 population and reaching up…

Chikungunya fever: pathogenesis and mechanisms underlying pain symptoms.

Chikungunya fever (CHIKF), caused by the Chikungunya virus (CHIKV) and transmitted by Aedes mosquitoes, has rapidly evolved from localized outbreaks to a significant global health threat. While the initial high fever is often debilitating, it is the severe and frequently long-lasting pain, affecting joints (arthralgia), muscles (myalgia), and sometimes nerves (neuropathic pain), that truly characterizes the disease's impact on sufferers. This review explores how CHIKV infection triggers both acute pain and persistent chronic pain. We examine the mechanisms by which the virus directly damages t…

Typhoid fever in pregnancy - a South Australian perspective.

Salmonella typhi in Australia is uncommon, with a reported incidence of less than 1 per 100,000. In pregnancy, untreated Salmonella can lead to complications including intrauterine fetal death and preterm labour. Described here is a woman who travelled home to India for a month in the second trimester of her pregnancy and contracted the illness there. She had a protracted incubation phase with the bacterium, initially having negative stool samples and then testing positive almost a month after her return. Despite a delay in antibiotic treatment, she went on to recover from her illness and have

Beyond the Usual Suspects: Neonatal Fever and Desaturation.

BACKGROUND: We report a 1-month-old infant admitted with fever, dyspnea, and desaturation after an unmonitored pregnancy in a migrant family. The patient rapidly required mechanical ventilation. Laboratory parameters were markedly elevated, suggesting an infectious etiology. METHODS: The clinical course is described with emphasis on diagnostic challenges, including imaging, laboratory findings, and the stepwise process of differential diagnosis. RESULTS: After common etiologies were excluded, rarer causes were considered. The final diagnosis was established through integration of clinical as…

[Fever, tetraparesis and confusion - neurological phenomena in influenza].

In this case study, we present an unusual course of a 40-year-old patient with clinical signs of a viral infection with leading gastrointestinal symptoms. In the short term, he developed a progressive ascending sensorimotor tetraparesis with autonomic symptoms as well as qualitative disturbance of consciousness and the need for intensive medical care. Imaging revealed transverse myelitis and a diffusion disorder of the splenium (so-called CLOCC lesion). A diagnosis of "mild encephalopathy with reversible splenial lesion (MERS)" and "longitudinally extensive transverse myelitis (LETM)" was made

Fever in Sickle Cell Disease: Thinking Beyond Bacteremia.

Sickle cell disease (SCD) is a complex pathophysiology with a wide range of symptoms. More than one-third of pediatric SCD emergency department encounters are due to fever. Patients with SCD are immunocompromised because of hyposplenism. Therefore, infection is a serious concern in any febrile patient with SCD. Although bacteremia is seen in only 1.1% of febrile patients with SCD, it remains the leading cause of death globally. However, several noninfectious causes of fever can be seen in SCD, which should be considered in appropriate settings. We present a case of an adolescent female patient…

Acute Rheumatic Fever in a Low Prevalence Setting.

BACKGROUND: Acute rheumatic fever is a systemic inflammatory condition caused by beta-hemolytic streptococcal infection. Its prevalence in high-income countries has fallen significantly as a result of widespread antibiotic use and improved living conditions. CASE SUMMARY: We report the case of an 18-year-old, otherwise healthy, Irish woman who presented to an Irish hospital with fever and general malaise on a background of recurrent pharyngitis. Her condition rapidly deteriorated in the hospital and she developed septic shock requiring intensive care admission for organ support. Investigation…

Acute rheumatic fever and Post-streptococcal reactive arthritis.

Acute rheumatic fever (ARF) and poststreptococcal reactive arthritis (PSRA) represent two distinct but related poststreptococcal conditions, both arising from an autoimmune response following Group A streptococcal (GAS) infection, typically pharyngitis. Although both illnesses have a shared trigger, their clinical presentations and long-term consequences differ markedly. ARF, a systemic inflammatory disorder, can impact several organs, particularly the heart, potentially resulting in chronic rheumatic heart disease with irreversible valve damage. The cornerstone of ARF treatment includes Strep…

[The 512th case: fever, cough, acute kidney injury].

A 70-year-old female patient presented with fatigue and edema for 3 months and was found to have elevated serum creatinine for 3 weeks. During the course of the disease, she had fever and cough. Examinations revealed multiple ground-glass opacities in both lungs and positivity for myeloperoxidase-anti-neutrophil cytoplasmic antibodies (ANCA), leading to a diagnosis of ANCA-associated vasculitis. The patient's condition initially improved after pulse glucocorticoid therapy combined with cyclophosphamide. During treatment, however, the patient developed hematochezia, and colonoscopy revealed mul…

Epidemiology of the Crimean-Congo hemorrhagic fever virus.

Crimean-Congo hemorrhagic fever virus (CCHFV) is endemic in >30 countries across Asia, Europe, the Middle East, and Africa. CCHFV causes Crimean-Congo hemorrhagic fever (CCHF), a disease characterized by bleeding and haemoptysis. Since the first diagnosis in 1944, 10,000-15,000 CCHF cases are reported worldwide annually. Around three billion people are at risk of CCHFV infection. The World Health Organization (WHO) announced CCHFV as a priority pathogen in December 2015 due to its widespread and significant healthcare threat. In this review, we track the outbreaks, discuss their origin, and an…
