Falls prevention for older adults in outdoor public spaces: an interdisciplinary Delphi consensus on risks, actions, and barriers.

BACKGROUND: Falls are the leading cause of accidental injury among older adults, 30% of community-dwelling adults aged 65 and over fall each year, with nearly half occurring outdoors. These falls are complex, understudied, and insufficiently addressed in current age-friendly cities or walkability frameworks. This study aimed to build interdisciplinary consensus on risks, preventive actions, and barriers to fall prevention in outdoor public spaces through a Delphi process. METHODS: A three-phase Delphi study was conducted with 64 participants in round 1, 60 in round 2, and 49 in round 3, inclu

Developing a prospective ethical governance framework for a discrete choice experiment involving children aged 6-12 years: the PREF-ANX Kids study.

BACKGROUND: Discrete choice experiments (DCEs) are quantitative preference-elicitation methods, developed largely within health economics, in which participants choose repeatedly between hypothetical scenarios composed of systematically varied attributes. Their growing use with children supports children's right to be heard in decisions affecting their care, yet it also raises ethical challenges that general paediatric research ethics guidance does not address in operational detail: sustained cognitive demand, potential emotional distress, assent that can blur under clinical stress, parental i

Affinal parenthood after divorce: a socio-legal study of stepparent visitation rights in Peruvian blended families.

INTRODUCTION: Blended families challenge legal systems that continue to organize parental status primarily through formal categories such as filiation, adoption, parental authority, custody, and legally recognized family status. This study examines how Peruvian judicial, legal, prosecutorial, public-defense, and psychosocial professionals interpret the possibility of granting post-divorce visitation rights to stepparents in blended families. It does not propose automatic legal parenthood for stepparents but analyzes whether affinal status, when accompanied by evidence of social or functional p

Recognition and Management of Hyperkalemia-Induced Tachyarrhythmia in Pediatric Spontaneous Tumor Lysis Syndrome: A Simulation Case.

INTRODUCTION: Prompt recognition and treatment of hyperkalemia-induced arrhythmias in children is vital, as these can be life-threatening. Tumor lysis syndrome (TLS) can present with severe hyperkalemia, which, if untreated, may lead to renal failure, arrhythmias, or multi-organ failure. Simulation offers a safe way for providers to practice managing such acute scenarios. While prior simulations address hyperleukocytosis, few focus on metabolic complications like hyperkalemia in spontaneous TLS. This high-fidelity simulation aimed to improve participants' confidence in recognizing and managing

One Size Does Not Fit All: Comparing CDH Team Structure Across Children's Hospitals in the United States of America.

BACKGROUND: Congenital diaphragmatic hernia (CDH) is a complex anomaly with high rates of morbidity and mortality. Successful management depends on interprofessional and multidisciplinary collaboration. Multiple previous publications have underscored the effect that center-specific factors can have on CDH outcomes. However, beyond institutional differences, optimal outcomes also rely on a well-composed team with both surgical and critical care expertise. Understanding the center effect requires identifying key drivers, with team structure being a major factor. While surgeons and neonatologists

Changes in low back pain, global sagittal alignment, and spinopelvic parameters following primary total hip arthroplasty: results of a prospective observational study with two-year follow-up.

PURPOSE: The hip-spine relationship is increasingly recognized as clinically relevant in patients undergoing total hip arthroplasty (THA), yet associations between THA and changes in low back pain (LBP), sagittal alignment, and functional disability remain incompletely understood. This prospective study evaluated two-year changes in LBP (primary clinical endpoint) and radiographic alignment, with global sagittal alignment and spinopelvic alignment defined as primary radiographic endpoints, as well as their associations with postoperative pain and disability. METHODS: This prospective observat

Visceral Metastases of Osteosarcoma in the Hepatopancreatobiliary System.

Background: Osteosarcoma (OS) is the most common primary malignant bone tumor, mainly affecting adolescents and young adults. While lung metastases are common, visceral metastases in the hepatopancreatobiliary system are extremely rare and usually associated with a poor prognosis. The limited diagnostic and therapeutic options for such metastases make the treatment of affected patients difficult. The possibility of very late metastatic onset in high-grade OS highlights the potential need for extended follow-up (FU) beyond established intervals. Methods: This study combines a retrospective anal

Development of a Nomogram to Integrate Molecular Testing and Clinical Variables to Improve Malignancy Risk Assessment Among Cytologically Indeterminate Thyroid Nodules.

Background: The introduction of molecular testing (MT) of cytologically indeterminate thyroid nodules (ITNs) alone has not impacted thyroidectomy rates. Due to this, we evaluated the incremental diagnostic value of various clinical variables in addition to MT results, in predicting the risk of malignancy (ROM) among ITNs. Methods: This prospective observational study included 1024 consecutive ITNs that underwent reflexive ThyroSPEC MT between Jul 30, 2020, and Oct 30, 2023. A multivariable logistic regression model was built to assess the relationship between histology outcomes and clinical va

Prediction of Ki-67 Expression in HIV-Associated Lung Adenocarcinoma Patients Using Multiple Machine Learning Models Based on CT Imaging Radiomics.

PURPOSE: The incidence of lung adenocarcinoma (LUAD) in HIV-infected individuals is significantly increased. However, invasive procedures for Ki-67 assessment may increase the risk of complications. Therefore, developing a non-invasive and accurate method for Ki-67 prediction holds significant clinical importance. This study aims to explore the feasibility and value of a radiomics model based on preoperative CT images in predicting Ki-67 expression levels in HIV-associated LUAD. PATIENTS AND METHODS: A total of 237 patients with HIV-associated LUAD were included. Of these, 102 were classified

Clinical Roles of Infectious Diseases Specialists in Ophthalmology.

OBJECTIVE: Ophthalmology is characterized by its organ-specific focus, reliance on specialized devices, and the requirement that intraocular examinations be performed exclusively by ophthalmologists. These factors limit interdisciplinary collaboration. However, infection screening prior to immunosuppressive therapy (IST), systemic infection management, and antimicrobial planning increasingly require input from infectious diseases (ID) specialists. This study aimed to analyze the indications and clinical outcomes of ID consultations initiated by ophthalmology subspecialties in a tertiary eye ho
