Carbachol injection into the pontine reticular formation depresses laryngeal muscle activities and airway reflexes in decerebrate cats.

To understand the role of cholinoceptive, medial pontine reticular formation (mPRF) neurons in the control of upper airway, pharyngolaryngeal reflexes, we measured activities of intrinsic laryngeal muscles (posterior cricoarytenoid, PCA; thyroarytenoid, TA), diaphragm (DIA), genioglossus (GG) and a neck muscle (trapezius) in unanesthetized, decerebrated, spontaneously breathing cats with and without mPRF carbachol injections. The ethimoidal nerve was electrically stimulated to evoke sneezing, and the superior laryngeal nerve to evoke the laryngeal reflex, swallowing, and coughing. Carbachol re

Feeding problems and malnutrition in spinal muscular atrophy type II.

The aim of the study was to conduct a survey using a dedicated questionnaire to assess feeding difficulties and weight gain in a population of 122 Spinal Muscular Atrophy (SMA) type II patients, aged between 1 and 47 years. All the answers were entered in a database and were analysed subdividing the cohort into age groups (1-5, 6-10, 11-14, 15-19, 20-29, and 30-50 years). Six out of our 122 patients (5%), all younger than 11 years, had weights more than 2SD above the median for age matched controls, whilst 45 (37%) had weights less than 2SD below the median. Chewing difficulties were reported

Preventive strategies for aspiration pneumonia in elderly disabled persons.

Pneumonia is the fourth leading cause of death despite the availability of potent new antimicrobials in Japan. Aspiration of oropharyngeal bacterial pathogens to the lower respiratory tract is one of the most important risk factors for pneumonia. Impairments in swallowing and cough reflexes among disabled older persons, e.g., related to cerebrovascular disease, increase the risk of pneumonia. Thus, strategies to reduce the volumes and pathogenicity of aspirated material should be pursued. Since both swallowing and cough reflexes are mediated by endogenous substance P contained in the vagal and

Effect of chronic and acute cigarette smoking on the pharyngoglottal closure reflex.

BACKGROUND: Injection of water into the pharynx at a threshold volume induces vocal cord adduction--the pharyngoglottal closure reflex (PGCR). This reflex together with other supraoesophageal reflexes may be helpful in preventing aspiration. Cigarette smoking has an adverse affect on the pharyngo-upper oesophageal sphincter contractile reflex and reflexive pharyngeal swallow. The effect of smoking on PGCR has not been studied previously. AIMS: To elucidate the effect of chronic and acute cigarette smoking on PGCR. SUBJECTS: We studied 10 chronic smokers and 10 non-smokers before and after re

Diabetes insipidus in a patient with a highly malignant B-cell lymphoma and stomatitis.

A 37-year-old male patient with a diffuse pleomorphic B-cell-lymphoma, which has been diagnosed two month earlier with the primary site at the pterygopalatine fossa on both sides with infiltration of the clivus and cavernous sinus was referred to our hospital for continuation of the third course of CHOP chemotherapy. At admission he reported about a recent history of painful swallowing and intermittent substernal chest pain. Alleviation of the pain on swallowing and the chest pain was apparently only possible by drinking 10 to 15 l of cold coca cola throughout the day and night, a regimen that

Functional outcome in soft palate reconstruction using a radial forearm free flap in conjunction with a superiorly based pharyngeal flap.

BACKGROUND: Speech and swallowing problems due to velopharyngeal incompetence may follow soft palate resection and reconstruction. Over the past 3 years, we have developed the use of a superiorly based pharyngeal flap in conjunction with a radial forearm flap for soft palate reconstruction. METHODS: This paper describes the technique in detail and compares the functional results in a study with patients undergoing soft palate resection for squamous cell carcinoma treated with or without a pharyngeal flap as an adjunct to a radial forearm free flap for soft palate reconstruction. Seven patient

[Therapy of central nervous system listeriosis in sheep].

Twenty-two sheep and 4 goats suffering from central nervous listeriosis were treated with a therapy that had proved to be successful in cattle. For one week they received daily subcutaneous injections of 50,000 IU Procaine Penicillin G per kg live weight and 5 to 10 mg Vitamin B1 per kg body weight. The base excess was tested by blood gas analysis, and it was compensated by intravenous infusion of Na-bicarbonate. Animals that could not eat or swallow received water and rumen liquid by stomach tube. Eight of 26 patients (31%) were healed. The prognosis of central nervous listeriosis depends mai

The assessment of oesophagitis in hiatus hernia patients.

The incidence of oesophagitis has been determined in 108 patients with sliding hiatus hernias using endoscopic, histological, and radiological criteria. Particular consideration has been given to the relationship between inflammatory disease and clinical symptoms. All the patients were attending a thoracic surgical clinic and the spectrum of disease encountered was fairly severe; over half of the cases had established strictures when first seen. The incidence of oesophagitis based on endoscopic evidence was 76·9%, while inflammatory change was noted on biopsy in 56·2% and at barium swallow in

Assessing vocal fold movement in healthy young children and infants using laryngeal ultrasound by speech and language therapists: a feasibility study.

BACKGROUND: Vocal fold motion impairment (VFMI) is a common laryngeal disorder in infants and young children and may impact respiration, phonation and swallowing. Flexible nasendoscopy (FNE) is the current gold standard for assessment; however, its invasive nature may limit its use for routine screening. Recent studies reported good sensitivity and specificity of laryngeal ultrasound (LUS), a minimally invasive alternative compared to FNE. However, its application by Speech and Language Therapists (SLTs) in the paediatric population remains underexplored. OBJECTIVES/HYPOTHESIS: This study aim

Brain-Computer Interface Applications in Craniofacial Nerve Functional Reconstruction: A Narrative Review.

BACKGROUND: Brain-computer interface (BCI) technology is increasingly relevant to craniofacial nerve functional reconstruction because it can decode cortical motor intent and convert it into physical or digital output when peripheral motor pathways are impaired. Facial nerve palsy, dysphagia, and oromandibular motor dysfunction remain difficult to treat when conventional nerve repair, muscle transfer, or electrical stimulation cannot restore coordinated and natural movement. METHODS: This narrative review synthesized peer-reviewed literature on BCI-related craniofacial functional reconstructi

Contributions of pharmaceutical interventions to the multidisciplinary dysphagia team: A retrospective observational study.

BACKGROUND: The 2022 revision of Japanese healthcare reimbursement removed pharmacists from the mandatory dysphagia team, despite emerging evidence of medication-related swallowing complications. Our previous pharmacovigilance analysis identified dopamine-blocking drugs as primary contributors to the risk of aspiration pneumonia. This study aimed to validate these findings through a clinical examination of pharmaceutical interventions performed by a multidisciplinary dysphagia team. METHODS: This retrospective observational study was conducted at a 97-bed community hospital in Osaka, Japan, f

Association between a malnutrition screening tool and mealtime observation checklist items in older people receiving oral intake support: A cross-sectional study of four long-term care facilities.

BACKGROUND: Multiple nutritional screening tools are available for older people; however, few screening tools include specific eating behaviours as risk factors that could lead to poor food intake. The 24-item mealtime observation checklist (MOCL), developed by the Japanese Ministry of Health, Labour and Welfare in 2015, comprises signs, symptoms and conditions during mealtime that reflect eating and swallowing functions and oral conditions. OBJECTIVES: To examine factors associated with malnutrition among the MOCL items in older people. METHODS: A cross-sectional study was conducted using d

How enteral nutrition modes influence nasopharyngeal carcinoma survivors with late dysphagia after radiotherapy: a randomized controlled study.

BACKGROUND: Palliation to late dysphagia after radiotherapy for nasopharyngeal carcinoma (NPC) continues to be a challenge. Intermittent oro-esophageal tube feeding (IOE) is an emerging enteral nutrition mode that can address nutrition and quality of life issues related to nasogastric tube feeding (NGT). OBJECTIVES: This study aims to investigate the effect of IOE versus NGT in late dysphagia after radiotherapy for NPC. METHODS: This randomized controlled study included 82 NPC survivors with late dysphagia after radiotherapy. The subjects were randomized divided into the IOE and NGT groups (

Relationship Between Oral Hypofunction and Nutritional Status in Patients Treated With Maxillofacial Prostheses.

Introduction Patients who have jaw or tongue defects owing to head and neck cancer or congenital disease suffer from oral dysfunction such as speech, swallowing, and mastication. Although maxillofacial prostheses are applied to improve oral functions, their oral functions remain compromised compared to those of patients without jaw or tongue defects. On the other hand, it has been reported that oral hypofunction could lead to malnutrition in the general elderly. However, to date, no reports have evaluated the relationship between oral function and nutritional status in patients with maxillofac
