Non-invasive blood pressure monitoring with in-ear infrasonic hemodynography for preventative cardiovascular care.

Infrasonic Hemodynography (IH) captures low-frequency vibrations detected by in-ear microphones, offering a new approach to blood pressure monitoring. We report a strong correlation (r = 0.95) between IH waveforms and aortic blood pressure measurements obtained through cardiac catheterization (CC). Temporal analysis confirms the aortic origin of IH signals, with minimal differences in aortic valve opening timings between IH and CC (1.6 ± 12.1 ms), rejecting the hypothesis of peripheral origins. We test an approach to predict systolic (SBP) and diastolic (DBP) blood pressure from waveform morph…

Risk Factors Associated with Middle Ear Barotrauma in Patients Undergoing Monoplace Hyperbaric Oxygen Therapy.

PURPOSE: Transurethral resection of bladder tumors (TUR-BT) requires follow-up evaluation by cystoscopy. We sought to evaluate the prognosis of non-muscle-invasive bladder cancer (NMIBC) patients within 6 months of surgery to identify the optimal timing for the first cystoscopy after TUR-BT. MATERIALS AND METHODS: In this retrospective analysis, patients newly diagnosed with NMIBC were divided into two groups according to whether they underwent cystoscopy within 6 months after TUR-BT. We considered four outcomes: recurrence, progression, cancer-specific mortality, and all-cause mortality. Inv

Surgical decision-making regarding hearing and ear reconstruction in craniofacial microsomia: Exploring caregiver narratives.

Treatment decision-making is an integral but complex part of healthcare, particularly in the context of craniofacial surgeries. The aim of the current study was to explore caregiver narratives to inform future surgical care delivery and best practice. 'Life Story' narrative interviews were conducted with US English- and Spanish-speaking caregivers (n = 62) of children aged 3-17 years with craniofacial microsomia (CFM). Extracts relating to treatment decision-making were inductively coded using Reflexive Thematic Analysis. Four themes were identified: 1) 'Grappling with Difference' exemplifies …

3D-Printed Scaffolds for Ear Reconstruction Using Decellularized Human Cartilage-Derived Bioink and Polycaprolactone.

Reconstructing auricular tissue is challenging because ear cartilage has few blood vessels and limited regenerative capacity. Traditional methods that utilize autologous costal cartilage or synthetic polymers often lead to donor site morbidity and suboptimal biocompatibility. In this study, we introduce 3D-printed scaffolds composed of decellularized human cartilage-derived bioink combined with polycaprolactone (PCL), designed to enhance both tissue regeneration and mechanical stability. The decellularization process effectively removed cellular components while preserving glycosaminoglycan an…

A Modified Two-Flap Method for First-Stage Ear Reconstruction in Lobule-Type Microtia.

BACKGROUND: Autologous ear reconstruction typically involves a lengthy treatment period, often spanning 2 to 4 stages or utilizing skin expansion, to achieve satisfactory results. Consequently, an alternative procedure capable of shortening this treatment period would be advantageous for patients. We have made significant enhancements to the safety and outcomes of single-stage autologous ear reconstruction through a multitude of technical improvements. Herein, we propose a novel procedure capable of reconstructing a complete ear contour with postauricular sulcus in the first stage, with the op…

Injectable polyplex-loaded glycol chitosan thermogel for efficient and safe inner ear gene delivery.

Gene therapy has emerged as a promising approach for treating inner ear disorders, such as hearing loss and balance impairments, which require precise and efficient delivery mechanisms. This study introduces a dual-component delivery system combining RH-PAMAM G2 dendrimer-based polyplexes with hexanoyl glycol chitosan (HGC) thermogel to enhance gene delivery to the inner ear. The RH-PAMAM G2 dendrimers, modified with histidine and arginine, demonstrated high DNA binding affinity, low cytotoxicity, and effective cellular uptake, facilitating stable plasmid DNA (pDNA) transfection in HEI-OC1 aud…

Volumetric atlas of the rat inner ear from microCT and iDISCO+ cleared temporal bones.

BACKGROUND: Volumetric atlases are an invaluable tool in neuroscience and otolaryngology, greatly aiding experiment planning and surgical interventions, as well as the interpretation of experimental and clinical data. The rat is a major animal model for hearing and balance studies, and a detailed volumetric atlas for the rat central auditory system (Waxholm) is available. However, the Waxholm rat atlas only contains a low-resolution inner ear featuring five structures. In the present work, we segmented and annotated 34 structures in the rat inner ear, yielding a detailed volumetric inner ear a…

Spatiotemporal expression of CWC27 spliceosome-associated protein (CWC27) in the developing mouse inner ear.

CWC27 is a member of cyclophilin-type peptidyl-prolyl cis-trans isomerase family and takes part in the pre-mRNA splicing. The mutation of its gene, Cwc27, has been verified to be related to cancer, craniofacial and skeletal anomalies, retinal degeneration, and hearing loss. However, expression pattern of Cwc27 in the development of inner ear has not been fully understood. In this study, we studied the spatiotemporal expression of Cwc27 mRNA and CWC27 protein in the mouse inner ear from embryonic day (E) 9 to postnatal 0-day by in situ hybridization and immunohistochemistry staining, respective…

Glue ear: an ongoing global program - closing the audiology gap among First Nations children.

Most children globally (~80%) are affected by Otitis Media with Effusion (OME; 'serous otitis media' or 'glue ear') by four years of age. Most episodes resolve in a timely manner are brief and uncomplicated. Chronic, recurrent and complicated OME has been linked to social disadvantage, with poorer educational and employment outcomes and increased criminal justice system contact. Specific populations are particularly at risk, even in High Income Countries, including children with craniofacial abnormalities, immunocompromise and First Nations children in colonised regions (e.g., Australia, New Z…

Human inner ear fluid imbalance detected by optical coherence tomography correlates with hearing loss.

Hearing loss and vertigo occur when there is an imbalance between the two inner ear fluids, endolymph and perilymph. The inner ear is a small delicate structure encased in dense bone deep in the base of the skull, making it challenging to image with high resolution. Because the fluid chambers are so small, there is no reliable way to measure their balance in a living patient to guide therapy. Here, we translated the technology of optical coherence tomography (OCT) for use in the human inner ear. Peering through the otic capsule bone during mastoid surgery, we imaged the lateral and posterior s…

Middle ear neuroendocrine tumor with multiple brain metastases: a case report and literature review.

Middle ear neuroendocrine tumor (MeNET) is a low-grade tumor with rare recurrence or metastasis. Here, we describe the case of a 29-year-old man who suffered from MeNET that recurred 3 times over 10 years and eventually metastasized to the brain. The patient was treated with surgical resection, radiotherapy, and chemotherapy. However, the tumor was not entirely removed as the brain metastatic tumor adhered tightly to the brainstem. Due to tumor rupture and bleeding after multiple brain tumor removal, profound coma developed. Finally, the patient died 10 months after the last surgery. To our kn…

Transcanal Endoscopic Ear Surgery for Pediatric Bilateral Congenital Cholesteatoma: A Report of Two Cases.

Bilateral congenital cholesteatoma (BCC) is rare among congenital cholesteatoma (CC) cases, and bilateral surgery poses a significant psychophysical burden on pediatric patients. Here, we present 2 pediatric cases of BCC that were successfully managed using simultaneous bilateral transcanal endoscopic ear surgery (TEES). The surgical approach provided clear visualization of the middle ear structures, enabling successful cholesteatoma removal with minimal invasiveness and a short operative time. In both cases, follow-up computed tomography showed no recurrence, and pure-tone audiometry results …

DEK::AFF2 fusion-associated middle ear non-keratinizing squamous cell carcinoma: A case report.

BACKGROUND: Primary squamous cell carcinoma (SCC) of the middle ear is rare, with non-keratinizing basaloid types being exceptionally uncommon. Distinguishing these cancers, often caused by viral factors (e.g., human papillomavirus or Epstein-Barr virus), or specific genetic alterations (e.g., bromodomain-containing protein 4-nuclear protein in testis fusion gene or Ewing sarcoma breakpoint region 1 gene fused with FLI chromosomal rearrangement), from other cranial conditions, is difficult. The recently identified DEK::AFF2 non-keratinizing SCC (NKSCC) is a novel subtype, fitting the World Hea…

Cortical mechanisms of across-ear speech integration investigated using functional near-infrared spectroscopy (fNIRS).

This study aimed to investigate integration of alternating speech, a stimulus which classically produces a V-shaped speech intelligibility function with minimum at 2-6 Hz in typical-hearing (TH) listeners. We further studied how degraded speech impacts intelligibility across alternating rates (2, 4, 8, and 32 Hz) using vocoded speech, either in the right ear or bilaterally, to simulate single-sided deafness with a cochlear implant (SSD-CI) and bilateral CIs (BiCI), respectively. To assess potential cortical signatures of across-ear integration, we recorded activity in the bilateral auditory co…
