Strength gains without muscle injury after strength training in patients with postpolio muscular atrophy.

We evaluated changes in the dynamic and isometric strength in the newly weakened quadriceps muscles and asymptomatic triceps muscles of 6 patients with postpolio muscular atrophy (PPMA) after 10 weeks of progressive resistance strength training. Alterations in muscle size were determined with magnetic resonance imaging. Serum creatine kinase levels were measured throughout training, and histological signs of muscle injury and changes in muscle fiber size and types were assessed with muscle biopsies before and after training. Exercise training led to an increase in dynamic strength of 41% and 6

Brachial plexus repair by peripheral nerve grafts directly into the spinal cord in rats. Behavioral and anatomical evidence of functional recovery.

Over the years, peripheral nerve grafts, a favorable environment for the support of axonal elongation, have attracted interest as a possible means of promoting spinal cord repair. In the experiments described here, rats underwent an avulsion injury of the brachial plexus, and the musculocutaneous nerve was repaired by direct insertion of peripheral nerve grafts into the spinal cord. After varying postoperative periods, the rats were submitted to a series of behavioral tests to evaluate forelimb and hindlimb function. They also underwent retrograde double-labeling studies. Nerve grafts were har

Neuromuscular Electrical Stimulation in Brachial Plexus Birth Injury Rehabilitation: A Systematic Review.

Background and Objectives: Brachial plexus birth injury (BPBI) is a peripheral nerve injury occurring during birth that may result in upper-extremity weakness and functional impairment. This systematic review aimed to evaluate the effects of neuromuscular electrical stimulation (NMES) on motor function, muscle strength, range of motion, and upper-extremity function in children with BPBI. Materials and Methods: This systematic review was conducted according to PRISMA guidelines and registered in PROSPERO. PubMed, CINAHL, Scopus, Web of Science, PEDro, and the Cochrane Library were searched from

Optimizing donor fascicle selection in Oberlin's procedure: A retrospective review of anatomical variability using intraoperative neuromonitoring.

BACKGROUND: Transfer of the fascicle carrying the flexor carpi ulnaris (FCU) branch of the ulnar nerve (UN) to the biceps/brachialis muscle branch of the musculocutaneous nerve (Oberlin's procedure), is a mainstay technique for elbow flexion restoration in patients with upper brachial plexus injury. Despite its widespread use, there are few studies regarding the anatomic location of the donor fascicle for Oberlin's procedure. Our report aims to analyze the anatomical variability of this fascicle within the UN, while obtaining quantifiable, objective data with intraoperative neuromonitoring (IO

Racial Disparities in 30-day Readmission After Orthopaedic Surgery: A 5-year National Surgical Quality Improvement Program Database Analysis.

BACKGROUND: Readmission rate after surgery is an important outcome measure in revealing disparities. This study aimed to examine how 30-day readmission rates and causes of readmission differ by race and specific injury areas within orthopaedic surgery. METHODS: The American College of Surgeon-National Surgical Quality Improvement Program database was queried for orthopaedic procedures from 2015 to 2019. Patients were stratified by self-reported race. Procedures were stratified using current procedural terminology codes corresponding to given injury areas. Multiple logistic regression was done

Surgical versus non-surgical treatment of humeral SHAFT fractures compared by a patient-reported outcome: the Scandinavian Humeral diAphyseal Fracture Trial (SHAFT)-a study protocol for a pragmatic randomized controlled trial.

BACKGROUND: The outcome of non-surgical treatment is generally good, but the treatment course can be long and painful with approximately a quarter of the patients acquiring a nonunion. Both surgical and non-surgical treatment can have disabling consequences such as nerve injury, infection, and nonunion. The purpose of the study is to compare patient-reported outcomes after surgical and non-surgical treatment for humeral shaft fractures. METHODS: A pragmatic randomized controlled trial (RCT) is planned with two study groups (SHAFT-Young and SHAFT-Elderly). A total of 287 eligible acute humeral

The Modification of Five-Flap Z-Plasty for Web Contracture.

BACKGROUND: Web contractures are fairly commonly encountered in those who have suffered from burn injury or other trauma. Numerous local flaps have been adopted previously. The five-flap Z-plasty is one that has been used frequently. To release the scar as much as possible, based on the traditional design, we developed a modified technique of the five-flap Z-plasty to reconstruct the axillary and elbow web contractures. Hence, the length of the axis of the cicatrix could be much lengthened. METHODS: Twenty patients (12 females and 8 males, 7 to 48 years-old) with 27 web contractures were arra

Outcomes after surgical treatment of missed Monteggia fractures in children.

BACKGROUND: Chronic dislocation of the radial head treatment in Monteggia fracture dislocation is still controversial. We present a large series of patients treated in our Institution. MATERIALS AND METHODS: The outcome of 22 children treated surgically between 1988 and 2011 for post-traumatic chronic radial head dislocation is reported. There were 12 girls and 10 boys with a mean age at surgery of 7.2 years (4.1-13.6). The mean interval between injury and treatment was 15.7 months (1-128). Nine patients underwent open reduction with removal of interposed tissue and repair (7) or Bell-Tawse r

Corrective and reconstructive surgery in patients with postburn heterotopic ossification and bony ankylosis: an evidence-based approach.

Articular heterotopic ossification (HO) and subsequent bony ankylosis (BA) are infrequent yet devastating complications of severe burn injuries. These conditions are diagnosed clinically and confirmed by imaging. Patients then begin active physical therapy until cleared for surgery, which remains the standard of care. This study critically reviews the existing literature on the diagnosis, treatment, and surgical outcomes of HO/BA in severely burned patients and provides an evidence-based treatment algorithm. A comprehensive search for all the studies addressing HO and BA after burn injury was

Biomechanical study of the extensor carpi ulnaris as a dynamic wrist stabilizer.

PURPOSE: To investigate the dynamic stabilizing effect of the extensor carpi ulnaris (ECU) on the distal radioulnar joint (DRUJ) and the ulnocarpal joint in a simulated model of triangular fibrocartilage complex (TFCC) injury. METHODS: Using 8 fresh-frozen cadavers, we fixed the humerus and ulna at 90° of elbow flexion, and the radius and hand were allowed to rotate freely around the ulna. Passive mobility was tested by translating the radiocarpal unit relative to the ulna along dorsal-palmar directions. Unit displacement was measured by an electromagnetic tracking device in different forearm

Axillary nerve repair by fascicle transfer from the ulnar or median nerve in upper brachial plexus palsy.

OBJECT: Nerve repair using motor fascicles of a different nerve was first described for the repair of elbow flexion (Oberlin technique). In this paper, the authors describe their experience with a similar method for axillary nerve reconstruction in cases of upper brachial plexus palsy. METHODS: Of 791 nerve reconstructions performed by the senior author (P.H.) between 1993 and 2011 in 441 patients with brachial plexus injury, 14 involved axillary nerve repair by fascicle transfer from the ulnar or median nerve. All 14 of these procedures were performed between 2007 and 2010. This technique wa

Partial ipsilateral C7 transfer to the upper trunk for C5-C6 avulsion of the brachial plexus.

BACKGROUND: Ipsilateral whole C7 root transfer has been reported in treating C5-C6 avulsion. To minimize donor deficits, partial ipsilateral C7 (PIC7) transfer was developed. OBJECTIVE: To investigate the long-term results of PIC7 transfer to the upper trunk in treating C5-C6 avulsion of the brachial plexus. METHODS: We prospectively studied 8 young adults with C5-C6 avulsion. Five patients (group A) who also had spinal accessory nerve (SAN) injury underwent PIC7 transfer to the upper trunk. The other 3 patients (group B) without SAN injury underwent a combination of PIC7 to the upper trunk

Fracture blisters: clinical and pathological aspects.

Fracture blisters are tense vesicles or bullae that arise on markedly swollen skin directly overlying a fracture. There is very little objective data in the literature detailing their characteristics and management. All fracture blisters that occurred over a 3 1/2-year period were studied retrospectively at four hospitals, of which three were level I trauma centers. A total of 53 blisters developed in 51 patients. They occurred in characteristic locations along the human musculoskeleton, most commonly overlying the tibia, ankle, and elbow. They arose within 24-48 h of acute injury in most inst

Intramedullary fixation of humeral shaft fractures.

In this paper we document seventy fractures of the humeral shaft that were treated by intramedullary fixation between 1970 and 1981. Complications developed in forty-seven (67 per cent) of the fractures, and forty-five (64 per cent) required at least one additional operative procedure. Of the sixty fractures that were internally fixed within six weeks after injury, nine (15 per cent) had a delayed union and five (8.3 per cent) had a non-union, two of which persisted despite subsequent surgery. Three of the ten fractures that were internally fixed more than six weeks after injury never united d

A novel single-portal arthroscopic technique for the management of pediatric humeral lateral condylar fractures.

BACKGROUND: Surgical intervention is indicated for significantly displaced lateral humeral condyle fractures (LHCFs) in children. Arthroscopic-assisted closed reduction represents a minimally invasive alternative; however, its widespread adoption has been limited by the technical challenges inherent in both pediatric fracture management and elbow arthroscopy. This study introduces a simplified technique utilizing a single proximal anterolateral portal for arthroscopic-assisted reduction, which has shown promising efficacy and safety. METHODS: A retrospective analysis was performed on 18 pedia

Radiographic identification method of safe medial pinning zone and its clinical efficacy in pediatric supracondylar humeral fracture fixation.

OBJECTIVES: Preventing iatrogenic ulnar nerve injury remains a critical challenge during medial pinning in pediatric supracondylar humeral fractures (SCHFs). This study seeks to define a safe zone for medial pin insertion and evaluate its effectiveness in minimizing complications. METHODS: Three-dimensional reconstructions of radiographs from a healthy elbow joint were generated using E3d software to delineate a safe medial pinning zone. Pediatric patients with modified Gartland type III or IV SCHFs who underwent closed reduction and percutaneous crossed pin fixation were categorized into two

Outcomes of Displaced Greater Tuberosity Fractures Treated with Screw Fixation and Double Row Suture Bridge Technique.

INTRODUCTION: Closed treatment of comminuted, displaced greater tuberosity fractures (GTFs) can result in nonunion or malunion, causing rotator cuff dysfunction and/or subacromial impingement. This study evaluates the outcomes and complications of combined suture anchor and screw fixation technique for the treatment of GTFs. MATERIALS AND METHODS: This retrospective case series included 13 patients with displaced GTFs (> 5 mm) who underwent screw and double-row suture bridge fixation from 2019 to 2024 at a single institution. Patient demographics, injury mechanisms, fracture characteristics,
