Sleep Classification With Artificial Synthetic Imaging Data Using Convolutional Neural Networks.

OBJECTIVE: We propose a new analytic framework, "Artificial Synthetic Imaging Data (ASID) Workflow," for sleep classification from a wearable device comprising: 1) the creation of ASID from data collected by a non-invasive wearable device that permits real-time multi-modal physiological monitoring on heart rate (HR), 3-axis accelerometer, electrodermal activity, and skin temperature, denoted as "Temporal E4 Data" (TED) and 2) the use of an image classification supervised learning algorithm, convolutional neural network (CNN), to classify periods of sleep. METHODS: We investigate ASID Workflow

Computer-aided autism diagnosis using visual attention models and eye-tracking: replication and improvement proposal.

BACKGROUND: Autism Spectrum Disorder (ASD) diagnosis can be aided by approaches based on eye-tracking signals. Recently, the feasibility of building Visual Attention Models (VAMs) from features extracted from visual stimuli and their use for classifying cases and controls has been demonstrated using Neural Networks and Support Vector Machines. The present work has three aims: 1) to evaluate whether the trained classifier from the previous study was generalist enough to classify new samples with a new stimulus; 2) to replicate the previously approach to train a new classifier with a new dataset

Disentangled and Side-Aware Unsupervised Domain Adaptation for Cross-Dataset Subjective Tinnitus Diagnosis.

EEG-based tinnitus classification is a valuable tool for tinnitus diagnosis, research, and treatments. Most current works are limited to a single dataset where data patterns are similar. But EEG signals are highly non-stationary, resulting in model's poor generalization to new users, sessions or datasets. Thus, designing a model that can generalize to new datasets is beneficial and indispensable. To mitigate distribution discrepancy across datasets, we propose to achieve Disentangled and Side-aware Unsupervised Domain Adaptation (DSUDA) for cross-dataset tinnitus diagnosis. A disentangled auto

Expectation of clinical decision support systems: a survey study among nephrologist end-users.

BACKGROUND: Chronic kidney disease (CKD), a major public health problem with differing disease etiologies, leads to complications, comorbidities, polypharmacy, and mortality. Monitoring disease progression and personalized treatment efforts are crucial for long-term patient outcomes. Physicians need to integrate different data levels, e.g., clinical parameters, biomarkers, and drug information, with medical knowledge. Clinical decision support systems (CDSS) can tackle these issues and improve patient management. Knowledge about the awareness and implementation of CDSS in Germany within the fi

Wearable-Based Assessment of Heart Rate Response to Physical Stressors in Patients After Open-Heart Surgery With Frailty.

Due to frailty, cardiac rehabilitation in older patients after open-heart surgery must be carefully tailored, thus calling for informative and convenient tools to assess the effectiveness of exercise training programs. The study investigates whether heart rate (HR) response to daily physical stressors can provide useful information when parameters are estimated using a wearable device. The study included 100 patients after open-heart surgery with frailty who were assigned to intervention and control groups. Both groups attended inpatient cardiac rehabilitation however only the patients of the

Temporal Micro-Action Localization for Videofluoroscopic Swallowing Study.

Videofluoroscopic swallowing study (VFSS) visualizes the swallowing movement by using X-ray fluoroscopy, which is the most widely used method for dysphagia examination. To better facilitate swallowing assessment, the temporal parameter is one of the most important indicators. However, most information of that acquire is hand-crafted and elaborated, which is time-consuming and difficult to ensure objectivity and accuracy. In this article, we propose to formulate this task as a temporal action localization task and solve it using deep neural networks. However, the action of VFSS has the followin

Analysis of a Deep Learning Model for 12-Lead ECG Classification Reveals Learned Features Similar to Diagnostic Criteria.

Despite their remarkable performance, deep neural networks remain unadopted in clinical practice, which is considered to be partially due to their lack of explainability. In this work, we apply explainable attribution methods to a pre-trained deep neural network for abnormality classification in 12-lead electrocardiography to open this "black box" and understand the relationship between model prediction and learned features. We classify data from two public databases (CPSC 2018, PTB-XL) and the attribution methods assign a "relevance score" to each sample of the classified signals. This allows

Cluster-Re-Supervision: Bridging the Gap Between Image-Level and Pixel-Wise Labels for Weakly Supervised Medical Image Segmentation.

Weakly supervised learning, releasing deep learning from highly labor-intensive pixel-wise annotations, has gained great attention, especially for medical image segmentation. With only image-level labels, pixel-wise segmentation/localization usually is achieved based on class activation maps (CAMs) containing the most discriminative regions. One common consequence of CAM-based approaches is incomplete foreground segmentation, i.e. under-segmentation/false negatives. Meanwhile, suffering from relatively limited medical imaging data, class-irrelevant tissues can hardly be suppressed during class

Sequential Active Contour Based on Morphological-Driven Thresholding for Ultrasound Image Segmentation of Ascites.

Paracentesis is a high-demanding and routine operation, which has great potentials and benefits if semi-autonomous procedures can be developed. One of the most important techniques that facilitate semi-autonomous paracentesis is to segment the ascites from ultrasound images accurately and efficiently. The ascites, however, is usually with significantly different shapes and noise among different patients, and its shape/size changes dynamically during the paracentesis. This makes most of existing image segmentation methods either time consuming or inaccurate for segmenting ascites from its backg

A Real-Time Bionic Method Inspired by Neural Oscillators for Estimation and Extraction of Pathological Tremor.

The typical representative of pathological tremor is Parkinson's disease. One of the pathogenesis is that the synchronized neural oscillations within and between brain areas are affected. Inspired by this, this work proposes an algorithm based on neural oscillator to extract voluntary motion and estimate tremor motion in real time, which is named as RTBNO. This algorithm is composed of multiple adaptive modified Hopf oscillators linear combiner. The combiner is divided into two parts: one is used to estimate tremor motion and the other is applied to estimate voluntary motion. As it is updated

Diagnosis of Peripheral Artery Disease Using Backflow Abnormalities in Proximal Recordings of Accelerometer Contact Microphone (ACM).

OBJECTIVE: The development of an accurate, non-invasive method for the diagnosis of peripheral artery disease (PAD) from accelerometer contact microphone (ACM) recordings of the cardiac system. METHODS: Mel frequency cepstral coefficients (MFCCs) are initially extracted from ACM recordings. The extracted MFCCs are then used to fine-tune a pre-trained ResNet50 network whose middle layers provide streams of high-level-of-abstraction coefficients (HLACs) which could provide information on blood pressure backflow caused by arterial obstructions in PAD patients. A vision transformer is finally int

Comparison of decision tree with common machine learning models for prediction of biguanide and sulfonylurea poisoning in the United States: an analysis of the National Poison Data System.

BACKGROUND: Biguanides and sulfonylurea are two classes of anti-diabetic medications that have commonly been prescribed all around the world. Diagnosis of biguanide and sulfonylurea exposures is based on history taking and physical examination; thus, physicians might misdiagnose these two different clinical settings. We aimed to conduct a study to develop a model based on decision tree analysis to help physicians better diagnose these poisoning cases. METHODS: The National Poison Data System was used for this six-year retrospective cohort study. The decision tree model, common machine learning

Leveraging Unlabelled Data in Multiple-Instance Learning Problems for Improved Detection of Parkinsonian Tremor in Free-Living Conditions.

Data-driven approaches for remote detection of Parkinson's Disease and its motor symptoms have proliferated in recent years, owing to the potential clinical benefits of early diagnosis. The holy grail of such approaches is the free-living scenario, in which data are collected continuously and unobtrusively during every day life. However, obtaining fine-grained ground-truth and remaining unobtrusive is a contradiction and therefore, the problem is usually addressed via multiple-instance learning. Yet for large scale studies, obtaining even the necessary coarse ground-truth is not trivial, as a

Design and Evaluation of Deep Learning Models for Continuous Acute Pain Detection Based on Phasic Electrodermal Activity.

The current method for assessing pain in clinical practice is subjective and relies on self-reported scales. An objective and accurate method of pain assessment is needed for physicians to prescribe the proper medication dosage, which could reduce addiction to opioids. Hence, many works have used electrodermal activity (EDA) as a suitable signal for detecting pain. Previous studies have used machine learning and deep learning to detect pain responses, but none have used a sequence-to-sequence deep learning approach to continuously detect acute pain from EDA signals, as well as accurate detecti

Early Prediction of Impending Exertional Heat Stroke With Wearable Multimodal Sensing and Anomaly Detection.

We employed wearable multimodal sensing (heart rate and triaxial accelerometry) with machine learning to enable early prediction of impending exertional heat stroke (EHS). US Army Rangers and Combat Engineers (N = 2,102) were instrumented while participating in rigorous 7-mile and 12-mile loaded rucksack timed marches. There were three EHS cases, and data from 478 Rangers were analyzed for model building and controls. The data-driven machine learning approach incorporated estimates of physiological strain (heart rate) and physical stress (estimated metabolic rate) trajectories, followed by rec

M-CSAFN: Multi-Color Space Adaptive Fusion Network for Automated Port-Wine Stains Segmentation.

Automatic segmentation of port-wine stains (PWS) from clinical images is critical for accurate diagnosis and objective assessment of PWS. However, this is a challenging task due to the color heterogeneity, low contrast, and indistinguishable appearance of PWS lesions. To address such challenges, we propose a novel multi-color space adaptive fusion network (M-CSAFN) for PWS segmentation. First, a multi-branch detection model is constructed based on six typical color spaces, which utilizes rich color texture information to highlight the difference between lesions and surrounding tissues. Second,

A Comparative Effectiveness Study on Opioid Use Disorder Prediction Using Artificial Intelligence and Existing Risk Models.

Opioid use disorder (OUD) is a leading cause of death in the United States placing a tremendous burden on patients, their families, and health care systems. Artificial intelligence (AI) can be harnessed with available healthcare data to produce automated OUD prediction tools. In this retrospective study, we developed AI based models for OUD prediction and showed that AI can predict OUD more effectively than existing clinical tools including the unweighted opioid risk tool (ORT). Data include 474,208 patients' data over 10 years; 269,748 were females with an average age of 56.78 years. Cases ar
