The authors present their own two-year experience with osteosynthesis of 54 fractures of the diaphysis of the leg in 51 patients, using a UTN nail without predrilling, at the Orthopaedic Clinic of the Institute for Postgraduate Medical Training in Prague Bulovka Hospital. We report on two sisters affected by congenital alopecia, nail dystrophy, and a severe T-cell immunodeficiency, presumably inherited as an autosomal-recessive disorder. We reviewed 63 patients with fractures of the distal tibial metaphysis, with or without minimally displaced extension into the ankle joint. The fractures had been caused by two distinct mechanisms, either a direct bending force or a twisting injury. This influenced the pattern of the fracture and its time to union. Ten patients with an acute tibial shaft fracture complicated by a compartment syndrome were treated with immediate decompressive fasciotomies, bony stabilization with a locked unreamed intramedullary nail, and early active joint motion. There were seven closed and three open injuries. All fractures healed. Knee motion was excellent, but ankle and subtalar motion were not uniformly normal. Nine patients were without any long-term neurologic sequelae. This treatment protocol simplifies the bony and soft tissue management in patients with this combination of injuries.

Five months after internal fixation, the patient developed a Staphylococcus aureus infection of the knee, which resolved after a 42-day course of IV antibiotics. Twelve patients, presenting with traumatic avulsion of the distal nail bed, repaired by a de-epithelialized palmar advancement flap, were reviewed with an average follow-up of 23 months. In a prospective randomised trial between September 1989 and June 1990 one hundred patients with per- and subtrochanteric fractures were consecutively treated by gamma-nail or DHS. After a review of the cases (57 between 1981 and 1988) the authors present the results of treatment of diaphyseal fractures of the lower limb by Grosse Kempf locked intramedullary nailing. Heterotopic ossification at the site of insertion of an intramedullary nail was observed in five central nervous system-injured patients with femoral fractures. Three of these patients experienced a reduction in the range of motion of the ipsilateral hip joint, which in two improved with physical therapy. Patients with central nervous system injury who have closed interlocking intramedullary nail fixation may be at increased risk of heterotopic ossification at the surgical site.

One hundred seventy-two patients with 204 slipped capital femoral epiphyses treated with nailing or pinning were evaluated an average of 28 years after surgery. Follow-up evaluation was obtained by questionnaire in 153 cases (181 hips) and by clinical and radiographic reexamination in 132 cases (157 hips). While intramedullary fixation may have less operative morbidity than sliding plate systems, there also may be a loss of stability and strength of the fixation. In a biomechanical study with simulated unstable intertrochanteric fractures in cadaver femurs, multiple Ender's pins were 18% stronger than the single Harris nail. Compression hip screws were three times stronger than the Harris nail and two and one-half times stronger than Ender's pins. The compression hip screw was five times more rigid than either condylocephalic system.

Intramedullary osteosynthesis is preferred for shaft fractures of the long bones of the lower extremities because it generally results in early weight-bearing stability, allowing full function of the leg and rapid rehabilitation. Records were reviewed on 445 patients with fracture of the hip admitted to St. Paul's Hospital, Vancouver, Canada, during the period 1973--1975 inclusive. The epidemiologic factors were analyzed: age, sex, where and when the accident occurred, site of the fracture, the treatment procedure, length of time in hospital, complications, associated conditions, and final placement of the patient.
