The real burden of restless legs syndrome: clinical and economic outcomes.

Restless legs syndrome (RLS) is a highly prevalent and substantially underdiagnosed sensorimotor disorder. Only relatively recently have the large impact on patient quality of life (QoL) and the economic burden associated with RLS become more widely recognized. QoL in patients with RLS has been shown to be worse than that of many other chronic conditions, including type 2 diabetes, clinical depression, and osteoarthritis. Sleep disturbance, a cardinal feature of RLS, is the most common and most destructive of its symptoms. More than two-thirds of RLS patients experience serious insomnia, and w

Efficacy of mirtazapine in obstructive sleep apnea syndrome.

STUDY OBJECTIVES: Decreased serotonergic facilitation of upper-airway motor neurons during sleep has been postulated as an important mechanism rendering the upper airway vulnerable to obstruction in patients with obstructive sleep apnea syndrome (OSA). Although serotonin reuptake inhibitors have been shown to produce modest reductions in the apnea-hypopnea index (AHI) during non-rapid eye movement (NREM) sleep, they have not been proven to be generally effective as treatments for OSA. Conversely, antagonists of type 3 (5-HT3) serotonin receptors effectively have been shown to reduce the freque

Sleep and circadian parameters in Behçet's syndrome: a comparative analysis using actigraphy and questionnaires.

OBJECTIVE: Sleep disturbances are highly prevalent in Behçet syndrome (BS) patients. Within this population, sleep disturbances are frequently associated with active disease and comorbid fibromyalgia. However, possible sleep impairments in BS patients without these conditions remain poorly explored, along with BS patients' obstructive sleep apnoea syndrome (OSAS) risk and circadian rhythm preferences. We aimed to address these research gaps through a cross-sectional study comparing sleep and circadian parameters between BS patients, with or without active disease and comorbid fibromyalgia, and

Association between mental health and sleep quality among Forcibly Displaced Rohingya pregnant women living in Cox's Bazar, Bangladesh: a cross-sectional observation.

BACKGROUND: Poor sleep quality during pregnancy is associated with adverse health and developmental outcomes for both mother and fetus. Various factors may influence sleep quality, including mental health problems, stressful life circumstances, pregnancy, illness, and socio-demographic conditions. These links have been explored in refugees and asylees residing in high-income countries. There is a notable gap in the literature regarding the association of mental health and sleep quality among conflict and displacement-affected populations residing in low- and middle-income countries and exposed

Examining the between- and within-person associations among perceived sleep quality and mental health symptoms in emergency medical service clinicians.

Disturbed sleep is a known risk factor for heightened mental health symptoms, and this association may be particularly problematic among emergency medical service (EMS) clinicians. Yet, associations between daily sleep quality and daily mental health symptoms are understudied among this vulnerable group. We used ecological momentary assessments to examine between- and within-person associations between perceived sleep quality and mental health symptoms in 79 EMS clinicians employed at a large agency in central New York. Participants completed eight daily assessments (558 total) on perceived sl

Daily associations between sleep quality, stress, and cannabis or alcohol use among veterans.

United States military veterans face heightened vulnerability to sleep disturbances due to factors such as irregular sleep schedules, combat-related stress, and co-occurring mental health disorders. These sleep disturbances are often exacerbated by substance use, including alcohol and cannabis, as veterans may rely on these substances to self-medicate for stress and sleep issues. However, the interplay between sleep quality, substance use, and perceived stress remains poorly understood, particularly on a day-to-day basis. This study aimed to explore the dynamic associations between these facto

Effect of vagus nerve stimulation (taVNS) on anxiety and sleep disturbances among elderly health care workers in the post COVID-19 pandemic.

BACKGROUND: Healthcare workers (HCWs) have been significantly impacted by the pandemic. Elderly health care workers carry out a variety of duties at work and have years of clinical expertise. Anxiety and insomnia are among the more commonly encountered problems in senior physicians and other geriatric medical professional populations. OBJECTIVE: The study aims to determine the effect of vagal nerve stimulation on anxiety and sleep disturbances among geriatric medical professionals. METHODS: 42 participants were enrolled in this study based on the inclusion and exclusion criteria. The partici

Association of β-Amyloid Burden With Sleep Dysfunction and Cognitive Impairment in Elderly Individuals With Cognitive Disorders.

IMPORTANCE: Evidence shows that sleep dysfunction and β-amyloid (Aβ) deposition work synergistically to impair brain function in individuals with normal cognition, increasing the risk of developing dementia later in life. However, whether Aβ continues to play an integral role in sleep dysfunction after the onset of cognitive decline in individuals with dementia is unclear. OBJECTIVE: To determine whether Aβ deposition in the brain is associated with subjective measures of sleep quality and cognition in elderly individuals with cognitive disorders. DESIGN, SETTING, AND PARTICIPANTS: A nested

Sleep and eating disorders among adults enrolled in a commercial weight loss program: associations with self-report and objective sleep measures.

PURPOSE: Some research suggests that eating disorders are related to poor sleep health. To increase knowledge on the relation between sleep and eating disorders, we used a multi-method approach to examine associations between sleep, chronotype, and eating disorder psychopathology. METHODS: We investigated associations between ED psychopathology, both diagnostic categories (ascertained through self-report data) and dimensional measures, and self-report and ambulatory measures of sleep. Adults currently enrolled in a commercial weight loss program completed self-report measures as well as 1 wee

Sleep in remitted bipolar disorder: a naturalistic case-control study using actigraphy.

INTRODUCTION: Findings from actigraphic studies suggesting that sleep and circadian rhythms are disrupted in bipolar disorder (BD) patients have been undermined by methodological heterogeneity and the failure to adequately address potential confounders. METHOD: Twenty-six euthymic BD cases and 29 healthy controls (HC), recruited from University Paris-Est and matched for age and gender, were compared on subjective (Pittsburgh Sleep Questionnaire Inventory; PQSI) and objective (mean scores and variability in actigraphy) measures of sleep as recorded by over 21 consecutive days. RESULTS: Multiv

Sleep disorders in chronic liver disease.

Sleep-related complaints and disturbances are increasingly recognized in the setting of chronic liver disease and have recently been shown to be an important prognostic factor in patients with advanced chronic liver disease. This article reviews the literature surrounding sleep disturbances and disorders in a variety of types of chronic liver disease. This includes the association of sleep disturbances with hepatitis C and antiviral therapy, primary biliary cirrhosis, and Wilson disease as well as the circadian rhythm abnormalities present in cirrhosis and hepatic encephalopathy. Sleep disorders in aging and dementia.

This paper reviews the characteristics of sleep disorders found in people at a greater risk of dementia: the elderly adult, patients with mild cognitive impairment (MCI) and those with neurodegenerative diseases. The frequency of sleep architecture modifications and circadian rhythm sleep disturbances increases with age. Although around 40% of older adults complain of poor sleep, true sleep disorders are far less prevalent in healthy older adults and are frequently associated with comorbidities. The sleep disorders observed in Alzheimer's disease (AD) patients are often similar to (but more in

Screening and evaluation of sleep disorders in children and adolescents.

Most pediatric professionals do not use a systematic screening, identification, or referral process to detect serious sleep problems and disorders in children and adolescents. Therefore, only 2% to 3% of children with sleep disorders are identified and treated. This article presents a screening, referral, and diagnostic process that helps detect and correct sleep disorders that impact children's learning, behavioral and emotional functioning, and health.

Sleep habits in Nigerian undergraduates.

BACKGROUND: Quantity of night sleep is shorter than 8 h in several developed countries, but similar data is not available for most African countries. The objective of this study was to describe the quantity of night sleep, factors that are associated with non-restorative sleep, and sleep habits in a population of undergraduates in Nigeria. METHODS: Questionnaires were used to collect information about bedtimes, waketimes, intra-night awakenings, non-restorative sleep, and afternoon naps over a period of 14 days. RESULTS: Mean duration of night sleep was 6.2 h (median 6.0, range 4.5-9.3), whi

Hypersomnia following paramedian thalamic stroke: a report of 12 patients.

Paramedian thalamic stroke (PTS) is a cause of organic hypersomnia, which in the absence of systematic sleep-wake studies has been attributed to disruption of ascending activating impulses and considered a "dearoused" state. However, an increasing mount of data suggests a role of the thalamus in sleep regulation and raises the possibility that a sleep disturbance contributes to hypersomnia in PTS. We evaluated 12 patients with magnetic resonance imaging-proven isolated PTS and hypersomnia with 10 to >20 hours of sleep behavior per day. Nocturnal polysomnographic findings paralleled the severit

Sleep, excessive daytime sleepiness and fatigue in Parkinson's disease.

The objective of this questionnaire-based survey was to evaluate the prevalence and causes of sleep disturbances in 90 nondepressive patients with Parkinson's disease (PD) and 71 age-matched healthy subjects. We also assessed the prevalence and characteristics of excessive daytime sleepiness (both groups) and excessive fatigue (PD patients). A high prevalence of sleep disturbances in PD patients was found; this is to a large extent probably the result of aging. As compared with controls, patients had a more severely disturbed sleep maintenance because of nycturia, pain, stiffness, and problems

Anthropometric characteristics, cardiac abnormalities and adrenergic activity in patients with primary disorders of sleep.

Sixty-one patients (pts) with sleep apnea (SA), 35 with narcolepsy (N) and 24 with idiopathic hypersomnolence (H) were studied in the Clinical Research Center. The height to body weight ratio was less than normal in SA pts (0.32 +/- 0.01 vs 0.45 +/- 0.01, p less than 0.01), but normal in N and H pts (0.45 +/- 0.02 and 0.45 +/- 0.01, respectively). Twenty-four hour urinary epinephrine (E) plus norepinephrine (NE) was greater than normal (p less than 0.01), but not different among SA, N and H pts. The incidence of mitral valve prolapse (MVP) was greater in N (49%) and H (58%) compared to SA (20%

Excessive daytime sleepiness: a 1-year study in an unselected inpatient population.

A prospective epidemiological investigation of excessive daytime sleepiness (EDS) was carried out in an unselected inpatient population admitted to a general hospital during a 1-year period. The study comprised 2518 patients, 1347 female and 1171 male, aged 6-92 years (mean, 55.2). On the basis of histories and clinical and polysomnographic data, EDS was found in 28 cases (1.11%). Of these, 25 (0.99%; 18 female and 7 male; mean age, 61.3) had sleep apnea syndromes (SAS) with predominantly obstructive apnea. Two patients (0.07%; one female and one male; mean age, 65.5) had idiopathic CNS hypers

Obstructive sleep apnea in family members.

Two sons and their father had severe hypersomnolence and obstructive sleep apnea. A third son, although asymptomatic, was shown to have upper-airway obstruction during sleep. Electromyographic recordings of genioglossus activity in the two symptomatic sons revealed loss of tonic activity in early stages of sleep at times when sleep apnea occurred. The asymptomatic son showed loss of tonic activity during rapid-eye-movement sleep, the sleep period when upper-airway obstruction occurred. Two sudden deaths occurred in this family. A 30-year-old brother died at home while asleep, and a child of th
