Tubeless uniportal subxiphoid thoracoscopic thymectomy via percutaneous suspension technique for anterior mediastinal masses.

OBJECTIVE: We previously developed the percutaneous suspension technique to improve the anterior mediastinal exposure in minimally invasive thymectomy. In this study, we aimed to integrate this technique with tubeless video-assisted thoracoscopic surgery (VATS) for uniportal subxiphoid thoracoscopic thymectomy and to evaluate perioperative outcomes of this combined approach. METHODS: This retrospective, multicenter study included patients with anterior mediastinal masses who underwent uniportal subxiphoid thoracoscopic thymectomy using the percutaneous suspension technique combined with tubel

Local flaps for partial breast reconstruction.

INTRODUCTION: Breast-conserving surgery (BCS) is often the first-line treatment for early-stage invasive breast cancer. Many studies claim that survival is non-inferior to radical treatments in selected cases. Although BCS preserves some breast tissue and often the nipple-areola complex, asymmetry and distortion of the breast contour may occur and reconstruction is sometimes required. The authors' aim is to conduct a review of main breast reconstruction options with local flaps after partial mastectomy. EVIDENCE ACQUISITION: A literature review was conducted on PubMed using the keywords: "par

Mechanical stress in a solid ellipsoid model of the lung after thoracoscopic surgery for spontaneous pneumothorax.

BACKGROUND: Newly formed bullae after video-assisted thoracoscopic surgery (VATS) bullectomy in primary spontaneous pneumothorax (PSP) are an important etiology for recurrence and are associated with mechanical stress along the stapling line. However, the distribution or pattern of stress after VATS bullectomy has not been thoroughly investigated. Our aim was to analyze the stress distribution following lung resection. METHODS: Using finite element method analyses in COMSOL Multiphysics software to evaluate the stress distribution along the stapling line, a solid ellipsoidal lung model was co

Computed tomography-guided suture anchor localizer placement for multiple pulmonary nodule localization.

INTRODUCTION: Computed tomography (CT)-guided suture anchor localizer (SAL) placement is increasingly used to facilitate preoperative localization of pulmonary nodules (PNs) before video-assisted thoracoscopic surgery (VATS). Although this approach is well established for single nodules, evidence regarding its application in multiple nodules remains limited. AIM: We aimed to evaluate the safety and efficacy of CT-guided SAL placement for simultaneous localization of multiple PNs. MATERIALS AND METHODS: A 2-center retrospective study was conducted enrolling patients who underwent CT-guided SA

A New Renorrhaphy Technique Using Barbed Sutures and Elongated Pledgets for Robotic Partial Nephrectomy.

BACKGROUND: Effective hemostasis and renorrhaphy with intracorporeal suturing are major technical challenges in robot-assisted partial nephrectomy (RAPN). This report outlines the authors' experience with a novel RAPN approach using renorrhaphy with barbed sutures (BSs) and elongated pledgets (R-BEP) in a video. METHODS: The R-BEP procedure comprised several steps, including a single layer of monofilament adsorbable running BSs used to close the renal parenchyma, reinforced by elongated polytetrafluoroethylene (PTFE) pledgets. Two PTFE pledgets, approximately 8 to 10 mm in width, were cut to

Massive spontaneous pneumothorax with mediastinal shift in a 14-year-old with pulmonary tuberculosis: A case report.

INTRODUCTION AND IMPORTANCE: Spontaneous pneumothorax is a life-threatening complication of pulmonary tuberculosis (TB), particularly in endemic regions. Secondary spontaneous pneumothorax (SSP) occurs due to underlying lung pathology, such as cavitary lesions in TB. This case highlights the presentation, diagnosis, and management of SSP in a TB-endemic setting. CASE PRESENTATION: A 14-year-old male, presented with acute dyspnea, pleuritic chest pain, and a history of chronic cough and weight loss. Clinical examination revealed diminished breath sounds and hyperresonance on the left hemithora

Application of methylene blue injection staining in the diagnosis of hepatic pleural effusion in patients with cirrhosis-a case report and literature review.

BACKGROUND: Hepatic pleural effusion is a common complication in patients with cirrhosis. The diagnosis of hepatic pleural effusion is limited by many factors. There are advantages and disadvantages to using radioisotope examination, contrast-enhanced ultrasound and video-assisted thoracoscopy surgery in detecting diaphragmatic defects. We reported that methylene blue injection was injected into the peritoneal cavity, and pleural effusion was turned pale blue with methylene blue. We inferred diaphragmatic rupture in patients with hepatic pleural effusion. Methylene blue injection staining is a

Surgical technique for uniportal VATS right S10 segmentectomy with the pulmonary ligament approach, performing the intersegmental division last.

Segmentectomy requires the transection of the pulmonary artery, pulmonary vein, and bronchus in the segment. These are generally dissected from the center to the periphery. In S10 segmentectomy, the textbook procedure is to address A10 and B10 from the interlobar side. However, this procedure requires some dissection of the lung parenchyma up to the branching point of A10 and B10, which increases the risk of alveolar air leak. In this report, we performed a uniportal video-assisted thoracoscopic surgery (VATS) right S10 resection from the pulmonary ligament side without interlobar intervention

Recurrent Spontaneous Pneumothorax as a Manifestation of the Birt-Hogg-Dubé Syndrome: A Case Report.

Birt-Hogg-Dubé syndrome (BHDS) is a rare autosomal dominant disorder caused by mutations in the folliculin (FLCN) gene, characterized by a clinical triad of pulmonary cysts with spontaneous pneumothorax, cutaneous fibrofolliculomas, and renal tumors. Pulmonary manifestations may occur in isolation, making early diagnosis challenging. This report describes the case of a 36-year-old woman with a history of right-sided spontaneous pneumothorax, managed with video-assisted thoracoscopic surgery (VATS) and pleurodesis, who re-presented seven years later with a left-sided pneumothorax. Conservative

Inferior pulmonary ligament dissection after thoracoscopic bullectomy for primary pneumothorax prevents recurrence.

BACKGROUND: The recurrence of primary spontaneous pneumothorax (PSP) after video-assisted thoracoscopic surgery (VATS) is correlated with larger postoperative residual apical pleural space (RAPS). We hypothesized that inferior pulmonary ligament dissection (IPLD) would facilitate postoperative lung expansion and lower the recurrence. METHODS: We retrospectively reviewed PSP patients who underwent VATS with or without IPLD from September 2009 to December 2016 at Ditmanson Medical Foundation Chia-Yi Christian Hospital. The primary endpoint was postoperative recurrence. Secondary endpoint was RA

Complications of Coexisting Thymoma and Myelodysplastic Syndrome: A Case Report.

A thymoma is a rare malignant tumor of the thymus gland, often associated with local invasion, recurrence, and autoimmune disorders. The interplay between thymoma and myelodysplastic syndrome (MDS) represents a complex clinical phenomenon, yet its underlying mechanisms and optimal management strategies remain incompletely understood. We present the case of a 69-year-old male with recurrent thymoma, initially classified as type B1/AB, complicated by the subsequent development of MDS. The patient, who had worked as a plumber for over 20 years, had previously undergone robotic video-assisted thor

Carbon dioxide gas-induced pneumothorax versus one-lung ventilation in minimally invasive esophagectomy: a multicenter propensity score matching cohort study.

BACKGROUND: Carbon dioxide gas-induced pneumoperitoneum might be the reason for the shorter postoperative survival of patients with malignant tumors. Whether CO 2 gas-induced pneumothorax has unfavorable impacts on the surgical and oncological outcomes of minimally invasive esophagectomy remains unclear. METHODS: Between 2010 and 2016, a total of 998 patients with squamous cell carcinoma of the esophagus who received video-assisted surgery were registered from three large-volume medical centers. The overall survival (OS) and disease-free survival (DFS) were compared after using propensity sco…

A Unique Association of Right-Sided Aortic Arch With Kommerell's Diverticulum, Diffuse Idiopathic Pulmonary Neuroendocrine Cell Hyperplasia, and Typical Carcinoid: A Case Report.

Our case presents a unique occurrence marking the first documentation of a connection between a typical carcinoid in the context of diffuse idiopathic pulmonary neuroendocrine cell hyperplasia (DIPNECH) and vascular anomalies, including a right-sided aortic arch with Kommerell's diverticulum. Kommerell's diverticulum is a rare congenital anomaly. The lusoria subclavian artery, another developmental anomaly, arises from the right aortic arch instead of the typical left side. Neuroendocrine cells may contribute to lung disease pathogenesis by altering their physiology before clinical symptoms ap
