Systemic Sclerosis Presenting With Chronic Atrophic Gastritis and Retinal Artery Occlusion: A Case Report.

Systemic sclerosis (SSc), also known as scleroderma, is a rare autoimmune disease characterized by fibroproliferative changes, vascular dysfunction, and small-vessel proliferation. It can affect multiple organ systems, including the gastrointestinal tract, skin, heart, eyes, kidneys, and lungs. We report a case of a 67-year-old woman with systemic sclerosis who tested negative for specific antibodies and simultaneously developed two rare complications: autoimmune atrophic gastritis resulting in megaloblastic anemia and acute retinal artery occlusion. Notably, she presented with megaloblastic a

Choroidal metastases and exudative retinal detachment in medullary thyroid carcinoma.

The uvea is the most common site for ocular metastasis. Within this, the choroid is commonly involved as compared with the iris and the ciliary body. This is due to the distribution of blood supply, which is greater in the choroid than in the iris and ciliary body. Ocular metastasis mostly occurs from breast and lung cancer. Choroidal metastasis from thyroid carcinoma is rare. The gold standard test for diagnosis of choroidal metastasis is fluorescein angiography. We present the case of a man in his early 30s who reported a concern of diminished vision in the right eye for 2 weeks. Fundus exam

Presumed Ocular tuberculosis masquerading as autoimmune retinopathy.

PURPOSE: To report a case of tuberculosis (TB) infection with an unusual posterior segment manifestation that mimicked autoimmune retinopathy (AIR). OBSERVATION: A 36-year-old male presented with blurred vision and constriction of visual field (VF) in both eyes for over 2 weeks. Multimodal imaging suggested he had AIR-like retinopathy; however, the initial local/systemic steroid treatment worsened his condition. He later tested positive for TB infection and received one month of anti-tuberculosis (ATT) monotherapy, followed by combined steroid treatment for an additional 5 months. He was foll

Pyogranulomatous endophthalmitis and posterior uveitis in a crested capuchin (Sapajus robustus) with systemic Paracoccidioides brasiliensis infection.

Herein, we describe ocular abnormalities in an adult male Crested Capuchin (Sapajus robustus) with partial unilateral loss of vision. On necropsy, there was a white, firm, and homogeneous plaque expanding the ciliary body of the left eyeball. White, irregular and firm nodules were observed in skeletal muscles, lymph nodes, lungs, heart, spleen, ileum, cecum, pancreas, and testes. Microscopically, diffuse severe pyogranulomatous endophthalmitis in the left eyeball and mild granulomatous posterior uveitis in the right eyeball, both with intralesional yeasts, were noted. Immunohistochemistry reve

[Development of an abdominal acupoint localization system based on AI deep learning].

This study aims to develop an abdominal acupoint localization system based on computer vision and convolutional neural networks (CNNs). To address the challenge of abdominal acupoint localization, a multi-task CNNs architecture was constructed and trained to locate the Shenque (CV8) and human body boundaries. Based on the identified Shenque (CV8), the system further deduces key characteristics of four acupoints: Shangwan (CV13), Qugu (CV2), and bilateral Daheng (SP15). An affine transformation matrix is applied to accurately map image coordinates to an acupoint template space, achieving precis

Intra-axial Cortical-Based Tumour Presented as Homonymous Hemianopia in a Young Patient: A Diagnostic Dilemma.

Intra-axial cortical-based tumours are rare tumours affecting children and young adults. These tumours can be classified as either low-grade or high-grade, depending on their aggressiveness and rate of growth. We report a case of homonymous hemianopia secondary to an intra-axial cortical-based tumour in a young patient. A 26-year-old lady presented with bilateral blurring of vision for three weeks associated with a headache. Visual acuity was 6/6 in both eyes. Bilateral optic nerve functions were normal. The Humphrey visual field test showed left-homonymous hemianopia. A CT scan and MRI of the

Pediatric refractive surgery: Review article.

Refractive surgery is one of the most commonly performed elective procedures in ophthalmology; it is mostly performed in patients who are older than 18 years of age. This routine procedure has been time-tested and is proven to be both safe and effective in adults. However, little is known about the safety and efficacy of refractive surgery in pediatric patients. Granted, refractive surgery should not be used in kids merely for the purpose of avoiding spectacles wear, but it can be of valuable help is preserving vision in patients with amblyopia and accommodative esotropia who otherwise couldn'

Phenotypic variability in two siblings with Poretti-Boltshauser syndrome.

INTRODUCTION: Poretti-Boltshauser Syndrome (PBS) is a rare neuro-ophthalmological disorder with autosomal recessive inheritance. It is characterized by non-progressive cerebellar ataxia, delay in neuropsychomotor development, intellectual disability, and vision abnormalities. PBS is caused by mutations in the LAMA1 gene, resulting in cerebellar abnormalities, including cerebellar cysts in most cases. CASE PRESENTATION: We present two siblings with LAMA1 mutations and distinct phenotypic presentation, with one of them showing no evidence of cerebellar cysts on magnetic resonance imaging (MRI).

[Clinical features of CAPOS syndrome caused by maternal ATP1A3 gene variation: a case report].

CAPOS syndrome is an autosomal dominant neurological disorder caused by mutations in the ATP1A3 gene. Initial symptoms, often fever-induced, include recurrent acute ataxic encephalopathy in childhood, featuring cerebellar ataxia, optic atrophy, areflflexia, sensorineural hearing loss, and in some cases, pes cavus. This report details a case of CAPOS syndrome resulting from a maternal ATP1A3 gene mutation. Both the child and her mother exhibited symptoms post-febrile induction, including severe sensorineural hearing loss in both ears, ataxia, areflexia, and decreased vision. Additionally, the pa

Comment on Dutta Majumder et al.'s 2019 'Ocular Syphilis: An Update'.

The article 'Ocular Syphilis: An Update (2019)' provides a thorough review of the challenges in diagnosing ocular syphilis. However, our research on 'Dermatological and Ocular Manifestations of Syphilis,' identifies a significant gap in both literature and clinical practice: the lack of recognition of dermatological signs during ophthalmological assessments. Ocular syphilis often mimics other conditions and can remain undiagnosed for months or years. Detecting dermatological signs, such as the characteristic palmar rash of secondary syphilis and extragenital chancres, could prompt earlier inve

Weil's disease in a temperate climate: diagnostic uncertainty and the importance of prompt antibiotic treatment.

A man in his 50s presents with a short history of rigors, back pain and dark urine. This was associated with scleral icterus. He was initially treated as urosepsis due to perinephric fat stranding on his first CT but continued to deteriorate with worsening sepsis requiring intensive care admission. He had a conjugated hyperbilirubinaemia (peak 708 µmol/L) with normal liver enzymes, anaemia, thrombocytopaenia, acute kidney injury requiring filtration and respiratory failure requiring ventilatory support. A subsequent CT revealed mediastinal lymphadenopathy and extensive ground-glass changes wit

Blue cone monochromacy and gene therapy.

Blue cone monochromacy (BCM) is a congenital vision disorder characterized by complete loss or severely reduced long- and middle-wavelength cone function, caused by mutations in the OPN1LW/OPN1MW gene cluster on the X-chromosome. BCM patients typically suffer from poor visual acuity, severely impaired color discrimination, myopia, and nystagmus. In this review, we cover the genetic causes of BCM, clinical features of BCM patients, genetic testing, and clinical outcome measurements for future BCM clinical trials. However, our emphasis is on detailing the animal models for BCM and gene therapy u

Microglial and macroglial dynamics in a model of retinitis pigmentosa.

In retinal degenerative diseases, such as retinitis pigmentosa (RP), the characteristic photoreceptor cell death is associated with changes of microglia and macroglia cells. Gene therapy, a promising treatment option for RP, is based on the premise that glial cell remodeling does not impact vision rescue. However, the dynamics of glial cells after treatment at late disease stages are not well understood. Here, we tested the reversibility of specific RP glia phenotypes in a Pde6b-deficient RP gene therapy mouse model. We demonstrated an increased number of activated microglia, retraction of mic

Optimization of Capillary-Based Western Blotting for MYO7A.

Myosin VIIA (MYO7A)-associated Usher syndrome type 1B (USH1B) is a severe disorder that impacts the auditory, vestibular, and visual systems of affected patients. Due to the large size (~7.5 kb) of the MYO7A coding sequence, we have designed a dual adeno-associated virus (AAV) vector-based approach for the treatment of USH1B-related vision loss. Due to the added complexity of dual-AAV gene therapy, careful attention must be paid to the protein products expressed following vector recombination. In order to improve the sensitivity and quantifiability of our immunoassays, we adapted our tradition

Pseudoxanthoma elasticum and retinitis pigmentosa in a patient with a novel mutation in the ABCC6 gene.

BACKGROUND: Pseudoxanthoma elasticum (PXE) is an autosomal recessive condition caused by mutations in the ABCC6 gene. Ocular features include angioid streaks, peau d'orange fundus, and drusen. We report a novel ABCC6 mutation causing PXE in a patient with a mixed phenotype of PXE and retinitis pigmentosa (RP). CASE: A 37-year-old female presented with decreased peripheral vision and nyctalopia. Ocular imaging revealed angioid streaks emanating from the optic nerve as well as peripheral pigmentary changes and bone spicules. Genetic testing revealed two mutations in ABCC6 in trans. No other mut

Evaluation of the Visual System by the Primary Care Provider Following Concussion.

Concussion is a common injury in childhood and has the potential for substantial impact on quality of life. Visual issues have been increasingly recognized as a common problem after concussion. Many children initially seek care for concussion with their pediatrician, making it even more important for pediatricians to recognize, evaluate, and refer children with visual issues after concussion. This clinical report is intended to support the recommendations in the companion policy statement on vision and concussion and provides definitions of some of the physiologic aspects of the visual system
