Clinical characteristics of internal carotid artery pseudoaneurysms in the sphenoid sinus.

BACKGROUND: Internal carotid artery (ICA) pseudoaneurysms are associated with high mortality if lack of proper management. Patients with ICA pseudoaneurysms in the sphenoid sinus often visit a hospital's ear, nose and throat (ENT) department due to nasal bleeding. In such cases, simple examination and therapy will lead to misdiagnosis. OBJECTIVE: This study sought to investigate the clinical characteristics, diagnostic methods and treatment of ICA pseudoaneurysms in the sphenoid sinus. METHODS: Various data, including clinical features, imaging examination results, and treatment and prognosi

Randomized trial of Legflow(®) paclitaxel eluting balloon and stenting versus standard percutaneous transluminal angioplasty and stenting for the treatment of intermediate and long lesions of the superficial femoral artery (RAPID trial): study protocol for a randomized controlled trial.

BACKGROUND: Restenosis after percutaneous transluminal angioplasty (PTA) of the superficial femoral artery (SFA) may occur in 45% of patients at 2 years follow-up. Paclitaxel-coated balloons have been found to reduce neointimal hyperplasia, and thus reduce restenosis. Recently, the Legflow(®) paclitaxel-coated balloon (Cardionovum Sp.z.o.o., Warsaw, Poland) (LPEB) has been introduced. This balloon is covered with shellac, a Food and Drug Administration (FDA) approved natural resin, to obtain an equally distributed tissue concentration of paclitaxel. The RAPID trial is designed to assess resten

Superficial temporal artery-middle cerebral artery bypass combined with encephalo-duro-myo-synangiosis in treating moyamoya disease: surgical techniques, indications and midterm follow-up results.

BACKGROUND: Surgical interventions for moyamoya disease include direct and indirect revascularizations. This study aimed to evaluate the therapeutic effect of superficial temporal artery-middle cerebral artery bypass combined with an indirect revascularization procedure, encephalo-duro-myo-synangiosis, in the treatment of moyamoya disease. METHODS: From October 2005 to November 2009, we performed this combined revascularization procedure in 111 patients with different types and stages of moyamoya disease. The superficial temporal artery, middle meningeal artery and the deep temporal artery we

Cerebral angiographic findings in patients with non-traumatic subarachnoid hemorrhage.

BACKGROUND: Even though ruptured intracranial aneurysm is a major cause of non-traumatic subarachnoid hemorrhage (SAH), non-aneurysmal SAH has a good prognosis with few neurologic complications. The gold standard for detecting the vascular pathology is digital subtraction cerebral angiography (DSA). OBJECTIVE: The primary objective of the present study was to clarify cerebral angiographic findings in patients with non-traumatic subarachnoid hemorrhage (SAH); to define the incidence of nonaneurysmal SAH. The secondary aim was to review the clinical data of all of the patients diagnosed with no

Evaluation of severe subclavian artery stenosis by color Doppler flow imaging.

This study evaluates the diagnostic value of the hemodynamic parameters of color Doppler flow imaging (CDFI) for severe (70 to 99%) subclavian artery stenosis (SAS) using digital subtraction angiography (DSA) as the reference standard. Two-hundred fifty-two patients with suspected SAS were recruited into the study and examined from June 2005 to December 2009. The degree of stenosis was classified as moderate (50 to 69%) or severe (70 to 99%) stenosis. By using CDFI, the residual diameter (Dr), peak systolic velocity (PSV1) and end diastolic velocity (EDV) at the stenotic vessel segments, as we

A form of dysplasia or a fortuitous association? A cerebral aneurysm inside an arachnoid cyst: case report.

OBJECTIVE: Although arachnoid cysts and intracranial aneurysms are very common lesions, their association in the same patient is rare. We present a case of a middle cerebral artery aneurysm ruptured into an arachnoid cyst. We found only six cases with intracystic hemorrhage reported in the literature. The presence of an arachnoid cyst can mislead clinical presentation. The patient presented a paradoxically small temporal fossa and thickening of the temporal and sphenoid bone. The authors suggest that this uncommon association (arachnoid cyst, atypical cranial vault, and "mirror-like" cerebral

Duplex scanning allows selective use of arteriography in the management of patients with severe lower leg arterial disease.

PURPOSE: Until April 1997 we routinely performed intra-arterial digital subtraction angiography (iaDSA) in all patients with severe lower leg ischemia requiring intervention. After a comparative study of duplex scanning (DS), pulsegenerated runoff, and iaDSA of the lower leg arteries, we postulated that management could be based on DS/pulse-generated runoff in 59% of patients. We prospectively evaluated the safety of such a noninvasive workup strategy. METHODS: All consecutive patients referred with severe lower leg ischemia between April 1997 and September 1998 were eligible. Management was

99Tcm-MAA lung scintigraphy in patients with Behçet's disease: its value and correlation with clinical course and other diagnostic modalities.

Behçet's disease, initially described as local vasculitis involving the oral, genital mucosa and eyes, can affect many organ systems with a varying clinical course. Pulmonary lesions associated with Behçet's disease are rare and reported to have a prevalence of 1.0-7.7%. In this prospective study, 29 patients aged 16-60 years (mean +/- s: 34 +/- 10.5) were investigated for pulmonary involvement with direct radiography, computerized tomography (CT) and intravenous digital subtraction angiography. Lung scintigraphy was also performed to determine its diagnostic value. In addition, patients had a

Factors Determining Periprocedural and Long-term Complications of High Risk Carotid Artery Stenting.

BACKGROUND AND PURPOSE: Carotid artery stenting (CAS) has been, historically, an alternative to open endarterectomy (CEA) for stroke prevention in high risk patients with carotid atherosclerosis. We sought to determine the rates of periprocedural and long-term stroke or death and the risk factors for complications after CAS in our high risk patient population. METHODS: Clinical and treatment variables of consecutive CAS procedures performed between 2002 and 2011 were analyzed. Using univariate and multivariate logistic regression analyses we examined how patient characteristics influenced out

The role of transcranial Doppler embolic monitoring in the management of intracranial arterial stenosis.

Intracranial arterial stenosis (IAS) is thought to be responsible for 8% of all ischemic stroke subtypes. The best medical treatment for this condition is still controversial. Transcranial Doppler (TCD) emboli monitoring may help to guide the treatment by measuring the frequency of microembolic signals (MES). We report a case of IAS where TCD emboli monitoring proved useful in establishing the mechanism of stroke by being artery-to-artery emboli and guiding therapy based on the frequent symptoms and number of MES.

Vasospasm probability index: a combination of transcranial doppler velocities, cerebral blood flow, and clinical risk factors to predict cerebral vasospasm after aneurysmal subarachnoid hemorrhage.

OBJECT: The goal in this study was to create an index (vasospasm probability index [VPI]) to improve diagnostic accuracy for vasospasm after subarachnoid hemorrhage (SAH). METHODS: Seven hundred ninety-five patients in whom aneurysmal SAH was demonstrated by computed tomography, and in whom one or more intracranial aneurysms had been diagnosed, underwent transcranial Doppler (TCD) studies between April 1998 and January 2000. In 154 patients angiography was performed within 24 hours of the TCD examination, and in 75 133Xe cerebral blood flow (CBF) studies were obtained the same day. Seven case

Angiographic demarcation of an occlusive lesion may predict recanalization after intra-arterial thrombolysis in patients with acute middle cerebral artery occlusion.

BACKGROUND AND PURPOSE: The aim of this study is to investigate whether angiographic demarcation of an occlusive lesion may predict successful or failed result of intra-arterial thrombolysis in acute middle cerebral artery (MCA) occlusion. METHODS: We reviewed retrospectively the angiography and clinical data of acute MCA occlusion patients who underwent intra-arterial thrombolysis from 1994 to 2004. Pretreatment angiographic findings at the occlusive lesions were classified as either blurred or sharp, depending on whether the proximal portion of the occlusive lesions had poorly or well demar

Abciximab as an adjunct to high-risk carotid or vertebrobasilar angioplasty: preliminary experience.

OBJECTIVE: Abciximab, a platelet glycoprotein IIb/IIIa receptor inhibitor, has been shown to reduce the risk of ischemic events associated with coronary intervention. However, its role in neurointerventional procedures needs to be defined. We prospectively evaluated our initial experience with the use of abciximab in a series of high-risk patients undergoing carotid, basilar, or vertebral artery angioplasty. METHODS: Patients were given an intravenous abciximab bolus (0.25 mg/kg), followed by infusion (10 microg/min) for a period ranging from 12 to 24 hours, as an adjunct to angioplasty in 20

Interactive image-guided surgical resection of intracranial arteriovenous malformations.

Surgical excision is the only treatment method that immediately prevents increased morbidity or mortality as a result of hemorrhage from arteriovenous malformations (AVMs). For those lesions located deep within the cerebral hemispheres or near eloquent areas, conventional surgical resection may be associated with an unacceptable degree of morbidity and mortality. Herein we report our experience in the resection of these lesions using interactive image guidance. There were five women and five men in the patient group. Their age ranged from 16 to 73 years (mean = 41). Clinical presentation inclu

Intracranial dural arteriovenous fistula mimicking neuromyelitis optica spectrum disorder: A case report.

Neuromyelitis optica spectrum disorder (NMOSD) is an immune-mediated disorder. It often develops acute myelopathy due to longitudinally extensive transverse myelitis (LETM), although other disorders can cause an LETM-like lesion. Here, we report a 76-year-old patient presenting with acute-onset, progressive myelopathy, which proved to be caused by an intracranial dural arteriovenus fistula (dAVF). Magnetic resonance imaging (MRI) revealed a longitudinally extensive spinal cord lesion, which was further extended rostrally to the medulla. Although cord surface flow voids were absent on T2-weight
