A Sustainable Treatment Protocol for Organophosphate Poisoning in Rural Kenya for Facilities without Intensive Care Units and Where Transfer Is Not Possible.

Organophosphate poisoning is a common method of attempting suicide in developing countries, particularly in rural areas. The current WHO protocols emphasize intubation and transfer to a facility with intensive care. Both recommendations are beyond the resources of many rural hospitals in developing countries and their poor patients. Because organophosphate poisoning is a commonly seen condition in Kenya and because intubation is beyond the equipment and training of many rural hospitals, a protocol involving atropine without intubation was developed and used. The protocol includes placement of …

The Sambia "turnim-man": sociocultural and clinical aspects of gender formation in male pseudohermaphrodites with 5-alpha-reductase deficiency in Papua New Guinea.

Continuing controversy surrounding the Dominican Republic studies of 5-alpha-reductase deficiency and the development of gender identity in male pseudohermaphrodites concerns the roles of culture and biology in determining the ambiguity of gender socialization in an unsophisticated population. The present paper provides a cross-cultural perspective on these problems through description of anthropological and clinical data for a sample (N = 14) of subjects suffering from 5-alpha-reductase deficiency. Nine of these male pseudohermaphrodites were reared ambiguously as male and five as female. Fem

Technical note: In Mexico, the majority of 147 traumatic spinal cord injuries occurred in the thoracic spine for young males.

BACKGROUND: Traumatic spinal cord injury (TSCI) is a devastating problem typically associated with multi-organ disorders. Studies regarding TSCI and their attendant comorbidities are scarce from developing countries. METHODS: The electronic files of 147 patients with TSCI (2017-2018) were reviewed; 78% of patients were males averaging 38 years of age. The following variables associated with the TSCI included age, sex, etiology of trauma, vertebral level, fracture classification, Frankel grade, treatment, complications, and mortality. RESULTS: Of interest, most cases involved thoracic injurie…

When surgeons are rarer than resources - our experience with improving access to thoracic surgery in an urban setting: A special report.

With progressive globalisation enabled by technology, there is an increased interest in finding viable solutions to the myriad health problems faced by developing countries. In countries like Pakistan, occasionally the challenge is not a dearth of material resources but rather unavailability of expertise. The current paper was planned to share a model that was successfully implemented in the urban setting of Karachi, Pakistan, from 2012 onwards which significantly improved access to thoracic surgery for underprivileged individuals. Our model focussed on a qualified thoracic surgeon reviving a …

Ebola Virus Disease Epidemic: What Can the World Learn and Not Learn from West Africa?

With over 4,500 deaths and counting, and new cases identified in two developed countries that are struggling and faltering in their handling of the epidemic, the 2014 Ebola Virus Disease (EVD) epidemic is unlike any of its kind ever encountered. The ability of some poor, resource-limited, developing countries in sub-Saharan Africa to efficiently handle the epidemic within their shores provides some lessons learned for the global health community. Among others, the 2014 EVD epidemic teaches us that it is time to put the "P" back in public and population health around the world. The global healt…

Mucormycosis (zygomycosis) of renal allograft.

Fungal infection is relatively common among renal transplant recipients from developing countries. Mucormycosis, also known as zygomycosis, is one of the most serious fungal infections in these patients. The most common of presentation is rhino-cerebral. Isolated involvement of a renal allograft is very rare. A thorough search of literature and our medical records yielded a total of 24 cases with mucormycosis of the transplanted kidney. There was an association with cytomegalovirus (CMV) infection and anti-rejection treatment in these patients and most of these transplants were performed in th…

Epidemiology of neglected tropical diseases in transplant recipients. Review of the literature and experience of a Brazilian HSCT center.

The rising success rate of solid organ (SOT) and haematopoietic stem cell transplantation (HSCT) and modern immunosuppression make transplants the first therapeutic option for many diseases affecting a considerable number of people worldwide. Consequently, developing countries have also grown their transplant programs and have started to face the impact of neglected tropical diseases (NTDs) in transplant recipients. We reviewed the literature data on the epidemiology of NTDs with greatest disease burden, which have affected transplant recipients in developing countries or may represent a threa…

Unusual cause of seizures in Singapore: neurocysticercosis.

Neurocysticercosis is the commonest cause for adult onset of seizures in developing countries, and is relatively uncommon in Singapore. However, with the influx of foreign workers from developing countries, we need to consider it as a differential diagnosis in our approach to adult-onset seizures in this group of patients. We describe a neurocysticercosis occurring in a 22-year-old Indian man who presented with a single episode of generalised tonic-clonic seizures. Magnetic resonance imaging of the brain showed a well-defined, hypointense lesion in the cortical and subcortical regions of the l…

[BCG vaccination in the world].

BCG vaccination programme and BCG vaccination coverage in the world were summarized mainly based on the published informations from official organizations, such as World Health Organization (WHO), International Union Against Tuberculosis and Lung Disease (IUATLD) and Centers for Disease Control and Prevention (CDC). From this review, we can see how widely BCG has been used for the prevention of tuberculosis in the world. In most of the developing countries, especially in Africa, the Americas, and Pacific Region, BCG vaccination is carried out to newborn babies soon after birth by intradermal i…

Multifaceted epidemic preparedness against cholera outbreak by One Health approach and genomic surveillance.

Many developing countries in Africa and Asia are hotspots and endemic for cholera. Over the past five years, these countries have had steady rises in the incidence of cholera, with case fatality ratios continuously exceeding the WHO recommendation of 1 %. Despite this, there aren't many non-outbreak investigations that try to check for Vibrio cholerae in asymptomatic individuals who could act as reservoirs and vectors of transmission. We address the fundamental causes of global cholera transmission following a systematic review of available literature with a focus on the ecological and epidemi…

Genome-wide association study: new genetic insights into HBV/HCV-related hepatocellular carcinoma genomes.

Hepatocellular carcinoma (HCC) is the third common cause of cancer-related death with highest prevalence in developing countries, such as Southeast China and Saharan African. The major pathogenic factors can be categorized into environmental effects and genetic variations, and it is mostly caused by hepatitis B or C virus (HBV and HCV). The geographic prevalence of chronic hepatitis B and C (CHB and CHC) varies, with HBV heavily-infected in developing countries and HCV prevalent in developed countries. The infection of either hepatitis virus B or C causes damage to the liver cells through cell…

Global epidemiology of canine rabies: past, present, and future prospects.

The rabies virus, a public health scourge from ancient times, is currently responsible for an estimated 59,000 human deaths a year, almost all transmitted via dog bites. It causes considerable economic impacts on developing countries, primarily in Africa and Asia, which can least afford these losses. However, despite its almost 100% case fatality rate, canine rabies is a completely preventable disease, and historic examples of canine rabies elimination in the developed world attest to this. Over the last decade, programs based on eliminating the source of the disease from dogs have shown succe…

Growing burden of diabetes in sub-saharan Africa: contribution of pesticides ?

The diabetes burden is growing in Sub-Saharan Africa (SSA). The low overall access to health care has been documented to contribute to the high diabetes-related mortality. Due to economic, demographic, epidemiological and nutrition transitions in SSA, the growing prevalence of diabetes appears to be related to obesogenic lifestyles and the intergenerational impact of malnutrition in women of childbearing age. Both overnutrition and undernutrition have been associated with the development of diabetes and other chronic diseases. Africans are also suspected of being genetically predisposed to dia

Experience with tetanus in a tertiary hospital in south east Nigeria.

BACKGROUND: Tetanus has for long been an avoidable source of morbidity and mortality particularly in developing countries. The aim of this study was to review the experience with managing tetanus in a regional tertiary hospital in Nigeria. METHODS: A retrospective study of patients who were admitted with a clinical diagnosis of tetanus between January 1999 and December 2003 was done. Relevant data were extracted from the patients'case records. RESULTS: A total of 12 patients were identified within the time frame with a diagnosis of tetanus. There were more male than female patients (ratio 1.…

Causality or coincidence: may the slow disappearance of helminths be responsible for the imbalances in immune control mechanisms?

Intestinal infection continues to be a problem worldwide and helminths, which currently infect billions of individuals, are primary culprits. The major burden of disease falls on the populations of developing countries, given that over the last four to five decades helminth infections are disappearing in industrialized societies. In developing countries, a major source of immunomodulatory signals in post-natal life are parasites, particularly helminths, which, unlike most bacteria and viruses, selectively stimulate Th2 function. Helminths and their eggs are probably the most potent stimulators…

Clinical complications and anthropometric indicators of chronic kidney disease in a global context: scoping review.

BACKGROUND: Pediatric chronic kidney disease (CKD) is a serious public health problem and represents a considerable burden on health systems, particularly in low- and middle-income countries (LMICs). Despite well-documented global disparities in CKD management, the extent of these differences across continents remains insufficiently explored, particularly in pediatric populations. OBJECTIVE: To explore the body of recent evidence on clinical complications and anthropometric indicators of CKD in childhood and to identify gaps in knowledge that can guide the development of future studies of CKD

Surgical site infection prevention: a scoping review of strategies, implementation barriers, and emerging approaches.

BACKGROUND: Surgical site infections (SSIs) remain a major cause of healthcare-associated morbidity, mortality, and increased healthcare costs worldwide, particularly in low- and middle-income countries (LMICs). Although evidence-based prevention guidelines are widely available, inconsistent implementation and variability in adherence continue to limit their effectiveness across healthcare settings. METHODS: This scoping review synthesized recent evidence on SSI prevention strategies published between 2019 and 2025. A systematic search of Scopus, Web of Science, and ScienceDirect identified 2

Characteristics of supermarket-based interventions to improve the dietary quality and environmental sustainability of diets in people living with obesity, overweight, and/or food insecurity in high-income countries: a scoping review.

Food insecurity (FI), defined as unreliable access to healthy, nutritious food, is a major health concern in higher-income countries, primarily due to its association with an increased risk of obesity. Supermarket-based interventions may influence population-level food purchasing behaviour, an antecedent to consumption. It is unclear whether there are specific characteristics that these interventions should employ to resonate with vulnerable groups. This scoping review aimed to explore the characteristics of supermarket-based interventions that sought to support healthier and/or more environme
