Unsupervised Multi-Latent Space RL Framework for Video Summarization in Ultrasound Imaging.

The COVID-19 pandemic has highlighted the need for a tool to speed up triage in ultrasound scans and provide clinicians with fast access to relevant information. To this end, we propose a new unsupervised reinforcement learning (RL) framework with novel rewards to facilitate unsupervised learning by avoiding tedious and impractical manual labelling for summarizing ultrasound videos. The proposed framework is capable of delivering video summaries with classification labels and segmentations of key landmarks which enhances its utility as a triage tool in the emergency department (ED) and for use

Robust and Interpretable General Movement Assessment Using Fidgety Movement Detection.

Fidgety movements occur in infants between the age of 9 to 20 weeks post-term, and their absence are a strong indicator that an infant has cerebral palsy. Prechtl's General Movement Assessment method evaluates whether an infant has fidgety movements, but requires a trained expert to conduct it. Timely evaluation facilitates early interventions, and thus computer-based methods have been developed to aid domain experts. However, current solutions rely on complex models or high-dimensional representations of the data, which hinder their interpretability and generalization ability. To address that

QRS-T Angles as Markers for Heart Sphericity in Subjects With Intrauterine Growth Restriction: A Simulation Study.

Changes induced by intrauterine growth restriction (IUGR) in cardiovascular anatomy and function that persist throughout life have been associated with a higher predisposition to heart disease in adulthood. Together with cardiac morphological remodelling, evaluated through the ventricular sphericity index, alterations in cardiac electrical function have been reported by characterization of the depolarization and repolarization loops, and their angular relationship, measured from the vectorcardiogram. The underlying relationship between the morphological remodelling and the angular variation of

A WeChat applet-based national remote emergency system for malignant hyperthermia in China: a usability study.

BACKGROUND: Malignant hyperthermia (MH) is a rare anesthetic emergency with a high mortality rate in China. We developed a WeChat applet-based National Remote Emergency System for Malignant Hyperthermia (MH-NRES) to provide a real-time emergency system to help Chinese anesthesiologists deal with MH crises. However, it is imperative that close attention should be paid to the usability of the applet. PURPOSE: The objectives of this study were to (1) evaluate the usability of the applet-based MH-NRES for anesthesiologists; and (2) to test the validity and reliability of a modified mHealth app us

Beyond Heart Murmur Detection: Automatic Murmur Grading From Phonocardiogram.

OBJECTIVE: Murmurs are abnormal heart sounds, identified by experts through cardiac auscultation. The murmur grade, a quantitative measure of the murmur intensity, is strongly correlated with the patient's clinical condition. This work aims to estimate each patient's murmur grade (i.e., absent, soft, loud) from multiple auscultation location phonocardiograms (PCGs) of a large population of pediatric patients from a low-resource rural area. METHODS: The Mel spectrogram representation of each PCG recording is given to an ensemble of 15 convolutional residual neural networks with channel-wise at

Accurate Cobb Angle Estimation on Scoliosis X-Ray Images via Deeply-Coupled Two-Stage Network With Differentiable Cropping and Random Perturbation.

Automated Cobb angle estimation on X-ray images is crucial to scoliosis diagnosis. The existing efforts are typically two extremes, which either laboriously detect the raw vertebral landmarks or directly regress Cobb angles from the entire image. In this paper, we propose a novel two-stage end-to-end method as a balanced solution, to avoid vulnerability to false landmarks, and to preserve flexibility in clinical usages. Concretely, we cascade two stages sequentially for detecting vertebrae and then regressing their bending directions instead of raw landmarks. In the detection stage, we combine

VLTENet: A Deep-Learning-Based Vertebra Localization and Tilt Estimation Network for Automatic Cobb Angle Estimation.

Scoliosis diagnosis and assessment rely upon Cobb angle estimation from X-ray images of the spine. Recently, automated scoliosis assessment has been greatly improved using deep learning methods. However, in such methods, the Cobb angle is usually predicted based on regression models that don't account for information of the spine structure. Alternatively, the Cobb angle can be estimated indirectly through landmark-detection and vertebra-segmentation, but this approach is still highly sensitive to small detection and segmentation errors. This paper proposes a novel deep-learning architecture, c

PVR-Vocoder: A Pathological Voice Repair Vocoder for Voice Disorders.

Vocoder-based speech synthesis has become a promising technique to accommodate the demands of high-quality speech analysis, manipulation, and synthesis. However, most existing works focus on how to synthesize normal human voice with high signal-to-noise ratio, neglecting individuals' pathological voice disorder in speech interaction. In this work, we propose a non-linear voice repair vocoder for pathological vowels and sentences, which takes the pathological speech as input and generates high-quality repaired speech. Our approach is specifically designed to enhance the speech quality and intel

Domain-Aware Dual Attention for Generalized Medical Image Segmentation on Unseen Domains.

Recently, there has been significant progress in medical image segmentation utilizing deep learning techniques. However, these achievements largely rely on the supposition that the source and target domain data are identically distributed, and the direct application of related methods without addressing the distribution shift results in dramatic degradation in realistic clinical environments. Current approaches concerning the distribution shift either require the target domain data in advance for adaptation, or focus only on the distribution shift across domains while ignoring the intra-domain

Towards a clinically-based common coordinate framework for the human gut cell atlas: the gut models.

BACKGROUND: The Human Cell Atlas resource will deliver single cell transcriptome data spatially organised in terms of gross anatomy, tissue location and with images of cellular histology. This will enable the application of bioinformatics analysis, machine learning and data mining revealing an atlas of cell types, sub-types, varying states and ultimately cellular changes related to disease conditions. To further develop the understanding of specific pathological and histopathological phenotypes with their spatial relationships and dependencies, a more sophisticated spatial descriptive framewor…

The automatic detection of diabetic kidney disease from retinal vascular parameters combined with clinical variables using artificial intelligence in type-2 diabetes patients.

BACKGROUND: Diabetic kidney disease (DKD) has become the largest cause of end-stage kidney disease. Early and accurate detection of DKD is beneficial for patients. The present detection depends on the measurement of albuminuria or the estimated glomerular filtration rate, which is invasive and not optimal; therefore, new detection tools are urgently needed. Meanwhile, a close relationship between diabetic retinopathy and DKD has been reported; thus, we aimed to develop a novel detection algorithm for DKD using artificial intelligence technology based on retinal vascular parameters combined wit

A Dynamic Prediction Model Supporting Individual Life Expectancy Prediction Based on Longitudinal Time-Dependent Covariates.

In the field of clinical chronic diseases, common prediction results (such as survival rate) and effect size hazard ratio (HR) are relative indicators, resulting in more abstract information. However, clinicians and patients are more interested in simple and intuitive concepts of (survival) time, such as how long a patient may live or how much longer a patient in a treatment group will live. In addition, due to the long follow-up time, resulting in generation of longitudinal time-dependent covariate information, patients are interested in how long they will survive at each follow-up visit. In

Mapping patient pathways and understanding clinical decision-making in dengue management to inform the development of digital health tools.

BACKGROUND: Dengue is a common viral illness and severe disease results in life-threatening complications. Healthcare services in low- and middle-income countries treat the majority of dengue cases worldwide. However, the clinical decision-making processes which result in effective treatment are poorly characterised within this setting. In order to improve clinical care through interventions relating to digital clinical decision-support systems (CDSS), we set out to establish a framework for clinical decision-making in dengue management to inform implementation. METHODS: We utilised process m

TRSA-Net: Task Relation Spatial Co-Attention for Joint Segmentation, Quantification and Uncertainty Estimation on Paired 2D Echocardiography.

Clinical workflow of cardiac assessment on 2D echocardiography requires both accurate segmentation and quantification of the Left Ventricle (LV) from paired apical 4-chamber and 2-chamber. Moreover, uncertainty estimation is significant in clinically understanding the performance of a model. However, current research on 2D echocardiography ignores this vital task while joint segmentation with quantification, hence motivating the need for a unified optimization method. In this paper, we propose a multitask model with Task Relation Spatial co-Attention (referred as TRSA-Net) for joint segmentati

CT-Guided Survival Prediction of Esophageal Cancer.

Survival prediction of esophageal cancer is an essential task for doctors to make personalized cancer treatment plans. However, handcrafted features from medical images need prior medical knowledge, which is usually limited and not complete, yielding unsatisfying survival predictions. To address these challenges, we propose a novel and efficient deep learning-based survival prediction framework for evaluating clinical outcomes before concurrent chemoradiotherapy. The proposed model consists of two key components: a 3D Coordinate Attention Convolutional Autoencoder (CACA) and an uncertainty-bas

LamNet: A Lesion Attention Maps-Guided Network for the Prediction of Choroidal Neovascularization Volume in SD-OCT Images.

Choroidal neovascularization (CNV) volume prediction has an important clinical significance to predict the therapeutic effect and schedule the follow-up. In this paper, we propose a Lesion Attention Maps-Guided Network (LamNet) to automatically predict the CNV volume of next follow-up visit after therapy based on 3-dimentional spectral-domain optical coherence tomography (SD-OCT) images. In particular, the backbone of LamNet is a 3D convolutional neural network (3D-CNN). In order to guide the network to focus on the local CNV lesion regions, we use CNV attention maps generated by an attention

Cross-Boosted Multi-Target Domain Adaptation for Multi-Modality Histopathology Image Translation and Segmentation.

Recent digital pathology workflows mainly focus on mono-modality histopathology image analysis. However, they ignore the complementarity between Haematoxylin & Eosin (H&E) and Immunohistochemically (IHC) stained images, which can provide comprehensive gold standard for cancer diagnosis. To resolve this issue, we propose a cross-boosted multi-target domain adaptation pipeline for multi-modality histopathology images, which contains Cross-frequency Style-auxiliary Translation Network (CSTN) and Dual Cross-boosted Segmentation Network (DCSN). Firstly, CSTN achieves the one-to-many translation fro
