1H-magnetic resonance spectroscopy markers of cognitive and language ability in clinical subtypes of autism spectrum disorders.

This study assessed metabolic functioning of regional brain areas to address whether there is a neurometabolic profile reflecting the underlying neuropathology in individuals with autism spectrum disorders, and if varied profiles correlate with the clinical subtypes. Thirteen children (7-16 years) with autism spectrum disorders and 8 typically developing children were compared on (1)H-magnetic resonance spectroscopy data collected from hippocampus-amygdala and cerebellar regions. The autism spectrum disorder group had significantly lower N-acetyl-aspartate/creatine ratios bilaterally in the hi

Unexpected right hemisphere language representation identified by the intracarotid amobarbital procedure in right-handed epilepsy surgery candidates.

The intracarotid amobarbital procedure (IAP) is used for language lateralization in the presurgical evaluation for intractable epilepsy. Some epilepsy surgical centers forgo IAP for right temporal lobectomies in right-handed patients with no personal/family history of left-handedness, implying that right hemisphere language representation does not occur in these patients. To test this hypothesis, a retrospective analysis was performed on 156 consecutive epilepsy surgery candidates who underwent IAP. Of the 156 candidates, 122 were right-handed, and 55 of the 122 demonstrated right hemisphere s…

Narrative skills, cognitive profiles and neuropsychiatric disorders in 7-8-year-old children with late developing language.

BACKGROUND: A community-representative sample of screened and clinically examined children with language delay at 2.5 years of age was followed up at school age when their language development was again examined and the occurrence of neuropsychiatric/neurodevelopmental disorder (attention deficit/hyperactivity disorder (ADHD) and/or autism spectrum disorder (ASD)) was documented. AIMS: (1) To determine whether these 7-8-year-old children with a history of language delay have deficits in narrative skills compared with the age norms of standardized tests; and (2) to analyse if there is a relati…

Subject-verb agreement and phonological processing in developmental dyslexia and specific language impairment (SLI): a closer look.

BACKGROUND: Problems with subject-verb agreement and phonological (processing) skills have been reported to occur in children with specific language impairment (SLI) and in those with developmental dyslexia, but only a few studies have compared such problems in these two groups. Previous studies have claimed a causal relationship between phonological processing deficits and morphosyntactic problems. AIMS: The following questions were addressed in this study: (1) Are children with developmental dyslexia and SLI comparable in the level of sensitivity to subject-verb agreement, phonological awar…

Naming speed and visual search deficits in readers with disabilities: evidence from an orthographically regular language (Italian).

The study examined rapid automatized naming (RAN) in 42 children with reading disabilities and 101 control children-all native speakers of Italian, a language with shallow orthography. Third-, 5th- and 6th-grade children were given a RAN test that required rapid naming of color, object, or digit matrices. A visual search test using the same stimulus material (but not requiring a verbal response) and an oral articulation test were also given. Readers with disabilities performed worse than controls on the RAN test. This effect was larger in higher grades than in lower ones. Readers with disabili…

How do individuals with Asperger syndrome respond to nonliteral language and inappropriate requests in computer-mediated communication?

Computer-mediated communication in individuals with Asperger syndrome, Tourette syndrome and normal controls was explored with a program called Bubble Dialogue (Gray, Creighton, McMahon, and Cunninghamn (1991)) in which the users type text into speech bubbles. Two scenarios, based on Happé (1994) were adapted to investigate understanding of figure of speech and sarcasm, and a third, developed by ourselves, looked at responses to inappropriate requests (lending money and disclosing home address on a first meeting). Dialogue transcripts were assessed by 62 raters who were blind to the clinical d

The application of cognitive event-related brain potentials (ERPs) in language-impaired individuals: review and case studies.

There is a substantial body of basic research that has utilized ERPs to investigate the neurological basis of cognition. This research has, in turn, led to the development of practical applications of cognitive ERPs in patient populations. In particular, recent work has focused on the development of ERP-based assessment measures for the neuropsychological assessment of dyslexia and language impairments secondary to stroke. This review describes the innovative assessment methods program (IAMP), an initiative to utilize ERPs for a neuropsychological assessment of patients who cannot be evaluated…

A dyspraxic deficit in specific language impairment and developmental coordination disorder? Evidence from hand and arm movements.

The extent to which children with either specific language impairment (SLI) or developmental coordination disorder (DCD) could be considered dyspraxic was examined using three tasks involving either familiar, or unfamiliar actions. SLI is diagnosed in children who fail to develop language in the normal fashion for no apparent reason, while the DCD diagnosis is applied to a child who experiences problems with movement in the absence of other difficulties. Seventy-two children aged between 5 and 13 years participated, falling into one of four groups: (1) children with specific language impairmen…

Cerebral organization of oral and signed language responses: case study evidence from amytal and cortical stimulation studies.

A normally hearing left-handed patient familiar with American Sign Language (ASL) was assessed under sodium amytal conditions and with left cortical stimulation in both oral speech and signed English. Lateralization was mixed but complementary in each language mode: the right hemisphere perfusion severely disrupted motoric aspects of both types of language expression, the left hemisphere perfusion specifically disrupted features of grammatical and semantic usage in each mode of expression. Both semantic and syntactic aspects of oral and signed responses were altered during left posterior tempo…

Maintaining ethnic identity amid additive multilingualism: language attitudes and ethnolinguistic vitality among the Hmong of northern Vietnam.

Research traditions in language attrition tend to read a minority mother tongue's retreat from public domains as a sign of one-directional decline. This study examines that equation through the case of the Hmong in northern Vietnam, where Vietnamese dominates public institutions while Hmong contracts toward family and community, and asks whether such a retreat is necessarily subtractive. The study draws on a qualitative, reflexive thematic analysis of semi-structured interviews with six Hmong speakers across three generations, with two comparison participants offering outsider and institutiona…

Characterizing Features of Vulnerability in Narrative Production of Cantonese-Speaking School-Aged Children With Developmental Language Disorder.

PURPOSE: Narratives show vulnerability in children with developmental language disorder (DLD) cross-linguistically. However, certain narrative measures revealing significant deficits in English and European DLD literature have been studied primarily in preschoolers, with limited evidence in school-aged children and no data in Cantonese. We examine features of vulnerability in narrative production of school-aged children learning a major Chinese language, Cantonese. METHOD: Twenty-five children with DLD (Mage = 9;7 [years;months]) and age-matched typically developing (TD) peers (Mage = 9;8) we…

Prolonged Transcortical Mixed Aphasia Following Awake Glioma Resection: Functional Language Network Disruption Associated with Persistent Epileptiform Activity.

Aphasia occurring after tumor resection in the language-dominant hemisphere is commonly interpreted as a consequence of irreversible structural injury. In awake craniotomy, intraoperative language mapping enables real-time functional monitoring and is expected to reduce the risk of postoperative aphasia. However, postoperative language impairment is not always attributable to structural damage alone, and functional network disturbances may also contribute. We report a case of prolonged aphasia following awake glioma resection in the dominant hemisphere, in which epileptiform activity was suspe…

Comparative Evaluation of Large Language Models in Responding to Common Misconceptions Regarding Periodontal Disease and Oral Hygiene.

Background/Objective: As large language models (LLMs) are increasingly used to obtain oral health information, their ability to provide accurate and comprehensive responses to patient-focused periodontal questions derived from commonly reported misconceptions is increasingly relevant to patient education. This study aimed to evaluate the performance of contemporary LLMs in answering such questions with respect to accuracy, comprehensiveness, readability, and temporal consistency. Methods: Following a comprehensive review of the existing literature, structured patient-focused periodontal questi…

Large Language Model-Assisted Review of Clinical Trial Informed Consent for Data Reuse: Retrieval-Augmented Generation Approach.

BACKGROUND: Regulatory frameworks such as the Belmont Report, the Common Rule, and the Declaration of Helsinki require informed consent to ensure participants understand a study's purpose and can make voluntary decisions about their involvement. Regulations, including the General Data Protection Regulation (Regulation [EU] 2016/679), further emphasize that consent must be freely given and revocable without disadvantage. Although informed consent forms (ICFs) are intended to be clear and accessible, they have become increasingly lengthy and complex. Large language models (LLMs) offer the potent…

A language model framework for sequence modeling of audit logs to characterize clinician-electronic health record interactions.

OBJECTIVE: Electronic health record (EHR) audit logs capture clinician-EHR interaction patterns, but most audit log research relies on aggregated measures (eg, total time). We investigated how audit logs could be modeled using large language model (LLM) architectures to learn underlying workflow sequences during clinician-EHR interactions. MATERIALS AND METHODS: Using >295 million EHR-based audit log actions from inpatient settings spanning 2019 to 2024, we fine-tuned Llama-3-8B under 2 encodings: (1) symbolic (field-based) tokens and (2) semantic (word-based) natural language-based audit log…

From conservation scores to language models: a feature engineering perspective on machine learning for genetic variant interpretation.

The interpretation of genetic variants remains central to precision medicine, and machine-learning-based variant effect prediction tools have become indispensable for supporting decision-making processes. In this review we trace the progression from hand-crafted features and traditional machine learning classifiers, through supervised ensembles, to deep-learning architectures and foundation models that learn representations directly from biological sequences. We classify methods for genetic variant effect prediction by underlying machine learning model and variant type handled, from missense t
