Superior long term functional and scar outcome of Meek micrografting compared to conventional split thickness skin grafting in the management of burns.

INTRODUCTION: Autologous skin grafting is the mainstay of treatment in burn patients. Extensive full thickness burns remains a challenge to the burns surgeon due to the lack of autologous skin donor sites. The conventional split thickness skin grafting (SSG) and the Meek micrografting (Meek) technique are part of the armamentarium of the burns surgeon to curtail the challenge of paucity of donor sites. With advances in burn care, mortality rates of burn patients have reduced. As a result, with more patients surviving acute burn, there is a paradigm shift of research towards assessment of funct

Unique complications of foot and ankle injuries secondary to warfare.

This article discusses the common complications associated with lower extremity trauma and amputations secondary to combat injuries. The complications include retained fragments, soft tissue adhesions, poor wound healing, painful bursae, neuroma formation, heterotopic ossification, and depleted uranium. Although there is some literature on these topics, most is based on noncombat injuries, indicating a need for further research into the management of these devastating injuries.

Altered muscle synergies during gait in pediatric acquired brain injury: Neurological and biomechanical contributions.

This study compared muscle synergies during gait in children with acquired brain injury (ABI) and typically-developing (TD) peers, and examined the influence of gait deviations and injury onset on synergy characteristics. Retrospective treadmill-based 3D gait analysis was conducted in 35 children with ABI in the chronic stage (mean age 11.1 ± 3.7 years; 6.0 ± 4.4 years post-injury) and 21 TD peers. Surface electromyography of seven bilateral lower-limb muscles was analyzed using non-negative matrix factorization. Synergies of the more affected leg were assessed with the dynamic-motor-control-i

Enhanced Reticulospinal Output in Subacute Spinal Cord Injury Patients with Spasticity.

In the chronic phase of spinal cord injury (SCI), damage to the reticulospinal and corticospinal tracts has been associated with the development of spasticity. Because spasticity can emerge within just a few weeks postinjury, this suggests that reorganization of these motor pathways may begin during the subacute phase. To test this hypothesis, we assessed the StartReact response-an involuntary release of a planned movement via a startling stimulus that engages the reticulospinal tract-by measuring reaction times from electromyographic activity, motor evoked potentials (MEPs) by stimulating the

Knee joint biomechanics under external focus instructions promoting a quiet, safe and soft landing.

BACKGROUND: Anterior cruciate ligament injuries are common in young female athletes, often occurring during landing tasks. Attentional focus instructions may influence landing biomechanics and reduce injury risk, but comparative effects of different external focus (EF) cues remain unclear. METHODS: Twelve novice female volleyball players (age: 13.6 ± 0.6 years) performed double-leg drop landings from a height of 50  cm under four attentional focus conditions: (1) as quietly as possible, (2) as safely as possible, (3) as softly as possible, and (4) a no-focus control. A Qualisys 3D motion capt

The relationship between ankle landing kinematics, isokinetic strength, muscle activity, and the prevalence of lower extremity injuries in university-level netball players during a single season.

BACKGROUND: Safe landing in netball is fundamental. Research on the biomechanics of multidirectional landings is lacking, especially among netball players. Furthermore, few studies reporting the associations between ankle kinematics, isokinetic ankle muscle strength, muscle activities, and injury prevalence in South African netball have been undertaken. OBJECTIVES: To determine the relationships between ankle kinematics, kinetics, isokinetic strength, and muscle activity during jump-landing tasks, as well as the prevalence of lower extremity injuries in university-level netball players during

Transfer of Soleus Muscular Branch of Tibial Nerve to Deep Fibular Nerve to Repair Foot Drop After Common Peroneal Nerve Injury: A Retrospective Study.

OBJECTIVE: Common peroneal nerve (CPN) injury that leads to foot drop is difficult to manage and treat. We present a new strategy for management of foot drop after CPN injury. The soleus muscular branch of the tibial nerve is directly transferred to the deep fibular nerve, providing partial restoration of motor function. METHODS: We retrospectively reviewed eight patients treated for CPN injury between 2017 and 2019. The soleus muscular branch of the tibial nerve was transferred to the deep fibular nerve to repair foot drop. Electrophysiology was conducted, and motor function was assessed. Mo

Femoral nerve transfers for restoring tibial nerve function: an anatomical study and clinical correlation: a report of 2 cases.

Sciatic nerve injuries cause debilitating functional impairment, particularly when the injury mechanism and level preclude reconstruction with primary grafting. The purpose of this study was to demonstrate the anatomical feasibility of nerve transfers from the distal femoral nerve terminal branches to the tibial nerve and to detail the successful restoration of tibial function using the described nerve transfers. Six cadaveric legs were dissected for anatomical analysis and the development of tension-free nerve transfers from femoral nerve branches to the tibial nerve. In 2 patients with compl

Integration of the functional movement screen into the National Hockey League Combine.

The sport of ice hockey requires coordination of complex skills involving musculoskeletal and physiological abilities while simultaneously exposing players to a high risk for injury. The Functional Movement Screen (FMS) was developed to assess fundamental movement patterns that underlie both sport performance and injury risk. The top 111 elite junior hockey players from around the world took part in the 2013 National Hockey League Entry Draft Combine (NHL Combine). The FMS was integrated into the comprehensive medical and physiological fitness evaluations at the request of strength and conditi

L5 spinal nerve injury caused by misplacement of outwardly-inserted S1 pedicle screws.

PURPOSE: To evaluate L5 nerve root injuries caused by outwardly misplaced S1 pedicle screws. Pedicle screws remain the criterion standard for fixation of L5-S1 to correct lumbosacral instability. When inserting S1 pedicle screws, it is possible to injure the L5 nerve root if screws are inserted outwardly and the tip of the screw perforates the anterior cortex of the sacrum. Despite this risk, to our knowledge this type of injury has never been reported as a case series. METHODS: We experienced 2 cases of L5 nerve root injury caused by outwardly-inserted S1 pedicle screws. In both cases, bilat

Measurement of functional recovery in individuals with acute anterior cruciate ligament rupture.

OBJECTIVES: To measure functional recovery following acute anterior cruciate ligament (ACL) rupture using a simple and reliable clinical movement analysis system. Clinic based methods that simultaneously quantify different aspects of movement over a range of activities and model functional recovery will help guide rehabilitation. METHODS: A longitudinal study was used to measure gait variables at initial physiotherapy attendance and then at monthly intervals using a digital camcorder and computer for quantitative analysis. Jogging and distance hopping were added during recovery. A sample of 6

Intact female rats are more susceptible to the development of tactile allodynia than ovariectomized female rats following partial sciatic nerve ligation (PSNL).

As previously reported, this laboratory has determined that female rats are more prone to develop tactile allodynia, an indicator of neuropathic pain, than male rats using the partial sciatic nerve ligation (PSNL) model. In order to further characterize this gender difference, the role of ovarian hormones in predisposing female rats to the development of tactile allodynia was investigated. In a double blind randomized trial, 12 intact and 12 ovariectomized (ovx) female rats underwent PSNL of the right rear leg. Sham 1 operated (exposure of the nerve without ligation; 2 intact, 2 ovx) and sham

Evolution of the Lower Extremity Reconstruction: From Limb Salvage to Functional Reconstruction.

Lower extremity reconstruction has evolved from a focus on limb salvage to prioritizing functional restoration. This shift has been driven by advances in microsurgery, fixation techniques, imaging, and the integration of orthopedic and plastic surgery expertise through the orthoplastic approach. This collaborative model emphasizes simultaneous bone stabilization and soft tissue reconstruction, recognizing their interdependence in healing. Evidence shows reduced infection rates, faster definitive treatment, and improved functional outcomes with orthoplastic care. Modern strategies favor the "re
