[Modified Küntscher's nail for the proximal femur fixation after corrective osteotomies in children (author's transl)].

The authors describe the development in the design of the modified Küntscher's nail with two fins on its upper part, assigned for the fixation of proximal femur after corrective osteotomies in children. Detailed explanation about the nails construction and its biomechanical characteristics are presented. The fixation of the nail in the proximal and distal femoral part, as well as on the site of osteotomy is particularly analyzed. On the base 423 operated cases, where the modified Küntscher's nail was applied, the authors conclusion is that the nail is very suitable for the correction of proxim…

[Valgus deformity of the femoral neck after nailing of infantile femoral shaft fractures (author's transl)].

In 6 patients out of 29 who had suffered fractures of the femoral shaft at childs age and were treated mainly by open nailing, considerable growth disturbances of the major trochanter and valgus deformities of the neck of the femur had been observed in check-ups. The origin and prevention of these complications in nailing and nailremoval are discussed.

A novel surgical technique for removal of intramedullary nails when there is a failure of the proximal extraction device: Report of two cases and review of literature.

Diaphyseal fractures of the tibia is a common injury with an incidence of 16.9/100,000 population. Surgical treatment with intramedullary interlocking nail is the treatment of choice in closed diaphyseal tibial injuries. Removal of the nail is required in non-union, infected implant, implant failure or during exchange nailing procedure. Implant removal surgery presents some unique challenges to the Orthopaedic surgeons like broken nails, broken screws and failure of the extraction devices to engage with the threads of the nail due to stripping of the threads in the proximal part of the nail. V…

How much is too much? A guide to appropriately bending ball tip guide wires when using intramedullary nails for the treatment of lower extremity long bone fractures.

INTRODUCTION: This study was to determine the angle of bend that can be placed in a ball-tip guide wire while still allowing passage of reamers and nails of several commonly utilized nailing systems. METHODS: Ball-tip guide wires, opening reamer tips, femoral nails, and tibial nails were collected from several manufacturers. Guide wires were incrementally bent 3 cm from the tip and passed through the reamer tip, tibial nail, and femoral nail until unable to pass. RESULTS: All systems tested demonstrated that the reamer, with its relative smaller diameter cannula as compared to the nails them…

Nonunion rate of first metatarsal-phalangeal joint arthrodesis for end-stage hallux rigidus with crossed titanium flexible intramedullary nails and dorsal static staple with immediate weight-bearing.

Myriad forms of fixation have been proposed for arthrodesis of the first metatarsal-phalangeal joint (MTPJ). Regardless of the fixation type, nonunion of the arthrodesis site has been purported to be a common complication. We performed a retrospective review of all patients undergoing arthrodesis of the first MTPJ for end-stage hallux rigidus with 2 crossed flexible titanium intramedullary nails and a dorsal static 10-mm titanium staple followed by immediate protected weight-bearing. The inclusion criteria were as follows: the exact internal fixation technique described was employed for end-st…

The use of gentamicin-coated nails in complex open tibia fracture and revision cases: A retrospective analysis of a single centre case series and review of the literature.

INTRODUCTION: Despite modern advances in fracture care, deep (implant-related) infection remains a problem in the treatment of tibia fractures. There is some evidence that antibiotic-coated implants are beneficial in the prevention of this sometimes devastating complication. In the following study we describe our results using a gentamicin-coated intramedullary tibia nail (Expert Tibia Nail (ETN) PROtect™) for the surgical treatment of complex open tibia fracture and revision cases. MATERIALS AND METHODS: We describe the outcome of patients treated between January 2012 and September 2013, usi

Evidence-based guidelines of the spanish psoriasis group on the use of biologic therapy in patients with psoriasis in difficult-to-treat sites (nails, scalp, palms, and soles).

Psoriatic lesions affecting the scalp, nails, palms, and the soles of the feet are described as difficult-to-treat psoriasis and require specific management. Involvement of these sites often has a significant physical and emotional impact on the patient and the lesions are difficult to control with topical treatments owing to inadequate penetration of active ingredients and the poor cosmetic characteristics of the vehicles used. Consequently, when difficult-to-treat sites are involved, psoriasis can be considered severe even though the lesions are not extensive. Scant information is available …

Cephalomedullary nail fixation of subtrochanteric femur fractures in older adults: Is 1 distal locking screw enough?

BACKGROUND: Subtrochanteric (ST) fractures are proximal femur fractures with strong deforming muscular forces, which complicate anatomical reduction and treatment. These fractures are fixed with intramedullary nails with 2 distal locking screws, although the need for both screws has never been researched. PURPOSE: This study's purpose was to investigate the use of 1 versus 2 distal locking screws on clinical and radiographic outcomes and complications. STUDY DESIGN/LEVEL OF EVIDENCE: Retrospective cohort, Level of Evidence III. METHODS: An IRB-approved retrospective review of a consecutive …
