The neuropsychiatry of Parkinson's disease.

The neuropsychiatry of Parkinson's disease (PD) and its correlates are reviewed. Dementia occurs in up to 30% and can be treated with cholinesterase inhibitors. Cognitive impairments involve executive, visuospatial, attentional, and memory dysfunctions. Apathy may respond to dopamine agonists or cholines-terase inhibitors. Cognitive impairment, psychosis, and depression predict quality of life. Visual hallucinations and paranoia are common, and respond to low dose clozapine. Depression is common and predicts caregiver burden and depression. The best data suggest the efficacy of nortriptyline a

Sleep, rest-activity rhythm, cognitive and emotional symptoms in adult ADHD: unraveling the links with an actimetry-based approach.

BACKGROUND: Attention Deficit Hyperactivity Disorder (ADHD) in adults is characterized by cognitive, behavioral and affective symptoms. Emotion dysregulation (ED), psychiatric comorbidities, and disturbances in sleep and circadian rhythms frequently co-occur with ADHD, yet their relationships have rarely been investigated using objective measures and are overlooked in therapeutic approaches. This study aimed to compare sleep and rest-activity rhythm parameters between adults with ADHD and control subjects and to examine their associations with cognitive and emotional symptoms in adults with AD

Sleep disorders in renal patients: a comparative analysis of dialysis and post-renal transplant outcomes.

Sleep disorders are common among individuals with end-stage renal disease (ESRD) on dialysis and can persist after renal transplantation. This study aims to compare the association of sleep disorders between matched groups of patients with ESRD on dialysis and those who have undergone renal transplants. This is a clinic-based cross-sectional study conducted in patients with post-organ transplant and patients with ESRD on dialysis. The study involved, in addition to demographic data, administering structured sleep questionnaires, including the Epworth Sleepiness Scale (ESS), Berlin Questionnair

Sleep duration and all-cause mortality: a systematic review and meta-analysis of prospective studies.

BACKGROUND: Increasing evidence suggests an association between both short and long duration of habitual sleep with adverse health outcomes. OBJECTIVES: To assess whether the population longitudinal evidence supports the presence of a relationship between duration of sleep and all-cause mortality, to investigate both short and long sleep duration and to obtain an estimate of the risk. METHODS: We performed a systematic search of publications using MEDLINE (1966-2009), EMBASE (from 1980), the Cochrane Library, and manual searches without language restrictions. We included studies if they were

Neuroimaging insights into the pathophysiology of sleep disorders.

Neuroimaging methods can be used to investigate whether sleep disorders are associated with specific changes in brain structure or regional activity. However, it is still unclear how these new data might improve our understanding of the pathophysiology underlying adult sleep disorders. Here we review functional brain imaging findings in major intrinsic sleep disorders (i.e., idiopathic insomnia, narcolepsy, and obstructive sleep apnea) and in abnormal motor behavior during sleep (i.e., periodic limb movement disorder and REM sleep behavior disorder). The studies reviewed include neuroanatomica

Cosleeping versus solitary sleeping in children with bedtime problems: child emotional problems and parental distress.

This study investigated sleep, behavioral and emotional problems, and parental relationships and psychological distress in a group of school-aged children with bedtime problems and persistent cosleeping, compared to solitary sleepers and controls. Participants were 148 school-aged children with bedtime problems (44 cosleepers, 104 solitary sleepers) and 228 healthy peers. Results suggested that cosleepers have a significantly later bedtime, shorter nighttime sleep duration, higher Children's Sleep Habits Questionnaire (CSHQ) bedtime resistance and sleep anxiety scores, and more behavioral and

Advances in the research of melatonin in autism spectrum disorders: literature review and new perspectives.

Abnormalities in melatonin physiology may be involved or closely linked to the pathophysiology and behavioral expression of autistic disorder, given its role in neurodevelopment and reports of sleep-wake rhythm disturbances, decreased nocturnal melatonin production, and beneficial therapeutic effects of melatonin in individuals with autism. In addition, melatonin, as a pineal gland hormone produced from serotonin, is of special interest in autistic disorder given reported alterations in central and peripheral serotonin neurobiology. More specifically, the role of melatonin in the ontogenetic e

Mandibular jaw movements as a non-invasive measure of respiratory effort during sleep: application in clinical practice.

Assessment of respiratory effort (RE) is key for characterization of respiratory events. The discrimination between central and obstructive events is important because these events are caused by different physio-pathological mechanisms and require different treatment approaches. Many of the currently available options for home sleep apnea testing either do not measure RE, or RE signal recording is not always reliable. This is due to a variety of factors, including for instance wrong placement of the respiratory inductance plethysmography (RIP) sensors leading to artifacts or signal loss. Monit

Changes in tongue morphology predict responses in pharyngeal patency to selective hypoglossal nerve stimulation.

STUDY OBJECTIVES: The major goal of the study was to determine whether changes in tongue morphology under selective hypoglossal nerve therapy for obstructive sleep apnea were associated with alterations in airway patency during sleep when specific portions of the hypoglossal nerve were stimulated. METHODS: This case series was conducted at the Johns Hopkins Sleep Disorders Center at Johns Hopkins Bayview Medical Center. Twelve patients with apnea implanted with a multichannel targeted hypoglossal nerve-stimulating system underwent midsagittal ultrasound tongue imaging during wakefulness. Chan

Obstructive sleep apnea and ageing Myotonometry demonstrates changes in the soft palate and tongue while awake.

Upper airway dilator muscles are important in the pathogenesis of obstructive sleep apnea syndrome (OSAS). The present study compares changes of tissue properties between the soft palate and tongue in different age groups of apnea patients and healthy subjects. Materials and methods: OSAS patients diagnosed by polysomnography (15 patients - aged 30-70 years; 10 patients - aged 18-29 years) and healthy volunteers (10 subjects - aged 30-70 years; 10 patients - aged 18-29 years) participated in the study. Computerized endopharyngeal myotonometry was used to measure the biomechanical properties -

The Relationship Between Sleep, Cognition, and Dementia Risk in People With Focal Epilepsy.

BACKGROUND AND OBJECTIVES: Sleep disruption and cognitive impairment are important comorbidities of focal epilepsy. However, the nature to which sleep affects cognition and long-term dementia risk in focal epilepsy, compared with other brain conditions, remains unclear. We examined the relationship between sleep, cognition, and dementia risk in patients with focal epilepsy compared with healthy controls and stroke patients. METHODS: We conducted an analysis of the prospective UK Biobank cohort study, with baseline assessments performed between 2006 and 2010 and follow-up until 2021. Study gro

Disruption of sleep macro- and microstructure in Alzheimer's disease: overlaps between neuropsychology, neurophysiology, and neuroimaging.

Alzheimer's disease (AD) is the leading cause of dementia, often associated with impaired sleep quality and disorganized sleep structure. This study aimed to characterize changes in sleep macrostructure and K-complex density in AD, in relation to neuropsychological performance and brain structural changes. We enrolled 30 AD and 30 healthy control participants, conducting neuropsychological exams, brain MRI, and one-night polysomnography. AD patients had significantly reduced total sleep time (TST), sleep efficiency, and relative durations of non-rapid eye movement (NREM) stages 2 (S2), 3 (S3),

Sleep Health among Children Adopted from Foster Care: The Moderating Effect of Parent-Child Sleep Interactions.

OBJECTIVES: Sleep disruption is prevalent among children placed in foster care, elevating risk for a range of deleterious outcomes. Theoretically, achieving permanency via adoption may have a positive influence on children's sleep via the presence of various factors, but little is known about the sleep health of children adopted from foster care, including predictors and moderators of sleep health. METHOD: The current study included 226 parents who adopted a child from foster care in the U.S. (aged 4-11 years) within the past two years and a propensity score matched sample of 379 caregivers o

Neural reward processing in self-reported short sleepers: examination of gambling task brain activation in the Human Connectome Project database.

STUDY OBJECTIVES: Much of what we assume about the effects of short sleep duration on neural reward processing derives from total sleep deprivation studies. Although total sleep deprivation appears rare, habitual short sleep is common: 30% of working US adults report habitually sleeping ≤ 6 hours/night. It remains largely unknown whether habitual short sleepers exhibit similar reward processing brain activation patterns to those observed following total sleep deprivation in prior studies. Therefore, our aim was to test objectively reward processing brain activation patterns associated with sel

Juvenile myoclonic epilepsy and sleep.

Juvenile myoclonic epilepsy (JME) is a sleep-related epilepsy syndrome, and only a few studies have addressed the relationship between JME and sleep disorders. In this review, the sleep characteristics of patients with JME were summarized based on the features of circadian rhythm, the possible cause of the early morning seizures, the common subjective and objective sleep disorders, the alterations in sleep architecture, and the effect of sleep deprivation and sodium valproate (VPA). The aims of this study were to summarize the interaction between JME and sleep, to reveal JME sleep characterist
