Development of a pathway to facilitate gastrostomy insertion for patients with MND.

A pathway has been developed using a multidisciplinary group from within specialist palliative care to ensure a comprehensive approach to the insertion of gastromy tubes for patients with motor neurone disease (MND) with swallowing difficulties. The pathway has ensured that there is a coordinated approach and the professionals involved are clear as to their responsibilities in the discussion and planning of the insertion, ensuring the best support for the patient and family.

A rudimentary epiglottis associated with Pierre Robin sequence.

Epiglottis anomaly associated with Pierre Robin sequence (PRS) is a rare occurrence. To the knowledge of the authors, this is the first reported case of epiglottic anomaly associated with PRS. Doctors should remain aware of this atypical presentation in a PRS patient, and the presented case highlights the clinical challenges that must be met to ensure effective treatment of the defect in terms of compromised respiration, swallowing and feeding.

Suprahyoid pharyngotomy for base of tongue carcinoma.

Carcinoma of the tongue remain one of the greatest management challenges for the head and neck surgeon because of the adverse effects of treatment on oral and pharyngeal function. In early carcinoma of the base of tongue however, the prognosis is encouraging and function of swallowing and speech is preserved despite surgery. Suprahyoid pharyngotomy is one of the surgical approaches advocated for resection of base of tongue tumours with primary anastomosis.

Leiomyomatous hamartoma of the posterior tongue: a case report.

A case of a rare leiomyomatous hamartoma arising in the posterior tongue of a sixteen-month-old male is reported. There has been no recurrence following simple excision and presenting symptoms of choking on swallowing have resolved. Most other leiomyomatous hamartomas in the upper aerodigestive tract have been reported in Japanese patients and have involved the maxillary gingiva and hard palate.

Recurrence of Treacher Collins' syndrome with sonographic findings.

Treacher Collins' syndrome, or mandibulofacial dysostosis, is a rare, autosomal-dominant disorder. We describe the recurrence of this anomaly in the fetus of a patient who previously had delivered a severely affected infant. The ultrasonographic findings of hydramnios, absence of fetal swallowing movements, and poor growth of the biparietal diameter and head circumference permitted us to suggest the diagnosis antenatally of another affected child.

[The characteristics of aerophagy and its treatment].

Clinical and roentgenological examination of esophagus and cervical spine were carried out in 27 patients with functional aerophagia. In 11 of them the air was found to pass through without any swallowing into the stomach and in remaining 16--into esophagus. All the patients revealed osteochondrosis, unstable and blocked spines. Manual therapy applied to cervical zone of spine promoted elimination of aerophagia.

[Deglutition and partial supracricoid laryngectomies].

The indications of partial supracricoid laryngectomy have become more numerous since the first descriptions made by Majer, Labayle and Piquet. The study of the published functional results emphasizes that false passages during deglutition are the main postoperative problem. This technical note is aimed at drawing attention to the various points to be respected intra- and postoperatively to allow swallowing in as physiological a manner as possible.

Treatment of chronic aspiration as a complication of cerebrovascular accident.

Chronic aspiration remains one of the most difficult complications to treat in the patient recovering from brain stem infarction. Aspiration in these patients is often due to an incompetent glottis in association with dysfunction of the pharyngeal phase of swallowing. A systematic approach to the evaluation and treatment of these patients is presented with illustrative case presentations. Such individual treatment protocols allow for optimum rehabilitation of the post-stroke victim.

Delayed halitosis-a rare cause.

A 38-year-old tuberculous male Pakistani presented with halitosis and a cough especially marked when lying on his left side. Barium swallow demonstrated a fistula between oesophagus and left main bronchus. Anti-tuberculous therapy and repeated cauterization failed to close the fistula. Thoracotomy confirmed a congenital oesophago-bronchial fistula. Division and suture resulted in cure.

[Lateral pharyngeal pouches (author's transl)].

Lateral pharyngeal "pouches" in young men are described. They are not to be confused with true laryngeal cells as often mentioned in the literature. The radiographic diagnostic of the pharyngeal "pouches" after barium swallow together with the Valsalva test is demonstrated in review of the own casuistic. Increased intraluminal pressure caused by instrument- and glassblowers may be an etiologic factor as well as an acquired defect in the lateral pharyngeal wall.

Gastric and esophageal dysfunction after ingestion of acid.

Esophageal and gastric function was measured in a patient who swallowed a household acid solution. Dysphagia, transient ulceration of the esophagus with luminal narrowing, and complete loss of peristalsis without loss of lower esophageal sphincter function were noted. Gastric dysfunction appeared 2 weeks after ingestion with complete obstruction, necessitating antral resection. The proximal stomach was relatively spared.

Dysphagia due to vertical subluxation of the axis in rheumatoid arthritis.

Two patients with rheumatoid arthritis developed swallowing difficulties. These were mild in one case but severe in the other. Associated pyramidal tract signs and striking upward translocation of the dens made it seem probable that dysphagia was attributable to medullary compression despite the absence of other bulbar features. A history of dysphagia may therefore have serious implications for patients with rheumatoid disease.

Zebras in Adult Dysphagia Workup: Where to Look When You Think You Have Looked Everywhere.

While many patients who present with dysphagia have a clinically identifiable cause of dysphagia, the etiology of swallowing difficulty is oftentimes a diagnostic enigma. The aim of this article is to review possible etiologies of dysphagia when objective evidence of dysphagia is lacking. Included in this discussion are cricopharyngeal spasm, retrograde cricopharyngeal dysfunction, muscle tension dysphagia, dysphagia secondary to medications, and functional dysphagia.

Ingestion of pointed objects in a complex suicide.

A 51-year-old woman with a psychiatric history of depression and drug abuse was found hanged in her bedroom. The ligature was a scarf attached to the door handle. The autopsy findings suggested that two methods of suicide had been used one after the other. In the first stage, numerous nails, pins and needles were swallowed; in the second stage, the suicide was completed by hanging.

"My two-week-old daughter is throwing up blood".

Swallowed maternal blood at the time of delivery or from cracked nipples during breastfeeding is the most common cause of suspected gastrointestinal bleeding in the neonate. In this case, the Apt-Downey test is a useful diagnostic tool. The Apt-Downey test can effectively differentiate neonatal from maternal hemoglobin based on the conversion of oxyhemoglobin to alkaline globin hematin when mixed with alkali.

Perforation of the oesophagus and aorta after eating fish: an unusual cause of chest pain.

This report describes perforation of the oesophagus after eating fish complicated by perforation of the aorta six days later. The patient had not knowingly swallowed a fish bone. Aorto-oesophageal fistula is almost universally fatal. In the case described here, the fistula was demonstrated on contrast computed tomography before surgery, thus informing surgical management. The patient is the eighth reported survivor.

Glossopharyngeal neuralgia caused by adhesive arachnoid.

Glossopharyngeal neuralgia (GPN) is an uncommon disorder characterized by a severe lancinating pain commonly induced by swallowing. When the pain is resistant to medical management, surgical treatment such as microvascular decompression (MVD) or partial rhizotomy is performed. We report a case of glossopharyngeal neuralgia caused by adhesive arachnoid, in which pain disappeared solely by dissection of the peripheral arachnoid around the glossopharyngeal nerve.
