Valvular heart disease: focus on women.

Calcific aortic stenosis is a disease of the elderly and aortic valve replacement should always be considered for severe symptomatic disease; comorbidities, not age, determine surgical risk. Aortic regurgitation imposes a pressure and volume load on the left ventricle so that close monitoring is needed to identify patients at risk for irreversible left ventricular dysfunction, although symptoms are the most common indication for valve replacement. Early intervention now is recommended for rheumatic mitral stenosis due to the effectiveness of percutaneous balloon valvotomy. Earlier intervention

A giant pseudoaneurysm becoming apparent after cardiac resynchronisation therapy.

A 57-year-old male patient with coronary artery disease developed a pseudoaneurysm after an inferior infarct in 1997. He underwent coronary bypass surgery and resection of the pseudoaneurysm located at the inferior wall. Unfortunately, the pseudoaneurysm recurred due to dehiscence of the patch, necessitating a second surgical intervention. After six years he developed progressive heart failure due to severe left ventricular dysfunction. He was referred to our institution for cardiac resynchronisation therapy (CRT) because of drug refractory heart failure which was associated with a left bundle

Long-term results of the medical treatment of coronary artery disease.

The natural history of patients with ischemic heart disease varies widely depending upon the anatomic lesion and extent of left ventricular dysfunction. Several clinical variables may also be used to help determine the likelihood of long-term survival. Individuals with high-grade isolated disease of the left anterior descending coronary artery, without significant impairment of ventricular function, have little morbidity and mortality for the 2-year period after arteriography. At the other end of the spectrum, medically treated patients with stenosis of the left main coronary artery or three-v

Diagnosis and prognosis of fetal cardiomyopathies: a review.

Cardiomyopathies (CM) are a very rare disease in fetuses with a very poor outcome. Only isolated case reports and small case series were reported. According with published studies we will describe the fetal CM starting from their echocardiographic presentation: dilated cardiomyopathy (DCM) with dilatation of either or both ventricles and impaired ventricular function, and hypertrophic cardiomyopathy (HCM) with different degree of disproportionate hypertrophy of the myocardial walls. The term of the "noncompaction" of the left ventricular myocardium, is used in cases with DCM with evidence of n

Early Inflammatory Biomarkers, Ventricular Dysfunction and In-Hospital Mortality in Patients with ST-Elevation Myocardial Infarction Undergoing Primary Percutaneous Coronary Intervention.

Background/Objectives: Inflammation plays a central role in the pathophysiology of ST-elevation myocardial infarction (STEMI) and may influence myocardial injury, ventricular dysfunction and clinical outcomes. Simple inflammatory biomarkers derived from routine laboratory tests have been proposed as potential prognostic indicators in patients undergoing primary percutaneous coronary intervention (PCI). Objective: This study aimed to evaluate the association between admission inflammatory biomarkers, echocardiographic markers of ventricular dysfunction and in-hospital mortality in patients with

Multimodal Framework of Left Heart-Pulmonary Vascular Remodeling Underlying Right Ventricular Failure in PH-HFpEF.

BACKGROUND: Right ventricular (RV) dysfunction in pulmonary hypertension due to heart failure with preserved ejection fraction (PH-HFpEF) leads to adverse outcomes, yet the mechanisms underlying RV failure remain incompletely defined. We aimed to develop a multimodal framework integrating vascular mechanics and myocardial transcriptomics for a mechanistic understanding of RV dysfunction in PH-HFpEF. METHODS: In a 2-step study, a predominantly retrospective PH-HFpEF cohort (n=48) underwent comprehensive assessment with clinical evaluation, echocardiography, cardiac magnetic resonance imaging (

Transcatheter cardiac valve interventions.

Currently aortic valve replacement is performed for patients with severe aortic stenosis and symptoms or objective pathophysiologic consequences such as left ventricular dysfunction. For transcatheter mitral valve interventions, the complex pathophysiology of mitral regurgitation with varying causes along with challenging imaging and delivery issues has led to slower than anticipated clinical introduction. Transcatheter pulmonary valve intervention was primarily designed to treat the difficult problem of right ventricular to pulmonary artery conduit stenosis in the congenital population. These

Risk factor analysis for second-stage palliation of single ventricle anatomy.

BACKGROUND: Single ventricle hearts can be surgically palliated by a series of operations culminating in the Fontan procedure, which establishes a total cavopulmonary connection. The second-stage procedure creates a physiologic connection between the superior vena cava and the pulmonary artery. METHODS: From 1998 to 2010, 557 patients with single ventricle heart disease underwent second-stage surgical palliation. This cohort was retrospectively analyzed to assess patient outcome by a number of anatomic, physiologic, and procedural factors. The analysis excluded patients undergoing hybrid firs

Left ventricular filling impairment in asymptomatic chronic alcoholics.

Systolic left ventricular dysfunction is relatively common in even asymptomatic alcoholics, but whether diastolic function is also altered is much less well-studied. We used M-mode and Doppler echocardiography to study left ventricular size, mass, systolic function and diastolic filling in 32 alcoholics free of clinically detectable heart disease and in 15 healthy control subjects. Left ventricular mass index and posterior wall thickness were higher in alcoholics than in controls, but there was no statistically significant difference either in end-diastolic size or in systolic ventricular func

Congenital diaphragmatic hernia-associated cardiac dysfunction.

There is increasing evidence that cardiac dysfunction is a key contributor to CDH pathophysiology. Dysfunction in both right and left ventricles is common in the early neonatal period, contributes to clinical disease severity, and is associated with adverse outcomes including death and ECMO use. Early and routine assessment of ventricular function and pulmonary artery pressure may guide individualized clinical decision-making, including use of pulmonary vasodilators, cardiotropes, ECMO, and timing of surgical repair. Minimizing cardiac dysfunction, whether by prenatal, postnatal or perinatal t

Do echocardiographic parameters predict mortality in patients with end-stage renal disease?

BACKGROUND: Left ventricular function predicts cardiovascular mortality both in the general population and those with end-stage renal disease. Echocardiography is commonly undertaken as a screening test before kidney transplantation; however, there are little data on its predictive power. METHODS: This was a retrospective review of patients assessed for renal transplantation from 2000 to 2009. A survival analysis using demographic and echocardiographic variables was undertaken using the Cox proportional hazards regression model. RESULTS: Of 862 patients assessed for transplantation, 739 had

[Cardiac amyloidosis. Invasive and noninvasive diagnosis].

Aim of this study is the analysis of clinical, morphologic and haemodynamic features of cardiac amyloidosis. Cardiac amyloidosis was demonstrated histologically in 7 of our patients: in 6 by endomyocardial biopsy during cardiac catheterization, in one at autopsy. The clinical picture was characterized in every patient by signs and symptoms of congestive heart failure. The electrocardiogram showed several non specific signs: low voltage of the QRS complexes, both in peripheral (4/7) and precordial leads (7/7 cases); marked leftward and upward deviation of the QRS axis (6/7 cases); first degree

Valvular heart disease in adults. When to call in the surgeon.

Valvular heart disease is now amenable to surgical intervention. Valve surgery needs to be considered at the appropriate time so that patients are not denied the benefit of operation. Patients with indications for surgery should be operated on before progressive left ventricular dysfunction or the complications attendant to valvular heart disease develop. Delay of indicated surgery may increase the operative risk. This does not mean that patients should be referred for surgery before the appropriate indications appear. It does mean, however, that patients with valvular heart disease should be

Effect of early initiation of dialysis on cardiac structure and function: results from the echo substudy of the IDEAL trial.

BACKGROUND: Abnormalities of cardiac structure and function are common in patients undergoing dialysis, and cardiovascular disease is the major cause of mortality in this group. Heart failure is a common clinical manifestation of cardiovascular disease and is preceded by left ventricular hypertrophy (LVH). There are variable reports about the impact of dialysis on LVH, both deleterious and beneficial. Our study investigated whether the timing of the initiation of dialysis therapy had an impact on cardiac structure and function. STUDY DESIGN: Randomized controlled trial. SETTING & PARTICIPANT

Impact of early versus class I-triggered surgery on postoperative survival in severe aortic regurgitation: An observational study from the Aortic Valve Insufficiency and Ascending Aorta Aneurysm International Registry.

OBJECTIVES: Class I triggers for severe and chronic aortic regurgitation surgery mainly rely on symptoms or systolic dysfunction, resulting in a negative outcome despite surgical correction. Therefore, US and European guidelines now advocate for earlier surgery. We sought to determine whether earlier surgery leads to improved postoperative survival. METHODS: We evaluated the postoperative survival of patients who underwent surgery for severe aortic regurgitation in the international multicenter registry for aortic valve surgery, Aortic Valve Insufficiency and Ascending Aorta Aneurysm Internat

Preoperative management of the neonate with critical aortic valvar stenosis.

Neonatal critical aortic stenosis is a rare form of CHD that often presents with cardiogenic shock. Although surgical and cardiac catheterisation-based interventions have been successful in alleviating left ventricular outflow tract obstruction, it remains associated with high morbidity and mortality. Critical aortic stenosis results in elevated left ventricular wall stress, which ultimately increases myocardial oxygen consumption and disrupts coronary artery perfusion during diastole, leading to ventricular dysfunction and cardiogenic shock. Critical care management before definitive interven

Microaxial Flow Pump and Ivabradine for Tachycardia-Induced Cardiogenic Shock Caused by Automatic Atrial Tachycardia.

BACKGROUND: Management of tachycardia-induced cardiomyopathy (TIC) complicated by cardiogenic shock is challenging because rate control and hemodynamic stabilization must be achieved simultaneously. CASE SUMMARY: A 32-year-old woman was transferred because of refractory narrow QRS tachycardia at a rate of 140 beats/min and cardiogenic shock with a left ventricular ejection fraction of 10% to 15%. P-wave morphology, failure to terminate with cardioversion, and a warm-up pattern suggested automatic atrial tachycardia originating near the sinus node. Severe left ventricular dysfunction was consi

Reversal of systolic dysfunction in noncompaction and Wolff-Parkinson-White syndrome after accessory pathway ablation.

In a previously healthy asymptomatic 18-year old male, Wolff-Parkinson-White syndrome (WPW) with left ventricular hypertrabeculation/noncompaction (LVHT) and systolic dysfunction was detected. Holter monitoring disclosed multiple long episodes of supraventricular tachycardia with a heart rate of about 110/min. After radiofrequency ablation of an epicardial posteroseptal accessory pathway with ante- and retrograde conduction, systolic function gradually normalized without any pharmacotherapy. After 32 months of follow-up, the patient remains asymptomatic with normal systolic function. WPW-induc
