Bi-level CPAP does not change central blood flow in preterm infants with respiratory distress syndrome.

BACKGROUND: Current literature provides limited data on the hemodynamic changes that may occur during bi-level continuous positive airway pressure (CPAP) support in preterm infants. However, the application of a positive end-expiratory pressure may be transmitted to the heart and the great vessels resulting in changes of central blood flow. OBJECTIVE: To assess changes in central blood flow in infants with respiratory distress syndrome (RDS) during bi-level CPAP support. DESIGN: A prospective study was performed in a cohort of 18 Very-Low-Birth-Weight Infants who were put on nasal CPAP suppo

Rationale and design of a multicenter echocardiographic study to assess the relationship between cardiac structure and function and heart failure risk in a biracial cohort of community-dwelling elderly persons: the Atherosclerosis Risk in Communities study.

BACKGROUND: Heart failure is an important public health concern, particularly among persons>65 years of age. Women and blacks are critically understudied populations that carry a sizeable portion of the heart failure burden. Limited normative and prognostic data exist on measures of cardiac structure, diastolic function, and novel measures of systolic deformation in older adults living in the community. METHODS AND RESULTS: The Atherosclerosis Risk in Communities (ARIC) study is a large, predominantly biracial, National Heart, Lung, and Blood Institute-sponsored epidemiological cohort study.

Pulmonary hypertension and cor pulmonale.

This article is directed primarily to primary care physicians. The challenge is to suspect, diagnose, and treat pulmonary arterial hypertension when treatment is most effective and before the effects of cor pulmonale become fully manifested. A good history and physical examination should be followed by electrocardiographic and roentgenographic evaluations. Of additional value are arterial blood gas measurements, two-dimensional echocardiography, first-pass radionuclide angiography, and catheterization of the right ventricle. Chronic obstructive pulmonary disease (COPD) is one of the more commo

Atrioventricular Block Treatment: Pacing Site, AV Synchrony, or Both?

Background/Objectives: Right ventricular pacing (RVP), leadless pacing (LL), and conduction system pacing (CSP) are treatment options for atrioventricular block (AVB), each with distinct characteristics. However, the long-term outcomes of these pacing strategies remain insufficiently compared. This study evaluates clinical and echocardiographic outcomes of patients with AVB treated with dual chamber RVP, His bundle pacing (HBP), or LL. Methods: This single-center observational registry study included 22 consecutive patients receiving LL with atrioventricular resynchronization functionality (Oc

The hemodynamic effect of intrinsic conduction during left ventricular pacing as compared to biventricular pacing.

OBJECTIVES: We sought to investigate the effect of intrinsic conduction over the right bundle on the maximum rate of left ventricular pressure rise (LVdP/dt(max)) during left ventricular (LV) pacing compared to biventricular (BiV) pacing. BACKGROUND: Simultaneous BiV pacing and LV pacing both improve LV function in patients with heart failure and LV asynchrony. We studied the hemodynamic effect of intrinsic conduction leading to ventricular fusion during LV pacing. METHODS: In 34 patients with New York Heart Association functional class III or IV, sinus rhythm with normal atrioventricular (A

Cardiac magnetic resonance feature tracking in Olympic athletes: a myocardial deformation analysis.

AIMS: Athlete's heart induces extreme cardiovascular remodelling, generating challenges for the differential diagnosis with early stages of cardiomyopathies. Advanced cardiac function analysis could be helpful, but data on healthy athletes and the impact of sports disciplines are lacking. The study aims to describe myocardial deformation by cardiac magnetic resonance (CMR) in a cohort of Olympic athletes and to evaluate possible differences based on sports disciplines and sex. METHODS AND RESULTS: A group of Olympic athletes with normal cardiovascular evaluation and a group of sedentary contr

A rare variant of a ruptured sinus of valsalva aneurysm forming an aorto-atrial fistula: a case report and review of literature.

BACKGROUND: Sinus of valsalva aneurysms (SOVAs) are infrequent findings and generally diagnosed incidentally. A SOVA may be at risk for rupture, which would lead to an aorto-cardiac shunt. These patients present similarly to decompensated heart failure. CASE PRESENTATION: We present a case of a 44-year-old female with a ruptured non-coronary SOVA diagnosed by echocardiogram during evaluation for exertional dyspnoea. A trans-oesophageal echocardiogram (TEE) revealed a 2.1 cm non-coronary SOVA with windsock communication to the right atrium. The patient refused surgery, and two years later, pre

Effects of mesenchymal stromal cell-conditioned media on measures of lung structure and function: a systematic review and meta-analysis of preclinical studies.

BACKGROUND: Lung disease is a leading cause of morbidity and mortality. A breach in the lung alveolar-epithelial barrier and impairment in lung function are hallmarks of acute and chronic pulmonary illness. This review is part two of our previous work. In part 1, we demonstrated that CdM is as effective as MSCs in modulating inflammation. Herein, we investigated the effects of mesenchymal stromal cell (MSC)-conditioned media (CdM) on (i) lung architecture/function in animal models mimicking human lung disease, and (ii) performed a head-to-head comparison of CdM to MSCs. METHODS: Adhering to t

Clinical Outcome Prediction Model in Tetralogy of Fallot Without Pulmonary Valve Replacement Using Contraction Fraction From the SCOUT-TOF Registry.

BACKGROUND: Few large scale prediction models of clinical outcomes in repaired tetralogy of Fallot (rTOF) exist. Further, contraction fraction, a novel parameter indexing stroke volume by mass reflecting myocardial efficiency, has not been studied. The goals of this study were to develop and validate an rTOF prediction model of clinical outcome from a single center, the SCOUT-TOF (Single Center Outcomes Using Cardiac Magnetic Resonance in Tetralogy of Fallot) registry, using readily available cardiac magnetic resonance parameters and explore novel metrics. METHODS AND RESULTS: We retrospectiv

Angiopoietin-2-mediated integrin α5β1 and FAK signaling contributes to pulmonary arterial hypertension pathogenesis.

Pulmonary arterial hypertension (PAH) is a progressive disorder characterized by pulmonary vascular remodeling and right heart failure, in which endothelial dysfunction plays a central role. Angiopoietin-2 (ANGPT2) has been implicated in endothelial injury and vascular remodeling in multiple diseases; however, its role in hypoxic PAH remains incompletely understood. This study investigated the role and underlying mechanisms of ANGPT2 in PAH, with a particular focus on endothelial integrity, permeability, and vascular remodeling. Gene expression profiling was performed using lung tissues from p

Biventricular myocardial strain analysis using cardiac magnetic resonance feature tracking (CMR-FT) in patients with distinct types of right ventricular diseases comparing arrhythmogenic right ventricular cardiomyopathy (ARVC), right ventricular outflow-tract tachycardia (RVOT-VT), and Brugada syndrome (BrS).

OBJECTIVES: As underlying heart diseases of right ventricular tachyarrhythmias, ARVC causes wall-motion abnormalities based on fibrofatty myocardial degeneration, while RVOT-VT and BrS are thought to lack phenotypic MR characteristics. To examine whether cardiac magnetic resonance (CMR) feature tracking (FT) in addition to ARVC objectively facilitates detection of myocardial functional impairments in RVOT-VT and BrS. METHODS: Cine MR datasets of four retrospectively enrolled, age-matched study groups [n = 65; 16 ARVC, 26 RVOT-VT, 9 BrS, 14 healthy volunteers (HV)] were independently assessed

Kaempferol alleviates right ventricular hypertrophy in high-altitude pulmonary hypertension rats by modulating the AMPK/ACC/CPT1B axis and the AMPK-mediated LDHA/PDHA1 pathway.

This study evaluates the protective effects of kaempferol on right ventricular (RV) structure and function in a rat model of high-altitude pulmonary hypertension (HAPH) and explores the underlying mechanisms. Male Sprague-Dawley rats (n = 57) were exposed to a simulated high-altitude environment (5000 m) for 28 days to induce HAPH, followed by the administration of kaempferol (25, 50, 100 mg kg-1 day-1) or sildenafil (30 mg kg-1 day-1). Echocardiography, hemodynamic measurements, and histopathological analyses revealed that kaempferol significantly reduced the RV/(LV + S) index, mean pulmonary

Alternative Polyadenylation Signatures Distinguish Maladaptive Right Ventricular Remodeling in Pulmonary Hypertension: Implications for RNA-Based Diagnostics and Therapeutics.

Increased pulmonary vascular pressures due to vascular remodeling, elevated vascular resistance, and vasoconstriction characterize Pulmonary Arterial Hypertension (PAH). The narrowing of the pulmonary arteries and obstruction of blood flow increase the Right Ventricular (RV) afterload, forcing the RV to undergo structural and functional changes. While adaptive remodeling leads to RV compensation by maintaining function, maladaptive remodeling leads to RV decompensation, characterized by worsening function and eventual failure. At present, there is no effective treatment for these patients as t
