Monitoring cochlear function intraoperatively using distortion product otoacoustic emissions.

Distortion product otoacoustic emissions (DPOAEs) elicited by a bitonal stimulus complex are low-level sounds that are generated within the cochlea, and are easily measured by a miniature microphone system placed in the external auditory canal. Under conditions of rapid measurement, DPOAEs have been shown in an animal model to be exquisitely sensitive to interruption of the blood supply to the inner ear. Currently, there is no real-time procedure available for monitoring hearing function intraoperatively during neurotologic surgery. The goal of the present study was to determine the utility of

An objective assessment of voice production after radiation therapy for stage I squamous cell carcinoma of the glottis.

Radiation therapy has been the primary treatment for early glottic carcinoma, especially when restoration of normal voice is essential; yet objective evidence of the status of vocal function after treatment is lacking. The purpose of this study was to assess vocal characteristics of patients with glottic carcinoma after they had undergone radiation therapy. Twenty males, who had previously been treated with external beam irradiation for T1N0M0 squamous cell carcinoma with no subsequent evidence of recurrence, volunteered for this study. Laryngovideostroboscopic, acoustic, aerodynamic, and perc

Voice outcomes following robotic transoral, robotic retroauricular, and open hemithyroidectomy.

PURPOSE: Considering the favourable prognosis of thyroid tumours, the importance of quality of life, including functional and voice outcomes, is critically important. The impact of remote-access thyroidectomy on postoperative voice function remains unclear, and direct comparative data across major approaches are limited. This study compared patient-reported and objective voice outcomes following robotic transoral (TO), robotic retroauricular (RA), and open hemithyroidectomy. METHODS: This retrospective observational cohort study included patients undergoing hemithyroidectomy between January 2

Deconstruction and reconstruction of degrading effects in ultrasound imaging: aberration, multiple reverberation, and trailing reverberation.

Ultrasound image degradation in the human body arises from the propagation and reflection of acoustic waves in a complex acoustic environment. The heterogeneous distribution of soft tissue and the variation in acoustic properties distort the ultrasonic beam causing deterioration in image quality, including loss of resolution and contrast. Here, we establish a framework to construct images based on a separable (additive or multiplicative) representation of aberration, multiple reverberation, and trailing clutter. A separable approach enables high modularity and flexibility when generating quant

Finite Element Simulation of Two Vowel Changes in a Male Vocal Tract Model With an Aramany Class I Maxillectomy.

BACKGROUND: Acoustic analysis of vowels offers an objective method for evaluating speech in maxillectomy patients. Prior studies have focused primarily on lower formants linked to vowel identification, while changes in higher formants-critical for voice quality and individualization-remain underexplored. OBJECTIVES: This study used finite element (FE) modeling to analyze vocal tract transfer function up to 10 kHz, acoustic mode shapes, and phonation of the vowels /aː/ and /iː/ in Aramany class I maxillectomy models. METHODS: Healthy and maxillectomy vocal tract models were created using magn

A case report of critical aortic stenosis diagnosed utilizing non-imaging continuous wave Doppler probe.

BACKGROUND: Aortic stenosis (AS) is the most commonly acquired valvular pathology in the western world. Aortic stenosis severity is typically evaluated with Doppler echocardiography. Evaluation of aortic gradients using standard Doppler echocardiography from the apical window may underestimate the true gradient due to misalignment of blood flow to the ultrasound beam and is often better evaluated from other imaging windows using a non-imaging continuous wave Doppler (NI-CWD) probe. Herein, we describe a unique case of AS being underestimated by dynamic acoustic shadowing from the apical window

Total Versus Completion Thyroidectomy: A Multidimensional Evaluation of Long-Term Vocal Alterations.

BACKGROUND: Total thyroidectomy (TT) and completion thyroidectomy (CT) are two common surgical operations that are frequently followed by vocal symptoms despite preservation of the recurrent laryngeal nerve (RLN) and of the external branch of superior laryngeal nerve (EBSLN). The aim of this study was to analyze vocal alterations through endoscopic findings, videolaryngostroboscopy (VLS), acoustic vocal parameters and impact on patients' quality of life after surgery in the absence of laryngeal nerve injury. METHODS: We enrolled 198 patients who underwent thyroidectomy by the same surgeon. On

Effect of corneal stiffening on Goldmann applanation tonometry and Tono-Pen measurements in canine eyes.

PURPOSE: To experimentally examine the effect of increased corneal stiffness on Goldmann applanation tonometry (GAT) and Tono-Pen (Reichert, Inc., Depew, NY) measurements of intraocular pressure (IOP) in a canine eye model. METHODS: Twenty globes were recovered from 10 dogs with no known diseases. For each dog, corneal stiffening was induced in one eye with glutaraldehyde/phosphate buffered saline (PBS) immersion while the other cornea was immersed in PBS only. Acoustic impedance was measured before and after treatment in all eyes. After treatment, IOP was measured by GAT and Tono-Pen at true

Characterizing the relative contributions of large vessels to total ocean noise fields: a case study using the Gerry E. Studds Stellwagen Bank National Marine Sanctuary.

In 2006, we used the U.S. Coast Guard's Automatic Identification System (AIS) to describe patterns of large commercial ship traffic within a U.S. National Marine Sanctuary located off the coast of Massachusetts. We found that 541 large commercial vessels transited the greater sanctuary 3413 times during the year. Cargo ships, tankers, and tug/tows constituted 78% of the vessels and 82% of the total transits. Cargo ships, tankers, and cruise ships predominantly used the designated Boston Traffic Separation Scheme, while tug/tow traffic was concentrated in the western and northern portions of th

Top-down processes during auditory phoneme categorization in dyslexia: a PET study.

While persistence of subtle phonological deficits in dyslexic adults is well documented, deficit of categorical perception of phonemes has received little attention so far. We studied learning of phoneme categorization during an activation H(2)O(15) PET experiment in 14 dyslexic adults and 16 normal readers with similar age, handedness and performance IQ. Dyslexic subjects exhibited typical, marked impairments in reading and phoneme awareness tasks. During the PET experiment, subjects performed a discrimination task involving sine wave analogues of speech first presented as pairs of electronic

Reproducibility of response to nasal lysine-aspirin challenge in patients with aspirin-induced asthma.

BACKGROUND: Peak nasal inspiratory flow (PNIF) and acoustic rhinometry objectively measure the effects of nasal provocation testing. Although the latter is conventionally used in nasal lysine-aspirin challenge, use of the former in aspirin-induced asthma (AIA) has never been evaluated. OBJECTIVE: To evaluate the reproducibility of PNIF and acoustic rhinometry following nasal lysine-aspirin challenge in AIA. METHODS: Fourteen patients with a clear-cut history of AIA underwent nasal lysine-aspirin challenge at 2 separate visits 1 week apart. Both PNIF and minimum cross-sectional area (MCA) wer

Changes in clinical and instrumental vestibular parameters following acute exposition to auditory stress.

Besides Tullio's phenomenon, resulting from anatomic changes in the labyrinth, a hypersensitivity to acoustic stimuli of the saccular structures appears to be the underlying cause of the vestibular responses detected in some patients. In order to evaluate the incidence of vestibular symptoms triggered by acute exposure to auditory stress (disco music), 40 subjects aged between 18 and 26 years, with no audiological and vestibular disorders, were submitted to otoneurologic tests. Subjects were exposed to disco music [intensity 128 dB (C)], for 3 hours. Tests have been carried out before and imme

Ataxic dysarthria.

Although ataxic dysarthria has been studied with various methods in several languages, questions remain concerning which features of the disorder are most consistent, which speaking tasks are most sensitive to the disorder, and whether the different speech production subsystems are uniformly affected. Perceptual and acoustic data were obtained from 14 individuals (seven men, seven women) with ataxic dysarthria for several speaking tasks, including sustained vowel phonation, syllable repetition, sentence recitation, and conversation. Multidimensional acoustic analyses of sustained vowel phonati

[Study on middle ear ventilation using positional tympanometry--post mastoidectomy ear].

The middle ear is a cavity surrounded by solid bone, lined with mucosa with a lumen filled with gas. To examine this unique ventilation system under atmospheric pressure, the middle ear pressure of 50 normal ears was previously examined. Positional tympanometry, whereby the middle ear pressure is increased as the subject assumes the lateral position under atmospheric pressure was used. As a result, (1) The middle ear pressure was elevated by the change from the sitting position to the lateral position. Venous pressure was regarded as causative factor of this pressure elevation. (2) The elevati
