Bile Duct Injury after Transcatheter Arterial Chemoembolization: Risk Factors and Clinical Implications.

BACKGROUND/AIMS: The current study aims to investigate the risk factors and clinical implications of bile duct injury after performing transcatheter arterial chemoembolization (TACE) for the treatment of patients with hepatic malignancy. METHODOLOGY: A total of 2340 patients with hepatic malignancies underwent 5656 TACE procedures without any radiographic evidence of biliary abnormalities pre-TACE. Of these, 40 patients developed bile duct injuries 3 weeks to 3 months after TACE. RESULTS: TACE-induced bile duct injuries occurred in 30 out of 348 patients with liver metastatic tumors (8.6%) a

Transarterial treatment of congenital renal arteriovenous fistulas.

OBJECTIVE: Congenital renal arteriovenous fistulas (CRAVF) represent a distinct clinical entity with characteristic hemodynamic and angiographic features. Treatment is warranted given potential for growth with renal and hemodynamic compromise. We report our experience in a rare series of treated symptomatic CRAVFs. METHODS: Over a 10-year period, patients treated for symptomatic CRAVFs (no history of predisposing renal pathology, instrumentation, neoplasm, or trauma) were retrospectively investigated for clinical presentation, imaging features, treatment outcomes, and complications. Technical

Renovascular hypertension: screening and modern management.

The diagnosis and management of patients with renovascular disease and hypertension continue to elude healthcare providers. The advent of novel imaging and interventional techniques, and increased understanding of the pathways leading to irreversible renal injury and renovascular hypertension, have ushered in commendable attempts to optimize and fine-tune strategies to preserve or restore renal function and control blood pressure. Large randomized clinical trials that compare different forms of therapy, and smaller trials that test novel experimental treatments, will hopefully help formulate i

Low outflow distensible venous malformations of the anterior orbit: presentation, hemodynamic factors, and management.

PURPOSE: To describe a subclass of venous malformations of the orbit whose hemodynamic properties make them ideal for treatment with the technique of intraoperative embolization using cyanoacrylate glue followed by direct surgical excision. METHODS: A retrospective, noncomparative clinical study of 3 patients with "low-outflow" venous lesions of the anterior orbit who underwent treatment with the above technique in January 2008 and were followed for 2 years. RESULTS: Three patients presented with venous malformations of the orbit and elected to undergo surgical intervention. All cases demons

[Hilar cholangiocarcinoma (Klatskin's tumor): up-to-date radiologic diagnosis, intervention and treatment possibilities].

HISTORY AND ADMISSION FINDINGS: A 53-year-old man with jaundice was referred with suspected cholangiocarcinoma of the porta hepatis. He was found to be in a reduced general and nutritional condition. Cholinesterase was decreased, total bilirubin, GOT, GPT and alkaline phosphatase were increased. INVESTIGATIONS: Abdominal contrast-enhanced computed tomography revealed a centrally located and contrast-enhancing mass at the proximal common hepatic duct. Metastases were not detectable. Endoscopic retrograde cholangiography showed a stenosis of the proximal common hepatic duct. An additional magne

Ruptured persistent sciatic artery aneurysm managed by endovascular embolization.

Persistent sciatic artery (PSA) is a rare vascular anomaly present in 0.025% to 0.05% of the population. They are particularly prone to aneurysmal degeneration, potentially leading to distal ischemia, sciatic neuropathy, or rarely rupture. Here, we describe a case of a ruptured PSA aneurysm managed by endovascular embolization. A 70-year-old man initially presented with acute left lower extremity ischemia. He was found to have a popliteal embolus originating from a complete persistent sciatic artery aneurysm. He underwent thrombolysis followed by a femoropopliteal bypass and ligation of the pr

Fenestrated stent-graft facilitates emergency endovascular therapy for blunt aortic injury.

BACKGROUND: Endovascular stent-grafting with intentional coverage of the left subclavian artery may be used to treat aortic isthmus injury, but this procedure may have serious neurologic sequelae and may not provide an adequate proximal landing zone. In 2005, in an effort to mitigate these problems, we began to use fenestrated stent-grafts for emergency repair of blunt aortic injury (BAI). METHODS: Between 2005 and 2007, all patients in our practice with a BAI with mediastinal hematoma (except young patients without an associated critical injury) were treated with immediate endovascular stent

Mast cell chymase in the ischemic kidney of severe unilateral renovascular hypertension.

Chymase degrades angiotensin I (AI) to form angiotensin II (AII), probably constituting a bypass of the renin-angiotensin cascade. Chymase activity increases in some vascular diseases. In the kidney, an increase in chymase activity was reported in an animal model of ischemic kidney of renovascular hypertension (RVH); however, no such evidence has been provided in humans. We treated a 64-year-old patient with severe unilateral RVH and atherosclerosis, for whom removal of the ischemic kidney was the only option. Using immunohistochemical staining, we investigated chymase activity in the removed

Screening angiography of supraaortic vessels performed by invasive cardiologists at the time of cardiac catheterization: indications and results.

An aging population makes multiple vascular distributions more likely in patients arriving at the cath lab for coronary artery angiography or complete cardiac catheterization. Whether or not screening angiography of supraaortic vessels can be performed at the time of cardiac catheterization by the invasive cardiologist is still debatable. We sought to determine safety and utility of performing angiography of supraaortic vessels during cardiac catheterization. Medical records of all patients undergoing combined coronary and noncoronary angiography between May 1998 and December 2002 was retrospe

Visceral artery aneurysms: a single center experience.

BACKGROUND: Splanchnic artery areurysm is a rare but clinically relevant disease, showing a high mortality rate in emergency surgery. Reports on splanchnic aneurysms are rare and often anecdotal. The aim of this study is to discuss data obtained from 16 patients in a single vascular surgery center. MATERIALS AND METHODS: Between January 1987 and December 2000, 16 patients underwent surgery for splanchnic artery aneurysms. In 13 patients the localization was single (in two associated with an infrarenal abdominal aortic aneurysm) and in 3 patients multiple. The arteries involved were: splenic (

Source of microembolic signals in patients with high-grade carotid stenosis.

BACKGROUND AND PURPOSE: In patients with both symptomatic and asymptomatic carotid artery stenoses, the relationship between carotid plaque characteristics and transcranial Doppler (TCD)-detected microembolic signals (MES) is unclear. The purpose of this study was to examine the relationship between macroscopically described plaque characteristics and MES in patients undergoing carotid endarterectomy. METHODS: Sequential patients scheduled for carotid endarterectomy underwent preoperative 30-minute TCD monitoring of the ipsilateral middle cerebral artery to detect MES. TCD signal analysis, by

Renal insufficiency: gadopentetate dimeglumine as a radiographic contrast agent during peripheral vascular interventional procedures.

Gadopentetate dimeglumine diluted 1:1 with 0.9% normal saline was used as the radiographic contrast agent in two patients with azotemia who underwent peripheral vascular interventional procedures. The patients had no evidence of contrast material-induced renal failure after the procedures. The radiographic attenuation of the diluted gadopentetate dimeglumine was equivalent to diatrizoate meglumine diluted to 40 mg iodine per milliliter. Gadopentetate dimeglumine is an alternative radiographic contrast material for azotemic patients.

Association between fetal type posterior cerebral artery and internal carotid artery stenosis.

OBJECTIVES: The internal carotid artery (ICA) serves as the main supply artery in individuals with bilateral complete fetal type posterior cerebral artery (BcFTP), a significant anatomical variation that can impact cerebral blood flow distribution. We aimed to determine whether BcFTP was associated with the occurrence of ICA stenosis. METHODS: In this observational study, we retrospectively reviewed patients from our comprehensive stroke center whose data were extracted from a prospectively collected digital subtraction angiography (DSA) registry from January 2011 to December 2022. They were
