Endoscopic management of lateral pharyngeal pouch.

A patient with dysphagia including symptoms of regurgitation of food and worsening pulmonary symptoms was found to have a lateral pharyngeal pouch. The diagnosis was made by barium swallow study and confirmed by endoscopy. The characteristic barium swallow findings include retention of barium in a sac or diverticulum. Endoscopy showed a pouch in the left lateral pharynx, adjacent to the vallecula. Surgical therapy consisting of endoscopic stapling of the mucosal band separating the pouch from the pharynx was performed successfully, and the patient improved dramatically.

Complications of transurethral electro-incision of posterior urethral valves.

The development of dense urethral strictures following transurethral resection of posterior urethral valves is a well known complication, especially in the neonate. In 85 boys with posterior urethral valves no strictures were found during follow-up. Urinary incontinence associated with transurethral incision occurred in 3 patients and a small urethral diverticulum was found in 1. The complication rate associated with the transurethral incision itself was 5%. Our technique for transurethral incision of posterior urethral valves is described.

[Intussusception in children and adults].

The authors describe several cases of intussusception in children and one in an adult, paying attention to the underlying pathology, also in children. This must be kept in mind at the radiological examination (so called "leading points") and at the operation. Different causes are Meckel's diverticulum, ectopic pancreatic tissue, tumours. Air insufflation (= chinese method) could be safer and in case of perforation it would be less dangerous. Surgery shall be undertaken in late diagnosis, difficult or impossible reductions by enema and signs of perforation or peritonitis.

[Abnormalities of the stomach].

Malformations of the stomach are rare, their complications can be very dangerous in infancy. Clinical picture and diagnosis are discussed using 5 own cases (one 2-years-old girl with membraneous atresia, one 3-day-old boy with pylorus atresia, one 2-years-old boy with bleeding diverticulum of the stomach and a newborn with tubular hypoplasia (i.e. microgastria)). The treatment of these malformations is always surgical. Only cases of membraneous stenosis of the stomach may be treated endoscopically.

[Experience and application of diagnostic pneumoperitoneum in the anomalies in the umbilical region (author's transl)].

By means of 30 pneumoperitoneograms performed "postmortem" and five performed for diagnosis, the exploratory technique, radiologic anatomy of umbilical region, and the usefulness of the process in the diagnosis in children are discussed. Pneumoperitoneum may be indicated in diagnosis of bleeding Meckel's diverticulum, in exclusion or confirmation of remnants of the omphalomesenteric duct in cronically moist lesions of the umbilicus resistent to symptomatic treatment, in cases of non-communicating urachal cysts which can not be diagnosed by cystogram, and in the differential diagnosis of abdomi…

Tummy pain is not often what we think - Imaging findings in uncommon causes of abdominal pain in children and adolescents - A case series.

Acute abdominal pain is a common complaint in children. We came across several unusual causes of acute abdomen including jejunal haematoma, perforation and abdominal abscess following hydrostatic intussusception reduction, twisting of mesenteric cyst, perforation of sigmoid colon and Meckel's diverticulum presenting with intussusception. In this article, we aim to present imaging features of these entities so that paediatric surgeons, radiologists and other health-care providers are aware of these unusual manifestations of acute abdomen.

A Rarely Seen Complication That Causes Increase in Morbidity in Tetraplegic Patients: Zenker Diverticula.

Although spinal cord injury (SCI) damages the spinal cord, physiological changes due to SCI can affect many organs and systems of the human body. While respiratory problems are common following cervical SCI, dysphagia is a relatively uncommon secondary complication that occurs after cervical SCI. We report a case of recurrent aspiration pneumonia due to Zenker diverticulum in 26-year-old tetraplegic patient with a chronic history of silent aspirations and dysphagia contributing to functional disability.

Multiple giant diverticula of the foregut causing upper gastrointestinal obstruction.

Small bowel diverticulosis represents an uncommon disorder (except for Meckel diverticulum) often misdiagnosed since it causes non-specific gastrointestinal symptoms. Most of times the diagnosis is carried out in case of related complications, such as diverticulitis, hemorrhage, perforation or obstruction. Intestinal obstruction can be caused by inflammatory stenosis due to repeated episodes of diverticulitis, volvulus, intussusception or jejunal stones. Herein we report a case of multiple jejunal diverticula causing chronic gastrointestinal obstruction.

Common bile duct pressure and Oddi sphincter pressure in patients with common bile duct stones with and without juxta-ampullar diverticula of the duodenum.

Resting common bile duct pressure and Oddi sphincter pressure were measured in 16 patients with common bile duct stones, 8 having in addition a juxta-ampullar diverticulum. Pressure measurements were performed with an infused catheter introduced through an endoscope under direct vision. No significant differences in fasting common bile duct pressures were observed between the two groups. The Oddi sphincter had a phasic activity, and the peak pressure was similar in both groups.

Sonographic Features of Acute Sigmoid Diverticulitis Detected During Pelvic Pain Evaluation.

Acute diverticulitis, which arises from inflammation of a colonic diverticulum, is a prevalent cause of acute abdominal pain and hospitalization in developed countries. Although most cases respond to conservative treatment, serious complications such as perforation and formation of large abscesses and fistulas can arise, which would require urgent intervention. In the United States, computed tomography (CT) is the diagnostic standard, offering sensitivity and specificity >95%, but ultrasound has also proven effective in identifying uncomplicated cases of diverticulitis.

[Enterolithiasis].

The term enterolithiasis refers to concretions formed within the gastrointestinal tract. Conditions causing stasis predispose formation of enteroliths, which may be either radiopaque or radiolucent. Thus, a wide spectrum of intestinal disorders are associated with enterolithiasis including intestinal diverticulum, Crohn's ileitis, intestinal tuberculosis, radiation enteritis and strictures after surgery. The authors report one patient with intestinal obstruction as a complication of enterolithiasis and diagnosed previously as gallstone ileus. The pathogenesis and management concerning this ent…

An unusual presentation of caecal diverticulitis.

Solitary caecal diverticulitis is a rare and often misdiagnosed cause of abdominal pain. A 63-year-old Caucasian woman was admitted with a 3-day history of left upper quadrant pain and constipation. Preoperative imaging identified a possible transverse colonic tumour. At laparotomy a long, mobile ascending colon resulted in the caecal pole lying in the left upper quadrant and an inflamed gangrenous solitary caecal diverticulum was found. A right hemicolectomy was performed and the patient recovered promptly.

Bronchogenic cyst of the right hemidiaphragm mimicking a hydatid cyst of the liver: report of the first pediatric case.

Bronchogenic cyst (BC), which develops from abnormal budding of the tracheal diverticulum or ventral foregut, is a congenital bronchopulmonary malformation. Localization of the BC varies depending on the level of the abnormal budding. Thoracic or abdominal-sited diaphragmatic lesions are the rarest presentations of the BC. We present a case of BC that originated from the diaphragm and mimicking hydatid cyst of the liver in a 19-month-old girl. Diagnosis of a diaphragmatic lesion was confirmed during laparotomy and complete resection was successful.

Carbimazole embryopathy-bilateral choanal atresia and patent vitello-intestinal duct: a case report and review of literature.

BACKGROUND: In utero exposure to carbimazole for maternal hyperthyroidism has been reported to cause choanal atresia. There are case reports of patent vitello-intestinal duct and Meckel's diverticulum in similar association. CASE: We report another such instance of an infant who was exposed to carbimazole in utero, presenting with bilateral choanal atresia and patent vitello-intestinal duct. CONCLUSIONS: As there seems to be no reports of a possible association between propylthiouracil and congenital malformations, it may be safer to use propylthiouracil instead of carbimazole.

Type IX Ehlers-Danlos syndrome: bladder diverticula with transitional cell carcinoma.

Patients with type IX Ehlers-Danlos syndrome have a tendency for development of diverticulae of the urinary bladder, and these often recur after surgical excision. We report on a patient with this syndrome in whom an extensively infiltrating transitional cell carcinoma developed in a diverticulum. To our knowledge, this is the first reported case of malignancy developing in a patient with type IX Ehlers-Danlos syndrome. The highly aggressive nature of the neoplasm is discussed in relation to the dampened desmoplastic response to the tumor, observed in this patient.

Ultrasound of intussusception with lead points.

Ultrasonography of 4 cases of intussusception in children with proven lead points were reviewed retrospectively. The lead points were due to lymphosarcoma, inverted Meckel's diverticulum, jejunal polyps and an inverted appendiceal stump. The lead points form a complex mass in the centre of the intussusception in both transverse and longitudinal sections, distinct from primary intussusception. The presence of such ultrasonographic findings are suggestive of secondary intussusception with a lead point and surgical reduction rather than hydrostatic reduction should be considered.

Tubular adenomatous metaplasia (nephrogenic adenoma) of the female urethra.

We present a case of tubular adenomatous metaplasia (nephrogenic adenoma) arising within a urethral diverticulum in a woman complaining of a vaginal mass with dyspareunia. This lesion, which uncommonly affects the urethra, may clinically manifest as a gynecologic condition and mimic a low-grade adenocarcinoma on pathologic assessment. Criteria for diagnosis and current concepts of pathogenesis are discussed. Our findings, including an immunocytochemical work-up, support the concept of a reactive metaplastic response rather than nephrogenic differentiation.

Meckel's diverticula in an adult surgical unit eleven years experience.

A retrospective review of Meckel's diverticula over an 11-year period in a surgical unit, to which children were seldom admitted, revealed 28 diverticula, seven of which were symptomatic from inflammatory disease or obstruction. There was no morbidity nor mortality in this group. Twenty-one diverticula were removed as incidental findings, four patients dying from the cause of their symptoms. The complications of Meckel's diverticulum can be satisfactorily managed and are uncommon in the adult. Prophylactic diverticulectomy is difficult to justify.

Midline pericardial cyst in the posterior mediastinum: sonographic and computed tomographic correlation.

A chest radiograph revealed an incidental finding of a large mediastinal mass located in the midline posterior to the lower aspect of the heart. The computed tomogram and sonographic examinations of the chest showed the mass to be cystic in nature, contiguous with the heart, and anterior to the displaced distal esophagus. At surgery this proved to be a pericardial cyst. The unusual location of this entity, which has never before been documented, now behooves us to include pericardial cyst and diverticulum in the differential diagnosis of mediastinal masses, posterior to the heart.
