The role of the specific and nonspecific afferent systems in the mechanism of changes in evoked cortical responses in the presence of vibration.

The interrelationship of the activity of specific and nonspecific afferent systems in the mechanism of changes in the electrical activity of the cortex under the influence of vibration was studied under chronic experimental conditions. The character of the postvibrational shifts in the vestibulo- and reticulocortical evoked responses in the presence of the isolated and combined disengagement of the specific (vestibular) and nonspecific afferent systems of the brain was investigated. It was established that vestibular afferentation plays a significant role in the mechanism of postvibrational sh

Research strategies for motion and space sickness.

Research is urged for several motion sickness topics now receiving relatively little attention. The role of the area postrema, if any, is not completely resolved. The hypothesis that the cerebral spinal fluid might be a necessary neurochemical conduit should be confirmed or put to rest. It is surprising that the cerebellum, implicated more than 40 years ago, has not been reevaluated for its role in motion sickness in view of the explosion of new data on this structure. The sensory rearrangement theory, known by several names, needs direct experimental verification. One such experiment should d

Intraoperative facial nerve monitoring in acoustic neuroma surgery.

Intraoperative facial nerve monitoring simultaneously using electromyography and mechanical pressure sensors is being used in retrosigmoid and translabyrinthine approaches for acoustic neuroma resection. Insulated electrified microsurgical instruments and air drills are used to stimulate the facial nerve with a pulsed, constant current through bone and tumor, before the facial nerve is visually encountered. Electrical stimulation is used to help locate the facial nerve, map the course of the facial nerve within tumor, warn the surgeon of unexpected facial nerve locations, and help predict faci

Effect of glycerol and mannitol on perilymphatic PO2 in guinea pig cochlea.

The effects of glycerol and mannitol on partial oxygen pressure (PO2) in perilymph were studied with a polarographic technique in the guinea pig cochlea. Following a transient reduction immediately after the intravenous injection of each drug, the oxygen tension increased to reach a peak 5 to 10 minutes later, and then decreased gradually. The magnitude and duration of the PO2 increases were larger after the administration of glycerol than after that of mannitol. These results corresponded well with the clinical experience that glycerol was more effective than mannitol in improving hearing in

Hearing deterioration in professional divers: an epidemiologic study.

Hearing acuity was measured in 116 professional divers. After approximately 6 yr they were retested. At most frequencies, the divers had higher hearing thresholds than otologically normal subjects at the same age, both at the first and at the final examination. The divers' hearing deteriorated faster than that of the otologically normal subjects. The young divers' hearing thresholds were lower than in unscreened nondivers at comparable age, but the gap closed gradually with increasing age. Accordingly, the divers' hearing deteriorated faster than that of unscreened nondivers. Some of the diver

Unilateral centrifugation of the otoliths as a new method to determine bilateral asymmetries of the otolith apparatus in man.

Ten healthy subjects were eccentrically rotated with constant speed on a Barany chair. Setting of a luminous line (LL) to the subjective vertical and ocular counter-roll (OCR) were evaluated. During eccentric position rotation subjects consistently reported illusory rotation and set the LL to an angle correlating to centrifugal force. At the same time an OCR of opposite direction was measured. In one patient, labyrinthectomized on the right side, only counterclockwise rotation of the luminous line was observed. Differences between "inner" and "outer" eye were evident for luminous line settings

Chronic labyrinthine ischemia.

The purpose of the present study was to describe the audiological and vestibular deficit in patients presumed to suffer from chronic impairment of the blood supply to the labyrinth. Thirty-nine subjects affected by various pathologies often impairing systemic blood circulation, which also presented labyrinthine damage of unclear origin, were evaluated for a possible relationship between these two conditions. At the time of this study 80 per cent of the subjects showed also a certain degree of increased blood viscosity. The most common finding in the group was slow and progressive bilateral sen

Internal auditory canal vascular loops and sensorineural hearing loss.

The relationship between vascular loops in the internal auditory canal and VIII nerve disorders was examined by performing pneumatic computerized tomographic (CT) cisternography in patients with idiopathic sudden deafness, idiopathic sensorineural hearing loss, and Meniere's disease. The incidence of vascular loops was higher in the diseased group than in the control group, viz. 19 of 61 cases (30%) in the former, compared with only 6 of 88 cases (7%) in the latter. The loops were classified as of Types I-IV according to the proximity to the fundus of the internal auditory canal. A large propo

Results of endolymphatic sac to mastoid shunt surgery for Menière's disease refractory to medical therapy.

In the ten-year period ending July 1, 1986, 5475 dizzy patients were evaluated and treated medically at the Colorado Ear Clinic. Of these, 214 patients (4%) were diagnosed as having classic Meniere's disease refractory to medical therapy and underwent endolymphatic sac to mastoid shunt surgery. These patients were followed for a minimum of one year after surgery. The average audiometric follow-up was 35.1 months. Based on the American Academy of Otolaryngology-Head and Neck Surgery 1985 criteria for reporting results at one year, 60.3% had lower audiometric thresholds postoperatively (H1) or u

Endolymphatic sac-mastoid shunt surgery. A nonspecific treatment modality?

The reason for the effectiveness of endolymphatic sac-mastoid shunt surgery in the treatment of patients with Meniere's disease is still open for debate. In a double-blind study, published in 1981, we could not demonstrate any difference between the effect of a simple mastoidectomy and a regular endolymphatic sac-mastoid Silastic sheet shunt. However, a significant reduction in symptoms could be demonstrated in both groups, and 70% of patients in both groups could be classified as successes. The patients were reexamined 3 years after surgery, and it was still not possible to demonstrate any di

[Interlabyrinthine asymmetry, vestibular dysfunction and space motion sickness].

The data available suggest that a disorder in the labyrinthine paired function can be regarded as a factor responsible for the initiation and development of space motion sickness. This concept is based on the assumption that in the norm the vestibular function is to a certain extent asymmetric which is made for by a compensatory center in the central nervous system. Exposure to an unusual space environment leads to a disorder of this compensation and development of a "new" vestibular asymmetry. This exposure involves: elimination of the difference in the weight of otolith membranes, disorder o

Medical and surgical management of Meniere's disease.

Meniere's disease has been the focus of much controversy concerning its etiology and pathophysiology, not to mention treatment. A brief review is presented of the major proposed causative factors and therapy, both medical and surgical, founded upon these theories. We also present the diagnostic evaluation and therapeutic modalities employed by The Otology Group, P.C., as well as long-term, follow-up data on our patients.

Sac surgery on the only or better hearing ear.

In a neuro-otological practice, many patients are seen with bilateral Meniere's disease. Some of these poor prognostic cases start in childhood or adolescence. Of all the conservative operations, endolymphatic sac surgery alone should be considered for the only or better hearing ear. The risk of surgical anacusis is less than 2 per cent, a risk outweighed by the significant short and long term benefit for these difficult patients. Including patients who undergo revision surgery, control of vertigo is very acceptable, and hearing gain, especially in only hearing ears, is often essential for reh

[Saccular aneurysm of the anterior-inferior (middle cerebellar)--internal auditory artery].

On the basis of data in the literature (4 cases) and one case observed by the authors themselves, a clinical characteristics is given of the rarely encountered aneurysm of the middle cerebellar-internal acoustic arteries. In the absence of subarachnoid hemorrhage the aneurysm produces a typical picture of neurinoma of the acoustic nerve. The patient described was operated on with the use of a surgical microscope. Clipping of the aneurysm resulted practically in full recovery of the patient.

Reversal of deafness after renal transplantation in Alport's syndrome.

Six patients (five men and one woman) with Alport's syndrome underwent successful renal transplantation (four received kidneys from cadaver donors and two received allografts from living, related donors). One patient who had received a cadaver kidney had substantial hearing improvement and the others had stabilization of hearing. Hearing loss in Alport's syndrome is progressive. The reversal of deafness in one of our patients and stabilization in the others made us wonder whether an inherited enzymopathy had been reversed, which then mitigated the deafness.

Endolymphatic sac surgery.

There is a disturbing variation in the reported success rate in Endolymphatic Sac Surgery in the literature. Relatively long-term (24 months to 10 years) follow-ups are given in a series of 66 patients in this paper. Results reveal only a 55% long-term relief of vertigo in these patients. Only 30% of this series of cases had any improvement in cochlear function. There was a definite trend towards recurrence of vertigo in the longer follow-up cases. It is concluded that Endolymphatic Sac Surgery still is important in the treatment of idiopathic endolymphatic hydrops, but is of questionable valu
