[Vertebral stability in management of spinal metastases. Criteria and strategies for operative interventions].

BACKGROUND: Metastases are responsible for most tumor manifestations of the spine. About 25% are symptomatic; however, due to interdisciplinary management the tumor-associated mortality is regressing. Associated acute spinal cord injury (SCI) syndromes raise patient morbidity with a loss of independence and quality of life associated with a fair potential for recovery. Therefore, the management is focused on the avoidance of SCI. The assessment of mechanical stability of vertebral bodies is a central part of decision-making when considering operative therapy. This review gives an update on the

Vestibular disorders and impaired path integration along a linear trajectory.

The goal of this study was to determine if people use vestibular information to keep track of their positions while walking through a simple course. Subjects were normals and patients with chronic peripheral vestibulopathies-- each of whom were tested once--and patients with acoustic neuromas tested pre-operatively and one and three weeks post-operatively. Subjects walked over a straight course, 7.62 m, with their eyes open and then with their eyes closed. The time needed for task performance, the forward distance subjects walked before veering, and the lateral distance subjects veered from th

Comparative evaluation of an innovative deflectable percutaneous kyphoplasty versus conventional bilateral percutaneous kyphoplasty for osteoporotic vertebral compression fractures: a prospective, randomized and controlled trial.

BACKGROUND CONTEXT: Osteoporotic vertebral compression fractures (OVCFs) can be treated with percutaneous kyphoplasty (PKP). In contrast to conventional PKP, the novel deflectable percutaneous kyphoplasty (DPKP), is a unilateral transpedicular approach procedure allowing a similar bilateral puncture effect, which owes to the deflectable curved bone expander creating a transcentral line cavity to achieve uniform distribution of cement and biomechanical balance while reducing operative time and radiation exposure. PURPOSE: The purpose of this study was to prospectively compare and evaluate an i…

Dual Bipolar Strategy and Postoperative Drain Output in Robot-assisted Radical Nephrectomy.

BACKGROUND/AIM: This study aimed to investigate the clinical utility of the dual bipolar method (DBM) in robot-assisted radical nephrectomy (RARN), with a particular focus on postoperative drain output. PATIENTS AND METHODS: This prospective observational cohort study included 50 consecutive patients who underwent RARN. Patients were classified into a DBM group (n=12) and a non-DBM group (n=38) according to DBM use. The primary outcome was postoperative drain output. Secondary outcomes included operative time, console time, estimated blood loss, length of hospital stay, time to oral intake, t…

Role of preoperative CT imaging in penetrating thoraco-abdominal injuries: A multicenter study of urban trauma centers.

INTRODUCTION: Management of penetrating thoracoabdominal (PTA) injuries with signs of hemorrhage have warranted operative intervention but improved imaging capabilities have redefined interventions required. We examined outcomes of hemodynamically stable patients undergoing preoperative CT imaging with the hypothesis that CT imaging would decrease OR time without delaying OR arrival. METHODS: A retrospective multicenter study was performed amongst four urban trauma centers examining hemodynamically stable patients with PTA injuries requiring operative intervention from January 2017-December 2

Novel use of the rectus abdominus muscle flap for vaginal reconstruction in complex obstetric fistula repair: A case series.

OBJECTIVE: Globally, obstetric fistula is a tragic outcome following obstructed labour. Failure of complex repair and post-operative incontinence are common. We describe an innovative surgical technique incorporating the rectus abdominus flap at the time of fistula repair. DESIGN: Retrospective case series. SETTING: Malawi, Fistula Care Centre. METHODS: Patients were followed for 3 months after discharge to determine continence and healing. RESULTS: Five of six patients were continent at 3 months and one was lost to follow-up by dry at a one month post-operative phone call. There were no m

Trautmann-focused mastoidectomy for a simple, safe presigmoid approach: technical note.

The presigmoid approach (PSA) is selected to obtain more lateral access to cerebellopontine angle tumors, brainstem cavernous malformations, or vertebrobasilar artery aneurysms than the standard retrosigmoid approach. However, mastoidectomy for the PSA can be considered time-consuming and to carry a higher risk of complications due to the anatomical complexity of the region. The authors established a method of minimized mastoidectomy focused on exposing Trautmann's triangle as the corridor for the PSA while maximizing procedural simplicity and safety and maintaining a sufficient operative view

Operative laparoscopy for unruptured ectopic pregnancy in a caesarean scar.

Eight women diagnosed by transvaginal ultrasonography with unruptured caesarean scar pregnancy underwent operative laparoscopy as an alternative treatment to laparotomy. The ultrasonographic diagnosis of caesarean scar pregnancy was confirmed in all women at laparoscopy. None of the women required conversion of the procedure to laparotomy. The total operative time ranged from 75 to 120 minutes. The total blood loss was limited, ranging from 50 to 200 ml. All women tolerated the operation well and had uneventful recoveries. Our results show that in the hands of a well-trained operator, laparosc…

Somatostatin: a new therapeutic option for the treatment of chylothorax.

BACKGROUND: The standard treatment of chylothorax in pediatric intensive care today includes conservative therapy with fat-free nutrition, total parenteral nutrition and, if this is not successful, operative treatment (pleurodesis, ligation of the duct, pleuroperitoneal shunt). PATIENTS: We describe four patients who were not in a suitable condition for operative treatment and who were treated with continuous infusion of somatostatin. RESULTS: In three patients, chylothorax ceased with the continuous somatostatin infusion without side effects. One patient was treated without success. CONCLU

Transhiatal oesophagectomy: a comparison of alternative techniques in 68 patients.

Over a 7-year period 68 patients with oesophageal carcinoma underwent transhiatal oesophagectomy, utilizing blunt dissection in 42 cases and eversion stripping in 26. The effects of the alternative techniques on mortality and morbidity have been analysed. There was no significant difference between the two techniques in terms of operative blood loss or time or the number of patients requiring post-operative ventilatory support. Stripping of the oesophagus was associated with significantly fewer cardiorespiratory complications and a shorter hospital stay. There was no significant difference in

[Congenital digital syndactylia (author's transl)].

Congenital syndactylia of the fingers are discussed and the differential diagnosis is presented. While correction may be required for functional or easthetic purposes, the age of 4-6 years is recommended as the optimum time for correction. Early surgical treatment may be needed in cases of Apert's syndrome and in the case of congenital annular scars (amniotic bands). The operative technique is carefully described and general anaesthesia and exsanguination is recommended. Post-operative management is of equal importance and the fingers are dressed with vaseline gauze and a compression bandage.

Patterns and operative treatment of recurrent Crohn's disease: a prospective longitudinal study.

BACKGROUND: It is believed commonly that Crohn's recurrences always occur at the site of a previous intestinal anastomosis; yet this concept is probably inaccurate and not mindful of the panintestinal nature of the disease. MATERIALS AND METHODS: Between October, 1984 and March, 2003, we carried out 1,132 procedures (464 for primary and 668 for recurrent disease) on 981 consecutive patients. Patients in whom we carried out both the original procedure as well as the procedure for recurrent disease constitute this study population. Operative procedures for perineal complications were excluded f
